Effect of resilience on quality of life and anxiety in patients with breast cancer

BACKGROUND The incidence of breast cancer is high,with serious implications in terms of lives and health.Relevant data show that there are approximately 1 million new cases of breast cancer reported annually,with a rising trend.Some patients have poor treatment effects and are prone to anxiety and other negative emotions,which affect their quality of life(QoL).AIM To explore the correlation between mental resilience,QoL,and anxiety in patients with breast cancer.METHODS Using convenience sampling,200 patients with breast cancer were selected from the First Affiliated Hospital of Hebei North University.These patients were investigated using the Conner-Davidson Resilience Scale,Self-Rating Anxiety Scale,and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire to analyze the impact of resilience in patients with breast cancer on their QoL and anxiety.RESULTS The mean(SD)mental resilience score of the patients with breast cancer was 59.68(±9.84)points,the anxiety score was 49.87(±8.26)points,and the QoL score was 59.73(±8.29)points.Overall,they showed low mental resilience,mild anxiety,and medium QoL.Anxiety was negatively correlated with mental resilience and QoL(r=-0.275,r=-0.289,P<0.05).QoL was positively correlated with mental resilience(r=0.513,P<0.05).Anxiety was a mediating variable between mental resilience and QoL,accounting for 8.58%of the mediating effect.CONCLUSION Regarding psychological elasticity,anxiety plays an intermediary role in QoL among patients with breast cancer.Medical staff can improve patients'mental resilience by reducing their anxiety and improving their QoL. 展开更多

BACKGROUND Breast cancer(BC)is the second leading cause of tumor-related mortality after lung cancer.Chemotherapy resistance remains a major challenge to progress in BC treatment,warranting further exploration of feasible and effective alternative therapies.AIM To analyzed the quality of life(QoL)and survival of patients with BC treated with integrated traditional Chinese and Western medicine(TCM-WM).METHODS This study included 226 patients with BC admitted to the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine between February 2018 and February 2023,including 100 who received conventional Western medicine treatment(control group)and 126 who received TCM-WM treatment(research group).The total effective rate,side effects(alopecia,nausea and vomiting,hepatorenal toxicity,and myelosuppression),QoL assessed using the European Organization for Research and Treatment of Cancer Core Quality of Life Questionnaire(EORTC QLQ-C30),1-year overall survival(OS),recurrence and metastasis rates,and serum inflammatory factors[interleukin(IL)-6,IL-10,and tumor necrosis factor alpha]were comparatively analyzed.RESULTS The research group showed statistically better overall efficacy,EORTC QoL-C30 scores,and 1-year OS than the control group,with markedly lower side effects and 1-year recurrence and metastasis rates.Moreover,the posttreatment levels of serum inflammatory in the research group were significantly lower than the baseline and those in the control group.CONCLUSION Overall,TCM-WM demonstrated significantly improved therapeutic efficacy while ensuring drug safety in BC,which not only improved patients’QoL and prolonged survival,but also significantly inhibited the inflammatory response. 展开更多

Changes in acute and late toxicity and patient-reported health-related quality of life following radiotherapy in women with breast cancer:A 1-year longitudinal study

Objective:The objective of this study was to investigate the frequency of acute and late toxicities,as well as changes in the quality of life(QOL)for breast cancer patients following radiotherapy(RT).Materials and Methods:A total of 108 breast cancer women were recruited for this prospective study.Data were collected at various intervals;prior to,and 1,3,6 months,and 1 year after radiation therapy.The primary outcomes were toxicity radiation therapy oncology group/European Organization for Research and Treatment of Cancer(EORTC)criteria.Our secondary outcome was QOL,measured using EORTC QLQ-C30 and Edmonton Symptom Assessment Scale.We employed Friedman’s two-way analysis to evaluate the changes in QOL over the course of 1 year.Results:The early toxicities that are most commonly experienced include pharyngeal,skin,and mucous membrane toxicity.Late toxicities frequently involve skin and submucosal toxicity.To measure patient functionality,all functional subscale scores except for the patient’s emotional state increased over time compared to pre-RT.Symptoms of the patients,which were included in the QOL symptom scale,decreased during the follow-up period,except for fatigue;however,changes in pain,insomnia,and loss of appetite did not significantly change.We identified the analogous symptom profiles in Edmonton.Although patients’overall health scores declined in the 1st and 3rd months after radiotherapy(RT),they rebounded at 6 and 12 months.Conclusion:For breast cancer patients,RT did not adversely affect functional capacity or exacerbate symptoms,but persistent fatigue did increase during the observation period.Health-care professionals ought to devise strategies to assist patients with skin toxicity and fatigue. 展开更多

Objective:To explore the current status and influencing factors of quality of life in patients with lung cancer after surgery in a tertiary hospital in Hainan province.Methods:To investigate the influencing factors of quality of life of lung cancer patients after surgery in a tertiary hospital in Hainan province by cross‑sectional survey method.Results:The scores of insomnia,appetite loss,constipation and pain in 186 lung cancer patients after surgery in a tertiary hospital in Hainan Province were significantly higher than the reference value.Multiple linear regression analysis showed that older patients(>60 years)had lower scores in physical function domain(β=-0.193),and female patients had more appetite loss symptoms(β=0.245).Compared with other minority ethnic groups,Han ethnic group had lower scores in role function domain(β=0.179),more severe fatigue symptoms(β=-0.162),and higher general health level(β=0.166).Patients with employee medical insurance had lower scores of emotional function(β=0.194),cognitive function(β=0.281),the lowest score in social function(β=0.188),and severe pain in other parts(β=-0.227).Smokers had less cough symptoms(β=0.175)and more arm and shoulder pain symptoms(β=-0.21)than non‑smokers.Patients with secondhand smoke exposure had lower cognitive function scores(β=-0.158)and more obvious symptoms of oral ulcer(β=0.185).Patients who drank alcohol frequently(drinking frequency>1 time/day)had more severe cough symptoms(β=0.27).Patients with small number of children(0‑1)had milder cough symptoms(β=0.178).Patients who did not understand the disease had obvious symptoms of arm and shoulder pain(β=0.151).Patients with early pathological stage(stageⅠ‑Ⅱ)had more severe shortness of breath(β=-0.159)and pain(β=-0.181).The symptoms of appetite loss were more obvious in patients living in cities(β=0.192).The symptoms of peripheral neuropathy were more obvious(β=0.174).Patients who often consumed pickulated food had severe pain symptoms(β=-0.219),and pain in other parts was obvious(β=-0.149).Male patients had obvious alopecia symptoms(β=-0.306).Conclusion:Age,ethnicity,residence,type of medical insurance,number of children,pathological stage of lung cancer,smoking,second‑hand smoke exposure,alcohol consumption,and frequent consumption of pickled food were related to the quality of life of lung cancer patients in hospital after surgery.Medical staff and family members should pay attention to the emotional communication of patients during the treatment of lung cancer patients in hospital after surgery.Patients should avoid exposure to smoking,alcohol and second‑hand smoke,and reduce consumption of pickled food. 展开更多

Objective: To understand the changing trajectory of quality of life(QOL) during the treatment courses of breast cancer patients during chemotherapy and to investigate the factors in each treatment course that affect QOL.Methods: The M.D. Anderson Symptom Inventory Scale, the Hospital Anxiety and Depression Scale(HADS), and the Functional Assessment of Cancer Therapy-Breast(FACT-B) scale were used to perform a survey on 174 breast cancer patients who received the TAC(docetaxel, Adriamycin, and cyclophosphamide) chemotherapy regimen before postoperative chemotherapy and 5-7 days after each chemotherapy course.Results: The QOL scores of the breast cancer patients were the lowest before the postoperative chemotherapy(81.2 ± 19.6) and the highest after the second chemotherapy course(94.5 ± 14.4). After the fourth and fifth chemotherapy courses, the scores were much lower again, with values of 82.7 ± 13.9and 82.6 ± 13.1, respectively. The scores improved again after the sixth chemotherapy course(93.9 ± 18.7). Furthermore, each treatment course had different related symptoms that affected the QOL of the patients.Conclusions: More attention should be paid to the changing trajectory of QOL of patients in all treatment courses and to the influence of treatment-related symptoms on the QOL of patients; moreover, interventions should be adopted by medical care personnel to increase QOL in cancer patients. 展开更多

BACKGROUND Systemic lupus erythematosus(SLE)is a heterogeneous autoimmune disorder with varied clinical courses and prognoses,not only did the patients suffer from physical impairment,but also various physical and psychiatric comorbidities.Growing evidence have suggested that mental disorders in SLE patients,can lead to various adverse consequences.AIM To explored the features and influencing factors of mental health in patients with SLE and clarifying the correlations between mental health and personality characteristics and perceived social support.The results would provide a basis for psychological intervention in patients with SLE.METHODS The clinical data of 168 patients with SLE admitted at the First Affiliated Hospital of Hainan Medical University between June 2020 and June 2022 were collected.Psychological assessment and correlation analysis were conducted using the Symptom Checklist-90(SCL-90)and Perceived Social Support Scale,and the collected data were compared with the national norms in China.The relevant factors influencing mental health were identified by statistical analysis.A general information questionnaire,the Revised Life Orientation Test,and Short-Form 36-Item Health Survey were employed to assess optimism level and quality of life(QoL),respectively.RESULTS Patients with SLE obtained higher scores for the somatization,depression,anxiety,and phobic anxiety subscales than national norms(P<0.05).A correlation was identified between total social support and total SCL-90 score or each subscale(P<0.05).The factors significantly affecting patients’mental health were hormone dosage and disease activity index(DAI)(P<0.05).The average optimism score of patients with SLE was 14.36±4.42,and 30 cases were in the middle and lower levels.A positive correlation was found between optimism level and QoL scores.CONCLUSION Patients with SLE develop psychological disorders at varying degrees,which are significantly influenced by hormone dosage and DAI.Patients’mental health should be closely monitored during clinical diagnosis and treatment and provided adequate support in establishing positive,healthy thinking and behavior patterns and improving their optimism level and QoL. 展开更多

Meanwhile, it provides theoretical and practical basis for medical personnel to develop and implement individualized comprehensive intervention programs. Patients and Methods: According to the inclusion and exclusion criteria, 200 patients with cancer pain at home are selected to treat, and their quality of life conditions are observed before treatment, 1 week after treatment and 1 month after treatment, and their influencing factors are analyzed. BACKGROUND Low anterior resection syndrome(LARS)is a common complication of anuspreserving surgery in patients with colorectal cancer,which significantly affects patients'quality of life.AIM To determine the relationship between the incidence of LARS and patient quality of life after colorectal cancer surgery and to establish a LARS prediction model to allow perioperative precision nursing.METHODS We reviewed the data from patients who underwent elective radical resection for colorectal cancer at our institution from April 2013 to June 2020 and completed the LARS score questionnaire and the European Organization for Research and Treatment of Cancer Core Quality of Life and Colorectal Cancer Module questionnaires.According to the LARS score results,the patients were divided into no LARS,mild LARS,and severe LARS groups.The incidence of LARS and the effects of this condition on patient quality of life were determined.Univariate and multivariate analyses were performed to identify independent risk factors for the occurrence of LARS.Based on these factors,we established a risk prediction model for LARS and evaluated its performance.RESULTS Among the 223 patients included,51 did not develop LARS and 171 had mild or severe LARS.The following quality of life indicators showed significant differences between patients without LARS and those with mild or severe LARS:Physical,role,emotional,and cognitive function,total health status,fatigue,pain,shortness of breath,insomnia,constipation,and diarrhea.Tumor size,partial/total mesorectal excision,colostomy,preoperative radiotherapy,and neoadjuvant chemotherapy were identified to be independent risk factors for LARS.A LARS prediction model was successfully established,which demonstrated an accuracy of 0.808 for predicting the occurrence of LARS.CONCLUSION The quality of life of patients with LARS after colorectal cancer surgery is significantly reduced. 展开更多

BACKGROUNDWeight gain is a potential negative outcome of breast-cancer treatment,occurringin 50%-to-96%of breast-cancer patients,although the amount of weight gain isinconsistently reported in the literature.Research has also shown a relationshipbetween overweight/obesity and breast-cancer mortality.Correspondingly,weight management is a self-care approach known to benefit quality of life(QOL).These research questions and analysis add to existing literature byexamining participants’body mass index(BMI)trend and its relationship withQOL indicators over seven years.AIMTo examine:(1)BMI trends among breast cancer survivors;and(2)The trends’relationship to QOL indicators over seven years.METHODSDuring the Breast Cancer and Lymphedema Project,378 patients’weight andheight were recorded by nurses prior to or just after beginning breast cancertreatment and repeated at quarterly-to-semiannual intervals over seven years.Additionally,participants annually completed the 36-Item Short Form HealthSurvey(SF-36),a valid and reliable tool assessing QOL and health concepts,including physical function,pain,and emotional well-being.BMI trends,changein BMI,and change in SF-36 subscales over seven years were calculated using arandom-intercept repeated-measures regression.Patients were placed into BMIcategories at each time point:Normal,Overweight and Obese.As patients’weights changed,they were categorized accordingly.RESULTSDuring the seven-year study and while controlling for age and residence,participants gained an average of 0.3534 kg/m^2(P=0.0009).This amountremained fairly consistent across BMI categories with those in the normal-weight category(n=134)gaining 0.4546 kg/m^2(P=0.0003);Overweight(n=190)gaining 0.2985 kg/m^2(P=0.0123);and obese(n=199)gaining 0.3147 kg/m^2,(P=0.0649).Age(under or over 55)and region(metro/micro vs small/rural)weresignificantly associated with BMI increase in both the normal and obesecategories.There were statistically significant(P<0.0100)changes in five of theeight SF-36 domains;however,the directions of change were different andsomewhat divergent from that hypothesized.Controlling for age and region,these five were statistically significant,so there were no change or differencesbetween the micropolitan/metropolitan and small town/rural groups.CONCLUSIONAlthough only modest increases in mean BMI were observed,mean BMI changewas associated with selected QOL indicators,suggesting the continued need forself-care emphasis during breast cancer survivorship. 展开更多

This phase II study assessed the clinical response and short-term quality of life of patients receiving first-line chemotherapy with epirubicin-docetaxel combination for metastatic breast cancer. Thirty-one breast cancer patients were treated with epirubicin (75 mg/m2 for 15 minutes) followed one hour later by a one-hour infusion of docetaxel (75 mg/m2) q3w. EORTC QLQ-C30 and EORTC QLQ-BR23 forms were filled in at baseline, and at the second and eighth cycle of chemotherapy. The combination of epirubicin and docetaxel provided a high degree of clinical benefit. Clinical response was observed in 17 patients (55%), including five (16%) complete responses and 12 (39%) partial responses. Of responding and stable patients 23 (74%) maintained the same status for at least six months (clinical benefit). The mean survival time was 40.8 months. During the treatment the emotional functioning improved and the concerns about the future were relieved. Some aspects of quality of life were impaired, with slightly decreased physical and cognitive functioning, distress related to body image and hair loss, and adverse effects of chemotherapy. Questionnaires were conducted with the general information questionnaire, Connor-Davidson Resilience Scale (CD-RISC) and Work-Related Quality of Life Scale-2 (WRQoL-2). <strong>Results:</strong> The total average score of the quality of work life of ICU nurses was (3.35 ± 0.44), which was at a medium level. Department, professional title, frequency of night shift, experience of workplace violence and psychological resilience were the influencing factors of the quality of work life of nurses in ICU, accounting for 39.4% of the variance. <strong>Conclusion:</strong> The quality of work life of ICU nurses needs to be improved. Nursing managers should pay attention to the work environment of ICU nurses, rationally allocate manpower, and improve the status of psychological resilience of nurses, so as to improve their quality of work life. 展开更多

Objective: To investigate the subthreshold depression and the quality of life in patients preliminarily diagnosed with breast cancer, and to explore their relationship. Methods: A total of 210 patients preliminarily diagnosed breast cancer were recruited by convenience sampling method to complete the self-made general questionnaire, Center for Epidemiological Studies Depression Scale (CES-D), Hamilton Rating Scale for Depression and Functional Assessment of Cancer Therapy-Breast (FACT-B). Results: The incidence rates of subthreshold depression was 48.6% in patients preliminarily diagnosed breast cancer, the total score of FACT-B in patients with subthreshold depression and non-subthreshold depression was (73.92 &plusmn;17.62) and (86.74 &plusmn;16.15). The total score of CES-D was significantly negatively correlated to the total score and factor scores of FACT-B (p Conclusion: The incidence rate of subthreshold depression in preliminarily diagnosed breast cancer patients is high, and it is closely related to the quality of life, so we should pay attention to the psychological status of preliminarily diagnosed breast cancer patients, to increase the psychological intervention, than improve their quality of life. 展开更多

Background:Quality of life is an important indicator in patients with breast cancer.Studies here reported that the quality of life in patients with breast cancer is low and many factors contribute to this poor quality of life.Purpose:To examine the relationships among demographic characteristics,optimism,social support,illness related factors,appraisal of illness,coping strategies and the quality of life of Chinese women with breast cancer residing in Wuhan,China.Methods:A convenience sample of 156 Chinese women with breast cancer was recruited from five teaching hospitals in Wuhan,China.Participants completed the Revised Life Orientation Test,the Perceived Social Support Scale,the Symptom Distress Scale,the Appraisal of Illness Scale,the Medical Coping Modes Questionnaire,and the Functional Assessment of Cancer Therapy-Breast.Path analysis was used to examine factors influencing quality of life.Results:Significant relationships were found between optimism,symptom distress,social support,appraisal of illness,a give-in coping mode and quality of life.Optimism,social support,symptom distress,lymph node status,appraisal of illness,and a give-in coping mode accounted for 66.6%of the variance in quality of life.Conclusions:The findings of this study underscore the importance of helping women reduce symptoms distress,appraise their illness positively,use less negative coping modes,and maintain optimism,maintain good social support,because all of these factors indirectly or directly affect their quality of life. 展开更多

Purpose:To investigate the overall quality of life level and identify influencing factors in patients with traumatic brachial plexus injury.Methods:One hundred three patients with brachial plexus injury were assessed using the World Health Organization Quality of Life-BREF(WHOQOL-BREF)and Family Adaptability and Cohesion Evaluation Scale(FACESII-CV)questionnaires.Results:The overall quality of life score from patients with brachial plexus injury was 65.7815.2.The scores for the physical,psychological and environmental factors were significantly lower than the norm(Ps<0.05).Regression analysis showed that age,injury located ipsilateral to the dominant hand,upper limb function score,score of family intimacy and family income were all factors influencing the of quality of life.Conclusions:A brachial plexus injury significantly compromises an individual’s quality of life.Multiple factors influence this quality,which should be targeted to augment the physical and psychological care provided. 展开更多

Objective: The purpose of this study was to elucidate clinical factors influencing quality of life (QOL) in anorexia nervosa (AN) patients. Methods: Twenty female patients with AN (median age = 30.0 years, quartile deviation = 6.8) and forty female healthy controls (HC) (median age = 30.0 years, quartile deviation = 8.6) participated in the study. QOL was assessed with the 36-Item Short Form Health Survey (SF-36) and social support was evaluated using the Multidimensional Scale of Perceived Social Support (MSPSS). Clinical symptoms were evaluated with the Structured Interview Guide for the Hamilton Depression Rating Scale (SIGH-D) and the Eating Disorder Inventory-2 (EDI-2). Results: Scores of the SF-36 Mental Component Summary (MCS) (U = 124.0, P U = 223.0, P = 0.006) and scores of total (U = 108.0, P < 0.0001) and subscales of the MSPSS were lower in AN group than HC group, and the SIGH-D score was higher (U = 46.0, P ρ = &minus;0.565, P < 0.05) and subscales of the EDI-2 Interoceptive Confusion (ρ = &minus;0.556, P ρ = &minus;0.581, P ρ = &minus;0.617, P < 0.05) were negatively correlated to MCS, and score of Interoceptive Confusion subscale showed a negative correlation to RCS (ρ = &minus;0.672, P < 0.05). Moreover, stepwise regression analysis showed that the SIGH-D score was an independent predictor of MCS and Interoceptive Confusion score predicted RCS. Conclusion: These results suggest that among a variety of clinical symptoms and psychopathologies, depressive symptoms, poor emotional awareness and impaired sense of control are the most important influencing factors on AN patients’ QOL. 展开更多

Objective:The aim of this study was to observe the physical training influence on the limbs of postoperative breast cancer patients with adjuvant chemotherapy and do dual-response quality of life(QOL) according to clinical characteristics of patients.Methods:from February 2007 to December 2008,169 patients with postoperative breast cancer were included.We put all the patients into two groups randomly,including 80 patients in the treatment group and 89 patients in the control group.When the cycle of chemotherapy has been completed or on the 28th day postoperatively,all the patients need to be measured the function of upper limbs.The patients in the treatment group perform the rehabilitative training by our rehabilitation gymnastics,and the patients in the control group perform the rehabilitative training by themselves.When 6 cycles of adjuvant chemotherapy have been completed,we evaluated the differences of the function of upper limbs in patients.At the same time,we hand out the QOL questionnaire to patients and their doctors for doing the two-way assessment.Results:Compared with patients in control group,patients in treatment group have better in the abductive angle of shoulders and myodynamia(P = 0.000).By analysis on 156(92.3%) questionnaires,we found that the match rate of doctors and patients with physiological functions and physical symptoms were better than mentation and relationships.The better match rate between doctors and patients in QOL questionnaires suggested that the QOL of patients would be improved in the higher rate.Conclusion:Exercising the function of limbs in early could be beneficial to the patients with postoperative breast cancer and effectively improve patient's QOL.Meanwhile,we put doctors' questionnaires in the assessment of patients' QOL,and the results show that the assessment of patients' QOL is more objective and complete. 展开更多

Objective: To analyze and discuss the quality of life of elderly patients with chronic diseases and its influencing factors. The aim of this study was to provide a theoretical basis for targeted intervention of influencing factors of chronic diseases and improvement of the quality of life of patients. Methods: A total of 1000 elderly patients with chronic diseases in our health center were randomly selected as the study subject. Questionnaire survey was conducted by self-designed questionnaire. The SF-36 quality of life scale was used to evaluate their quality of life;and linear regression model was used to explore the relevant influencing factors. Result: The SF-36 score showed that there was a correlation between the six dimensions, including overall health, physiological function, somatic pain, vitality, social function and mental health, and the number of chronic diseases in elderly patients. With the increase of chronic diseases, the scores of each dimension showed a downward trend. Linear regression analysis showed that age, anxiety and poor sleep quality were the main factors affecting the quality of life in elderly patients with chronic diseases. Conclusion: The quality of life of elderly patients with chronic diseases in our city is poor. In community work, targeted nursing and intervention should be given according to the types of diseases suffered by the elderly. 展开更多

Research on Influence of Clopidogrel on Life Quality in Patients with Transient Ischemic Attack and its Influencing Factors

Purpose:To analyze the basic situation and health related quality of life(HRQOL)results of patients with transient ischemic attack(TIA),and to study the influencing factors of health related quality of life in patients receiving clopidogrel.Method:Divide the TIA patients into clopidogrel group,aspirin group and untreated group.Measure HRQOL scores of TIA patients using the short form 36 questionnaire(SF-36)scale and analyze influencing factors of HRQOL with one-way ANOVA and multivariate stepwise linear regression statistical methods.Results:The differences of HRQOL scores among three groups are of statistical significance(F=4.29,P=0.00).There is no difference in HRQOL score between clopidogrel group and aspirin group(t=5.35,P=0.00),but HRQOL scores of clopidogrel group and aspirin group are higher than those of the untreated group(t=6.14,P=0.00;t=5.16,P=0.00).The HRQOL scores of clopidogrel group are positively correlated with diet,exercise,gender and family harmony(P<0.05),but negatively correlated with diabetes,hypertension,smoking,drinking,hyperlipidemia,age,career and ABCD2 score(P<0.05).Conclusion:TIA patients who received secondary prevention with clopidogrel and aspirin show better life quality results than those who did not.There were many factors influencing clopidogrel's treatment effect.Paying attention to middle-aged and elderly,the obese,mental workers and female TIA patients,teach TIA patients to quit smoking,drinking and eating low-salt and low-fat food,actively treating their hypertension,hyperlipidemia and diabetes and laying emphasis on psychological counseling and exercise can significantly improve the treatment effect of clopidogrel. 展开更多
