目的探讨妊娠期肝内胆汁淤积症(intrahepatic cholestasis of pregnancy,ICP)患者血清及胎盘组织中人类含I型血小板结合蛋白基序的解聚蛋白样金属蛋白酶1(human depolymerizin-like metalloproteinase-1 containing the type I platelet...目的探讨妊娠期肝内胆汁淤积症(intrahepatic cholestasis of pregnancy,ICP)患者血清及胎盘组织中人类含I型血小板结合蛋白基序的解聚蛋白样金属蛋白酶1(human depolymerizin-like metalloproteinase-1 containing the type I platelet-binding protein motif,ADAMTS-1)和ADAMTS-4的表达水平。方法选择2021年7月至2022年3月苏北人民医院产科收治的妊娠晚期ICP患者(ICP组,22例)和同期住院正常待产孕妇(对照组,20例),采用酶联免疫吸附试验(enzyme linked immunosorbent assay,ELISA)和免疫组化试验(immunohistochemistry,IHC)分别分析ADAMTS-1和ADAMTS-4的表达水平,比较两组血清及胎盘组织中ADAMTS-1和ADAMTS-4表达水平的差异。结果ICP组年龄24~34岁,其中初产妇12例,经产妇10例。对照组年龄21~34岁,其中初产妇11例,经产妇9例。与对照组相比,ICP组平均产时孕周提前([38.0±1.5)周比(39.7±0.9)周]、血清总胆汁酸(total bile acid,TBA)较高([27.33±16.94)μmol/L比(2.85±1.58)μmol/L]、血清ADAMTS-1较高([49.41±4.39)ng/ml比(33.01±3.13)ng/ml]、血清ADAMTS-4较高([134.47±17.47)ng/ml比(87.39±12.35)ng/ml],差异均有统计学意义(P<0.05)。与对照组相比,ICP组胎盘组织ADAMTS-1和ADAMTS-4强阳性比例较高(36.36%比10%,22.73%比15.00%),差异均有统计学意义(P<0.05)。结论ADAMTS-1、ADAMTS-4在ICP患者血清及胎盘中的表达明显增加,提示二者可能参与ICP的发生发展。展开更多

Objective:To observe the expression of CTX-I(Ⅰcollagen C-terminal foreign body peptide)and TRACP(tartrate-resistant acid phosphatase)in different syndromes of knee osteoarthritis and their correlation with ADAMTS-4(p...Objective:To observe the expression of CTX-I(Ⅰcollagen C-terminal foreign body peptide)and TRACP(tartrate-resistant acid phosphatase)in different syndromes of knee osteoarthritis and their correlation with ADAMTS-4(polyproteoglycan enzyme-4)and MMPS-3(matrix metalloproteinase-3).Methods:A total of 98 patients diagnosed with knee osteoarthritis were enrolled from the Rehabilitation Pain Department and Rheumatology Department of Wuhan Caidian District People's Hospital.According to their syndrome types,they were divided into 40 cases of Yang deficiency and cold constipation type,28 cases of qi stagnation and blood stasis type,and 30 cases of kidney jing deficiency type.The contents of bone metabolism indexes,ADAMTS-4 and MMPS-3 in different syndromes of the three groups were compared.Correlation of bone metabolism indexes with ADAMTS-4 and MMPS-3.Results:There were statistically significant differences in the contents of CTX-I,TRACP,ADAMTS-4 and MMPS-3 among three groups with different syndromes(P<0.05).There was a positive correlation between CTX-I,TRACP,ADAMTS-4 and MMPS-3(P<0.05).The syndrome of qi stagnation and blood stasis was positively correlated with TRACP and ADAMTS-4,and negatively correlated with MMPS-3(P<0.05).The renal essence deficiency syndrome was positively correlated with CTX-I,ADAMTS-4 and MMPS-3(P<0.05).Bone metabolic indexes(CTX-I,TRACP)were positively correlated with ADAMTS-4 and MMPS-3(P<0.05).Conclusion:Different syndromes of knee osteoarthritis are correlated with bone metabolism indexes,ADAMTS-4 and MMPS-3,which can provide a certain objective basis for clinical TCM syndrome differentiation and classification。展开更多

目的:观察电针“痛敏穴”联合热敏灸对膝骨性关节炎的临床疗效及对血脂水平、聚蛋白多糖酶4(a disintegrin and metalloprotease with thrombospondin motifs,ADAMTS-4)、基质金属蛋白酶-3(matrix metalloproteinase-3,MMPs-3)影响。方...目的:观察电针“痛敏穴”联合热敏灸对膝骨性关节炎的临床疗效及对血脂水平、聚蛋白多糖酶4(a disintegrin and metalloprotease with thrombospondin motifs,ADAMTS-4)、基质金属蛋白酶-3(matrix metalloproteinase-3,MMPs-3)影响。方法:将80例膝骨性关节炎患者随机分为治疗组40例与对照组40例。治疗组给予电针联合热敏灸,每周6次,连续治疗4周;对照组给予关节腔内注射玻璃酸钠,每周1次,连续治疗4周。结果:两组在治疗期,视觉模拟评分量表(visual analogue scale,VAS)均呈现下降趋势,组内比较,差异具有统计学意义(P<0.05);在治疗2周、治疗4周、随访2周,治疗组得分均低于对照组(P<0.05)。两组治疗后(美国西安大略麦马斯特大学骨性关节炎指标(Western Ontario and McMaster Universities osteoarthritis index,WOMAC)各项目得分均明显下降,治疗组疗效优于对照组(P<0.05)。治疗组在4周治疗后,总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、低密度脂蛋白胆固醇(LDL-C)含量下降,高密度脂蛋白胆固醇(HDL-C)含量上升,其改善程度优于对照组(P<0.05)。两组治疗后ADAMTS-4、MMPs-3含量均下降,组间比较,差异有明显统计学意义(P<0.05);治疗组有效率为87.5%,对照组有效率为60.0%,治疗组疗效更佳(P<0.05)。结论:电针“痛敏穴”联合热敏灸能明显改善膝骨性关节炎患者的症状,调节血脂水平,安全性高,值得临床推广使用。展开更多
