Introduction: High blood pressure is defined as blood pressure greater than or equal to 140 mm Hg for systolic and or 90 mm Hg for diastolic. It constitutes a major public health problem, the leading chronic disease in the world. The objective was to determine the prevalence, treatment rate and control of hypertension. Methods: This was a cross-sectional and descriptive study which took place over a period of 6 months in a hospital environment and in the general population. Results: Of the 1000 participants, 637 had hypertension, giving a prevalence of 63.70% with a female predominance. Thirty-three percent (33%) were unaware of their high blood pressure. The age group 60 and more was the most represented (44%). A proportion of 33 and 23.20 were overweight and obese participants, respectively. Male subjects were more overweight than female, unlike obesity which was more common among female subjects. Sixty-two percent (62%) of hypertensives were treated, of whom 44% were non-compliant. The excessively high cost and consumption of medications as needed were the main factors in therapeutic non-compliance. Twenty-two percent (22%) of all hypertensive patients and 35% of treated hypertensive patients were controlled. Women were more treated but less observant and less controlled than men. Therapeutic coverage and combination therapy rates were lower in rural areas. Hypertensives who had a high level of education were better treated and controlled than those who had no level. Conclusion: High blood pressure remains a real public health problem in Mali. It is more common in people aged 60 and over and in females. One in three hypertensives were unaware of their hypertension. The majority received antihypertensive treatments, but only a minority of them had their hypertension controlled. 展开更多

The two pathologies may influence each other. The aim was to study the correlation between glycemic control and blood pressure control and to determine the factors associated with blood pressure control. Methodology: This was a descriptive cross-sectional study with an analytical focus over 7 months. Patients were recruited as outpatients and all underwent ambulatory blood pressure measure, glycated hemoglobin and creatinine measurements, and assessment of compliance with treatment. Results: During this period 116 patients were collected. The predominance was female 69%. The mean age of the patients was 62 ± 7 years with a peak between 60 and 70 years. The average age of hypertension was 12 years and that of diabetes 6 1/2 years. The most frequently associated cardiovascular risk factor was a sedentary lifestyle (71.5%) after age. 57.8% of patients were not controlled at the office, with a predominance of systolic hypertension (58.2%). 61.6% of patients were controlled by ambulatory blood pressure measure, a rate of 47.8% of white coat hypertension. Glycemic control was observed in 42.2% of cases and 87% of patients had good renal function (glomerular filter rate ≥ 60 ml/mn). Therapeutic compliance was good in 53.4% of cases and dual therapy was the most used therapeutic modality 44.8% (52 patients) followed by triple therapy. The factors associated with poor blood pressure control were glycemic imbalance, non-compliance and monotherapy. Dual therapy had a protective effect. Conclusion: The association of hypertension and type 2 diabetes is frequent. The risk of occurrence increases with age. Ambulatory blood pressure measure is the best method to assess blood pressure control. Optimization of blood pressure control should also include optimization of glycemic control. 展开更多

This study was done to assess whether self monitoring of blood glucose is associated with better glycaemic control in type 2 diabetic Trinidadians. Data was collected from 146 type 2 diabetics via interviews and medical files. Variables extracted included fasting blood glucose, HbA1c and lipid profile. Recent users of the glucometer had better glycemic control compared with longstanding users (HbA1c: 8.4% ± 0.28% vs 9.4% ± 0.62%). Diabetics using a glucometer for >11 years had the highest cholesterol (234 ± 27mg/dL) while those using it 6 months to 5 yrs had the lowest values (206 ± 8 mg/dL). Using the glucometer occasionally or once a week had little impact on these indices. Usage of insulin preparation was positively correlated with HbA1c (r = 0.295;p = 0.001) while those relying on oral hypoglycaemic agents showed a negative correlation (r = -0.265). Further, those who took their medication as prescribed had a lower HbA1c than those who took their medication occasionally. Ironically, HbA1c fell with increasing duration between clinic visits with those visiting annually having the lowest HbA1c (9.0% ± 0.2% vs 7.1% ± 0.7%). Compliance with lifestyle and exercise resulted in lower HbA1c and blood lipids. Poor client education on glucometer usage, administration of insulin and non-compliance with diet and exercise were associated with higher HbA1c. 展开更多

BACKGROUND Type 1 diabetes is one of the most common chronic diseases in childhood.The number of type 1 diabetes patients in China still ranks fourth in the world.Therefore,children with type 1 diabetes in China are a group that needs attention.The management of type 1 diabetes mellitus(T1DM)involves many aspects of daily life.It is extremely challenging for children and their families.T1DM children have complex medical care needs.Despite the continuous development of therapeutic medicine and treatment technologies,blood glucose control in children with T1DM is still not ideal.They and their parents need to acquire more knowledge and skills before being discharged.AIM To explore the influence of hospital discharge plan based on parental care needs of children with T1DM on discharge readiness,quality of discharge education and blood glucose control level.METHODS In total,102 parents of children with type 1 diabetes were divided into control group and intervention group according to admission time.Fifty cases from February to June 2019 were selected as the control group,and 52 cases from July to October 2019 were selected as the intervention group to implement the discharge plan.The Readiness for Hospital Discharge Scale,Hospital Discharged Education Quality Scale and children's blood glucose metabolism indicators were used to compare the differences in discharge preparation,discharge education quality and blood glucose control between the two groups of children and their parents.RESULTS On the day of discharge,the two groups of children had the following scores of readiness for discharge:The intervention group score was 225.34±32.47,and the control group score was 208.68±29.31.The P value was 0.007,and the difference was statistically significant.The discharge education quality scores were as follows:The intervention group score was 135.11±19.86,the control group score was 124.13±15.56,the P value was 0.002 and the difference was statistically significant.Three months after discharge,the blood glucose metabolism indicator showed that the glycosylated hemoglobin value of the two groups was(7.45%±1.04%),and that of the control group was(8.04%±1.27%),P=0.012.Therefore,the improvement of parents'readiness for discharge,quality of discharge education and blood glucose metabolism indicators(glycosylated hemoglobin,fasting blood glucose and postprandial blood glucose)in the intervention group were better than those in the control group(P<0.05),and the difference was statistically significant.CONCLUSION The discharge plan for children with T1DM can help the children and their families realize the transition from hospital care to home self-management and improve the parents'readiness for discharge,thereby improving children’s blood glucose control levels. 展开更多

Objective Current clinical evidence on the effects of home blood pressure telemonitoring(HBPT)on improving blood pressure control comes entirely from developed countries.Thus,we performed this randomized controlled trial to evaluate whether HBPT plus support(patient education and clinician remote hypertension management)improves blood pressure control more than usual care(UC)in the Chinese population.Methods This single-center,randomized controlled study was conducted in Beijing,China.Patients aged 30-75 years were eligible for enrolment if they had blood pressure[systolic(SBP)≥140 mmHg and/or diastolic(DBP)≥90 mmHg;or SBP≥130 mmHg and/or DBP≥80 mmHg with diabetes].We recruited 190 patients randomized to either the HBPT or the UC groups for 12 weeks.The primary endpoints were blood pressure reduction and the proportion of patients achieving the target blood pressure.Results Totally,172 patients completed the study,the HBPT plus support group(n=84),and the UC group(n=88).Patients in the plus support group showed a greater reduction in mean ambulatory blood pressure than those in the UC group.The plus support group had a significantly higher proportion of patients who achieved the target blood pressure and maintained a dipper blood pressure pattern at the12th week of follow-up.Additionally,the patients in the plus support group showed lower blood pressure variability and higher drug adherence than those in the UC group.Conclusion HBPT plus additional support results in greater blood pressure reduction,better blood pressure control,a higher proportion of dipper blood pressure patterns,lower blood pressure variability,and higher drug adherence than UC.The development of telemedicine may be the cornerstone of hypertension management in primary care. 展开更多

Methods: A total of 138 patients with newly diagnosed T2DM who were treated in the hospital between March 2016 and April 2017 were divided into routine group (n=69) and combined treatment group (n=69) by random number table method. Routine group were treated with metformin alone and combined treatment group received DPP-4 inhibitor combined with metformin therapy. The differences in blood glucose control as well as oxidative stress-related indicator and inflammatory factor contents were compared between the two groups before and after treatment. Results: Before treatment, the differences in blood glucose index levels in peripheral blood as well as the oxidative stress index and inflammatory mediator contents in serum were not statistically significant between the two groups. After 4 weeks of treatment, blood glucose indexes FBG and HOMA-IR levels in peripheral blood of combined treatment group were lower than those of routine group;oxidative stress indexes MDA and LHP contents in serum were lower than those of routine group whereas GSH-Px and T-AOC contents were higher than those of routine group;inflammatory mediators hs-CRP, IL-1 and IL-6 contents in serum were lower than those of routine group. Conclusion: DPP-4 inhibitor combined with metformin therapy can effectively control the blood glucose and suppress the systemic oxidative stress and inflammatory response in T2DM paients. 展开更多

In recent decades, due to the increasing risk of diabetes, the measurement and control of the blood sugar is of great importance. In typeI diabetes, because of the lack of insulin secretion, the cells cannot absorb glucose, and thus have a low level of glucose. To control blood glucose, the insulin must be injected to the body. In fact, the injection must be in a completely controlled environment. If the level of the insulin exceeds the physiological limits, it may cause death. This paper presents an online approach to control the blood glucose level using a nonlinear model predictive control. This method, maintains the level of blood glucose concentration within a normal range. Thus, the blood glucose level is measured in each minute and predicted for the next time interval. If that is not in the normal range, amount of the insulin which must be injected will be determined. The proposed control approach includes important features such as model uncertainties and prevents acute decrease in the blood glucose level, and instability. In order to assess performance of the proposed controller, computer simulations have been carried out in Matlab/Simulink. Simulation results will reveal the effectiveness of the proposed nonlinear model predictive controller in adjusting the blood glucose level by injecting required insulin. So if the nutrition of the person decreases instantly, the hypoglycemia does not happen. 展开更多

[Objectives]To investigate the clinical effect of different doses of Xuesaitong combined with autologous platelet-rich gel(APG)on patients with diabetic foot(DF).[Methods]90 patients with diabetic foot admitted to our hospital from February 2017 to February 2019 were enrolled in the study.According to the random number table method,the subjects were divided into study group A and B and control group C.Group A was given a low dose of Xuesaitong combined with APG,while group B was given high-dose Xuesaitong combined with APG and group C was treated only with APG.Patients in the three groups were observed and the changes of related indexes were detected.[Results]After treatment,with regard to the three groups,the fasting blood glucose,2-h postprandial blood glucose level,HbA1c,TNF-α,Hcy,blood urea nitrogen(BUN),creatinine(Cr)and 24-h urine protein levels were all decreased,yet AT-III level was increased,and granulation tissue coverage and thickness,wound clearance rate were increased,while the repair time of ulcer surface was significantly reduced.The above indexes were all significantly different(P<0.05).Compared with the control group C,the changes of the indexes in the study group A and B were the same as above,and the difference was significant(P<0.05).Compared with the study group A,the level of AT-III in group B was significantly increased(P<0.05),while TNF-α,Hcy,BUN,Cr and 24-h urine protein levels were significantly decreased(P<0.05).There was no significant difference in the other indexes between groups A and B(P>0.05).[Conclusions]Xuesaitong combined with APG could effectively reduce the blood sugar level of DF patients,improve the clinical indexes,promote wound healing,and the high-dose group had more significant advantages and was worthy of promotion. 展开更多

Introduction: The management of hypertension is mostly based on pharmacotherapy and hygienic and dietary measures (HDMs) for which little data is available in Cameroon. The concern to improve the quality of life of hypertensive patients led us to study the knowledge, compliance and effect of HDMs among Cameroonian hypertensive patients. Methods: This was a cross-sectional study carried out at the Douala General Hospital;the census of patients was carried out from 05 March to 10 May 2018. The data evaluated were knowledge and compliance with HDMs with an inference of their effect on blood pressure control. Results: We recruited 330 participants at mean age of 60 ± 11 years, 37.9% men;57.3% with blood pressure (BP) controlled. Out of 330 subjects, 308 (93.3%) who had been educated about dietary health measures for hypertension were assessed on knowledge, compliance and effect of these measures against 22 (6.7%) who had never heard about them. Around 85.7% of participants had good knowledge of HDMs and 78.9% had good compliance with them. There was no statistically significant influence of knowledge and compliance with HDMs on blood pressure control. Conclusion: The level of knowledge and adherence to HDMs of hypertensive patients at the Douala General Hospital was appreciable. It is however appropriate for physicians to intensify patient education on HDMs and BP control. 展开更多

Objective:To determine the effects of the Self-Management Training Program on self-management behavior and blood pressure(BP)levels among elderly people with hypertension(HT).Methods:This study was based on a quasi-experimental design using sampling groups composed of elderly residents of Pathum Thani Province with HT.Simple random sampling was applied to two sampling groups,an experimental group and a control group.However,the experimental group had been on the Self-Management Training Program based on the Creer concept.The Program’s contents included small-group health-education sessions and the patients’manual,Self-Management Training Skills for Self-Management Behavior.Small group discussions were held for self-reflection,to stimulate follow-up for self-management behavior training during home visits.The Self-Management Training Program was compared with programs involving the more usual care.Data were collected using the Self-Management Behavior for Hypertension Control Questionnaire and BP assessment tests.Data were analyzed using a descriptive statistic,chi-square test,pair t-test,repeated-measures Analysis of Variance(ANOVA),and repeated-measures Analysis Covariance(ANCOVA).Results:It was found that at 13 weeks after entering the program,the experimental group demonstrated a significantly lower systolic and diastolic BP with a baseline statistical significance(P<0.005),and that at the 4th week and the 13th week,there was a decrease in BP in the experimental group.The experimental group also had above-baseline self-management behavior scores that were higher than in the control group(P<0.01).Conclusions:Results from this study show that the Self-Management Program was useful for BP control and promoted sustainable self-management behavior. 展开更多

The differences in blood glucose control, adipocytokine secretion and oxidative stress response were compared between the two groups before and after intervention.Results: Before intervention, the differences in blood glucose index levels in peripheral blood as well as the contents of adipocytokines and oxidative stress indexes in serum were not statistically significant between the two groups of patients. After 3 months of intervention, blood glucose indexes FPG, FINS and 2hPBG levels in peripheral blood of HIIT group were lower than those of control group;adipocytokine APN content in serum was higher than that of control group whereas LEP, Resistin and CHEM contents were lower than those of control group;oxidative stress indexes MDA and ROS contents in serum were lower than those of control group whereas T-AOC content was higher than that of control group. Conclusion: Routine hypoglycemic therapy combined with high-intensity interval training can further control the blood glucose levels, regulate the secretion of adipocytokines and reduce the systemic oxidative stress response. 展开更多

The prevention of complications depends on hypertension control. Aim: To assess blood pressure control, complication presence, and their influencing factors. Methods: This cross-sectional study was conducted on patients with hypertension attending a private medical facility, a governmental facility, and a primary health care center in Khartoum, Sudan, between September 2022 and February 2023. Statistical Package for Special Sciences version 25 was used for data analysis;frequency and percentage were used to describe the qualitative variables. A chi-square test was used for co-relation analysis;a P-value ≤ 0.05 was considered statistically significant, and the confidence interval was 95%. Results: The study included 250 participants;55.6% were males. Their age was 58.77 ± 6.80 years, and they had hypertension for 10.7 ± 8.51 years. The duration of the disease was 10.7 ± 8.51 years. Systolic blood pressure was 127.1 ± 11.3 mm hg, and diastolic blood pressure was 81.6 ± 7.1 mm hg. Of the participants, 76.4% had controlled hypertension, while 27.6% had complications due to hypertension. Controlled blood pressure was associated with females, age group 40 - 60 years, normal waist circumference, and high adherence level (P ≤ 0.05). The presence of complications was associated with males, participants older than 60 years, and a low adherence level (P ≤ 0.05). Conclusions: In Sudan, the level of control is good;nonetheless, not all patients achieve it. Availability and access to treatment facilities and medications should be improved. Proper patient counseling should be offered, as well as continuous care. 展开更多

Objective:To investigate the effect of different blood glucose levels on viscera injury, oxidative stress response and Wnt5a inflammatory pathway in children with sepsis.Methods:70 children with sepsis treated in our hospital between January 2014 and December 2015 were retrospectively collected and divided into the sepsis group A (blood glucose>10 mmol/L) (n=29) and the sepsis group B (blood glucose ≤10 mmol/L) (n=41) according to the different blood glucose levels, and 50 healthy children who accepted vaccination and were with normal blood glucose levels in our hospital during the same period were selected as healthy control group. The day after admission, RIA method was used to determine liver function indexes;automatic biochemical analyzer was used to determine oxidative stress indexes;enzyme-linked immunosorbent assay (ELISA) was used to detect Wnt5a inflammatory pathway index levels.Results: Liver function indexes total bilirubin (TBIL), alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels of sepsis group A were higher than those of sepsis group B and healthy control group (P<0.05);oxidative stress indexes advanced protein oxidation products (AOPPs), lipid hydrogen peroxide (LHP), reactive oxygen species (ROS), and malondialdehyde (MDA) levels were higher than those of sepsis group B and healthy control group while glutathione peroxidase (GSH-Px) level was lower than that of sepsis group B and healthy control group (P<0.05);serum Wnt5a inflammatory pathway indexes interleukin-1β (IL-1β), interleukin-6 (IL-6), interleukin-12 (IL-12) and macrophage inflammatory protein-1β (MIP-1β) levels were higher than those of sepsis group B and healthy control group (P<0.05).Conclusions:Sepsis children with high blood glucose are with severe liver function damage as well as intense oxidative stress and inflammatory response, which are the important indexes for severe illness and poor prognosis. 展开更多

Objective:To study the correlation of anxiety state with blood glucose control, microinflammation and oxidative stress in patients with type 2 diabetes mellitus.Methods:A total of 138 patients with type 2 diabetes mellitus were divided into the non-anxiety group (n=43) (SAS score < 50 points), mild-to-moderate anxiety group (n=71) (SAS score 50-69 points) and severe anxiety group (n=24) (SAS score 70 points) according to the self-rating anxiety scale (SAS) score. The differences in levels of glucose metabolism indexes, microinflammation indexes and oxidative stress indexes were compared among the three groups.Results: Peripheral blood FPG, P2hPG, HOMA-IR, CRP, ASAA, IL-6, MDA and ROS levels of mild-to-moderate anxiety group and severe anxiety group were higher than those of non-anxiety group while CAT and GSH-Px contents were lower than those of non-anxiety group;peripheral blood FPG, P2hPG, HOMA-IR, CRP, ASAA, IL-6, MDA and ROS levels of severe anxiety group were higher than those of mid-to-moderate anxiety group while CAT and GSH-Px contents were lower than those of mid-to-moderate anxiety group.Conclusion:The increase of anxiety in patients with type 2 diabetes mellitus is a direct factor leading to the abnormal glucose metabolism as well as the aggravation of microinflammatory state and oxidative stress state. 展开更多

Objective:To explore the risk factors of diabetic nephropathy and its correlation with blood pressure control.Methods:A retrospective analysis of 80 patients with diabetic nephropathy(diabetic nephropathy group)and another 80 patients with diabetes(diabetic group)who were admitted to the Department of Nephrology and Endocrinology at our hospital from October 2021 to October 2022 was conducted.The general data of the two groups were compared,the influencing factors associated with the two groups were analyzed unilaterally,and unconditional dichotomous logistic regression was performed to analyze the influencing factors in patients with diabetic nephropathy.Results:There were no significant differences in high-density lipoprotein,systolic blood pressure,diastolic blood pressure,and creatinine between the two groups(P>0.05);however,compared with the diabetic group,the DN group had significantly elevated glycated hemoglobin,low-density lipoprotein,24-h urine protein,insulin resistance,and diabetes duration≥10 years(P<0.05).Conclusion:The clinical research on the correlation between the incidence of hypertension and the control of blood pressure in patients with diabetic nephropathy should be strengthened in order to formulate reasonable and feasible treatment plans. 展开更多

AIM: To determine gastric emptying, blood pressure, mesenteric artery blood flow, and blood glucose responses to oral glucose in Parkinson&#x02019;s disease.METHODS: Twenty-one subjects (13 M, 8 F; age 64.2 &#x000b1; 1.6 years) with mild to moderate Parkinson&#x02019;s disease (Hoehn and Yahr score 1.4 &#x000b1; 0.1, duration of known disease 6.3 &#x000b1; 0.9 years) consumed a 75 g glucose drink, labelled with 20 MBq <sup>99m</sup>Tc-calcium phytate. Gastric emptying was quantified with scintigraphy, blood pressure and heart rate with an automated device, superior mesenteric artery blood flow by Doppler ultrasonography and blood glucose by glucometer for 180 min. Autonomic nerve function was evaluated with cardiovascular reflex tests and upper gastrointestinal symptoms by questionnaire.RESULTS: The mean gastric half-emptying time was 106 &#x000b1; 9.1 min, gastric emptying was abnormally delayed in 3 subjects (14%). Systolic and diastolic blood pressure fell (P &#x0003c; 0.001) and mesenteric blood flow and blood glucose (P &#x0003c; 0.001 for both) increased, following the drink. Three subjects (14%) had definite autonomic neuropathy and 8 (38%) had postprandial hypotension. There were no significant relationships between changes in blood pressure, heart rate or mesenteric artery blood flow with gastric emptying. Gastric emptying was related to the score for autonomic nerve function (R = 0.55, P &#x0003c; 0.01). There was an inverse relationship between the blood glucose at t = 30 min (R = -0.52, P &#x0003c; 0.05), while the blood glucose at t = 180 min was related directly (R = 0.49, P &#x0003c; 0.05), with gastric emptying.CONCLUSION: In mild to moderate Parkinson&#x02019;s disease, gastric emptying is related to autonomic dysfunction and a determinant of the glycaemic response to oral glucose. 展开更多

To investigate the effects of impaired glucose metabolism (IGM) on cardiovascular autonomic nervous systems in essential hypertensive (EH) patients by comparing heart rate variability (HRV) and blood pressure variability (BPV) in EH patients with or without type 2 diabetes mellitus (T2DM). Simultaneous 24-h recordings of ambulatory ECG and blood pressure monitoring were performed in 36 male old patients with simple EH and 33 male old patients with EH combined with T2DM. HRV analysis included time domain parameters such as SDNN, SDANN, SDNNi, rMSSD and pNN50, and total spectral power (TP) of HRV, which mainly consists of VLF, LF and HF component along with LF/HF ratio, was also obtained. The value of ambulatory blood pressure was represented as the mean blood pressure (mean systolic/mSBP, diastolic/mDBP and pulse pressure/mPP) during different periods (24 h/24 h, day time/d and night time/n). Standard deviation (SD) as well as coefficient of variance (CV) of blood pressure during each above-mentioned period were obtained to reflect the long-term BPV. Our result showed that SDNN, SDNNi, SDANN, rMSSD, PNN50, TP and HF of HRV in cases of EH with T2DM were all significantly lower than those in simple EH subjects (P〈0.05). No significant differences in VLF or LF was found between the two groups (P〉0.05), while LF/HF ratio was significantly higher in EH with T2DM patients than in simple EH subjects (P〈0.01). Moreover, dmSBP, 24 h-mPP and dmPP were all significantly higher in EH with T2DM patients than in simple EH subjects (P〈0.05), while nmSBP, 24 h-mSBP, 24 h-mDBP, dmDBP, nmDBP or nmPP showed no significant difference between this two groups of patients (P〉0.05). And dSBPSD, dSBPCV and 24 h-SBPSD were all significantly higher in EH with T2DM patients than in simple EH subjects (P〈0.05), while the other BPV indexes showed no significant difference between this two groups (P〉0.05). It is concluded that the cardiovascular autonomic nervous systems in EH patients was further impaired by T2DM, displaying lowering of HRV and enlargement of BPV, which in turn induced abnormal structural and functional changes of cardiovascular systems. Therefore, improving cardiovascular autonomic nervous systems might reduce the occurrence of cardiovascular complications in the EH patients with IGM. 展开更多
