A Study of the Teaching Effects of Case Analysis in Cross-Cultural Communication Class-Taking Individualism-Collectivism Cultural Dimension Class as an Example

This study introduces the individualism-collectivism dimension of the cultural dimension of cross-cultural communication initiated by Geert Hofstede.Different cultures must develop a way of correlating that strikes a balance between caring for themselves and showing concern for others.Individualist culture encourages uniqueness and independence while collectivist culture emphasizes conformity and mutual assistance.This article introduces how to use case analysis method to effectively carry out classroom teaching in this cultural dimension.

Mycobacterium gordoniasis of the cervical lymph nodes:A case report

BACKGROUND Owing to the advancement in bacterial identification techniques,the detection rate of non-tuberculous mycobacterium(NTM)has been on the rise.Different from Mycobacterium tuberculosis,the clinical symptoms of NTM are not easily detected,and the clinical efficacy and prognosis are somewhat heterogeneous.To report a case of Mycobacterium gordoniasis of cervical lymph node diagnosed in Anhui Chest Hospital in July 2022.CASE SUMMARY Upon examination,the patient who weighed 67.5 kg,was human immunodeficiency virus negative,healthy,without hypertension,diabetes,heart disease and other basic diseases microscopic analysis revealed granulomatous inflammation with coagulation necrosis in the lymphocyte,and tuberculosis was not ruled out.Plain computed tomography scans of the neck and chest indicated the presence of a single grayish-yellow and grayish-brown tissue,the dimensions of which was top of form 10.5 cm×3.0 cm×1.5 cm.After pathological consultation in our hospital,the diagnosis was confirmed as NTM infection.CONCLUSION This case report and the clinical epidemiological research on improving NTM have important guiding significance for improving decision-making in clinical treatments.

Clinical review and literature analysis of hepatic epithelioid angiomyolipoma in alcoholic cirrhosis: A case report

BACKGROUND Hepatic epithelioid angiomyolipoma(HEA)has a low incidence and both clinical manifestations and imaging lack specificity.Thus,it is easy to misdiagnose HEA as other tumors of the liver,especially in the presence of liver diseases such as hepatitis cirrhosis.This article reviewed the diagnosis and treatment of a patient with HEA and alcoholic cirrhosis,and analyzed the literature,in order to improve the understanding of this disease.CASE SUMMARY A 67-year-old male patient with a history of alcoholic cirrhosis was admitted due to the discovery of a space-occupying lesion in the liver.Based on the patient’s history,laboratory examinations,and imaging examinations,a malignant liver tumor was considered and laparoscopic partial hepatectomy was performed.Postoperative pathology showed HEA.During outpatient follow-up,the patient showed no sign of recurrence.CONCLUSION HEA is difficult to make a definite diagnosis before surgery.HEA has the poten-tial for malignant degeneration.If conditions permit,surgical treatment is recom-mended.

Clinical manifestations and the prenatal diagnosis of trisomy 7 mosaicism:Two case reports

BACKGROUND The clinical manifestations of trisomy 7 mosaicism are diverse and nonspecific,so prenatal diagnosis is very difficult.CASE SUMMARY Two pregnant women with abnormal prenatal screening results were included.One was a 22-year-old woman(G1P0).At 31st week of gestation,ultrasound revealed that the posterior horn of the left lateral ventricle was 10 mm and the right renal pelvis had a separation of 7 mm.The other pregnant woman was 33 years old(G2P1L1A0),and her fetus was found to have a cardiac malformation at the 24th week of gestation.Copy number variation sequencing,whole-exome sequencing and karyotype analysis were carried out after amniocentesis,and both fetuses were diagnosed with trisomy 7 mosaicism.After parental counseling,one woman continued the pregnancy,and the other woman terminated the pregnancy.CONCLUSION In trisomy 7 mosaicism,the low proportion of trisomy does not lead to abortion,but can result in abnormal fetal development,which can be detected via ultrasound.Therefore,clinicians need to pay more attention to various aspects of fetal growth and development,combining with imaging,cellular,molecular genetics and other methods to perform comprehensive evaluations of fetuses to provide more reliable genetic counseling for pregnant women.

The Spatial Distribution Pattern Analysis of City Infrastructure Cases of Urban Management

The effective disposal of daily city infrastructure cases is an important issue for urban management. To more effectively utilize a large amount of historical cases data collected and accumulated in the urban grid management system, and to analyze their spatial distribution pattern information for city managers, this study used the comparative kernel density analysis method in two types of cases (i.e. power facilities and traffic guardrail) in Xicheng District, Beijing for the year 2016 and 2017. This research analyzes them at different scales (300 m, 600 m, 1,200 m), and the experiment results show that the method of comparative kernel density analysis is able to provide an intuitively spatial visualization distribution analysis of city infrastructure related cases. The quantitative information of spatial agglomeration degree is helpful for city managers making decision.

3D Nonlinear Analysis of the Preloading Filling Spiral Case of the Jinping-I Hydropower Station

In consideration of the contact between the steel spiral casing and the peripheral reinforced concrete, a nonlinear analysis for the combined bearing structure of the JinPing-I preloading filling spiral case has been made with ANSYS. Contrasts of the stress of the reinforcing bars, the biggest width of the crack and the outspread section of the crack has been made with the different parameters of preloading pressure and reinforcement scheme, resulting in a reasonable preloading pressure and reinforcement scheme. This conclusion has been applied to the spiral casing of JinPing-I hydropower station.

Analysis on the Evolution Path of Transformation of Manufacturing Enterprises through Servitization——A Case Study Based on ShaanGu Group

Service-oriented manufacturing is an emerging innovative development model,and its core is the deep integration of products and services.It has become a hot issue in current research.In order to deeply explore the evolution path of servitization transformation of manufacturing enterprises,ShaanGu Group is taken as a case,and discusses practical application of service-oriented manufacturing model in different development processes of ShaanGu Group.Thus,four stages of transformation of ShaanGu Group through servitization are summarized:specialized after-sales service stage,rental service stage,engineering complete service stage and financial service stage.Through comparative research,it is found that general evolution path of manufacturing enterprises to service transformation is:the primary stage(providing product-oriented product service system)→the intermediate stage(providing application-oriented product service system)→the advanced stage(providing utility-oriented product service system)→the completed stage(providing pure service).On this basis,some suggestions are provided for manufacturing enterprises to integrate productive services in the process of transformation.

Complications of mono center laparoscopic cholecystectomy and their prevention and treatment: An analysis of 18 726 cases

Objective To analyze the complications of laparoscopic cholecystectomy (LC) and their affecting factors and investigate their prevention and treatment. Methods The clinical data of 18 726 patients treated with LC between March 1992 to January 2001 in our hospital were retrospectively analyzed. Results The complications were common bile duct injury in 28 cases (0. 15%), bile leakage in 62 (0. 33% ), introabdominal hemorrhage in 6 (0.03) and postoperative introabdominal abscess in 4 (0. 02). The causes for these complications were improper choice of indications and surgeon’s back of knowledge and experience in identifying the abnormality of bile duct anatomy. Conclusion The major complications of LC are bile duct leakage and injury. The injury can be avoided through careful performance of LC. Immediate discovery and proper treatment will result in good outcome.7 refs.1 fig,2 tabs.

Isolated mitral valve replacement with Guangdong-Ⅰ porcine bioprostheses in rheumatic heart valve disease: analysis of 166 cases with long-term follow-up

Objective To study the long-term effects of mitral valve replacement with bioprostheses in rheumatic heart valve disease. Methods 166 patients with rheumatic heart valve disease underwent isolated mitral valve replacement from Jan. 1978 to Dec. 1985. 79 Patients were male and 87 female. Patients’ age ranged from 11 to 53 years [mean (29.4 ± 9.9) years]. The patients were classified into two groups: group 1 (age【 30 years) included 84 patients, group 2 (age≥30 years) 82 cases. Mean CTR is 0.68 *0.08. Most patients were in NYHA function class Ⅲ-Ⅳ. 76. 3% of patients had atrial fibrillation. 6 patients had pre-operative cerebral vascular accidents. Postoperative deaths were excluded. Follow-up extended from 0.4 year to 19.4 years. Results Actuarial estimate of survival rate at 14 years is (52.5 ± 7.0)%. Cumulative logit model in the analysis of endometrial cancer under a matched pair case-control design

Background: Binary as well as polytomous logistic models are widely used for estimating odds ratios when the exposure of prime interest assumes unordered multiple levels under matched pairs case-control design. In our previous studies, we have shown that the use of a polytomous logistic model for estimating cumulative odds ratios when the outcome (response) variable is ordinal (in addition to being polytomous) under matched pairs case-control design. The cumulative odds ratios were estimated based on separate fitting of the model at each of the cutpoint level as compared to less than equal to that level. In this paper we propose an alternative method of estimating the cumulative odds ratios and reanalyze the Los Angeles Endometrial Cancer data in the context of dose levels of conjugated oestrogen exposure and development of endometrial cancer under the matched pair case-control design. Methods: In the present study, the cumulative logit model is fitted using a single multinomial logit model for the data. For this, the full maximum likelihood estimation procedure is adopted. A test for equality of the cumulative odds ratios across the exposure levels is proposed. Results: The analysis revealed that there is a strong evidence of risk for developing endometrial cancer due to oestrogen exposure above each of the three dose level as compared to less than equal to that level. The estimated values at the three cutpoint levels were found to be 6.17, 3.60 and 5.16 respectively. Conclusions: The odds of developing endometrial cancer are very high for the users of any amount of oestrogen, even if it is the least dose, as compared to the non-users.

Gastrointestinal stromal tumor metastasis at the site of a totally implantable venous access port insertion:A rare case report

BACKGROUND The totally implantable venous access port(TIVAP)is an important device in patients for injecting blood products,parenteral nutrition or antineoplastic chemotherapy.Metastatic spread at the site of the insertion of a TIVAP is extremely rare.CASE SUMMARY We report the case of 33-year-old male with advanced gastrointestinal stromal tumor(GIST)who underwent radical tumor resection after neoadjuvant imatinib therapy.However,a solitary GIST metastasis at the site of a TIVAP insertion developed during adjuvant imatinib treatment.Mutational analysis showed secondary mutation in KIT exon 13(V564 A),which is resistant to imatinib treatment.To our knowledge,this is the first case report of a patient with advanced GIST developing GIST metastasis at the site of a TIVAP insertion.CONCLUSION This case highlights that when a patient with advanced,high metastatic GIST requires TIVAP insertion,we should realize that there is a risk of developing tumor metastasis at the site of a TIVAP insertion.

Effect of CBL Combined with Case Analysis in the Teaching of Clinical Anesthesia

Objective:To evaluate the effect of CBL(case-based learning)combined with case analysis in the teaching of clinical anesthesia.Methods:From June 2018 to June 2020,a total of 100 clinical anesthesia interns in Shaanxi Provincial People’s Hospital were selected as the research subjects.They were divided into two groups,the reference group(50 interns)and the study group(50 interns),by random sampling method.The reference group was under the traditional teaching method,whereas for the study group,CBL combined with case analysis was implemented.The teaching satisfaction and achievement of the interns in both the groups were evaluated.Results:The teaching satisfaction of the study group was 98%,which was significantly higher than that of the reference group(86%)(P<0.05);before teaching,there was no significant difference in the theoretical and clinical practice scores between the two groups(P>0.05);however,after teaching,the theoretical and clinical practice scores of the two groups increased significantly,in which the change of the research group was significantly greater than that of the reference group(P<0.05).Conclusion:The application of CBL combined with case analysis in the teaching of clinical anesthesia has an ideal effect,improving the teaching satisfaction and achievement.It is a high-quality teaching method and is worthy of application.
