Objective:To study the effect of Traditional Chinese medicine(TCM)nursing intervention combined with acupoint patch in the treatment of acute exacerbation of chronic obstructive pulmonary disease(COPD).Methods:A total of 60 patients with acute exacerbation of COPD admitted between September 2022 and September 2023 were selected and randomly grouped into a control group(conventional care and treatment)and an observation group(joint interventions:traditional Chinese medicine nursing interventions, acupoint compresses),with 30 patients each. The arterial blood gas indexes of the two groups, hospitalization time, pulmonary function indexes, and TCM symptom scores were analyzed and compared between the two groups. Results:The arterial oxygen pressure(PaO_(2))(9.52±1.02 kPa)and partial pressure of carbon dioxide(PaCO_(2))(5.01±1.02 kPa)of the observation group were better than those of the control group after the intervention(P<0.05).The forced expiratory volume(FEV1)(3.38±0.15%),the FEV1% prediction value(72.52±2.25),and the FEV1/forced vital capacity(FVC)(79.52±1.41%)were higher than those of the control group(P<0.05).The hospitalization time(12.16±1.02 d)and TCM symptom score(4.12±1.26)of the observation group were better than those of the control group(P<0.05).Conclusion:Significant nursing effects were achieved by carrying out combined interventions(Chinese medicine nursing intervention and acupoint application)during the acute exacerbation of COPD. 展开更多

Objective To measure the QOL in patients with AECOPD and the frequency of potential risk factors, and to evaluate the association of risk factors with poor QOL in patients with AECOPD. Methods A study sample of 196 patients with moderate to severe COPD admitted for acute exacerbations to two large general hospitals were studied. The St George QOL (SGQOL) scale, socio-demographic, clinical and patient care characteristics, including depression and spirometry were ascertained in the stable state before discharge and at one-month post discharge. Results There was a high prevalence of current or ex-heavy smokers, depression and consumption of psychotropic drugs, and low prevalence of care giver support, pulmonary rehabilitation and vaccination. The mean scores for the different domains were 55.9 for Symptoms; 65.1 for Activity; 32.9 for Impact; and the mean of overall Total scores was 46.5. Multiple regression analysis showed that CMH, male, depression, previous frequent hospital readmissions and poor therapy compliance were independently related to worse Symptoms Scores. Previous frequent readmissions, depression, severe dyspnea and older age (>72 years) were related to worse Activity Scores of SGQOL. Depression, previous frequent readmissions, severe dyspnea, long COPD duration(≥5years) and severe smoking were related to worse Impact Scores of SGQOL. Depression, previous frequent readmissions, severe dyspnea and long COPD duration(≥5years) were independently related to worse Total Scores of SGQOL. Conclusion Poor QOL in patients with COPD exacerbation was associated with disease severity, psychosocial and health care factors which are modifiable. 展开更多

Objective:To observe the effect of Elephantopus scaber Linn on chronic obstructive pulmonary disease(COPD),to investigate its effect on the MUC5AC hypersecretion of airway mucus mediated by neutrophil elastin in patients with acute exacerbation of COPD,and to observe its effect on lung function(FEV1,FEV1/FVC),so as to provide new ideas and theoretical basis for the prevention and treatment of COPD.Methods:160 patients with acute exacerbation of COPD were randomly divided into observation group and control group with 80 cases each according to the numerical table method. The observation group was treated with aerosol inhalation of Elephantopus scaber Linn on the basis of conventional treatment. Both groups were treated for 2 weeks.During the same period,80 healthy patients were selected as the healthy control group. The expression of NE and MUC5AC in serum of observation group, control group and healthy control group before and after treatment was detected by enzymelinked immunosorbent assay(ELISA).The changes of lung function(FEV1,FEV1/FVC)after treatment in the observation group and the control group were compared and analyzed, and the efficacy of Elephantopus scaber Linn in patients with acute exacerbation of copd was observed. Results:Compared with the control group, the total effective rate of the observation group was significantly better than that of the control group, with statistical significance(P<0.05).Compared with the healthy control group, there were significant differences in serum NE and MUC5AC between the observation group and the control group(P<0.05).Serum NE,MUC5AC,FEV1 and FEV1/FVC were significantly different in the observation group before and after treatment(P<0.05).Serum NE,MUC5AC,FEV1 and FEV1/FVC were significantly different in the observation group before and after treatment(P<0.05).Compared with the control group after treatment, there were significant differences in serum NE and MUC5AC in the observation group after treatment(P<0.05),and the increase of FEV1/FVC and FEV1 was significantly better than that of the control group after treatment(P<0.05).Conclusion:Elephantopus scaber Linn can significantly improve the clinical treatment effect of patients with acute exacerbation of COPD,improve lung function and airway mucus hypersecretion. It can effectively inhibit the expression of NE and MUC5AC in the blood. There were significant differences in serum NE and MUC5AC between the observation group and the control group after treatment(P<0.05),showing a positive correlation, suggesting that inhibition of MUC5AC secretion by Elephantopus scaber Linn in patients with COPD may be related to NE-mediated pathways. 展开更多
