Prevention and Control Strategies of Nosocomial Infection and Effectiveness Evaluation in a Tertiary Teaching Hospital during the Epidemic of COVID-19

Objective: To evaluate the role of prevention and control strategies for nosocomial infection in a tertiary teaching hospital during the sudden outbreak of Corona Virus Disease 2019 (COVID-19). Methods: The hospital initiated an emergency plan involving multi-departmental defense and control. It adopted a series of nosocomial infection prevention and control measures, including strengthening pre-examination and triage, optimizing the consultation process, improving the hospital’s architectural composition, implementing graded risk management, enhancing personal protection, and implementing staff training and supervision. Descriptive research was used to evaluate the short-term effects of these in-hospital prevention and control strategies. The analysis compared changes in related evaluation indicators between January 24, 2020 and February 12, 2020 (Chinese Lunar New Year’s Eve 2020 to lunar January 19) and the corresponding lunar period of the previous year. Results: Compared to the same period last year, the outpatient fever rate increased by 1.85-fold (P P Conclusion: The nosocomial infection prevention and control strategies implemented during this specific period improved the detection and control abilities for the COVID-19 source of infection and enhanced the compliance with measures. This likely contributed significantly to avoiding the occurrence of nosocomial infection.

Research on the Evaluation of VBSE Practical Training Teaching Effectiveness Based on CIPP Model at X University

Based on the CIPP(context,input,process,and product)model,this paper constructs an index system suitable for evaluating the teaching effect of VBSE(virtual business social environment)practical training from four aspects:context evaluation,input evaluation,process evaluation,and product evaluation.Taking the students participating in VBSE practical training in X university as the survey population,381 valid sample data were obtained through an online questionnaire survey,and the index weights were determined by factor analysis method.The score value of the VBSE practical training teaching effect was calculated based on the evaluation mean value of three indexes.The results showed that the context evaluation score was 1.56,the input evaluation score was 1.54,the process evaluation score was 1.51,and the product evaluation score was 1.48.Subsequently,this paper put forward some countermeasures from the aspects of optimizing course arrangement,improving hardware facilities,and enhancing team cooperation to provide a guideline for improving the effect of VBSE practical training.

Practical effect of different teaching modes in teaching gastrointestinal surgery nursing

BACKGROUND With the continuous development and progress of medical technology,the position of surgical nursing in the field of clinical medicine is becoming in-creasingly prominent.As an important branch of the surgical field,the nursing requirements and difficulty of gastrointestinal surgery are also increasing.In order to improve the teaching quality of nursing care in gastrointestinal surgery,many educators and researchers are actively exploring new teaching methods.Among them,the teaching method case-based learning(CBL),scene-simulated learning(SSL),task-based learning(TBL),combining self-evaluation and training mode is considered as an effective method.This method aims to help students to better master knowledge and skills and improve their comprehensive quality by cultivating their self-evaluation ability.AIM To explore the practical effect of CBL-SSL-TBL combined with training mode and student self-assessment in nursing teaching of gastrointestinal surgery.METHODS Seventy-one nursing interns in our hospital from December 2020 to December 2021 were selected.According to different teaching modes,they were divided into observation group CBL-SSL-TBL combined with training mode combined with student self-assessment and control group(conventional teaching mode),of which 36 were in observation group and 35 were in control group.The results of operational skills,theoretical knowledge,nursing students'satisfaction,learning effectiveness questionnaire and teaching effect were compared between the two groups.RESULTS Compared between the two groups,the operational skills and theoretical knowledge scores of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).Compared between the two groups,the total satisfaction ratio of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Compared between the two groups,the observation group was lower than the control group in the questionnaire results of learning efficacy,and the difference was statistically significant(P<0.05).Compared between the two groups,the proportion of thinking ability,subjective initiative and understanding of theoretical knowledge in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).CONCLUSION The use of CBL-SSL-TBL combined with training mode and student self-assessment in gastrointestinal surgery nursing teaching can improve the operational skills of nursing interns,theoretical knowledge and satisfaction scores of nursing students,improve the results of learning efficiency questionnaire and teaching effect,which can be popularized in clinical teaching.

The migration of healthcare professionals,including nurses,is a global phenomenon.It is driven by various factors,including the pursuit of better opportunities,living conditions,and professional development,as well as political instability or conflict in their home countries.The World Health Organization(WHO)has noted that high-income countries often rely on foreign-trained nurses to fill gaps in their healthcare systems[1].For instance,as of 2021,over 40%(52 million)of all nurses in the United States(US)were expatriates[2].In the United Kingdom(UK),the percentage of expatriate nurses was even higher,reaching approximately 18%in 2021[3].Owing to globalization and migration,healthcare providers must possess cultural competence to deliver improved care[4,5].Culturally responsive teaching(CRT)is rooted in the idea that culture plays a vital role in shaping people’s behaviors,beliefs,values,and communication styles[6].Furthermore,these cultural factors influence patients’perspectives on health,illness,healing,and their preferences for care and communication[7].By recognizing and embracing these cultural differences,nurses can provide more effective and compassionate care to their diverse patient population[8].

Review on Hydroxyurea Usage in Young Children with Sickle Cell Disease: Examining Hemoglobin Induction, Potential Benefits, Responses, Safety, and Effectiveness

Sickle cell disease (SCD) is a prevalent condition, particularly in the countries of sub-Saharan Africa, where the presence of specific genes associated with Malaria contributes to its high prevalence. Patients with sickle cell disease frequently experience painful episodes necessitating hospitalization, and their hemoglobin levels are typically lower than those of the general population. There are different treatment options available to manage complications, such as transfusing blood, hydroxyurea, and strong anti-pains. However, with all these treatments, patients still commonly experience pain crises and suffer from organ damage. Hydroxyurea, the sole approved medication for sickle cell anemia in developed and developing countries, is widely used in children despite being primarily indicated for adults. Multiple studies have demonstrated the efficacy of hydroxyurea in inducing HbF production in young children with SCD. Elevated HbF levels have been associated with improved clinical outcomes, including a reduction in vaso-occlusive crises, acute chest syndrome, and the need for blood transfusions. Furthermore, increased HbF levels have been shown to ameliorate disease-related organ damage, such as pulmonary hypertension and sickle cell retinopathy. The response to hydroxyurea treatment in young children with SCD is variable. Some patients achieve substantial increases in HbF levels and experience significant clinical benefits, while others may have a more modest response. Factors influencing the response include baseline HbF levels, genetic modifiers, treatment adherence, and dose optimization. Safety is a crucial consideration when using hydroxyurea in young children. Studies have shown that hydroxyurea is generally well-tolerated, with the most common adverse effects being myelosuppression, gastrointestinal symptoms, and dermatological manifestations. However,long-term effects and potential risks, such as renal dysfunction and reproductive impacts, require further investigation. The effectiveness of hydroxyurea in young children with SCD has been demonstrated in various clinical trials and observational studies. These studies have shown a significant reduction in disease-related complications and improved quality of life. However, optimal dosing, treatment duration, and long-term outcomes are still areas of ongoing research. This review focuses on recent studies investigating the benefits, effectiveness, responses, and safety of hydroxyurea in pediatric individuals diagnosed with sickle cell disease.

In order to better carry out research on education and teaching,the author consulted relevant literature on blended teaching mode from 2011-2021 through CNKI,Web of Science and other websites,summarized and analyzed the research status of blended teaching mode,in order to lay a good foundation for studying blended teaching mode in course teaching.

Effectiveness of Oral English Blended Teaching Supported by Digital Technology in Universities

An empirical study of oral English blended teaching was conducted in a university.The experiment class,which was under the blended teaching mode supported by digital technology,made significant improvements in language accuracy and range,discourse length and coherence,as well as flexibility and appropriateness.On the other hand,there was no significant improvement in the scores of the control class.Through the analysis of results by means of independent t-test and paired sample t-test,we drew the following conclusions:the oral performance of university students from the experiment class significantly improved;there was remarkable difference in the total oral English scores between the experiment class and the control class;the improvement in the total score and the score of each module showed that blended teaching had significant effect on the improvement of university students’oral English performance.

A Structured Method of Teaching Epidural Block reduces the incidence of accidental dural puncture in inexperienced trainees.

Background: The rate of accidental dural puncture is particularly high during the period of training, especially in novices. The structured epidural teaching model (SETM) includes three standardized video lessons, the construction of a 3D epidural module by trainees and practical training by using an epidural simulator with and without the CompuFlo™ Epidural instrument. In this study we report the retrospective analysis of the accidental dural puncture rate of inexperienced trainees during their 6 months clinical practice rotation in obstetrics before and after the introduction of the SETM in our Institution. Method: We evaluated the incidence of accidental dural puncture before the introduction of the SETM methodology and afterwards by analyzing our departmental database from February 2019 to January 2023. All epidural blocks were executed by trainees who had never previously performed an epidural block and were about to begin their obstetrics rotation. Results: We analyzed 7415 epidurals: 3703 were performed before the introduction of the SETM methodology (control group) and 3712 afterwards (study group). The incidence of accidental dural puncture was 0.37% for the control group and 0.13% for the study group (p<.05). The probability of making an accidental dural puncture was 64% (OR: 0.36) lower for trainees who had the training than for those who did not. Conclusions: After the introduction of the structured teaching method, we observed a significant reduction of accidental dural puncture during the training period. We hope that our observation will encourage a constructive discussion among experts about the need to use standardized and validated tools as a valuable aid in teaching epidural anesthesia.

Effectiveness of Participatory Teaching Methods in the Standardized Training of Respiratory Medicine Residents

Objective:To explore and analyze the application effect of participatory teaching method in the standardized training of respiratory medicine residents.Methods:The study period was from May 2022 to May 2023.20 doctors in the standardized training of respiratory medicine residency in our hospital were taken as research subjects and grouped into the participatory group(n=10)and the routine group(n=10)by using the random number lottery method.The participatory group practised participatory teaching methods,and the routine group practised routine teaching methods.The results of discharge assessment,teaching evaluation,and mini-Clinical Evaluation Exercise(mini-CEX)assessment were compared between the groups.Results:The theoretical assessment,operational assessment,and total scores of the participatory group were significantly higher than those of the routine group,and the difference was statistically significant(P<0.05).The teaching evaluation of independent learning,theoretical mastery,teamwork,clinical thinking,communication,and learning interest of the participatory group was significantly higher than that of the routine group,and the difference was statistically significant(P<0.05).The mini-CEX assessments of history inquiry,organizational effectiveness,professionalism,physical examination,clinical diagnosis,communication skills,and overall clinical competence of the participatory group were significantly better than those of the routine group,and the difference was statistically significant(P<0.05).The teaching satisfaction of the participatory group was significantly higher than that of the routine group,the difference was statistically significant(P<0.05).Conclusion:Participatory teaching methods can improve the overall level of doctors in the respiratory medicine residency standardized training,and the teaching satisfaction is improved.

Study on the Effectiveness of the Application of Perineal Wound Model in Teaching Midwife Interns

Objective:To discuss and analyze the effectiveness of the application of perineal wound model in teaching midwife interns.Methods:The research subjects in this study were 40 midwife interns who enrolled in our hospital from January 2020 to December 2022,which were divided into a control group(consisted of 20 interns,taught with the conventional method)and a research group(consisted of 20 participants,taught with the perineal wound model)using the random number table method.The outcomes of these two groups were compared.Results:The examination results of the research group were better than control group(P<0.05);the interns of the research group were more satisfied towards the teaching method compared to the control group(P<0.05).Conclusion:Applying the perineal wound model in teaching midwife interns could effectively improve their examination achievements and teaching satisfaction,which brings a significant clinical value.

Source localization in signed networks with effective distance

While progress has been made in information source localization,it has overlooked the prevalent friend and adversarial relationships in social networks.This paper addresses this gap by focusing on source localization in signed network models.Leveraging the topological characteristics of signed networks and transforming the propagation probability into effective distance,we propose an optimization method for observer selection.Additionally,by using the reverse propagation algorithm we present a method for information source localization in signed networks.Extensive experimental results demonstrate that a higher proportion of positive edges within signed networks contributes to more favorable source localization,and the higher the ratio of propagation rates between positive and negative edges,the more accurate the source localization becomes.Interestingly,this aligns with our observation that,in reality,the number of friends tends to be greater than the number of adversaries,and the likelihood of information propagation among friends is often higher than among adversaries.In addition,the source located at the periphery of the network is not easy to identify.Furthermore,our proposed observer selection method based on effective distance achieves higher operational efficiency and exhibits higher accuracy in information source localization,compared with three strategies for observer selection based on the classical full-order neighbor coverage.

Analysis of the Application of the Ladder Teaching Method Combined with Case Teaching in Otolaryngology Nursing Teaching

Objective:To analyze the feasibility of adopting a combined ladder and case teaching method in otolaryngology nursing teaching.Method:Nursing students in the otolaryngology department were selected.According to the admission time of nursing students,30 nursing students formed a control group(conventional nursing teaching),and 30 nursing students who were admitted later formed an observation group(ladder and case teaching method).The learning performance and teaching effectiveness of nursing students under different teaching methods were analyzed.Result:The academic performance and teaching effectiveness of the observation group were higher than those of the control group(P<0.05).Conclusion:The implementation of the ladder and case teaching method during the teaching of otolaryngology nursing significantly improved the student’s academic performance and teaching effectiveness.This method has great practical value.

Models to Simulate Effective Coverage of Fire Station Based on Real-Time Travel Times

In recent years,frequent fire disasters have led to enormous damage in China.Effective firefighting rescues can minimize the losses caused by fires.During the rescue processes,the travel time of fire trucks can be severely affected by traffic conditions,changing the effective coverage of fire stations.However,it is still challenging to determine the effective coverage of fire stations considering dynamic traffic conditions.This paper addresses this issue by combining the traveling time calculationmodelwith the effective coverage simulationmodel.In addition,it proposes a new index of total effective coverage area(TECA)based on the time-weighted average of the effective coverage area(ECA)to evaluate the urban fire services.It also selects China as the case study to validate the feasibility of the models,a fire station(FS-JX)in Changsha.FS-JX station and its surrounding 9,117 fire risk points are selected as the fire service supply and demand points,respectively.A total of 196 simulation scenarios throughout a consecutiveweek are analyzed.Eventually,1,933,815 sets of valid sample data are obtained.The results showed that the TECA of FS-JX is 3.27 km^(2),which is far below the standard requirement of 7.00 km^(2) due to the traffic conditions.The visualization results showed that three rivers around FS-JX interrupt the continuity of its effective coverage.The proposed method can provide data support to optimize the locations of fire stations by accurately and dynamically determining the effective coverage of fire stations.

Effectively modulating spatial vortex four-wave mixing in a diamond atomic system

Due to the spatial characteristics of orbital angular momentum,vortex fields can be applied in the fields of quantum storage and quantum information.We study the realization of spatially modulated vortex fields based on four-wave mixing in a four-level atomic system with a diamond structure.The intensity and spiral phase of the vortex field are effectively transferred to the generated four-wave mixing field.By changing the detuning of the probe field,the phase and intensity of the generated vertex four-wave mixing field can be changed.When the probe field takes a large detuning value,the spatial distribution of the intensity and phase of the vertex four-wave mixing field can be effectively tuned by adjusting the Rabi frequency or detuning value of the coupled field.At the same time,we also provide a detailed explanation based on the dispersion relationship,and the results agree well with our simulation results.

Impact of effective stress on permeability for carbonate fractured-vuggy rocks

To gain insight into the flow mechanisms and stress sensitivity for fractured-vuggy reservoirs,several core models with different structural characteristics were designed and fabricated to investigate the impact of effective stress on permeability for carbonate fractured-vuggy rocks(CFVR).It shows that the permeability performance curves under different pore and confining pressures(i.e.altered stress conditions)for the fractured core models and the vuggy core models have similar change patterns.The ranges of permeability variation are significantly wider at high pore pressures,indicating that permeability reduction is the most significant during the early stage of development for fractured-vuggy reservoirs.Since each obtained effective stress coefficient for permeability(ESCP)varies with the changes in confining pressure and pore pressure,the effective stresses for permeability of four representative CFVR show obvious nonlinear characteristics,and the variation ranges of ESCP are all between 0 and 1.Meanwhile,a comprehensive ESCP mathematical model considering triple media,including matrix pores,fractures,and dissolved vugs,was proposed.It is proved theoretically that the ESCP of CFVR generally varies between 0 and 1.Additionally,the regression results showed that the power model ranked highest among the four empirical models mainly applied in stress sensitivity characterization,followed by the logarithmic model,exponential model,and binomial model.The concept of“permeability decline rate”was introduced to better evaluate the stress sensitivity performance for CFVR,in which the one-fracture rock is the strongest,followed by the fracture-vug rock and two-horizontalfracture rock;the through-hole rock is the weakest.In general,this study provides a theoretical basis to guide the design of development and adjustment programs for carbonate fractured-vuggy reservoirs.

Empowerment of Artificial Intelligence in Teaching Reform of Ideological and Political Courses in Universities

The rapid development of artificial intelligence(AI)technology has brought new opportunities and challenges to the field of education.As an important link in cultivating students’comprehensive quality and socialist core values,it is necessary to carry out continuous teaching reform and innovation in ideological and political courses in colleges and universities.Based on the concept of AI empowering the teaching reform of ideological and political courses,this study aims to explore how to use artificial intelligence technology to improve the teaching effect and learning experience of ideological and political courses.The research first analyzes the application status of artificial intelligence technology in education,and then discusses the application potential of artificial intelligence in ideological and political courses.Subsequently,the teaching reform strategy of ideological and political courses based on artificial intelligence is proposed,including the use of virtual reality technology,the application of intelligent auxiliary teaching tools to enhance personalized teaching,and the construction of an intelligent learning management system.Lastly,a case analysis is conducted to explore the implementation effect of the teaching reform of ideological and political courses in universities.The results showed that the application of artificial intelligence technology can effectively improve the teaching effect and learning experience of ideological and political courses,and provide new ideas and methods for the teaching reform of ideological and political courses in universities.
