Myocardial fibrosis detection in arrhythmogenic right ventricular dysplasia/cardiomyopathy by 3. 0T magnetic resonance imaging

Arrhythmogenic right ventricular dysplasia/cardiomyopathy (ARVD/C) is characterized by fibro-fatty replacement of the right ventricle.However,the feasibility and significance of myocardial fibrosis detec-ted by delayed enhancement (DE) using 3.0T magnetic resonance imaging (MRI) in.ARVD /C is seldomly studied.Methods Twenty-seven consecutive patients were prospectively evaluated for ARVD /C.Magnetic reso-nance imaging was performed on a 3.0T scanner.Ten minutes after intravenous administration of 0.2 mmol /kg of gadodiamide,DE-MRI was obtained.Diagnosis of ARVD /C was based upon the Task Force criteria and in-cluded MRI findings.Results Seventeen(59% ) of 27 patients met the Task Force criteria for ARVD /C.Right ven-tricle DE was found in all (100% ) ARVD /C patients compared with none (0%) of the 10 patients without ARVD /C (P <0.001) .Additional left ventricular DE was found in 8/17 ARVD/C patients while without left ventricular mor-phological and functional abnormalities detected by echocardiography or MRI.Conclusions DE using 3.0T MRI could effectively detect myocardial fibrosis in the right and left ventricular myocardium in ARVD /C patients.Detection of myocardial fibrosis may have an important clinical significance in ARVD/C diagnosis.Histological left ventricle in-volvement may be easily missed by echocardiography. 展开更多

目的评价3T磁共振血管成像(3.0-Telsa magnetic resonance angiography,3T MRA)对糖尿病患者下肢动脉病变(peripheral artery disease,PAD)的诊断价值。方法根据大血管彩超结果,对下肢血管狭窄>50%的46例2型糖尿病患者做3T MRA检查,... 目的评价3T磁共振血管成像(3.0-Telsa magnetic resonance angiography,3T MRA)对糖尿病患者下肢动脉病变(peripheral artery disease,PAD)的诊断价值。方法根据大血管彩超结果,对下肢血管狭窄>50%的46例2型糖尿病患者做3T MRA检查,并用数字减影血管造影(digital subtraction angiography,DSA)作为对照,评价其对髂总、股、小腿和足动脉等4段血管病变诊断的意义。结果3T MRA能完整显示整个下肢PAD,对踝以上动脉血管病变显像清晰,且病变分布广泛、远端重于近端、呈多节段性狭阻。3T MRA显示的股动脉、小腿动脉的狭窄>50%病变节段数与DSA无明显差异(P=0.965,P=0.846),但其诊断的足部动脉狭阻节段数明显多于DSA(P=0.006).3T MRA诊断狭窄>50%的动脉病变的敏感度和特异度分别为:股动脉皆为100%,小腿动脉为95.62%和100%,足动脉为100%和72.22%。两种方法诊断不同节段动脉病变的κ值依次为:股动脉1.000(P<0.001),小腿动脉0.949(P<0.001),足部动脉0.635(P<0.05)。结论3T MRA可清楚显示糖尿病患者踝以上的动脉病变,对股及小腿动脉病变的诊断价值与DSA类似,但对足部动脉可能有夸大效应。 展开更多

目的分析脑动脉CT血管成像(CT angiography,CTA)、磁共振血管成像(Magnetic resonance angiography,MRA)分别联合血清基质金属蛋白酶9(matrix metalloproteinase 9,MMP-9)、天冬氨酸特异性半胱氨酸蛋白酶3(Caspase-3)在颅内动脉瘤诊断... 目的分析脑动脉CT血管成像(CT angiography,CTA)、磁共振血管成像(Magnetic resonance angiography,MRA)分别联合血清基质金属蛋白酶9(matrix metalloproteinase 9,MMP-9)、天冬氨酸特异性半胱氨酸蛋白酶3(Caspase-3)在颅内动脉瘤诊断中的应用价值。方法选取上海市第一人民医院收治的86例疑似颅内动脉瘤患者,术前常规检查血清MMP-9、Caspase-3水平,并行CTA与MRA诊断,以手术病理为"金标准",比较CTA、MRA分别联合血清MMP-9、Caspase-3诊断准确性、特异度、灵敏度,观察2种联合诊断方式对动脉瘤分布的检查情况。结果CTA联合血清MMP-9、Caspase-3诊断颅内动脉瘤灵敏度97.37%,特异度90.00%,准确性96.51%,Kappa值0.84;MRA联合血清MMP-9、Caspase-3诊断灵敏度85.53%,特异度70.00%,准确性83.72%,Kappa值0.41;CTA联合血清MMP-9、Caspase-3诊断灵敏度、准确性显著高于MRA联合血清MMP-9、Caspase-3(P<0.05),且在诊断动脉瘤分布情况方面优于MRA联合血清MMP-9、Caspase-3。结论与MRA相比,CTA联合血清MMP-9、Caspase-3对颅内动脉瘤诊断灵敏度、准确性更高,CTA在动脉瘤分布情况显示方面具有更大优势。 展开更多

MR Visualization of Spinal Dural Arterio-Venous Fistula Using T2-Weighted 3D SPACE—A Spin-Echo Technique

Pre- and post-contrast MR imaging of the spin... Spinal dural arteriovenous fistulas (SDAVF) account for about 70% of all spinal AV shunts. We re-port on a 56-year-old male patient with progressive neurological symptoms. Pre- and post-contrast MR imaging of the spine, including contrast-enhanced MR angiography was done on a 1.5 T MR scanner. In addition a special (pre-contrast) 3D turbo-spin-echo sequence with variable flip angle evolution (SPACE) was performed providing a slice thickness of 0.9 mm. T2-weighted images showed extensive widened and tortuous vascular structures with typical flow voids intradurally due to dilated pial veins. The origin of the spinal dural arteriovenous fistula was found to be the radiculomeningeal artery at level D10/11 on the left side, visualized by the T2-weighted 3D SPACE sequence and CE MRA. After imaging the patient was referred to an interventional neuroradiology department for therapy. With the detailed information from MR imaging concerning the fistula level the interventional radiologist was able to find the fistula immediately and consequently treated it successfully by endovascular therapy. Interpreting the MR images of this obvious case of a SDAVF, SPACE sequence after post processing with 3D MPR software directly led to the location of the AV shunt at D10/11 on the left side, better than 3D contrast-enhanced MR angiography did. Therefore, SPACE can be a useful adjunct in further evaluation of spinal dural arteriovenous fistula and especially in defining fistula level. 展开更多

目的探讨颅内动脉瘤的临床表现及诊治经验。方法 回顾性分析2007年10月至2010年3月我院103例颅内动脉瘤患者发病、临床表现、影像资料。结果 103例患者中共查出126个颅内动脉瘤。其中单发84例,多发性颅内动脉瘤19例(15例有2个动脉瘤,... 目的探讨颅内动脉瘤的临床表现及诊治经验。方法 回顾性分析2007年10月至2010年3月我院103例颅内动脉瘤患者发病、临床表现、影像资料。结果 103例患者中共查出126个颅内动脉瘤。其中单发84例,多发性颅内动脉瘤19例(15例有2个动脉瘤,4例有3个动脉瘤)。1例合并动静脉畸形,1例合并颈内动脉颅外段夹层动脉瘤,1例合并烟雾病。未破裂的颅内动脉瘤15例,占14.6%(15/103)。后交通动脉瘤最多见,占32.5%(41/126)。动脉瘤直径1.5~35mm不等,其中2.0~4.9mm者占61.9%(78/126)。结论 采用适宜的影像学检查及时诊断颅内动脉瘤并及时血管内治疗,可明显降低致死率和致残率。 展开更多

目的探讨三维动态增强磁共振血管成像(3D DCE MRA)三期扫描技术,评价其在肝移植术前的诊断价值.方法对拟进行肝移植的183例患者进行术前3D DCE MRA成像扫描,采用Siemens Symphony 1.5T超导MRI扫描机,圆形极化相控阵体线圈,3D DCE MRA三... 目的探讨三维动态增强磁共振血管成像(3D DCE MRA)三期扫描技术,评价其在肝移植术前的诊断价值.方法对拟进行肝移植的183例患者进行术前3D DCE MRA成像扫描,采用Siemens Symphony 1.5T超导MRI扫描机,圆形极化相控阵体线圈,3D DCE MRA三期(动脉期、门脉期和静脉期)扫描技术,即对所得图像进行综合评价.结果全部病例均获得了满意的血管成像图像,肝动脉可显示2~3级分支,门静脉可显示2~5级分支,肝静脉可显示1~2级分支.183例肝移植患者,5例显示肝动脉变异,其中2例起自肠系膜上动脉,2例直接起自腹腔干,1例起自胃左动脉.单纯肝硬化门脉高压103例,其中,冠状静脉和食道胃底静脉曲张23例、脐周静脉曲张5例、肠系膜静脉曲张2例、脾门周围静脉曲张30例,门静脉玻璃样变性1例;原发性肝癌79例,5例肝动脉包埋、僵直、推移,2例肝内动-静脉瘘,门静脉右支癌栓23例,门静脉左支癌栓7例,门静脉主干癌栓3例,同时发生门静脉左右支癌栓的2例,MRA表现为门静脉呈半月形或杯口形缺损或不显影.7例肝静脉出现栓塞,5例下腔静脉受压推移,1例下腔静脉瘤栓.结论 3D DCE MRA三期扫描能很好的显示肝动脉、门静脉、肝静脉及下腔静脉系统病变,肝移植术前应用3D DCE MRA,基本达到临床要求,是术前血管评估的有效方法. 展开更多

Modern post-mortem investigations use an increasing number of digital imaging methods,which can be collected under the term“post-mortem imaging”.Most methods of forensic imaging are from the radiology field and are therefore techniques that show the interior of the body with technologies such as X-ray or magnetic resonance imaging.To digitally image the surface of the body,other techniques are regularly applied,e.g.three-dimensional(3D)surface scanning(3DSS)or photogrammetry.Today’s most frequently used techniques include post-mortem computed tomography(PMCT),post-mortem magnetic resonance imaging(PMMR),post-mortem computed tomographic angiography(PMCTA)and 3DSS or photogrammetry.Each of these methods has specific advantages and limitations.Therefore,the indications for using each method are different.While PMCT gives a rapid overview of the interior of the body and depicts the skeletal system and radiopaque foreign bodies,PMMR allows investigation of soft tissues and parenchymal organs.PMCTA is the method of choice for viewing the vascular system and detecting sources of bleeding.However,none of those radiological methods allow a detailed digital view of the body’s surface,which makes 3DSS the best choice for such a purpose.If 3D surface scanners are not available,photogrammetry is an alternative.This review article gives an overview of different imaging techniques and explains their applications,advantages and limitations.We hope it will improve understanding of the methods. 展开更多
