不同入路小骨窗开颅显微手术治疗基底节区高血压脑出血的效果比较

目的分析不同入路小骨窗开颅显微手术治疗基底节区高血压脑出血的效果。方法选取2019年1月至2022年3月丰城市人民医院收治的88例基底节区高血压脑出血患者作为研究对象,随机分为观察组与对照组,每组44例。两组均行小骨窗开颅显微手术,... 目的分析不同入路小骨窗开颅显微手术治疗基底节区高血压脑出血的效果。方法选取2019年1月至2022年3月丰城市人民医院收治的88例基底节区高血压脑出血患者作为研究对象,随机分为观察组与对照组,每组44例。两组均行小骨窗开颅显微手术,对照组采用经颞叶皮质入路手术治疗,观察组采用经侧裂下Rolandic点-岛叶入路手术治疗,比较两组手术情况、血肿清除率、再出血率、术后并发症、术后1个月格拉斯哥昏迷量表(Glasgow coma score,GOS)分级情况及术后1、3、6个月的美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)评分及世界卫生组织生存质量测定量表(World Health Organization on quality of life brief scale,WHOQOL-BREF)评分。结果两组手术时间、术中出血量、引流管放置时间、行大骨瓣减压例数、住院时间比较差异均无统计学意义;观察组开始手术至颅内压下降时间长于对照组,差异有统计学意义(P<0.05)。观察组术后24 h血肿清除率明显高于对照组,差异有统计学意义(P<0.05);两组再出血率比较差异无统计学意义。观察组术后1个月预后良好率为81.82%,高于对照组的61.36%,差异有统计学意义(P<0.05)。术后1、3、6个月,观察组NIHSS评分均低于对照组,WHOQOL-BREF评分均高于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率为6.82%,低于对照组的25.00%,差异有统计学意义(P<0.05)。结论经侧裂下Rolandic点-岛叶入路小骨窗开颅显微手术治疗基底节区高血压脑出血的效果显著,有助于提升血肿清除率,减少术后并发症发生率,促进术后神经功能的恢复,提高患者预后生存质量。 展开更多

改良模块化剑突下胸腔镜胸腺肿瘤切除术临床应用

目的 总结剑突下入路胸腔镜胸腺肿瘤切除术的临床疗效。方法 2021年2月~2023年3月我院行胸腔镜胸腺肿瘤切除术病人93例。按照手术入路分为剑突下组(40例)和侧进胸组(53例)。比较两组手术结果、围术期指标及术后疼痛情况。结果 两组病人... 目的 总结剑突下入路胸腔镜胸腺肿瘤切除术的临床疗效。方法 2021年2月~2023年3月我院行胸腔镜胸腺肿瘤切除术病人93例。按照手术入路分为剑突下组(40例)和侧进胸组(53例)。比较两组手术结果、围术期指标及术后疼痛情况。结果 两组病人均完成胸腔镜胸腺全切或胸腺扩大切除术,无中转开胸。剑突下组胸腺全切手术时间为(60.32±1.53)分钟,胸腺扩大切除手术时间为(114.52±11.63)分钟,术后带管时间为(3.20±0.91)天,术后住院时间为(4.00±0.16)天,侧进胸组分别为(66.62±1.45)分钟、(138.76±6.35)分钟、(4.00±1.22)天和(4.70±0.18)天,两组比较差异有统计学意义(P<0.05)。剑突下组术后24小时VAS评分为(3.8±0.8)分、48小时VAS评分为(3.0±0.7)分、72小时VAS评分为(2.6±0.9)分,侧进胸组分别为(5.7±0.9)分、(4.7±0.8)分和(3.2±0.8)分,两组比较差异有统计学意义(P<0.05)。两组病人术中失血量、住院费用、并发症发生率、术后30天感觉异常情况比较差异无统计学意义(P>0.05)。结论 改良模块化剑突下胸腔镜胸腺肿瘤切除术安全可靠,具备明显的围术期优势,尤其适合伴有重症肌无力的MasaokaⅠ、Ⅱ期胸腺瘤病人。 展开更多

低低温电除尘器装机容量已超15万MW,具有高除尘效率和高SO_(3)脱除效率的效果,但由于电除尘性能与煤、灰成分和烟气温度等多种因素有关,存在选型设计难的问题。在结合常规电除尘选型技术和低低温电除尘提效作用的基础上,运用Visual Basi... 低低温电除尘器装机容量已超15万MW,具有高除尘效率和高SO_(3)脱除效率的效果,但由于电除尘性能与煤、灰成分和烟气温度等多种因素有关,存在选型设计难的问题。在结合常规电除尘选型技术和低低温电除尘提效作用的基础上,运用Visual Basic语言研发低低温电除尘选型设计软件,应用此软件可对烟气酸露点、灰硫比、表观趋近速度等关键技术参数进行计算,完成电除尘器进口烟气温度、比集尘面积的计算,实现低低温电除尘器的选型设计。软件应用后,通过对26个工程案例的统计对比分析,实际运行效率大于软件选型设计保证效率的项目数量占总数的92%,满足实际工程应用的需求,提高了低低温电除尘的选型命中率。 展开更多

Volume parameter is the basic content of a spatial body object morphology analysis.However,the challenge lies in the volume calculation of irregular objects.The point cloud slicing method proposed in this study effectively works in calculating the volume of the point cloud of the spatial object obtained through three-dimensional laser scanning(3DLS).In this method,a uniformly spaced sequent slicing process is first conducted in a specific direction on the point cloud of the spatial object obtained through 3DLS.A series of discrete point cloud slices corresponding to the point cloud bodies are then obtained.Subsequently,the outline boundary polygon of the point cloud slicing is searched one by one in accordance with the slicing sequence and areas of the polygon.The point cloud slice is also calculated.Finally,the individual point cloud section volume is calculated through the slicing areas and the adjacent slicing gap.Thus,the total volume of the scanned spatial object can be calculated by summing up the individual volumes.According to the results and analysis of the calculated examples,the slice-based volume-calculating method for the point cloud of irregular objects obtained through 3DLS is correct,concise in process,reliable in results,efficient in calculation methods,and controllable on accuracy.This method comes as a good solution to the volume calculation of irregular objects. 展开更多

目的 研究两种不同针刀入路治疗(lumbar disc herniation LDH)的临床疗效差异。方法 选取符合纳入标准的70例LDH患者,使用随机数字表法分为痛点入路组(35例,脱落1例)与椎间孔点入路组(35例,脱落2例),分别采用痛点入路和椎间孔点入路针... 目的 研究两种不同针刀入路治疗(lumbar disc herniation LDH)的临床疗效差异。方法 选取符合纳入标准的70例LDH患者,使用随机数字表法分为痛点入路组(35例,脱落1例)与椎间孔点入路组(35例,脱落2例),分别采用痛点入路和椎间孔点入路针刀松解治疗。在治疗前、治疗后、随访3个月时,分别采用VAS评分和JOA评分评定疼痛及功能,并在治疗后、随访3个月时评估疗效。结果 治疗后,痛点组VAS评分显著优于椎间孔点组(P<0.05);两组JOA评分及疗效差异无统计学意义(P>0.05);随访3个月时,痛点组VAS、JOA评分及疗效均显著优于椎间孔点组(P<0.05)。结论 两者短期疗效总体相当,但痛点入路在改善疼痛方面效果明显,且在远期疗效上更有优势。 展开更多

This article deals with the use of an interdisciplinary approach to modelling and creation of a complex technical system of different physical nature in relation to the kinematics of cutting and shaping. The professor of the National Technical University of Ukraine, Kuznetcov Iu. N., proposed the approach based on generalization of knowledge, methodological basis of which is the theory of evolution of the systems and methods of genetic analysis and synthesis. For generalization of the knowledge in the fundamental sciences is based on the principles of a limited number of elementary generic structures with the introduction of the gene concept. The modelling and synthesis of kinematic cutting schemes are providing the efficiency and viability of genetic and morphological approach. Its greatest advantage is that the covariance of causality encapsulated in any piece of semantic intelligence is provided with a great diversity of its individuality viewed as the properties of the current response and its reproducibility viewed as causality encapsulated in any of the homeostatic patterns. 基于能源、交通、人口和GDP等统计资料,考虑工业、交通、建筑、人体新陈代谢4种来源,估算了2019年中国内地地级市的人为热排放总量.采用自上而下的能源消费清单法,通过Cubist机器学习算法融合不同类别的兴趣点(POI)数据和多源遥感数据,... 基于能源、交通、人口和GDP等统计资料,考虑工业、交通、建筑、人体新陈代谢4种来源,估算了2019年中国内地地级市的人为热排放总量.采用自上而下的能源消费清单法,通过Cubist机器学习算法融合不同类别的兴趣点(POI)数据和多源遥感数据,分别对不同来源的人为热排放通量构建空间化模型,获得2019年中国内地1km空间分辨率的人为热通量网格数据,并分析不同地理、环境变量与来自不同热源的人为热排放通量之间的关系.结果表明:人为热通量的高值区通常分布于一线城市和工业比较发达的城市,城市中心的人为热通量值在80~200W/m^(2)之间,一些重工业区的人为热通量值甚至高于市区,极值高达519.2W/m^(2);Cubist机器学习算法能更好地刻画人为热排放通量与影响因子之间的非线性关系,且能对影响因子的变量重要性进行度量;POI是人为热通量空间分布的最重要指示性指标,通过融合POI数据可以更加有效地反映出不同来源人为热排放通量的空间分布特征.新的2019年人为热排放通量数据可以为中国城市天气气候、城市大气环境的数值模拟研究提供有力支持. 展开更多

目的比较Rolandic点-岛叶入路与颞叶皮层入路小骨窗经侧裂开颅术治疗高血压基底节区出血的疗效及预后。方法回顾性分析2020年8月至2021年8月在复旦大学附属华山医院收治的112例高血压基底节区出血患者的临床资料,所有患者均进行小骨窗... 目的比较Rolandic点-岛叶入路与颞叶皮层入路小骨窗经侧裂开颅术治疗高血压基底节区出血的疗效及预后。方法回顾性分析2020年8月至2021年8月在复旦大学附属华山医院收治的112例高血压基底节区出血患者的临床资料,所有患者均进行小骨窗经侧裂开路手术治疗,根据患者手术入路不同分为A组(Rolandic点-岛叶入路)57例和B组(颞叶皮层入路)55例,比较两组患者的临床疗效、围术期指标、手术前及术后两周的神经因子水平、预后及术后并发症发生情况。结果A组与B组患者的治疗总有效率分别为98.25%、96.36%,差异无统计学意义(P>0.05);A组患者的手术时间、住院时间分别为(3.41±0.60)h、(22.23±4.49)d,明显短于B组的(4.09±1.23)h、(31.08±5.15)d,差异均有统计学意义(P<0.05);A组患者的血肿清除率(术后24h)、术后1周格拉斯哥昏迷量表(GCS)评分分别为94.74%、(11.75±0.69)分,明显高于B组的80.00%、(10.81±0.57)分,差异均具有统计学意义(P<0.05);手术后,两组患者的S-100β蛋白(S-100β)、胶质纤维酸性蛋白(GFAP)、水通道蛋白4(AQP4)水平均降低,且A组患者的上述指标分别为(0.12±0.07)ng/mL、(3.62±0.48)pg/mL、(105.38±14.18)μg/L,明显低于B组的(0.16±0.06)ng/mL、(3.86±0.52)pg/mL、(111.19±12.73)μg/L,差异均具有统计学意义(P<0.05);A组患者的预后优良率为94.74%,明显高于B组的81.82%,差异具有统计学意义(P<0.05);A组与B组术后并发症发生率分别为7.02%、10.91%,差异无统计学意义(P>0.05)。结论与颞叶皮层入路小骨窗经侧裂开颅术比较,Rolandic点-岛叶入路方式可缩短高血压基底节区出血患者手术时间、住院时间,提高血肿清除率,改善神经因子水平,改善预后。 展开更多

In this article,we developed sufficient conditions for the existence and uniqueness of an approximate solution to a nonlinear system of Lorenz equations under Caputo-Fabrizio fractional order derivative(CFFD).The required results about the existence and uniqueness of a solution are derived via the fixed point approach due to Banach and Krassnoselskii.Also,we enriched our work by establishing a stable result based on the Ulam-Hyers(U-H)concept.Also,the approximate solution is computed by using a hybrid method due to the Laplace transform and the Adomian decomposition method.We computed a few terms of the required solution through the mentioned method and presented some graphical presentation of the considered problem corresponding to various fractional orders.The results of the existence and uniqueness tests for the Lorenz system under CFFD have not been studied earlier.Also,the suggested method results for the proposed system under the mentioned derivative are new.Furthermore,the adopted technique has some useful features,such as the lack of prior discrimination required by wavelet methods.our proposed method does not depend on auxiliary parameters like the homotopy method,which controls the method.Our proposed method is rapidly convergent and,in most cases,it has been used as a powerful technique to compute approximate solutions for various nonlinear problems. 展开更多

The paired approach is a kind of efficiency approach to closely spaced parallel runways(CSPRs),and the point merge system has the powerful interval management function,which is effective to realize the converge of traffic flows from different approach directions.In order to improve the operation efficiency of the airport terminal area,a model of paired approach sequencing based on point merge is proposed to investigate the problem of increasing the operation capacity of the closely spaced parallel runways.Taking the minimum average flight delay time as the objective,the flight distance on sequencing legs,wake turbulence separation and paired approach safety separation as constraints,the genetic algorithm is used to optimize the paired approach sequencing of arrival flights.Taking the closely parallel runways of Shanghai Hongqiao International Airport run south as an example,the point merge program is designed and the effect of model was analyzed.The results show that after optimization,the average delay time and average landing time are reduced by 40.6%and 51.8%respectively,the capacity of the closely spaced parallel runways are 1.1 times higher than the actual,the flight uptime rate can reach 100%.It is concluded that the proposed model is feasible,which can effectively reduce delay times and alleviate congestion in terminal areas. 展开更多
