Therefore, greater therapeutic efficacy for cerebral palsy could be obtained by selecting the five Shu acupoints, which are located at the qi-intersecting point of the twelve meridians below the elbow and knee. OBJECTIVE: To treat children with spastic cerebral palsy using acupuncture and massage at the Shu and He points through a combination of rehabilitative training, and to compare the effects with routine acupuncture and massage. DESIGN: A randomized controlled clinical trial. SETTING: Rehabilitation Center for Cerebral Palsy, Department of Pediatrics, Affiliated Hospital of Liaoning University of Traditional Chinese Medicine. PARTICIPANTS: Sixty children with spastic cerebral palsy were selected from the Rehabilitation Center for Cerebral Palsy, Department of Pediatrics, Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from May 2003 to February 2006. There were 38 boys and 22 girls, aged 10 months to 4 years old, muscle strength ranging from grade I to grade IV. The children were randomly divided into a treatment group (n =30) and a control group (n =30). Informed consent for the therapeutic program was obtained from the relatives of all enrolled children. The study was approved by the Hospital's Ethical Committee. METHODS: All children were given exercise and occupational therapy based on standard treatments. (1) In the control group, the children were additionally treated with routine acupuncture and massage during the one-month basic rehabilitative training. The acupuncture needles were manipulated alternately at the acupoints of Jianyu, Shousanli, Binao, Hegu, Huantiao, Xuanzhong, and Biguan. They were treated once per day, 6 days a week, followed by one day of rest. The children's limbs were first massaged by pressing, stretching, rolling, and kneading. Then, the same acupuncture points were pressed with the thumb for 30 minutes each time, once a day, 6 days a week, followed by one day of rest. The treatment continued for 1 month. (2) In the treatment group, the children were administered acupuncture and massage at the Shu and He points during the one-month basic rehabilitative treatment. For acupuncture of the upper limbs, the following acupoints were used: Sanjian and Quchi of the Large Intestine Meridian of Hand-Yangming, and Houxi and Xiaohai of the Small Intestine Meridian of Hand-Taiyang. The acupoints were acupunctured alternately every other day. For acupuncture of the lower limbs, the following acupoints were used; Zulinqi and Yanglingquan of the Gall bladder Meridian of Foot-Shaoyang, Taibai and Yinlingquan of the Spleen Meridian of Foot-Taiyang, or Xiangu and Zusanli of the Stomach Meridian of Foot-Yangming. The acupoints were acupunctured alternately every other day. Three needles were for the medial malleolus and three needles for the lateral malleolus. They were treated once a day, 6 days a week, followed by one day of rest. The children's limbs were first massaged by means of pressing, stretching, rolling, and kneading. The same acupuncture points were then pressed with the thumb for 0.5-1 minute at each point, 30 minutes each time, once a day, 6 days a week, followed by one day of rest. MAIN OUTCOME MEASURES: Therapeutic efficacy was evaluated using gross motor function measurements, as well as activity of daily living scale before and after treatment. RESULTS: All 60 enrolled children with spastic cerebral palsy were included in the final analysis of results. (1) Comparison of gross motor function scores: The gross motor function score after treatment was significantly higher in both the treatment group and control group than before treatment (t = -3.86 and -8.882, respectively, P 〈 0.05). The scores in the treatment group were significantly higher than the control group scores (t = -7.166, P 〈 0.01). (2) Changes in activity of daily living scores: The scores for activity of daily living in the treatment group was significantly higher after treatment than before treatment (t = -3.933, P 〈 0.01), and was also significantly higher than the control group (t = -0.671, P 〈 0.05). CONCLUSION: Acupuncture and massage at the Shu and He acupoints can significantly ameliorate motor function deficits and movement disabilities in children with spastic cerebral palsy. The therapeutic efficacy at these acupoints is better than routine acupuncture and massage.展开更多

The central nervous system (CNS) alterations mainly interfere with the anatomical organization of areas undergoing a phase of intense postnatal cell proliferation, disrupting plastic processes like learning, memory, and phonation. In the rat pup, prenatal malnutrition interferes with the elaboration of ultrasonic vocalizations (USVs) by poorly understood mechanisms. The neuronal caudal portion of the nucleus ambiguus (Ambc) innervates the laryngeal intrinsic muscles to produce phonation, a basic USV communication system. During postnatal development, enhanced plasticity phenomena play a fundamental role in improving brain function. Thus, the massage stimulation (MS) may accelerate growth and induce neurogenesis in different areas of the brain. The current study analyzed the effects of a daily 10-min MS on the dendritic tree and perikarya measurements of Ambc multipolar motoneurons (Golgi-Cox) of perinatally underfed (U), control (C), control massage-stimulated (CMS), and underfed massage-stimulated (UMS) groups at postnatal days (PDs) 8, 12, and 15. The data indicated that the dendritic scores were reduced (p < 0.05) in both number and density at PD8 and PD15 in the U subjects and that MS increased the values of these parameters (p < 0.05). In addition, MS induced body weight gain in both U and CMS groups, and it enhanced the dendritic density in CMS subjects. These results show that MS during the pre-weaning period restores the plastic properties of the Ambc over the hypoplastic multipolar motoneuron after the alterations caused by perinatal undernutrition.展开更多

Therefore, interventions that increase the probability of intact perineum are necessary. The aim of study was to determine the effect of perineal massage with a sterile lubricant on the incidence of episiotomy and perinea laceration. Materials: This clinical trial study was performed on 145 nulliparous women who referred to Amol Emam Ali teaching center for normal delivery. They were randomly participating in interventional group (massage with lubricant) (45 cases) or control group (100 cases). In massage group when they progressed to full dilatation of the cervix, the midwife inserted two fingers inside vagina and using a sweeping motion gently stretched the perineum with lubricant 5 up to 10 minutes, in and between mother’s pushing in the second stage of labour. In control group just Ritgen Maneuver was applied. At last, we compared the rate of intact perineum, episiotomy and laceration, mean duration of the second stage of labor and Apgar score in 1 and 5 minutes between two groups. Statistical analyses were performed using t-test, Chi Square to determine potentially significant associations, and a p value less than 0.05 was considered significant. Results: The incidences of intact perineum, episiotomy and laceration were 22.2% (10), 44.4% (20), 33.3% (15) respectively in interventional group. In control group, intact perineum, episiotomy and laceration were: 20.2% (20), 49.3% (71), 28.3% (28) respectively. This difference was not statis- tically significant. Rate of first-degree laceration was 33.3% (15) in massage group, while this percent was 28.3% (28) in control group. This difference was not statistically significant. In massage and control groups, second, third and fourth-degree lacerations did not occur. Conclusion: The results showed that massage with a sterile lubricant provides no apparent and significant advantage or disadvantage in reducing perineal trauma. Therefore, the use of massage as technique for perineal control is safe based on labour criteria and woman’s preference during delivery.展开更多

文摘BACKGROUND: Based on clinical findings and syndromes, it has been previously shown that therapeutic methods for cerebral palsy can be greatly improved by selecting fewer, but more accurate acupoints. The therapeutic efficacy at these acupoints is better than routine acupuncture and massage.
文摘Objective:This meta-analysis aims to systematically evaluate the effects of massage on infants with jaundice. Methods:Multiple electronic databases, including Cochrane Library,PubMed,EMBASE,Web of Science,China Biology Medicine (CBM),Wan Fang Data,VIP Database for Chinese Technical Periodicals and China National Knowledge Infrastructure (CNKI),were used to search for studies on the effects of massage on infants with jaundice.Data were analysed by Rev Man 5.3.Results:A total of 14 randomised controlled trials with 1889 patients were included.Statistically significant difference in percutaneous bilirubin [MD =-1.21,95% CI (-1.90,-0.52),P< 0.05;MD =-2.00,95% CI (-2.68,-1.32),P<0.05;MD=-2.00,95% CI (2.56,-1.44),P<0.05;MD=-1.93,95% CI (-2.44,-1.43),P< 0.05] was found between two groups at 48,72,96 and 168 h.Studies on the serum total bilirubin level were divided into two subgroups according to sample size, and the results of subgroup analysis showed that the serum total bilirubin level in the intervention group was significantly lower than that in the control group [MD =-52.06,95% CI (-57.76,-46.36),P< 0.05 and MD =-10.65,95% CI (-14.66,-6.63),P < 0.05].Statistically significant difference in defecation frequency was observed between the two groups at 48 h after birth[SMD =0.44,95%CI (0.02,0.87),P< 0.05].Conclusion:Massage can decrease serum total bilirubin and percutaneous bilirubin levels and increasing defecation frequency. However, due to heterogeneity among studies, numerous multi-centre,largesample and high-quality randomised controlled trials are needed to verify the effects of massage.

文摘Massage therapy is an alternative treatment for chronic pain that is potentially related to brain plasticity. However, the underlying mechanism remains unclear. We established a peripheral nerve injury model in rats by unilateral sciatic nerve transection and direct anastomosis. The experimental rats were treated over the gastrocnemius muscle of the affected hindlimb with a customized massage instrument(0.45 N,120 times/min,10 minutes daily, for 4 successive weeks).Resting-state functional magnetic resonance imaging revealed that compared with control rats, the amplitude of low-frequency fluctuations in the sensorimotor cortex contralateral to the affected limb was significantly lower after sciatic nerve transection. However, amplitudes were significantly higher in the massage group than in a sham-massage group. These findings suggest that massage therapy facilitated adaptive change in the somatosensory cortex that led to the recovery of peripheral nerve injury and repair. This study was approved by the Animal Ethics Committee of Shanghai University of Traditional Chinese Medicine of China(approval No.201701001)on January 12,2017.
文摘Undernutrition produces morphological brain alterations and cognitive deficiencies in children of underdeveloped countries. These results show that MS during the pre-weaning period restores the plastic properties of the Ambc over the hypoplastic multipolar motoneuron after the alterations caused by perinatal undernutrition.
文摘Background: Perineal traumas particularly caused by following vaginal delivery are associated with short and long term morbidity for women. Therefore, the use of massage as technique for perineal control is safe based on labour criteria and woman’s preference during delivery.
文摘Objective: To evaluate the effects of abdominal massage on gastrointestinal function in the intensive care unit(ICU) patients. Methods: Randomized controlled trials about the effects of abdominal massage on gastrointestinal function in ICU patients were included from multiple electronic databases: PubMed, Web of Science, the Cochrane Library, Embase, CINAHL, China Academic Journals Full-Text Database(CNKI), Wanfang Database, and CQVIP, until November 2018. Studies were selected according to inclusion and exclusion criteria, extracting data and assessing. Data were analyzed by RevMan 5.3.Results: Nine studies with 720 patients were included. The results of meta-analysis in the intervention group were as follows: abdomen circumference: mean difference(MD)=-4.22, 95% confidence interval(CI)=(-6.20,-2.24), P<0.00001;abdominal distension: MD=0.34, 95% CI=(0.22, 0.52), P<0.00001;gastric residual: MD=-41.51, 95% CI=(-55.86,-29.15), P=0.001;gastric retention: MD=-0.23, 95% CI(-0.30,-0.15), P<0.00001;and vomiting: MD=0.12, 95% CI=(0.04, 0.35), P=0.0001.Conclusions: Abdominal massage is effective in reducing abdominal distension, gastric residual, and vomiting. When the intervention period was <7 days, abdominal massage could not reduce the abdominal circumference in ICU patients, and when the intervention period was equal to 7 days, abdominal massage could reduce the abdominal circumference in ICU patients.
