Functional magnetic resonance imaging evaluation of brain function reorganization in cerebral stroke patients after constraint-induced movement therapy

In this study, stroke patients received constraint-induced movement therapy for 3 weeks. Before and after constraint-induced movement therapy, the flexibility of their upper limbs on the affected side was assessed using the Wolf motor function test, and daily use of their affected limbs was assessed using the movement activities log, and cerebral functional reorganization was assessed by functional magnetic resonance imaging. The Wolf motor function test score and the movement activities log quantity and quality scores were significantly increased, while action performance time in the Wolf motor function test was significantly decreased after constraint-induced movement therapy. By functional magnetic resonance imaging examination, only scattered activation points were visible on the affected side before therapy. In contrast, the volume of the activated area was increased after therapy. The activation volume in the sensorimotor area was significantly different before and after therapy, and the activation area increased and appeared adjusted. In addition to the activated area around the lesions being decreased, there were also some new activated areas, including the supplementary movement area, premotor area and the ipsilateral sensorimotor area. Our findings indicate that constraint-induced movement therapy significantly improves the movement ability and daily use of the affected upper limbs in stroke patients and promotes cerebral functional reorganization. 展开更多

目的:观察双侧训练对于急性期脑卒中患者的康复治疗效果。方法:36例急性期脑卒中患者分成对照组和观察组各18例,均给予常规康复训练,对照组增加单侧肢体及手运动功能训练,观察组增加双侧肢体及手运动功能训练。2组患者在治疗前后采用改... 目的:观察双侧训练对于急性期脑卒中患者的康复治疗效果。方法:36例急性期脑卒中患者分成对照组和观察组各18例,均给予常规康复训练,对照组增加单侧肢体及手运动功能训练,观察组增加双侧肢体及手运动功能训练。2组患者在治疗前后采用改良Ashworth评分-肩、肘、腕、手(MAS-S,MAS-E,MAS-W,MAS-H)、Fugl-Meyer评定量表肩/肘/前臂、腕手(FM-SEF,FM-WH)及改良Bathel指数(MBI)进行评分。结果:训练12周后,2组FM-SEF,FM-WH评分,MAS各项评分及MBI评分均较训练前改善(P<0.05),观察组训练后FM-SEF,及MAS-S,MAS-E,MAS-W评分较对照组改善更显著(P<0.05)。结论:双侧训练应用于脑卒中急性期的患者可以有效地降低肩、肘及腕关节肌张力,并且对于上肢腕手运动功能恢复有一定效用。 展开更多

目的探讨等速肌力训练联合双侧上肢强制性运动训练对缺血性脑卒中伴上肢本体感觉功能障碍患者的上肢本体感觉功能、运动功能、日常生活能力的影响。方法选取2019年7月至2020年6月在本院就诊的缺血性脑卒中伴上肢本体感觉功能障碍患者96... 目的探讨等速肌力训练联合双侧上肢强制性运动训练对缺血性脑卒中伴上肢本体感觉功能障碍患者的上肢本体感觉功能、运动功能、日常生活能力的影响。方法选取2019年7月至2020年6月在本院就诊的缺血性脑卒中伴上肢本体感觉功能障碍患者96例作为研究对象,采用随机数字法分为观察组及对照组,每组48例。对照组患者给予脑卒中后常规药物治疗及等速肌力训练,观察组患者在对照组治疗和训练的基础上,给予双侧上肢强制性运动训练,1周训练5 d,持续训练4周。比较两组患者干预前后的本体感觉功能、FM量表上肢运动及感觉功能评分、Carroll上肢功能测试(UEFT)评分及改良Barthel(MBI)指数评分。结果干预前,两组患者的平均轨迹误差(ATE)、测试执行时间(Time)比较,差异无统计学意义(P>0.05);干预4周后,两组患者的ATE、Time均显著降低,观察组患者的水平显著低于对照组,差异有统计学意义(P<0.05)。干预前,两组患者的FM量表运动、感觉功能评分比较,差异无统计学意义(P>0.05);干预4周后,两组患者的FM量表运动、感觉功能评分均显著增高,观察组患者的评分显著高于对照组,差异有统计学意义(P<0.05)。干预前,两组患者的UEFT、MBI评分比较,差异无统计学意义(P>0.05);干预4周后,两组患者的UEFT、MBI评分均显著增高,观察组患者的评分显著高于对照组,差异有统计学意义(P<0.05)。结论等速肌力训练结合双侧上肢强制性运动训练有助于缺血性脑卒中伴上肢本体感觉功能障碍患者改善其上肢本体感觉功能,促进运动功能恢复,提高日常生活能力。 展开更多
