In patients with NAFLD, especially those with non-alcoholic steatohepatitis(NASH), most often have one or more components of the metabolic syndrome, but this is not universal. Although most patients with NAFLD share many clinical features, only a subset of patients develops significant liver inflammation and progressive fibrosis. On the other hand, not all patients with NASH exhibit insulin resistance. NASH can be seen in patients who are lean and have no identifiable risk factors. Many clinical studies have tried numerous drugs and alternative medicine, however, investigators have failed to identify a safe and effective therapy for patients with NASH. As summarized, the heterogeneity of pathogenic pathways in individual patients with NASH may warrant the development of an individualized treatment according to the underlying pathogenic pathway. The differentiation of pathogenetic targets may require the development of diagnostic and prognostic biomarkers, and the identification of genetic susceptibilities. At present, evidence-based medicine provides only a few options including life-style modifications targeting weight loss, pioglitazone and vitamin E in non-diabetic patients with biopsy-proven NASH.展开更多

The simulation results show that a sudden expansion of cross section has remarkable mixing intensification effect within a limited period(on the sub-second scale) or tube-length(on the millimeter scale), corresponding to the generation of considerable local vortices determined by both the flow capacity and the ratio of cross section change; a sudden contraction of cross section has instantaneous but weak mixing intensification effect; through introducing a local expansion structure with proper length, as the combination of sudden expansion and sudden contraction, their mixing intensification effects could be superposed. Besides, the rationality and importance are experimentally verified to explore the time profile of mixing intensity and carry out the vortex analysis by simulation for enhancing the selectivity of a complicated reaction system. These progresses may lead to more meaningful quantitative description of mixing process in a flow microreactor for some specific chemical processes.展开更多

The largest changes in meteorology, ozone and its precursors are found in the 21st century, however, also significant changes are found in the 22nd century. At surface level the ozone concentration is projected to increase due to climate change in the areas where substantial amounts of ozone precursors are emitted. Elsewhere a significant decrease is projected at the surface. In the free troposphere a general increase is found in the entire Northern Hemisphere except in the tropics, where the ozone concentration is decreasing. In the Arctic the ozone concentration will increase in the entire air column, which most likely is due to changes in atmospheric transport. Changes in temperature, humidity and the naturally emitted Volatile Organic Compounds (VOCs) are governing the changes in ozone both in the past, present and future century.展开更多

文摘BACKGROUND Dyslipidemia was strongly linked to stroke, however the relationship between dyslipidemia and its components and ischemic stroke remained unexplained.AIM To investigate the link between longitudinal changes in lipid profiles and dyslipidemia and ischemic stroke in a hypertensive population.METHODS Between 2013 and 2014,6094 hypertension individuals were included in this, and ischemic stroke cases were documented to the end of 2018.Longitudinal changes of lipid were stratified into four groups:(1)Normal was transformed into normal group;(2)Abnormal was transformed into normal group;(3)Normal was transformed into abnormal group;and(4)Abnormal was transformed into abnormal group. To examine the link between longitudinal changes in dyslipidemia along with its components and the risk of ischemic stroke,we utilized multivariate Cox proportional hazards models with hazard ratio(HR)and 95%CI.RESULTS The average age of the participants was 62.32 years±13.00 years, with 329 women making up 54.0%of the sample.Over the course of a mean follow-up of 4.8 years,143 ischemic strokes happened. When normal was transformed into normal group was used as a reference, after full adjustments, the HR for dyslipidemia and ischemic stroke among abnormal was transformed into normal group, normal was transformed into abnormal group and abnormal was transformed into abnormal Wei CC et al.Dyslipidemia changed and ischemic stroke WJCC https://www.wjgnet.com 2 February 6,2025 Volume 13 Issue 4 group were 1.089(95%CI:0.598-1.982;P=0.779),2.369(95%CI:1.424-3.941;P<0.001)and 1.448(95%CI:1.002-2.298;P=0.047)(P for trend was 0.233),respectively.CONCLUSION In individuals with hypertension, longitudinal shifts from normal to abnormal in dyslipidemia-particularly in total and low-density lipoprotein cholesterol-were significantly associated with the risk of ischemic stroke.
文摘BACKGROUND Atypical optic neuritis, consisting of neuromyelitis optica spectrum disorders(NMOSD)or myelin oligodendrocyte glycoprotein antibody disease(MOGAD),has a very similar presentation but different prognostic implications and longterm management strategies.Vascular and metabolic factors are being thought to play a role in such autoimmune neuro-inflammatory disorders, apart from the obvious immune mediated damage. With the advent of optical coherence tomography angiography(OCTA),it is easy to pick up on these subclinical macular microvascular and structural changes.AIM To study the macular microvascular and structural changes on OCTA in atypical optic neuritis.METHODS This observational cross-sectional study involved 8 NMOSD and 17 MOGAD patients, diagnosed serologically,as well as 10 healthy controls.Macular vascular density(MVD)and ganglion cell+inner plexiform layer thickness(GCIPL)were studied using OCTA.RESULTS There was a significant reduction in MVD in NMOSD and MOGAD affected as well as unaffected eyes when compared with healthy controls.NMOSD and MOGAD affected eyes had significant GCIPL thinning compared with healthy controls.NMOSD unaffected eyes did not show significant GCIPL thinning compared to healthy controls in contrast to MOGAD unaffected eyes.On comparing NMOSD with MOGAD,there was no significant difference in terms of MVD or GCIPL in the affected or unaffected eyes.CONCLUSION Although significant microvascular and structural changes are present on OCTA between atypical optic neuritis and normal patients, they could not help in differentiating between NMOSD and MOGAD cases.
文摘The underlying molecular changes that result in minimal change disease(ne-phrotic syndrome)require an in-depth analysis.Current molecular studies have shown the involvement of zinc fingers and homeobox transcriptional factors in its pathogenesis. The application of therapeutic drugs relies on understanding the cascade of molecular events to determine their efficacy in managing the clinical condition.

文摘Glacier dynamics in the Himalayan midlatitudes, particularly in regions like the Shishapangma, are not yet fully understood, especially the localized topographic and climatic impacts on glacier deformation. This study analyzes the spatiotemporal characteristics of glacier surface deformation in the Shishapangma region using the Small Baseline Subset(SBAS)Interferometric Synthetic Aperture Radar(In SAR)technique. The analysis reveals an average deformation rate of-4.02±17.65 mm/yr across the entire study area, with glacier regions exhibiting significantly higher rates of uplift(16.87±13.20 mm/yr)and subsidence(20.11±14.55 mm/yr)compared to non-glacier areas. It identifies significant surface lowering on the mountain flanks and localized uplift in certain catchments, emphasizing the higher deformation rates in glacial areas compared to non-glacial ones.We found a strong positive correlation between temperature and cumulative deformation(correlation coefficient of 0.63),particularly in glacier areas(0.82).The research highlights the role of temperature as the primary driver of glacier wastage, particularly at lower elevations, with strong correlations found between temperature and cumulative deformation. It also indicates the complex interactions between topographic features, notably, slope gradient, which shows a positive correlation with subsidence rates, especially for slopes below 35°.South-,southwest-,and west-facing slopes exhibit significant uplift, while north-,northeast-,and east-facing slopes predominantly subside.Additionally,we identified transition zones between debris-covered glaciers and clean ice as areas of most intense deformation, with average rates exceeding 30 mm/yr,highlighting these as potential high-risk zones for geohazards. This study comprehensively analyzes the deformation characteristics in both glacier and non-glacier areas in the Shishapangma region, revealing the complex interplay of topographic, climatic, and hydrological factors influencing glacier dynamics.

文摘Air temperature is an important indicator to analyze climate change in mountainous areas.ERA5 reanalysis air temperature data are important products that were widely used to analyze temperature change in mountainous areas. However, the reliability of ERA5 reanalysis air temperature over the Qilian Mountains(QLM)is unclear. In this study,we evaluated the reliability of ERA5 monthly averaged reanalysis 2 m air temperature data using the observations at 17 meteorological stations in the QLM from 1979 to 2017.The results showed that:ERA5 reanalysis monthly averaged air temperature data have a good applicability in the QLM in general(R2=0.99).ERA5 reanalysis temperature data overestimated the observed temperature in the QLM in general.Root mean square error(RMSE)increases with the increasing of elevation range, showing that the reliability of ERA5 reanalysis temperature data is worse in higher elevation than that in lower altitude.ERA5 reanalysis temperature can capture observational warming rates well. All the smallest warming rates of observational temperature and ERA5 reanalysis temperature are found in winter, with the warming rates of 0.393°C/10a and 0.360°C/10a,respectively.This study will provide a reference for the application of ERA5 reanalysis monthly averaged air temperature data at different elevation ranges in the Qilian Mountains.

文摘Due to irrational human activities and extreme climate, the Qinghai-Xizang Plateau,China, faces a serious threat of desertification.Desertification has a detrimental effect on the ecological environment and socioeconomic development. In this study, the desertification sensitivity index(DSI)model was established by integrating the spatial distance model and environmentally sensitive area index evaluation method, and then the model was used to quantitatively analyze the spatial and temporal characteristics of desertification sensitivity of the Qinghai-Xizang Plateau from 1990 to 2020.The results revealed that:(1)a general increasing tendency from southeast to northwest was identified in the spatial distribution of desertification sensitivity. The low-sensitivity areas were mostly concentrated in the Hengduan and Nyaingqêntanglha mountains and surrounding forest and meadow areas. The high-sensitivity areas were located mainly in the Kunlun and Altun mountains and surrounding decertified areas. The center of gravity of all types of desertification-sensitive areas moved to the northwest, and the desertification sensitivity showed a decreasing trend as a whole;(2)the area of highly sensitive desertification areas decreased by 8.37%,with extreme sensitivity being the largest change among the sensitivity types. The desertification sensitivity transfer was characterized by a greater shift to lower sensitivity levels(24.56%)than to higher levels(2.03%),which demonstrated a declining trend;(3)since 1990,the change in desertification sensitivity has been dominated by the stabilizing type Ⅰ(29.30%),with the area of continuously increasing desertification sensitivity accounting for only 1.10%,indicating that the management of desertification has achieved positive results in recent years;and(4)natural factors have had a more significant impact on desertification sensitivity on the Xizang Plateau, whereas socioeconomic factors affected only localized areas. The main factors influencing desertification sensitivity were vegetation drought tolerance and aridity index.Studying spatiotemporal variations in desertification sensitivity and its influencing factors can provide a scientific foundation for developing strategies to control desertification on the Qinghai-Xizang Plateau.Dividing different desertification-sensitive areas on the basis of these patterns of change can facilitate the formulation of more targeted management and protection measures, contributing to ecological construction and sustainable economic development in the area.

文摘Studying the spatiotemporal variation and driving mechanisms of vegetation net primary productivity(NPP)in the Guanzhong Plain Urban Agglomeration(GPUA)of China is highly important for regional green and low-carbon development. This study used the Theil-Sen trend analysis,Mann-Kendall trend test, coefficient of variation,Hurst index, and machine learning method(eXtreme Gradient Boosting and SHapley Additive exPlanations(XGBoost-SHAP))to analyze the spatiotemporal variation of NPP in the GPUA from 2001 to 2020 and reveal its response to climate change and human activities. The results found that during 2001-2020,the averageNPP in the GPUA showed a significant upward trend, with an annual growth rate of 10.84 g C/(m^(2)•a).The multi-year average NPP in the GPUA was 484.83 g C/(m^(2)•a),with higher values in the southwestern Qinling Mountains and lower values in the central and northeastern cropland and built-up areas. The average coefficient of variation of NPP in the GPUA was 0.14,indicating a relatively stable state overall, but 72.72%of the study area showed weak anti-persistence,suggesting that NPP in most areas may have declined in the short term.According to XGBoost-SHAP analyses, elevation, land use type and precipitation were identified as the main driving factors of NPP.Appropriate precipitation and higher temperatures promote NPP growth, whereas extreme climates, high population density, and nighttime lighting inhibit NPP.This study has important theoretical and practical significance for achieving regional sustainable development, offers a scientific basis for formulating effective ecological protection and restoration strategies, and promotes green, coordinated, and sustainable development in the GPUA.
