Therefore, greater therapeutic efficacy for cerebral palsy could be obtained by selecting the five Shu acupoints, which are located at the qi-intersecting point of the twelve meridians below the elbow and knee. OBJECTIVE: To treat children with spastic cerebral palsy using acupuncture and massage at the Shu and He points through a combination of rehabilitative training, and to compare the effects with routine acupuncture and massage. DESIGN: A randomized controlled clinical trial. SETTING: Rehabilitation Center for Cerebral Palsy, Department of Pediatrics, Affiliated Hospital of Liaoning University of Traditional Chinese Medicine. PARTICIPANTS: Sixty children with spastic cerebral palsy were selected from the Rehabilitation Center for Cerebral Palsy, Department of Pediatrics, Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from May 2003 to February 2006. There were 38 boys and 22 girls, aged 10 months to 4 years old, muscle strength ranging from grade I to grade IV. The children were randomly divided into a treatment group (n =30) and a control group (n =30). Informed consent for the therapeutic program was obtained from the relatives of all enrolled children. The study was approved by the Hospital's Ethical Committee. METHODS: All children were given exercise and occupational therapy based on standard treatments. (1) In the control group, the children were additionally treated with routine acupuncture and massage during the one-month basic rehabilitative training. The acupuncture needles were manipulated alternately at the acupoints of Jianyu, Shousanli, Binao, Hegu, Huantiao, Xuanzhong, and Biguan. They were treated once per day, 6 days a week, followed by one day of rest. The children's limbs were first massaged by pressing, stretching, rolling, and kneading. Then, the same acupuncture points were pressed with the thumb for 30 minutes each time, once a day, 6 days a week, followed by one day of rest. The treatment continued for 1 month. (2) In the treatment group, the children were administered acupuncture and massage at the Shu and He points during the one-month basic rehabilitative treatment. For acupuncture of the upper limbs, the following acupoints were used: Sanjian and Quchi of the Large Intestine Meridian of Hand-Yangming, and Houxi and Xiaohai of the Small Intestine Meridian of Hand-Taiyang. The acupoints were acupunctured alternately every other day. For acupuncture of the lower limbs, the following acupoints were used; Zulinqi and Yanglingquan of the Gall bladder Meridian of Foot-Shaoyang, Taibai and Yinlingquan of the Spleen Meridian of Foot-Taiyang, or Xiangu and Zusanli of the Stomach Meridian of Foot-Yangming. The acupoints were acupunctured alternately every other day. Three needles were for the medial malleolus and three needles for the lateral malleolus. They were treated once a day, 6 days a week, followed by one day of rest. The children's limbs were first massaged by means of pressing, stretching, rolling, and kneading. The same acupuncture points were then pressed with the thumb for 0.5-1 minute at each point, 30 minutes each time, once a day, 6 days a week, followed by one day of rest. MAIN OUTCOME MEASURES: Therapeutic efficacy was evaluated using gross motor function measurements, as well as activity of daily living scale before and after treatment. RESULTS: All 60 enrolled children with spastic cerebral palsy were included in the final analysis of results. (1) Comparison of gross motor function scores: The gross motor function score after treatment was significantly higher in both the treatment group and control group than before treatment (t = -3.86 and -8.882, respectively, P 〈 0.05). The scores in the treatment group were significantly higher than the control group scores (t = -7.166, P 〈 0.01). (2) Changes in activity of daily living scores: The scores for activity of daily living in the treatment group was significantly higher after treatment than before treatment (t = -3.933, P 〈 0.01), and was also significantly higher than the control group (t = -0.671, P 〈 0.05). CONCLUSION: Acupuncture and massage at the Shu and He acupoints can significantly ameliorate motor function deficits and movement disabilities in children with spastic cerebral palsy. The therapeutic efficacy at these acupoints is better than routine acupuncture and massage.展开更多

However, there are few reports addressing changes in neurotransmitter expression following manual acupuncture and electroacupuncture against visceral traction pain.OBJECTIVE: To explore changes in neurotransmitter expression in the ileum and protein expression in the medullary visceral zone of visceral traction pain rats undergoing pretreatment of emulational manual acupuncture, and to investigate the differences between emulational manual acupuncture and electroacupuncture.DESIGN, TIME AND SETTING: The randomized, controlled study was performed at the Biomedical Engineering Laboratory, Shanghai University of Traditional Chinese Medicine, Shanghai, China from August 2008 to July 2009.MATERIALS: G6805 electroacupuncture apparatus (Shanghai Medical Electronic Machine Factory, China) and ZSF-I acupuncture manipulation simulation therapeutic system (Chinese Medical Engineering Room, Shanghai University of Traditional Chinese Medicine, Shanghai China) were used in the present study.METHODS: A total of 40 male Sprague Dawley rats were equally and randomly assigned to sham surgery, model, emulational manual acupuncture and electroacupuncture groups. In the emulational manual acupuncture and electroacupuncture groups, emulational manual acupuncture and electroacupuncture were applied at bilateral Zusanli (ST 36) acupoints for 30 minutes, and models of visceral traction pain were established immediately.MAIN OUTCOME MEASURES: Substance P expression, c-Fos and glial fibrillary acidic protein expression were measured using immunohistochemistry. Acetylcholine esterase activity was examined utilizing a colorimetric method. Leucine-enkephalin content was detected using a radioimmune assay. Degree of pain in rats was assessed by pain score.RESULTS: Pain score, substance P expression in the ileum, acetylcholine esterase activity, expression of c-Fos protein and glial fibrillary acidic protein in the medullary visceral zone were significantly decreased following pretreatment of emulational manual acupuncture and electroacupuncture in rats with visceral traction pain (P〈0.05). Compared with the electroacupuncture group, the leucine-enkephalin content was significantly increased, and pain score was significantly diminished in the emulational manual acupuncture group (P〈0.05).CONCLUSION: Emulational manual acupuncture pretreatment decreases acetylcholine esterase activity, increases leucine-enkephalin release, downregulates expression of c-Fos protein and glial fibrillary acidic protein and ultimately inhibits visceral traction pain by reducing substance P release. The effectiveness in inhibiting visceral traction pain is greater when using emulational manual acupuncture compared with electroacupuncture. This is because emulational manual acupuncture effectively increases leucine-enkephalin release.展开更多

Methods: In the present study, triple-center, single-blind, randomized and control methods were adopted. Qualified subjects were randomly assigned to three groups: acupuncture-massage (acu-mas) group, acupuncture group, and massage group (60 subjects in each group). Bilateral Fengchi (风池GB 20), Jiaji (夹脊EX-B 2, C_ 4-7) and Baihui (百会GV 20) were punctured in combination with massage at the neck-shoulder and back region. The treatment was conducted once daily, 14 times altogether. Results: After treatment, of the 60 cases in each of acu-mas, acupuncture and massage groups, 28, 11 and 13 were cured, 13, 20 and 17 markedly effective, 12, 14 and 16 effective, and 7, 15 and 14 ineffective, with the total effective rates being 88.3%, 75.0% and 76.7% respectively. The therapeutic effect of acu-mas group was significantly superior to that of simple acupuncture and massage groups (P<0.05). The scores of clinical symptoms and signs in 3 groups all declined significantly after treatment (P<0.01), and the score of symptoms of acu-mas was markedly lower than those of acupuncture and massage groups (P<0.05). No significant differences were found among 3 groups in the score of clinical signs and between acupuncture and massage groups in the score of clinical symptoms (P>0.05). The time for producing therapeutic effects in acu-mas group was significantly shorter than those of acupuncture and massage groups (P<0.01, 0.05), meaning a faster of acu-mas group in producing clinical effect. No adverse events were found in all the 3 groups. Conclusion: Acupuncture and massage are effective and safe in the treatment of VACS, and the therapeutic effect of acupuncture combined with massage is superior to that of simple acupuncture and simple massage therapy.展开更多

This is because emulational manual acupuncture effectively increases leucine-enkephalin release.

The use of ibuprofen, VAS score, Melle score, TCM syndrome score and serum inflammatory factors before and after treatment were analyzed and compared in each group of patients to evaluate the clinical efficacy. Results:shoulder joint pain and activity limitation were significantly improved in each group, VAS score, Melle score, TCM syndrome score and serum inflammatory factor levels were significantly decreased compared with before treatment (P < 0.05), and the improvement in the combined treatment group was significantly better than that in other groups (P < 0.05). Conclusion: Warm acupuncture combined with manipulation can significantly improve the clinical symptoms of periarthritis of shoulder, improve the mobility of shoulder joint, and inhibit the internal inflammatory response, which is significantly better than using warm acupuncture or manipulation alone and conventional physical therapy.
文摘Objective: To observe the therapeutic effect of traction combined with acupuncture on lumbar intervertebral disc herniation (LIDH). Methods:Eighty-three patients diagnosed as LIDH were randomly divided into treatment group (n=42)and control group (n=41). In the treatment group, besides lumbar traction, electroacupuncture at Jiaji (EX-B 2, L 3~5), Shenshu (BL 23), Qihai (CV 6), Mingmen (GV 4), Huantiao (GB 30), Chengshan (BL 57) and Yanglingquan (GB 34) was added. In the control group, only lumbar traction was applied. Results: In treatment group, the cure plus markedly effective rate was 80.95% with a total effective rate of 92.86%, while in control group, it was 39.02% with a total effective rate of 80.48%. The therapeutic effect in the treatment group was obviously better than that in the control group (P<0.001). Conclusion: Lumbar traction plus EA can effectively alleviate or even eliminate clinical symptoms and signs of lumbar intervertebral disc herniation.
文摘Objective: To observe the therapeutic effect of acupuncture combined with massage for neurasthenia. Methods: A total of 88 cases of neurosism outpatients were randomly divided into acu(puncture)-massage group (n=58) and medication group (n=30). In acu-massage group, scalp-points as Dingzhongxian (顶中线MS 5), Ezhongxian (额中线MS 1), Zhenshang Zhengzhongxian (枕上正中线 MS 12), etc, and body acupoints Taiyang (太阳EX-HN 5), Yintang (印堂EX-HN 3), Shanzhong (膻中CV 17), etc. acupoints were punctured, followed by performing massage from Yintang (EX-HN 3) to Shenting (神庭GV 24) and to the superciliary arch and Taiyang (EX-HN 5), etc. for 6-9 times. The treatment was conducted once daily with 10 times being a therapeutic course. In medication group, the patients were asked to take Alprazolam (0.4 mg, t.i.d. in the 1st 10 days; 0.8 mg, t.i.d. thereafter), etc. Results: Following 3 courses of treatment, of the 58 and 30 cases in acu-massage and medication groups, 22 (37.93%) and 8 (26.67%) were cured, 18 (31.63%) and 11 (36.37%) had marked improvement in clinical symptoms, 16 (27.59%) and 4 (13.33%) had improvement, 2 (3.45%) and 7 (23.33%) failed, with the total effective rates being 96.55% and 76.67% respectively.
