Patient: This is a 27-year-old patient from a district located 660 km from...The aim of our work was to establish the diagnostic difficulty of isolated nonspecific retroperitoneal congestive adenitis on medical imaging. Patient: This is a 27-year-old patient from a district located 660 km from Bamako with no known medical history or notion of travel. The onset of the disease dates back to 3 years marked by pain in the right hypochondrium and the epigastrium radiating to the back in the form of torsion, of medium intensity associated with periodic vomiting and jaundice. She was referred to the medical imaging department for pain in the right hypochondrium and cholestatic jaundice. The abdominal CT scan with and without contrast injection showed an oval retroperitoneal mass, with regular contours sitting behind the head of the pancreas and in front of the right kidney with its hypo-dense center of density equal to 23 HU measuring 44 × 28 mm, with a thick wall enhanced after injection of contrast product. It was associated with a distended gallbladder with a thin wall and hypo-dense content and a moderate dilation of the intrahepatic bile ducts. Following these examinations, the diagnostic hypothesis of cystic dilation of the common bile duct was retained. Surgery made it possible to cure the retroperitoneal lesion, discectomy of the gallbladder and biopsy of the lesions. The histological examination made it possible to make the diagnosis of isolated nonspecific reactive congestive adenitis. Conclusion: Isolated nonspecific congestive retropancreatic adenitis with a necrotic center poses a diagnostic problem for imaging in our environment because of its similarity to the bile duct cyst. This study, which is a very illustrative case, showed us a diagnostic discrepancy between the result of cross-sectional imaging and that of surgery and pathology.展开更多

Case report: A 17-year-old male patient presented with periumbilical abdominal pain for 2 days, of abrupt onset and moderate intensity, who gave away with a common analgesic, which progressed to the right iliac fossa (RIF) and increased of intensity. He also reported liquid diarrhea, with several episodes a day, without mucus and/or blood, 38°C fever, anorexia and vomiting. He was hospitalized for investigating a possible diagnostic of Acute Appendicitis. In the exams, it was considered mesenteric adenitis, and antibiotic therapy was prescribed. The patient had complete improvement at the end of treatment. Conclusions: Given its importance as a differential diagnosis of numerous pathologies, as well as controversial before the therapeutic and diagnostic conduct, it is essential to report these cases in order to elucidate the specificities of this disease.展开更多

It is only in recent times, with the more severe f...Over the centuries the idea of recurrent fevers has mainly been associated with malaria, but many other fevers, such as typhoid and diphtheria were cause for concern. It is only in recent times, with the more severe forms of fever from infectious origin becoming less frequent or a cause for worry that we started noticing recurrent fevers without any clear infectious cause, being described as having a pathogenesis of autoinflammatory nature. The use of molecular examinations in many cases can allow a diagnosis where the cause is monogenic. In other cases, however the pathogenesis is likely to be multifactorial and the diagnostic-therapeutic approach is strictly clinical. The old fever tree paradigm developed to describe fevers caused by malaria has been revisited here to describe today's periodic fevers from the periodic fever adenitis pharyngitis aphthae syndrome to the more rare autoinflammatory diseases. This model may allow us to place cases that are yet to be identified which are likely to be of multifactorial origin.展开更多

Based on Moderate Resolution Imaging Spectroradiometer on-board aqua platform (MO- DIS Aqua) data andsynoptic observations in the Red Sea, Chl a varies from north to south, with the northern part appearing to be oligotrophic. This is likely due to the absence of strong mixing and low nutrient intru- sion in comparison to the southern part during winter. In the Gulf of Aden, the emergence of upwelling cell is clearly evident along the coast of Yemen, and is only distinct from the summer-autumn seasons. Most notable is the pulsating nature of the upwelling, with warm and cold events clearly distinguished with phytoplankton response to this physical forcing also evident. The phytoplankton biomass distribution varies considerably between the two regions of study. In both study areas, water temperature and prevailing winds control nutrient concentrations.展开更多

In this work, a 3D Moho depth model of the study area is constructed for the first time by inverting gravity data from the Earth Gravitational Model(EGM2008) using the ParkerOldenburg algorithm. This model indicates the shallow zone is situated at depths of 20 km to 24 km beneath coastal plains, whereas the deep zone is located below the plateau at depths of 30 km to 35 km and its deepest part coincides mainly with the Dhamar-Rada ’a Quaternary volcanic field. The results also indicate two channels of hot magmatic materials joining both the Sana’a-Amran Quaternary volcanic field and the Late Miocene Jabal An Nar volcanic area with the Dhamar-Rada’a volcanic field. This conclusion is supported by the widespread geothermal activity(of mantle origin) distributed along these channels,isotopic data, and the upper mantle low velocity zones indicated by earlier studies.展开更多

A cross sectional survey was done during the period,...To assess the impact of the armed conflict of 2015-2016 on the availability of medicines and health service in Aden Governorate whilst the rebels over-run the city. A cross sectional survey was done during the period, September-November 2016. The survey included 63 medicinal preparations as marker medicines in 142 public health facilities including private pharmacies. Direct questions and a retrospective study of the United Nations and other organizations that were working in area reporting were studied. I.V. fluids had the highest availability in the market whilst vaccines had the lowest average availability at 60%. Medicine prices were increased by 71%. The function of health facilities was almost on the verge of collapse. The four districts containing the main hospital and the medical stores were inaccessible and completely controlled by the rebels. Two out of six hospitals remain functional. Dengue and malaria rampantly spread and approximately 65,250 civilians were affected. The situation was further aggravated by the absence of basic services such as electricity, water and food.展开更多
