She had previously been treated for arterial high blood pressure, parathyroidectomy and rheumatoid arthritis by a long corticotherapy treatment. The lesion was caused by an indirect mechanism in an overweight context. The diagnosis of a pseudarthrosis of the base of the fifth metatarsals was maintained after 7-month treatment. The patient received a treatment of both pseudarthrosis. The post-operative periods were simple. After a 9-month follow-up, the treatment allowed consolidation with painless feet allowing her to resume work.展开更多

目的比较空心螺钉双皮质固定与非手术治疗对第5跖骨基底部I区骨折疗效影响。方法回顾性分析2016年2月—2017年6月陕西中医药大学第二附属医院收治的30例第5跖骨基底部I区骨折患者临床资料。男性19例,女性11例;年龄18~63岁,平均31.87岁;...目的比较空心螺钉双皮质固定与非手术治疗对第5跖骨基底部I区骨折疗效影响。方法回顾性分析2016年2月—2017年6月陕西中医药大学第二附属医院收治的30例第5跖骨基底部I区骨折患者临床资料。男性19例,女性11例;年龄18~63岁,平均31.87岁;均属于Lawrence and Botte I区骨折。30例患者按照治疗方法分为非手术组和手术组,非手术组14例给予石膏外固定,手术组16例采用空心螺钉双皮质固定。对两组患者恢复结果进行评估,包括VAS评分、骨折愈合时间、完全负重时间、功能恢复以及相关并发症,功能恢复采用美国骨科足踝协会(AOFAS)中足评分标准评定。结果随访12~24个月,平均14.5个月。非手术组1例畸形愈合,其余29例均Ⅰ期愈合。两组患者治疗后1个月VAS评分差异无统计学意义(P=0.102>0.05),12个月后手术组VAS评分[(0.51±0.25)分]显著低于非手术组[(1.32±0.36)分],P=0.001<0.05;AOFAS评分6个月后手术组[(88.44±2.58)分]优于非手术组[(84.29±2.30)分],P=0.003<0.05,12个月后差异无统计学意义(P=0.124>0.05);手术组骨折愈合时间[(45.44±4.18)d]短于非手术组[(56.57±5.24)d],P=0.002<0.05;完全负重时间手术组(56.5±3.88)d早于非手术组(67.07±4.48)d(P=0.002<0.05)。手术组16例患者未发生感染、骨折延迟愈合或不愈合、足畸形等并发症,无皮肤激惹症状;术后平均8.3个月(6~14个月)取出内固定装置,未发生再次骨折。结论空心螺钉双皮质固定在治疗第5跖骨基底部I区骨折中疗效确切,是一种可供选择的手术方式。展开更多
