目的探讨外周血CD34阳性(CD34^(+))细胞计数对普乐沙福自体干细胞动员效果的预测价值。方法回顾性分析2021年5月—2023年7月中山大学附属第七医院使用人粒细胞集落刺激因子(G-CSF)联合普乐沙福进行自体干细胞动员的13例患者临床资料,分... 目的探讨外周血CD34阳性(CD34^(+))细胞计数对普乐沙福自体干细胞动员效果的预测价值。方法回顾性分析2021年5月—2023年7月中山大学附属第七医院使用人粒细胞集落刺激因子(G-CSF)联合普乐沙福进行自体干细胞动员的13例患者临床资料,分析普乐沙福动员前后外周血CD34^(+)细胞计数的变化及干细胞采集情况。结果共有13例患者纳入研究,包括淋巴瘤10例和多发性骨髓瘤3例。多发性骨髓瘤患者中1例为新诊断,另2例为复发患者;淋巴瘤患者中3例为套细胞淋巴瘤,6例为弥漫大B细胞淋巴瘤(包括1例复发),1例为B细胞淋巴瘤(不能明确类型)。本研究纳入的患者均使用G-CSF动员,在使用普乐沙福后CD34^(+)细胞计数均升高,使用普乐沙福前中位CD34^(+)细胞计数为13.3(2.5~76.1)/μL,使用普乐沙福后中位CD34^(+)细胞计数为73.6(10.4~208.70)/μL,升高4.18(1.99~13.60)倍。13例患者中有2例患者在使用普乐沙福前外周血CD34^(+)细胞计数<5/μL,均动员失败。Spearman相关分析结果显示,使用普乐沙福后CD34^(+)细胞计数与使用普乐沙福前CD34^(+)细胞数呈正相关(rs=0.769,P=0.003)。多元线性回归分析显示,使用普乐沙福后CD34^(+)细胞计数能较好地预测采集结果(P=0.004)。结论监测外周血CD34^(+)细胞计数可预测普乐沙福自体干细胞动员效果,使用普乐沙福后CD34^(+)细胞计数越多,CD34^(+)细胞采集量越大。 展开更多

Objective: To explore the effect of homoharringtonine (HHT) on bone morrow CD34^+CD7^+ cells in chronic granulocytic leukemia (CGL). Methods: The changes of bone morrow CD34^+CD7^+ cells were observed after the treatment of HHT in 23 cases with CGL. The proliferation and apoptosis of CD34^+CD7^+ cells treated with HHT in vitro were studied. Results: The proportion of CD34^+CD7^+ cells in CGL (0.145±0.021) was higher than that of normal control (0.052±0.013). The proportion of CD34^+CD7^+ cells in patients who got cytogenetic responses to HHT (0.072±0.020) decreased remarkably, but not in those patients who did not got cytogenetic responses to HHT, (0.137±0.023). the proliferation of CD34^+ cells was inhibited and the proportion of CD34^+CD7^+ cells decreased after cultured with HHT (0.134 in 24 h, 0.126 in 48 h and 0.102 in 72). The apoptosis rate of CD34^+CD7^+ cells was higher than that in CD34^+CDT cells (35.39%±4.39% versus 24.57%±4.01%, P〈0.05) 72 h after culture with HHT. Conclusion: The proportion of CD34^+CD7^+ cells in CGL was higher than that of normal control and HHT may inhibit the proliferation and induce apoptosis of bone marrow CD34^+CD7^+ cells. 展开更多

目的应用E6流式细胞仪建立检测CD34^+细胞百分比和绝对计数的方法并进行验证。方法收集2019年1月至2019年12月在深圳市儿童医院血液肿瘤科因重型β-地中海贫血行异基因造血干细胞移植的患儿89例,以移植患儿供的骨髓或外周血为研究对象,... 目的应用E6流式细胞仪建立检测CD34^+细胞百分比和绝对计数的方法并进行验证。方法收集2019年1月至2019年12月在深圳市儿童医院血液肿瘤科因重型β-地中海贫血行异基因造血干细胞移植的患儿89例,以移植患儿供的骨髓或外周血为研究对象,应用E6流式细胞仪建立ISHAGE法检测CD34^+细胞百分比,应用"流式直接体积法"计算CD34^+细胞的绝对计数,并结合白细胞手工计数对CD34^+细胞绝对计数的结果进行验证。结果结合白细胞手工计数对CD34^+细胞的绝对计数结果的正确性进行验证,Pearson相关分析结果显示:供者外周血干细胞或骨髓干细胞CD34^+细胞绝对计数流式结果与CD34^+细胞绝对计数计算结果高度相关(r=0.93,R2=0.87,P<0.0001);两次供者外周血干细胞或骨髓干细胞白细胞计数和单个核百分比结果高度一致;同样,两次供者外周血干细胞或骨髓干细胞CD34^+细胞流式计数百分比和绝对计数结果亦高度一致。结论应用E6流式细胞仪的"流式直接体积法"计算CD34^+细胞绝对计数结果准确,可以作为造血干细胞移植术前计算供者外周血干细胞或骨髓干细胞CD34^+细胞数目的可靠依据。 展开更多

Exploring hematopoietic stem cell population in human milk and its benefits for infants:A scoping review

Objective:To comprehensively explore hematopoietic stem cells(HSCs)in human milk,understanding their molecular markers,isolation methods,benefits for infants,and potential medical applications.Methods:We conducted a scoping literature review following the PRISMA-ScR guidelines.This review included studies investigating HSCs in human milk,utilizing molecular markers such as CD34^(+),CD113^(+),and CD117^(+)for characterization.Both in vitro and in vivo studies exploring the morphology,function,and clinical implications of these cells were considered.The diverse range of papers reviewed were indexed in PubMed,Science Direct,Scopus,Sage Journals,and Google Scholar,published between 2010 and 2023.Results:This scoping review explored 577 articles and selected 13 studies based on our inclusion criteria,focusing on HSCs in human milk.Most studies dilute samples prior to HSC isolation,followed by detection using markers such as CD34^(+),CD113^(+),and CD117^(+),with flow cytometry serving as the primary analysis tool,focusing on their isolation and detection methods.While no definitive benefits have been conclusively established,there is a strong belief in the potential of HSCs to positively impact infant immunity,growth,and tissue repair.Conclusions:This review presents significant evidence supporting the presence of HSCs in human milk,identified by markers such as CD34^(+),CD113^(+),and CD117^(+).These cells show considerable potential in enhancing infant health,including immunity,tissue repair,cognitive development,and gastrointestinal health.Despite methodological variations in isolation and detection techniques,the collective findings underscore the potential clinical relevance of HSCs in human milk.Moreover,this review highlights the noninvasive accessibility of human milk as a source of HSCs and emphasizes the need for further research to unlock their therapeutic potential. 展开更多

They have widespread effects on asthmatic airway inflammation. However, little is known about the effects of corticosteroids on the production of bone marrow inflammatory cells in asthma. This study observed the effects of glucocorticoid and cysteinyl leukotriene 1 receptor antagonist on CD34+ hematopoietic cells, so as to explore the possible effectiveness of a bone marrow-targeted anti-inflammatory strategy. METHODS: Balb/c mice were sensitized and challenged with ovalbumin (OVA) to establish an asthmatic model. For two consecutive weeks, asthmatic mice were challenged with OVA while being given either prednisone, montelukast, prednisone plus montelukast, or sterile saline solution. The mice were killed 24 hours after the last challenge with OVA, and bronchoalveolar lavage fluid (BALF), peripheral blood, and bone marrow were collected. Eosinophils in peripheral blood and BALF, and nucleated cells in BALF, peripheral blood, and bone marrow were counted. The percentages of CD34+ cells, CD4+ T lymphocytes and CD8+ T lymphocytes among nucleated cells in peripheral blood and bone marrow were counted by flow cytometry. Immunocytochemistry and in situ hybridization were employed to detect expression of CD34 and interleukin (IL)-5Ralpha mRNA (CD34+ IL-5Ralpha mRNA+ cells) among bone marrow hematopoietic cells. Background CD4^+T cell counts have been used as the indicator of human immunodeficiency virus type 1 (HIV-1) disease progression and thereby to determine when to start highly active antiretroviral therapy (HAART). Whether and how the baseline CD4^+T cell count affects the immunological and viral responses or adverse reactions to nevirapine (NVP)-containing HAART in Chinese HIV-1 infected adults remain to be characterized. Methods One hundred and ninety-eight HIV-seropositive antiretroviral therapy (ART)-naive subjects were enrolled into a prospective study from 2005 to 2007. Data were analyzed by groups based on baseline CD4^+T cell counts either between 100-200 cells/μl or 201-350 cells/μl. Viral responses, immunologic responses and adverse events were monitored at baseline and at weeks 4, 12, 24, 36, 52, 68, 84, 100. Results Eighty-six and 112 subjects ranged their CD4^+T cell counts 100-200 cells/μl and 201-350 cells/μl, respectively. The pre-HAART viral load in CD4 201-350 cells/μl group was significantly lower than that in CD4 100-200 cells/μl group (P=0.000). After treatment, no significant differences were observed between these two groups either in the plasma viral load (pVL) or in the viral response rate calculated as the percentage of pVL less than 50 copies/ml or less than 400 copies/ml. The CD4^+T cell counts were statistically higher in the 201-350 group during the entire follow-ups (P 〈0.01) though CD4^+ T cell count increases were similar in these two groups. After 100-week treatment, the median of CD4^+ T cell counts were increased to 331 cells/μl for CD4 100-200 cells/μl group and to 462 cells/μl for CD4 201-350 cells/μl group. Only a slightly higher incidence of nausea was observed in CD4 201-350 cells/μl group (P=0.05) among all adverse reactions, including rash and liver function abnormality. Conclusions The pVLs and viral response rates are unlikely to be associated with the baseline CD4^+T cell counts. Initiating HAART in Chinese HIV-1 infected patients with higher baseline CD4^+T cell counts could result in higher total CD4^+T cell counts thereby achieve a better immune recovery. These results support current guidelines to start HAART at a threshold of 350 cells/μl. 展开更多

Summary: The characteristics for the ex vivo expansion of the endothelial progenitor cells (EPCs) were explored, CD34^+ cells were selected from umbilical cord blood mononuclear cells (MNC) by MiniMACS system, expanded under the same conditions as those for total MNC, coincubation of CD34^+ and CD34 from the same donor for EPCs. In addition, the effects of vessel endothelial growth factor (VEGF) and passage on cell differentiation, expansion kinetics and apoptosis were examined, EPCs were determined and quantified by immunocytochemistry and flow cytometry, The results showed that both coculture of CD346+ and CD34^- and total MNC led to a significant increase in the expansion of CD34^+ cells as compared with CD34 enrichment (P〈0.05). There was a tendency toward decreased apoptosis in cultures when early passage was performed immediately after cord like structures appeared. VEGF had no significant effect on apoptosis (P〉0.05), These differentiated EPCs were positive for CD34^+, von Willebrand factor (vWF), KDR, CD31 staining and phagocytized acetylated low-density lipoprotein (LDL). CD34^+ cells accounted for (68.2±6,3) % of attaching (AT) cells at day 7 of culture. It was suggested the most efficient method to ex vivo expansion of EPCs was coculture of CD34^+ and CD34^- or total MNC. Early passage makes cell apoptosis rate decrease. VEGF had no significant effect on ex vivo expansion of EPCs. 展开更多

目的探讨移植物CD34^+细胞数对自体外周血造血干细胞移植(APBSCT)后非霍奇金淋巴瘤(NHL)患者造血重建及预后的影响。方法回顾性分析2010年5月至2016年5月于山西省肿瘤医院行APBSCT的60例NHL患者相关资料,其中B-NHL 32例,T-NHL 28例。患... 目的探讨移植物CD34^+细胞数对自体外周血造血干细胞移植(APBSCT)后非霍奇金淋巴瘤(NHL)患者造血重建及预后的影响。方法回顾性分析2010年5月至2016年5月于山西省肿瘤医院行APBSCT的60例NHL患者相关资料,其中B-NHL 32例,T-NHL 28例。患者依据受试者工作特征曲线(ROC)所得CD34+细胞数阈值分组,分析移植后造血重建情况;分析移植物CD34+细胞数与预后的关系;采用单因素及多因素分析法分析预后影响因素。结果根据ROC确定CD34+细胞数阈值为4.35×10^6/kg。CD34+细胞数≥4.35×10^6/kg组(20例)和CD34+细胞数<4.35×10^6/kg组(40例)粒细胞植活时间分别为(9.9±1.2)d、(12.5±3.7)d(P=0.031),血小板植活时间分别为(9.4±1.7)d、(13.8±2.9)d(P=0.012),3年总生存(OS)率分别为85.0%、55.0%(P=0.024),3年无进展生存(PFS)率分别为85.0%、57.5%(P=0.016)。B-NHL患者中CD34^+细胞数≥4.35×10^6/kg组(11例)和CD34^+细胞数<4.35×106/kg组(21例)3年PFS率分别为81.8%和42.9%(P=0.037)。T-NHL患者中CD34+细胞数≥4.35×10^6/kg组(9例)与CD34+细胞数<4.35×10^6/kg组(19例)3年OS率分别为77.8%和36.8%(P=0.049)。单因素生存分析结果显示,年龄>60岁、Ann Arbor分期Ⅲ~Ⅳ期、国际预后指数(IPI)评分>2分、移植物CD34+细胞数<4.35×10^6/kg均为影响患者PFS和OS的危险因素(均P<0.05)。多因素分析结果显示,IPI评分和移植物CD34+细胞数为影响PFS的独立因素(RR=0.333,95%CI 0.112~0.994,P=0.049;RR=0.190,95%CI 0.047~0.773,P=0.020);IPI评分是影响OS的独立因素(RR=0.095,95%CI 0.011~0.837,P=0.034)。结论移植物中CD34+细胞数影响NHL患者APBSCT后的造血重建时间及成分血输注,对患者预后具有一定预测价值。 展开更多

目的调查临床实验室开展CD34^(+)细胞计数的检测现状,分析存在的问题,为制定质量改进方案提供依据。方法以参加CD34^(+)细胞计数全国室间质量评价活动的101家实验室为调查对象,通过问卷调查和质评物检测,收集相关检测方法学信息及质评... 目的调查临床实验室开展CD34^(+)细胞计数的检测现状,分析存在的问题,为制定质量改进方案提供依据。方法以参加CD34^(+)细胞计数全国室间质量评价活动的101家实验室为调查对象,通过问卷调查和质评物检测,收集相关检测方法学信息及质评物检测结果。参考国内外指南文件确定CD34^(+)细胞计数的质量控制要求,对调查实验室的遵循情况进行分析。分组统计质评物检测结果,并与美国病理家学会(CAP)质评数据进行比较。结果共收到97家实验室回报的调查问卷信息,99家实验室回报的质评物检测结果。问卷调查数据显示实验室对设门方案、移液方式和获取细胞数等关键质量控制要求的遵循比率较高,分别为92.8%、83.9%和82.5%;而对采用全血质控品开展室内质量控制、溶血素的选择、处理样品是否洗涤、是否报告绝对计数以及仪器的质量控制等要求的遵循比率较低,分别为5.2%、28.9%、39.2%、46.4%和55.7%。质评物检测结果显示,国内实验室CD34^(+)相对计数的变异系数与CAP质评数据相近,但绝对计数的变异系数大于CAP质评数据。结论临床实验室对部分质量控制要求的遵从性较差,绝对计数结果的实验室间差异较大,建议实验室加强CD34^(+)细胞计数质量控制相关培训,特别是绝对计数方法和技术要求的培训。 展开更多

OriginalTranslation t(8;21)(q22;q22)acute myeloid leukemia(AML)is a highly heterogeneous hematological malignancy with a high relapse rate in China.Two leukemic myeloblast populations(CD34^(+)CD117^(dim) and CD34^(+)CD117^(bri))were previously identified in t(8;21)AML,and CD34^(+)CD117^(dim) cell proportion was determined as an independent factor for this disease outcome.Here,we examined the impact of CD34^(+)CD117^(dim)/CD34^(+)CD117^(bri) myeloblast-associated gene expression on t(8;21)AML clinical prognosis.In this study,85 patients with t(8;21)AML were enrolled.The mRNA expression levels of CD34^(+)CD117^(dim)-associated genes(LGALS1,EMP3,and CRIP1)and CD34^(+)CD117^(bri)-associated genes(TRH,PLAC8,and IGLL1)were measured using quantitative reverse transcription PCR.Associations between gene expression and clinical outcomes were determined using Cox regression models.Results showed that patients with high LGALS1,EMP3,or CRIP1 expression had significantly inferior overall survival(OS),whereas those with high TRH or PLAC8 expression showed relatively favorable prognosis.Univariate analysis revealed that CD19,CD34^(+)CD117^(dim) proportion,KIT mutation,minimal residual disease(MRD),and expression levels of LGALS1,EMP3,CRIP1,TRH and PLAC8 were associated with OS.Multivariate analysis indicated that KIT mutation,MRD and CRIP1 and TRH expression levels were independent prognostic variables for OS.Identifying the clinical relevance of CD34^(+)CD117^(dim)/CD34^(+)CD117^(bri) myeloblast-associated gene expression may provide new clinically prognostic markers for t(8;21)AML. 展开更多

本研究旨在观察不同动员方法对健康供者外周血造血干细胞的动员效果、采集过程中的不良反应及移植后受者造血功能恢复的影响。2008年1月-2013年5月期间本院43例异基因造血干细胞移植供者分为单纯动员和联合动员两组。单纯动员组采用粒... 本研究旨在观察不同动员方法对健康供者外周血造血干细胞的动员效果、采集过程中的不良反应及移植后受者造血功能恢复的影响。2008年1月-2013年5月期间本院43例异基因造血干细胞移植供者分为单纯动员和联合动员两组。单纯动员组采用粒细胞集落刺激因子5-10μg/(kg·d)皮下注射,动员4-6天开始采集;联合动员组在单纯动员基础上于采集前2-4 h给予静脉滴注地塞米松10 mg。观察不同组采集的MNC、CD34+细胞数及其与采集前外周血MNC数的关系,观察采集过程中的不良反应和回输不同组供者造血干细胞后受者造血重建情况。结果表明:两组供者采集造血干细胞数均满足移植需要,单纯动员组采集的MNC及CD34+细胞数均高于联合动员组。两组采集物中MNC与采集前外周血MNC计数均呈正相关;联合动员组采集后血红蛋白及血小板下降幅度较单纯动员组明显。单纯动员组采集过程中不良反应轻微,可以耐受及逆转,联合动员组未出现不良反应。在两组患者预处理方案无统计学差异的情况下,联合动员组相应的受者造血重建时间较单纯动员组明显缩短。结论:在G-CSF动员供体外周血干细胞时加用地塞米松,可以减少外周血造血干细胞采集的不良反应,可采集到足够的造血干细胞数,采集前外周血中MNC计数仍可以作为评估采集物中MNC高低的一项参考指标,特别是联合地塞米松动员干细胞对于受者造血重建有积极意义。 展开更多

目的探讨骨髓增生异常综合征(MDS)患者骨髓细胞凋亡累及的细胞类型,以及其与肿瘤坏死因子相关凋亡诱导配体(TRAIL)表达水平的关系。方法用流式细胞术检测骨髓有核细胞总凋亡率、CD3+、CD3+4细胞凋亡率和TRAIL表达水平,运用CellQuest及Mo... 目的探讨骨髓增生异常综合征(MDS)患者骨髓细胞凋亡累及的细胞类型,以及其与肿瘤坏死因子相关凋亡诱导配体(TRAIL)表达水平的关系。方法用流式细胞术检测骨髓有核细胞总凋亡率、CD3+、CD3+4细胞凋亡率和TRAIL表达水平,运用CellQuest及ModFit软件对所测得结果进行数据分析。结果MDS患者骨髓有核细胞总凋亡率为(17.39±10.17)%,CD+3、CD3+4细胞凋亡率分别为(20.41±11.61)%、(21.32±12.17)%,TRAIL表达水平为(7.55±3.10)%,均显著高于正常对照组[(5.88±1.38)%、(5.47±1.65)%、(4.92±2.11)%、(1.26±0.46%)],P<0.05。早期MDS有核细胞总凋亡率为(21.95±9.44)%、TRAIL表达水平为(9.00±2.47)%,均高于进展期[(8.26±2.27)%、(4.65±2.04)%],P<0.05。骨髓有核细胞总凋亡率与骨髓原始细胞数呈负相关(r=-0.65,P<0.05),与TRAIL表达水平呈正相关(r=0.85,P<0.05)。结论MDS存在过度凋亡,凋亡累及早期造血细胞和淋巴细胞;TRAIL可能是引起凋亡增加的原因之一。 展开更多

目的探讨多发性骨髓瘤(MM)患者外周血CD34阳性细胞计数对普乐沙福干细胞动员效果的预测价值。方法回顾性分析2019年12月至2021年2月广西医科大学第一附属医院使用普乐沙福进行干细胞动员的12例MM患者临床资料。分析普乐沙福动员前后外周... 目的探讨多发性骨髓瘤(MM)患者外周血CD34阳性细胞计数对普乐沙福干细胞动员效果的预测价值。方法回顾性分析2019年12月至2021年2月广西医科大学第一附属医院使用普乐沙福进行干细胞动员的12例MM患者临床资料。分析普乐沙福动员前后外周血CD34阳性细胞计数的变化及干细胞采集情况。结果共有12例患者纳入研究,国际分期系统(ISS)分期Ⅱ~Ⅲ期,诱导方案以VRD为主。不论在普乐沙福前用何种方案进行干细胞动员,患者在使用普乐沙福后CD34阳性细胞计数均升高,使用普乐沙福前后中位CD34阳性细胞计数分别为3.63/μl(0.72~13.53/μl)和32.11/μl(8.52~53.68/μl),差异有统计学意义(Z=-0.40,P<0.001),中位升高倍数为11.50倍(1.61~23.71倍)。12例患者中有1例动员失败,该患者在使用普乐沙福前外周血CD34阳性细胞计数<1/μl,使用普乐沙福后CD34阳性细胞计数虽然提高了11.83倍,但仍<10/μl。Pearson相关分析结果显示,在使用普乐沙福前CD34阳性细胞计数<4/μl的患者中,使用普乐沙福前后CD34阳性细胞计数呈正相关(r=0.80,P=0.032)。结论MM患者外周血CD34阳性细胞计数对普乐沙福干细胞动员效果有一定的预测价值。 展开更多

目的探讨深度脐带消毒方案与常规消毒方案对脐带血采集后的污染率和脐带血质量的影响。方法将2019年5-12月在南方医科大学附属何贤纪念医院分娩的孕妇400例随机分成两组,试验组200例采用深度脐带消毒方案消毒后采集脐带血,对照组200例... 目的探讨深度脐带消毒方案与常规消毒方案对脐带血采集后的污染率和脐带血质量的影响。方法将2019年5-12月在南方医科大学附属何贤纪念医院分娩的孕妇400例随机分成两组,试验组200例采用深度脐带消毒方案消毒后采集脐带血,对照组200例采用常规消毒方案消毒后采集脐带血,采集后的脐带血运送至广东省脐带血库进行细菌/霉菌污染率和脐带血质量的检测。结果两组产妇的年龄、分娩结局和新生儿体质量比较差异无统计学意义(P>0.05)。常规消毒对照组的脐带血细菌/霉菌污染率为2.5%,深度脐带消毒试验组的脐带血细菌/霉菌污染率为1.0%,试验组脐带血采集后的污染率更低。试验组与对照组的脐带血冻前有核细胞计数、CD34^(+)细胞含量、冻前有核细胞活性和粒细胞及巨噬细胞集落形成单位(CFUGM)细胞计数等比较,差异均无统计学意义(P>0.05)。结论深度脐带血消毒方案可以降低脐带血采集后的污染率且不影响脐带血质量,值得脐带血采集领域的关注。 展开更多
