Various treatment options are available, although often the acetabular defect cannot be corrected with a single option alone. Precise assessment of the bone loss and a suitable combination of methods are needed. Here we report a case of multiple failures with impaction bone grafting reconstruction for an acetabular bone defect of American Academy of Orthopedic Surgeons classification type III. We finally reconstructed the acetabulum with three femoral head allografts and a Kerboull-type acetabular reinforcement device. The allograft was a casted, jet-type helmet-like shape. A year later the patient was able to walk without a cane and perform light agricultural work. Accurate evaluation of the acetabular bone loss and appropriate reconstruction is important.展开更多

Introduction: Pseudarthrosis (PSA) of the diaphysis of long bones still remains a current problem, despite improvements in the treatment of these fractures. Our study aims to study the epidemiological and therapeutic ...Introduction: Pseudarthrosis (PSA) of the diaphysis of long bones still remains a current problem, despite improvements in the treatment of these fractures. Our study aims to study the epidemiological and therapeutic aspects of PSA of the diaphysis of long bones. Method: This retrospective work concerns 30 cases of non-union of the diaphysis of long bones treated in the orthopedic and trauma surgery department at Donka National Hospital, during a period of 18 months from January 1, 2019 to June 30, 2020. Results: We recruited 30 patients, 80% of whom were male, with an average age of 39.9 years. Public road accidents (AVP) represented the main cause of fractures of the diaphysis of long bones 87%, they were open in 25 cases or 83%. The fractures were located in the middle 1/3 of the diaphysis of the long bones in 50% of cases. Treatment of initial fractures was traditional in 21 cases, orthopedic in 2 cases and surgical in 7 cases. It was aseptic nonunion in 28 cases (93%) and septic nonunion in 2 cases. They were hypertrophic in 7 cases, slightly hypertrophic in 5 cases, oligotrophic in 11 cases, atrophic in 6 cases and with bone defect in 1 case. The treatment was based on osteosynthesis including 16 cases of screwed “PV” plate: 7 cases of centromedullary “ECM” nailing, 2 cases of external fixator, 1 case of broaching and 4 cases of Plastering. The results according to ASAMI criteria on an anatomical level were excellent in 19 cases, good in 3 cases and poor in 3 cases, with a union rate of 76%. And 5 patients undergoing consolidation. Conclusion: Based on the literature data and the experience of our department, the true treatment of PSA requires correct management of the initial fracture without forgetting the interest in preventing AVP which appears to be an element essential, making it possible to reduce the incidence of fractures of the diaphysis.展开更多

The results showed that (1) 5, 10 and 20 days after transplantation, the TNFα mRNA expressions were positive, andthe positive rate of expression was the highest by 10 days (P<0. 05 ). (2)There was strong hybridization signal localization to the nuclei of morphologically ldentifiable monocytes and multinucleated giant cells. (3)Similar activity was detected in the cytoplasm and (or) nuclei of partial adjacent mesenchymal cells, fibroblasts as well as striated muscle fibers. This finding tended to indicate that mRNA encoding for TNFα was intensely expressed in several kinds of cells and that TNFα seemed to be of importance for the modulation of local cellular immunity in the region of implanted xenogeneic bone.展开更多

Background Cavity reconstruction after benign bone tumor removal is varied and controversial.AIIograft is widely used but is associated with complications.New bone substitutes,such as calcium sulfate artificial bone,h...Background Cavity reconstruction after benign bone tumor removal is varied and controversial.AIIograft is widely used but is associated with complications.New bone substitutes,such as calcium sulfate artificial bone,have been introduced for bone tumor operation.However,the bone healing response of artificial bone has not been compared with allograft bone.We therefore compared calcium sulfate grafts (study group) with bone allografts (control group) for the treatment of benign bone tumors.Methods We retrospectively reviewed 50 patients who underwent calcium sulfate reconstruction and 50 patients who underwent allograft cancellous bone reconstruction.The two groups were well matched.The mean follow-up time of the study group was 19.9 (12-55) months.We investigated bone healing response,complications,and factors affecting bone healing.Results At the last follow-up,84% (42/50) of cases in the study group and 62% (31/50) of cases in the control group had achieved clinical healing (P=0.013).The initial healing rate showed no significant difference between the two groups (100% vs.96%,P=0.153).The mean healing times for calcium sulfate and allograft bone were 9.6 (3-42) months and 13.8 (3-36) months,respectively (P <0.01).Complications in the study group were minor and resolved.Implant volume was a significant factor affecting bone healing.Conclusion The calcium sulfate bone substitute showed a satisfactory healing outcome and safety profile in reconstruction of bone defects after benign bone tumor curettage,especially in smaller cavities.展开更多
Background Giant cell tumors (GCTs) most commonly occur around the knee.The most beneficial procedure for this disease has been extensive curettage with reconstruction.However,since many GCTs may compromise the subc...Background Giant cell tumors (GCTs) most commonly occur around the knee.The most beneficial procedure for this disease has been extensive curettage with reconstruction.However,since many GCTs may compromise the subchondral bone,surgery can further jeopardize the articular cartilage and result in secondary osteoarthritis.In this study,we aimed to determine the factors associated with the development of degenerative arthritis and the effect of bone grafting on the prevention of secondary osteoarthritis.Methods We retrospectively analyzed 76 patients with GCT around the knee.The mean age at first diagnosis was 31.1 years.Surgical treatments included extensive curettage and cementation with or without bone grafting in the subchondral bone.Patient follow-up was a median duration of 35 months,ranging from 18 to 113 months.Results The local recurrence rate was 5.3% (4/76).Secondary degenerative changes occurred in 30.3% (23/76) of the patients.Less than 10 mm of the residual thickness of the remaining subchondral bone was correlated with secondary degenerative changes in 57 patients (P <0.001).Of these 57 patients,56.5% (13/23) treated with bone cement reconstruction alone developed secondary degenerative changes; following bone grafting,the rate decreased to 29.4%(10/34),with a statistically significant difference (P=0.041).Conclusions GCT patients with less residual thickness of the subchondral bone are more likely to develop degenerative arthritis after curettage.Bone grafting in the subchondral bone area is recommended when the residual thickness of the subchondral bone is less than 10 mm.展开更多

The search terms were "impaction bone grafting", "particle size", "mechanical property"and "biological behavior".Study selection Articles relevant to the choices of allografts and their results for bone defect reconstruction on the acetabular side were selected.Results Different choices of allografts, including the particle size, process of irradiation or fat reduction, composition and particle grade, are made to improve the survival rate of a prosthesis in acetabular revision surgery. This review,which compares both mechanical and biological factors, summarizes the experimental and clinical results for different techniques.Conclusions Fresh frozen cancellous allografts with particle sizes ranging from 7 to 10 mm are a favorable choice for reconstruction of bone defects of American Academy of Orthopedic Surgeons (AAOS) types Ⅱ (cavitary defect) and Ⅲ(combined cavitary and segmental defect) on the acetabular side. A fat-reducing procedure with saline or solvent/detergent is controversial. Adding autologous marrow into irradiated allografts, which provides reliable mechanical stability and biological safety, may be a substitute for fresh frozen allografts. Cortical bone can be a supplementary material in cases of insufficiency of cancellous allografts. Cartilage should be excluded from the graft material. Further research is required to demonstrate the best particle grade, and randomized controlled trials in clinical practice are required to obtain more information about the selection of allografts.展开更多
Curettage of benign bone tumor is a common cause for bone defect.For such bone defect repair,autogenous bone,allogeneic bone and traditional artificial bone graft substitutes have many disadvantages.In recent years,a ...Curettage of benign bone tumor is a common cause for bone defect.For such bone defect repair,autogenous bone,allogeneic bone and traditional artificial bone graft substitutes have many disadvantages.In recent years,a biomimetic mineralized collagen(MC)with similar composition and microstructures to the natural bone matrix was developed and used for treating various bone defects.In this work,a retrospective study analyzed clinical outcomes of patients treated with curettage of benign bone tumors and bone grafting with MC,in comparison to another group treated with the same surgical method and autogenous bone.Lane-Sandhu X-ray score of the autogenous bone group was superior to the MC group at 1 month after the operation,but the two groups had no statistical difference at 6 and 12 months.The MC group was better in Musculoskeletal Tumor Society scoring at 1 and 6 months after the operation,and the two groups had no statistical difference at 12 month.Therefore,the MC performed not as good as autogenous bone in early stage of bone healing but achieved comparable outcomes in long-term follow-ups.Moreover,the MC has advantages in function recovery and avoided potential complications induced by harvesting autogenous bone.展开更多
BACKGROUND Giant cell tumor(GCT)is a benign lesion and rarely involves the patella.This disease is characterized by a relatively high recurrence rate after primary treatment.En bloc resection has been a predominant op...BACKGROUND Giant cell tumor(GCT)is a benign lesion and rarely involves the patella.This disease is characterized by a relatively high recurrence rate after primary treatment.En bloc resection has been a predominant option for recurrent GCT.However,total patellectomy can lead to disruption of the knee.Therefore,exploration of functional reconstruction of the extensor mechanism is worthwhile.CASE SUMMARY A 54-year-old woman presented with right knee pain and swelling,and was diagnosed as having a GCT in the patella following curettage and autograft.Medical imaging revealed a lytic and expanded lesion involving the whole patella with focal cortical breaches and pathological fracture.Based on the combination of histological,radiological,and clinical features,a diagnosis of recurrent GCT in the patella was made(Campanacci grade III).After a multidisciplinary team discussion,three-dimensional(3D)-printed custom-made patellar endoprosthesis was performed following en bloc resection for reconstructing the extensor mechanism.The patient was followed for 35 mo postoperatively.No evidence of local recurrence,pulmonary metastasis,or osteoarthritis of the right knee was observed.The active flexion arc was 0°-120°,and no extension lag was detected.A favorable patellar tracking and height(Insall-Salvati ratio 0.93)were detected by radiography.CONCLUSION We depict a case of a GCT at the right patella,which was successfully treated by patellectomy and 3D-printed custom-made endoprosthetic replacement.The patella normal reconstruction,the precise-fit articular design,and gastrocnemius flap augmentation could lead to satisfactory knee function and a low rate of complications in the short-term follow-up.展开更多

To determine the app...Introduction: Iliac particulate cancellous bone and marrow (PCBM) is commonly used as a high-quality reconstruction material;however, PCBM cannot be extracted in sufficient amounts to meet demand. To determine the appropriate amount of iliac PCBM to be collected, we used digital technology to measure the volume required for jaw reconstruction before surgery. Clinical Case: The patient, a 23-year-old man, underwent surgery for a calcifying odontogenic cyst. A maxillary cyst occupied the left anterior-premolar region (tooth 21 - 25) and the deciduous canine remained;a permanent canine was included in the cyst. We planned to preserve the teeth except for the impacted canine, completely excise the maxillary cyst, and preserve the alveolar ridge morphology. Preoperative digital imaging was used to determine the amount of alveolar ridge reconstruction required and accordingly determine the amount of iliac cancellous bone to be harvested. We used a titanium mesh tray and grafts of iliac particulate cancellous bone and marrow to reconstruct the alveolar ridge. The amount of iliac cancellous bone that needed to be collected was clarified and the supply amount could be collected in just the right amount;thus, the cortical bone of the iliac inner plate could be preserved. The alveolar bone morphology was reconstructed to allow the placement of dental implants as per the preoperative digital surgery. Three years after the operation, no sign of recurrence has been observed. Conclusion: Minimally invasive surgery was performed by clarifying the amount of iliac cancellous bone graft that needs to be harvested, which improved the accuracy of surgery.展开更多

Here we present a case treated with free vascularized fibular graft (FVFG) after this method. A 23-year-old man developed osteosarcoma in his left distal tibia. There was nonunion after frozen autograft reconstruction, which we treated with FVFG. Twenty-four months later, bridging between the host bone and the frozen autograft was achieved. Our department has achieved bone union in almost all cases, but we sometimes encounter cases of nonunion after this method because of delayed blood supply. In these instances, reconstruction using FVFG may represent an attractive choice for salvage treatment.展开更多
目的:分析和总结自体股骨头结构植骨重建髋臼辅助全髋关节置换术(total hip arthroplasty,THA)治疗改良Crowe TypeⅣB型成人髋关节发育不良性脱位(developmental dysplasia of the hip,DDH)患者的临床疗效。方法:按照改良Crowe分型,选...目的:分析和总结自体股骨头结构植骨重建髋臼辅助全髋关节置换术(total hip arthroplasty,THA)治疗改良Crowe TypeⅣB型成人髋关节发育不良性脱位(developmental dysplasia of the hip,DDH)患者的临床疗效。方法:按照改良Crowe分型,选取山东大学齐鲁医院德州医院关节外科2015年8月至2023年3月收治的TypeⅣB型DDH患者26例,其中男25例,女1例,采用自体股骨头结构植骨重建髋臼辅助THA,记录患者手术时间、术中失血量、术中术后输血量、术后血红蛋白、手术相关并发症和骨愈合时间等,并行骨盆正位X线片了解假体位置、假体骨长入、假体松动以及骨愈合情况等,采用视觉模拟评分表(visual analogue scale,VAS)评价髋关节的疼痛不适,采用髋关节Harris评分和Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)来评价髋关节功能及临床疗效。结果:所有患者的平均随访时间(9.73±8.35)个月,术中出血平均为(715.38±143.37) mL,术中平均输血(415.38±282.41) mL,手术时间平均为(118.62±18.27) min,术后平均输血为(192.31±236.51) mL。所有患者转子下骨端、自体股骨头和假臼之间均骨愈合良好。髋关节VAS评分从术前6.73±0.45,至术后末次随访时VAS评分1.73±0.53,差异有统计学意义(P=0.000),髋关节活动度均较术前明显改善,髋关节Harris评分从术前24.27±1.66,至术后末次随访时Harris评分74.77±2.89,差异有统计学意义(P=0.000),WOMAC术前术后评分分别为130.08±5.72和67.85±3.23,差异均有统计学意义(P=0.000)。结论:自体股骨头结构植骨重建髋臼辅助THA治疗改良Crowe TypeⅣB型DDH,具有操作相对简单、固定牢固、手术相对安全和疗效确切的优点。展开更多
目的通过分析原发性恶性骨肿瘤患者在瘤段切除后经不同重建方式治疗的随访资料,探讨手术方案的选择对临床疗效的影响。方法以2016年7月1日至2022年7月1日东部战区总医院单中心收治的四肢原发性恶性骨肿瘤患者为研究对象,根据瘤段切除后...目的通过分析原发性恶性骨肿瘤患者在瘤段切除后经不同重建方式治疗的随访资料,探讨手术方案的选择对临床疗效的影响。方法以2016年7月1日至2022年7月1日东部战区总医院单中心收治的四肢原发性恶性骨肿瘤患者为研究对象,根据瘤段切除后重建方式分为肿瘤型假体置换组和生物重建组。回顾患者的临床随访资料,对比2组患者至随访截止时间的复发率、转移率、生存率以及最终保肢率,并总结生物重建失败的影响因素。同时,采用美国肌肉骨骼肿瘤协会评分(MSTS)、多伦多保肢评分(TESS)评价并比较不同组间患者术后功能的恢复情况。结果共收集并筛选入组患者87例,其中男52例,女35例,年龄8~71岁,平均(38.3±21.2)岁。随访15~70个月,平均随访时间26.5个月。至随访截止时间,共存活67例,死亡20例,总生存率为77.01%;无瘤存活45例,无瘤生存率为51.72%。其中,假体置换组74例、生物重建组13例(瘤体骨灭活再植钢板重建2例、自体腓骨移植钢板重建11例)。生物重建组术后1年的MSTS评分及TESS评分均明显高于假体置换组(28.33±2.09 vs 24.16±4.51、85.62±6.71 vs 80.82±3.85,P<0.01)。假体置换组术后生存率达到了75.68%。而生物重建组患者术后生存率更达到了84.62%,且根据多因素Cox比例风险回归分析结果,不同的手术方式对无瘤生存(TFS)、总生存(OS)均无显著影响(P>0.05),复发和转移发生与否才是影响OS的预测因素(P=0.003和P=0.001)。而进一步的研究显示,两种不同手术方式的选择与患者的生存率(P=0.0843)、复发率(P=0.5206)、转移率(P=0.3217)无显著相关性。假体置换组与生物重建组的二次手术率(29.73%vs 38.46%)及最终保肢率(83.78%vs 84.62%)比较,差异均无统计学意义(P>0.05)。行瘤段切除后自体腓骨移植钢板重建的患者有11例,其中出现移植骨骨不连2例;愈合时间与年龄、术前肿瘤生长部位、术中截骨长度、取骨部位及植骨量均有相关性(P<0.05)。结论随着治疗理念和手术技术的不断进步,针对原发性恶性骨肿瘤瘤段切除后的骨缺损,采用术前精准测量和周密设计的生物重建手术,可以发挥其在关节稳定、肢体功能、保障疗效等各方面的优势,从而取得较假体置换手术更佳的治疗效果。展开更多

The aim of this study was to conduct a systematic literature review on th...Aim: Autogenous iliac crest bone graft is frequently used in immediate reconstruction of post ablation defect of benign mandibular pathologies. The aim of this study was to conduct a systematic literature review on the complication and failure rates with this technique and factors associated with failure. Methods: The initial literature search in PubMed and Cochrane databases identified 915 articles. Result: Of these, 7 were included in the final review. The majority of the studies were retrospective in nature. These articles encompassed 127 procedures with non-vascularized iliac crest bone graft;with complication rate of 13.3%and failure rate of 3.1%, most complications did not result in failure. All failures were due to infection with no main factor associated with failure. Conclusion: Use of non-vascularized iliac crest bone graft for immediate mandibular reconstruction appears to be associated with low complication and failure rates in carefully selected cases.展开更多

The results according to ASAMI criteria on an anatomical level were excellent in 19 cases, good in 3 cases and poor in 3 cases, with a union rate of 76%. And 5 patients undergoing consolidation. Conclusion: Based on the literature data and the experience of our department, the true treatment of PSA requires correct management of the initial fracture without forgetting the interest in preventing AVP which appears to be an element essential, making it possible to reduce the incidence of fractures of the diaphysis.
文摘The objective of this study was to determine if mRNA encoding for tumor necrosls factor-α(TNFα) was present at the site of implanted bovine cancellous bone and to observe the cellular localizations. The particles of bovine cancellous bone treated by special chemical reagents were implanted in the mouse’s muscle pouch. removed 5.10 and 20 days after implantation, and the specimens were processed for determining the expression and cellular localizations of TNFα mRNA, which was performed by a nonradioactive in situ hybridization technique. The results showed that (1) 5, 10 and 20 days after transplantation, the TNFα mRNA expressions were positive, andthe positive rate of expression was the highest by 10 days (P<0. 05 ). (2)There was strong hybridization signal localization to the nuclei of morphologically ldentifiable monocytes and multinucleated giant cells. (3)Similar activity was detected in the cytoplasm and (or) nuclei of partial adjacent mesenchymal cells, fibroblasts as well as striated muscle fibers. This finding tended to indicate that mRNA encoding for TNFα was intensely expressed in several kinds of cells and that TNFα seemed to be of importance for the modulation of local cellular immunity in the region of implanted xenogeneic bone.
文摘Giant cell tumour of the distal radius is the 3rd most common site after proximal tibia and distal femur. It is locally aggressive and is associated with a high rate of recurrence. Although it is usually treated with various modalities of treatment, wide resection and reconstruction with proximal fibular autograft is most commonly accepted in recurrent cases. The following is a case report of such a case with surgical management.
文摘Background Cavity reconstruction after benign bone tumor removal is varied and controversial.AIIograft is widely used but is associated with complications.New bone substitutes,such as calcium sulfate artificial bone,have been introduced for bone tumor operation.However,the bone healing response of artificial bone has not been compared with allograft bone.We therefore compared calcium sulfate grafts (study group) with bone allografts (control group) for the treatment of benign bone tumors.Methods We retrospectively reviewed 50 patients who underwent calcium sulfate reconstruction and 50 patients who underwent allograft cancellous bone reconstruction.The two groups were well matched.The mean follow-up time of the study group was 19.9 (12-55) months.We investigated bone healing response,complications,and factors affecting bone healing.Results At the last follow-up,84% (42/50) of cases in the study group and 62% (31/50) of cases in the control group had achieved clinical healing (P=0.013).The initial healing rate showed no significant difference between the two groups (100% vs.96%,P=0.153).The mean healing times for calcium sulfate and allograft bone were 9.6 (3-42) months and 13.8 (3-36) months,respectively (P <0.01).Complications in the study group were minor and resolved.Implant volume was a significant factor affecting bone healing.Conclusion The calcium sulfate bone substitute showed a satisfactory healing outcome and safety profile in reconstruction of bone defects after benign bone tumor curettage,especially in smaller cavities.
文摘Background Giant cell tumors (GCTs) most commonly occur around the knee.The most beneficial procedure for this disease has been extensive curettage with reconstruction.However,since many GCTs may compromise the subchondral bone,surgery can further jeopardize the articular cartilage and result in secondary osteoarthritis.In this study,we aimed to determine the factors associated with the development of degenerative arthritis and the effect of bone grafting on the prevention of secondary osteoarthritis.Methods We retrospectively analyzed 76 patients with GCT around the knee.The mean age at first diagnosis was 31.1 years.Surgical treatments included extensive curettage and cementation with or without bone grafting in the subchondral bone.Patient follow-up was a median duration of 35 months,ranging from 18 to 113 months.Results The local recurrence rate was 5.3% (4/76).Secondary degenerative changes occurred in 30.3% (23/76) of the patients.Less than 10 mm of the residual thickness of the remaining subchondral bone was correlated with secondary degenerative changes in 57 patients (P <0.001).Of these 57 patients,56.5% (13/23) treated with bone cement reconstruction alone developed secondary degenerative changes; following bone grafting,the rate decreased to 29.4%(10/34),with a statistically significant difference (P=0.041).Conclusions GCT patients with less residual thickness of the subchondral bone are more likely to develop degenerative arthritis after curettage.Bone grafting in the subchondral bone area is recommended when the residual thickness of the subchondral bone is less than 10 mm.
文摘Objective To review the choices of allografts for bone defect reconstruction in acetabular revision surgery using the technique of impaction bone grafting.Data sources The data cited in this review were mainly obtained from articles listed in PubMed that were published from January 1993 to July 2009. The search terms were "impaction bone grafting", "particle size", "mechanical property"and "biological behavior".Study selection Articles relevant to the choices of allografts and their results for bone defect reconstruction on the acetabular side were selected.Results Different choices of allografts, including the particle size, process of irradiation or fat reduction, composition and particle grade, are made to improve the survival rate of a prosthesis in acetabular revision surgery. This review,which compares both mechanical and biological factors, summarizes the experimental and clinical results for different techniques.Conclusions Fresh frozen cancellous allografts with particle sizes ranging from 7 to 10 mm are a favorable choice for reconstruction of bone defects of American Academy of Orthopedic Surgeons (AAOS) types Ⅱ (cavitary defect) and Ⅲ(combined cavitary and segmental defect) on the acetabular side. A fat-reducing procedure with saline or solvent/detergent is controversial. Adding autologous marrow into irradiated allografts, which provides reliable mechanical stability and biological safety, may be a substitute for fresh frozen allografts. Cortical bone can be a supplementary material in cases of insufficiency of cancellous allografts. Cartilage should be excluded from the graft material. Further research is required to demonstrate the best particle grade, and randomized controlled trials in clinical practice are required to obtain more information about the selection of allografts.

文摘Curettage of benign bone tumor is a common cause for bone defect.For such bone defect repair,autogenous bone,allogeneic bone and traditional artificial bone graft substitutes have many disadvantages.In recent years,a biomimetic mineralized collagen(MC)with similar composition and microstructures to the natural bone matrix was developed and used for treating various bone defects.In this work,a retrospective study analyzed clinical outcomes of patients treated with curettage of benign bone tumors and bone grafting with MC,in comparison to another group treated with the same surgical method and autogenous bone.Lane-Sandhu X-ray score of the autogenous bone group was superior to the MC group at 1 month after the operation,but the two groups had no statistical difference at 6 and 12 months.The MC group was better in Musculoskeletal Tumor Society scoring at 1 and 6 months after the operation,and the two groups had no statistical difference at 12 month.Therefore,the MC performed not as good as autogenous bone in early stage of bone healing but achieved comparable outcomes in long-term follow-ups.Moreover,the MC has advantages in function recovery and avoided potential complications induced by harvesting autogenous bone.

文摘BACKGROUND Giant cell tumor(GCT)is a benign lesion and rarely involves the patella.This disease is characterized by a relatively high recurrence rate after primary treatment.En bloc resection has been a predominant option for recurrent GCT.However,total patellectomy can lead to disruption of the knee.Therefore,exploration of functional reconstruction of the extensor mechanism is worthwhile.CASE SUMMARY A 54-year-old woman presented with right knee pain and swelling,and was diagnosed as having a GCT in the patella following curettage and autograft.Medical imaging revealed a lytic and expanded lesion involving the whole patella with focal cortical breaches and pathological fracture.Based on the combination of histological,radiological,and clinical features,a diagnosis of recurrent GCT in the patella was made(Campanacci grade III).After a multidisciplinary team discussion,three-dimensional(3D)-printed custom-made patellar endoprosthesis was performed following en bloc resection for reconstructing the extensor mechanism.The patient was followed for 35 mo postoperatively.No evidence of local recurrence,pulmonary metastasis,or osteoarthritis of the right knee was observed.The active flexion arc was 0°-120°,and no extension lag was detected.A favorable patellar tracking and height(Insall-Salvati ratio 0.93)were detected by radiography.CONCLUSION We depict a case of a GCT at the right patella,which was successfully treated by patellectomy and 3D-printed custom-made endoprosthetic replacement.The patella normal reconstruction,the precise-fit articular design,and gastrocnemius flap augmentation could lead to satisfactory knee function and a low rate of complications in the short-term follow-up.
文摘Introduction: Iliac particulate cancellous bone and marrow (PCBM) is commonly used as a high-quality reconstruction material;however, PCBM cannot be extracted in sufficient amounts to meet demand. To determine the appropriate amount of iliac PCBM to be collected, we used digital technology to measure the volume required for jaw reconstruction before surgery. Clinical Case: The patient, a 23-year-old man, underwent surgery for a calcifying odontogenic cyst. A maxillary cyst occupied the left anterior-premolar region (tooth 21 - 25) and the deciduous canine remained;a permanent canine was included in the cyst. We planned to preserve the teeth except for the impacted canine, completely excise the maxillary cyst, and preserve the alveolar ridge morphology. Preoperative digital imaging was used to determine the amount of alveolar ridge reconstruction required and accordingly determine the amount of iliac cancellous bone to be harvested. We used a titanium mesh tray and grafts of iliac particulate cancellous bone and marrow to reconstruct the alveolar ridge. The amount of iliac cancellous bone that needed to be collected was clarified and the supply amount could be collected in just the right amount;thus, the cortical bone of the iliac inner plate could be preserved. The alveolar bone morphology was reconstructed to allow the placement of dental implants as per the preoperative digital surgery. Three years after the operation, no sign of recurrence has been observed. Conclusion: Minimally invasive surgery was performed by clarifying the amount of iliac cancellous bone graft that needs to be harvested, which improved the accuracy of surgery.
文摘Recently we have been performing biological reconstruction for malignant bone tumors of the extremities using frozen autografts. Here we present a case treated with free vascularized fibular graft (FVFG) after this method. A 23-year-old man developed osteosarcoma in his left distal tibia. There was nonunion after frozen autograft reconstruction, which we treated with FVFG. Twenty-four months later, bridging between the host bone and the frozen autograft was achieved. Our department has achieved bone union in almost all cases, but we sometimes encounter cases of nonunion after this method because of delayed blood supply. In these instances, reconstruction using FVFG may represent an attractive choice for salvage treatment.
文摘目的通过分析原发性恶性骨肿瘤患者在瘤段切除后经不同重建方式治疗的随访资料,探讨手术方案的选择对临床疗效的影响。方法以2016年7月1日至2022年7月1日东部战区总医院单中心收治的四肢原发性恶性骨肿瘤患者为研究对象,根据瘤段切除后重建方式分为肿瘤型假体置换组和生物重建组。回顾患者的临床随访资料,对比2组患者至随访截止时间的复发率、转移率、生存率以及最终保肢率,并总结生物重建失败的影响因素。同时,采用美国肌肉骨骼肿瘤协会评分(MSTS)、多伦多保肢评分(TESS)评价并比较不同组间患者术后功能的恢复情况。结果共收集并筛选入组患者87例,其中男52例,女35例,年龄8~71岁,平均(38.3±21.2)岁。随访15~70个月,平均随访时间26.5个月。至随访截止时间,共存活67例,死亡20例,总生存率为77.01%;无瘤存活45例,无瘤生存率为51.72%。其中,假体置换组74例、生物重建组13例(瘤体骨灭活再植钢板重建2例、自体腓骨移植钢板重建11例)。生物重建组术后1年的MSTS评分及TESS评分均明显高于假体置换组(28.33±2.09 vs 24.16±4.51、85.62±6.71 vs 80.82±3.85,P<0.01)。假体置换组术后生存率达到了75.68%。而生物重建组患者术后生存率更达到了84.62%,且根据多因素Cox比例风险回归分析结果,不同的手术方式对无瘤生存(TFS)、总生存(OS)均无显著影响(P>0.05),复发和转移发生与否才是影响OS的预测因素(P=0.003和P=0.001)。而进一步的研究显示,两种不同手术方式的选择与患者的生存率(P=0.0843)、复发率(P=0.5206)、转移率(P=0.3217)无显著相关性。假体置换组与生物重建组的二次手术率(29.73%vs 38.46%)及最终保肢率(83.78%vs 84.62%)比较,差异均无统计学意义(P>0.05)。行瘤段切除后自体腓骨移植钢板重建的患者有11例,其中出现移植骨骨不连2例;愈合时间与年龄、术前肿瘤生长部位、术中截骨长度、取骨部位及植骨量均有相关性(P<0.05)。结论随着治疗理念和手术技术的不断进步,针对原发性恶性骨肿瘤瘤段切除后的骨缺损,采用术前精准测量和周密设计的生物重建手术,可以发挥其在关节稳定、肢体功能、保障疗效等各方面的优势,从而取得较假体置换手术更佳的治疗效果。
文摘Aim: Autogenous iliac crest bone graft is frequently used in immediate reconstruction of post ablation defect of benign mandibular pathologies. The aim of this study was to conduct a systematic literature review on the complication and failure rates with this technique and factors associated with failure. Methods: The initial literature search in PubMed and Cochrane databases identified 915 articles. Result: Of these, 7 were included in the final review. The majority of the studies were retrospective in nature. These articles encompassed 127 procedures with non-vascularized iliac crest bone graft;with complication rate of 13.3%and failure rate of 3.1%, most complications did not result in failure. All failures were due to infection with no main factor associated with failure. Conclusion: Use of non-vascularized iliac crest bone graft for immediate mandibular reconstruction appears to be associated with low complication and failure rates in carefully selected cases.
