COVID-19疫情背景下,常态化核酸检测可以有效实现感染者“早发现、早报告、早隔离”的目标,现已成为各地疫情防控的主要措施。通过核酸采样点布局优化,可以更好地提高核酸采样效率。首先,结合疫情防控实际要求提出一类全新的核酸采样点布局问题,其在传统“选址-分配”问题上,进一步集成考虑了采样点的服务能力、人员配置和工作时长等优化决策。其次,将该问题构建为一类非线性混合整数模型,进一步通过添加变量和约束的方法对其线性等价转换。再者,采用Cplex软件求解模型,进一步通过网格测试发现,在3600秒内最多可求解出包含525个节点的中等规模问题;最后,结合苏州市双塔街道核酸采样相关数据进行仿真分析,算例结果验证了模型的有效性。 展开更多

The coronavirus disease 2019(COVID-19)mortality rate in 55 African countries is almost 4.5 times lower than in the coronavirus disease 2019(COVID-19)despite Africa having over 4.2 times more people.This mortality paradox is also evident when comparing Nigeria,a heavily populated,poorly vaccinated and weakly mandated country to Israel,a small,highly vaccinated and strictly mandated country.Nigeria has almost 4 times lower COVID mortality than Israel.In this Field of Vision perspective,I explain how this paradox has evolved drawing upon my academic,clinical and social experience.Since April 2020,I’ve developed and been using the Egyptian immune-modulatory Kelleni’s protocol to manage COVID-19 patients including pediatric,geriatric,pregnant,immune-compromised and other individuals suffering from multiple comorbidities.It’s unfortunate that severe acute respiratory syndrome coronavirus 2 is still evolving accompanied by more deaths.However in Africa,we’ve been able to live without anxiety or mandates throughout the pandemic because we trust science and adopted early treatment using safe,and effective repurposed drugs that have saved the majority of COVID-19 patients.This article represents an African and Egyptian tale of honor. 展开更多

Objective:To explore on the association between nucleic acid turning-negative-time and traditional Chinese medicine(TCM)symptoms in coronavirus disease 2019(COVID-19)patients with the Omicron variant.Method:For this retrospective cross-sectional study,we enrolled 189 patients with COVID-19(age 20-90 years)were included.multiple linear regression models were used to investigate the TCM symptoms affecting the nucleic acid turning-negative-time of COVID-19 patients,during the process of data analysis,taking with nucleic acid turning-negative-time as the dependent variable,and TCM symptoms as independent variables,and at the same time,sex,age,hypertension,diabetes,and coronary heart disease were as confounding variables.Results:The study found that the most common TCM symptoms of COVID-19 patients with Omicron were cough,dry throat,expectoration,fever,sore throat,pharyngeal itching,running nose,and nasal congestion.Regression analysis showed that the fit regressive equation showed a significant difference(F=4.286,P<0.001),R=0.400,the adjusted R^(2)=0.123,and three variables in the regression equation showed significant difference(P<0.05).The results showed that nucleic acid turningnegative-time was mostly related to constipation,fever,and expectoration.If the patients had the symptoms of constipation,fever,and expectoration;that is,if patients showed these symptoms,the turning-negative-time of nucleic acid in patients with Omicron will be prolonged.Conclusions:Treatment based on symptoms for patients with constipation,fever,and expectoration may have important clinical significance for the COVID-19 patients with Omicron. 展开更多

As a newly emerged disease,COVID-19's in-depth understanding is still being explored.And also as the epidemic changes,various new situations will be encountered.Atypical patients have brought difficulties to our prevention and control of this disease.Among them,asymptomatic patients are a special category,and puerperae are a special population.What happens when a puerpera is infected with asymptomatic COVID-19?This study describes 1 case of COVID-19 asymptomatic infection with positive nucleic acid and negative antibodies in puerpera.The aim is to remind clinicians to take special cases seriously,such as puerperae and pregnant women,AIDS,and other immunocompromised patients.It may not be enough to rely solely on antibody detection,but more than two repeated nucleic acid tests are necessary,so that no suspicious patient can be easily let off. 展开更多

目的:报告某地区收治的30例普通型新冠肺炎患者的临床特点,分析新型冠状病毒核酸转阴的影响因素。方法:收集2020年1月1日~2020年2月29日期间收治的30例普通型新冠肺炎患者的病历资料。记录病史、接触史、症状、体征等信息。结果:30例患... 目的:报告某地区收治的30例普通型新冠肺炎患者的临床特点,分析新型冠状病毒核酸转阴的影响因素。方法:收集2020年1月1日~2020年2月29日期间收治的30例普通型新冠肺炎患者的病历资料。记录病史、接触史、症状、体征等信息。结果:30例患者中,病毒核酸转阴时间小于10天的有10例(33.3%),大于10天的有20例(66.7%)。发病年龄超过30岁对病毒核酸转阴时间的长短有影响,差异具有统计学意义(P=0.009),但患者的性别对病情的转归无影响(P=0.70)。发病时最常见的症状为发热(29/30,96.7%)、咳嗽(18/30,60.0%)和乏力(13/30,43.3%),发热的最高温度通常集中在38℃左右,腹泻(2/30,6.7%)一般较不常见。有一半的患者存在基础疾病如糖尿病、高血压和其他疾病如脑血栓、疾病术后及肿瘤化疗等(15/30,50%)。结论:对于确诊为普通型新冠肺炎的患者,特别是发烧3天以上、年龄大于30岁的患者,核酸转阴时间延长,应考虑加强对该类患者的监护。 展开更多

目的研究56℃30 min热灭活后的新型冠状病毒肺炎(COVID-19)咽拭子及提取的核酸在4℃保存的稳定性。方法采用多重荧光定量PCR方法对一例COVID-19患者的咽拭子样本,56℃30 min热灭活后进行COVID-19双靶点(ORF1ab和N基因)核酸检测,把原始... 目的研究56℃30 min热灭活后的新型冠状病毒肺炎(COVID-19)咽拭子及提取的核酸在4℃保存的稳定性。方法采用多重荧光定量PCR方法对一例COVID-19患者的咽拭子样本,56℃30 min热灭活后进行COVID-19双靶点(ORF1ab和N基因)核酸检测,把原始咽拭子样本及提取的核酸保存在4℃,分别于24,48,72和96h再次检测咽拭子样本及提取核酸中新冠病毒核酸的含量(Ct值),分析热灭活后咽拭子样本及提取核酸在4℃保存的稳定性。结果COVID-19患者的咽拭子样本,56℃30 min热灭活后核酸检测结果为ORF1ab和N基因双阳性,Ct值分别为31.27和30.64,咽拭子样本4℃保存24,48,72和96h后再次检测ORF1ab和N基因的结果为24h(31.41和30.68),48h(34.13和34.28),72h(34.06和34.11)和96h(36.22和40.02)。提取的核酸4℃保存24,48,72和96h后的检测结果为24h(31.64和30.06),48h(31.62和29.95),72h(31.72和29.75)和96h(31.36和30.09)。56℃30 min热灭活的咽拭子样本在4℃保存24h后,对COVID-19核酸的检测无明显影响(偏倚<1%),但4℃保存48~72h,ORF1ab基因核酸降解约7倍,N基因降解约11倍,4℃保存96h,ORF1ab和N基因核酸分别降解30.91倍和666.29倍。但是提取的核酸4℃保存24~96h后,对核酸的检测结果无明显影响(偏倚均小于3%)。结论研究结果表明56℃30 min热灭活后提取的核酸的稳定性比原始咽拭子样本好,热灭活后的咽拭子样本可在4℃保存24h。 展开更多

目的探讨新冠肺炎临床特点,为临床工作提供指导。方法回顾性分析苏州大学附属第一医院首诊确诊的9例(包括1例重症)新型冠状病毒肺炎人群特点、流行病学史、临床表现、辅助检查、诊断与治疗、转归等。结果9例新冠肺炎患者均有武汉接触史,... 目的探讨新冠肺炎临床特点,为临床工作提供指导。方法回顾性分析苏州大学附属第一医院首诊确诊的9例(包括1例重症)新型冠状病毒肺炎人群特点、流行病学史、临床表现、辅助检查、诊断与治疗、转归等。结果9例新冠肺炎患者均有武汉接触史,9例患者中合并高血压1例,脂肪肝3例,合并糖尿病、肝囊肿、胆囊结石、肝硬化各1例。8例新型冠状病毒患者首发症状为发热,1例首发症状为干咳,1例出现腹泻,1例重症患者出现胸闷气促,低氧血症;部分患者还表现为乏力,咽痛,肌肉酸痛,鼻塞、流涕等症状。实验室检查:9例患者白细胞总数正常或减少,5例淋巴细胞计数减少,8例C反应蛋白升高,7例CT可见双侧肺部感染,2例单一右肺或左肺感染,多以磨玻璃影、斑片影为主要表现,部分表现出条索状高密度影和纵膈淋巴结肿大。9例患者均给予洛匹那韦/利托那韦(克力芝)治疗,常用药物包括泼尼松、莫西沙星、连花清瘟胶囊、干扰素等。9例全部治愈出院,随访期间无一复阳。结论患者大部分具有流行病学史,症状相对较轻,影像学表现及核酸检查符合确诊标准,在诊断治疗以及出院后随访上仍存在一些问题,值得大家共同探讨。 展开更多

To explore the impact of ursodeoxycholic acid(UDCA)on severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection and clinical outcomes in patients with autoimmune liver disease(AILD).Patients diagnosed with AILD were enrolled and divided into a UDCA group and a non-UDCA group based on whether they received UDCA treatment.Relevant data were collected regarding AILD diagnosis,treatment,biochemical indicators,and imaging examination.The incidence of SARS-CoV-2 infection and the prognosis of AILD patients were observed.A total of 1,138 patients completed follow-up.The usage rate of hormone(P=0.003)and immunosuppressant(P=0.001)used for treating AILD in the non-UDCA group was markedly lower than in the UDCA group.The UDCA usage rate was markedly lower in SARS-CoV-2 infected patients than in uninfected patients(P=0.003).The rate of SARS-CoV-2 infection in the non-UDCA group was significantly higher than in the UDCA group(P=0.018).Logistic regression analysis showed that UDCA use(P=0.003)was correlated to a lower incidence of SARS-CoV-2,while immunosuppressant use(P=0.017)increased the incidence.Recovery time from SARS-CoV-2 infection was markedly longer for those receiving UDCA treatment than those in the non-UDCA group(P=0.018).UDCA is associated with low SARS-CoV-2 incidence in AILD patients,while immunosuppressant increases its incidence instead.Patients receiving UDCA treatment have a longer recovery time after being infected. 展开更多

Background:In a few discharged patients with coronavirus disease 2019(COVID-19),the nucleic acid test shows positive results again.Whether this is due to relapse of the disease,reinfection by the virus,or a false-positive result at hospital discharge is worth exploring.Case presentation:A woman with COVID-19 was discharged from the hospital after integrative treatment with traditional Chinese and Western medicine because she met the discharge standards.However,she obtained positive results on a nucleic acid test 22 days later.Conclusion:Based on this positive test result in a discharged patient with COVID-19,anal tests and coronavirus antibody tests should be combined with throat swab tests to further develop the diagnosis and discharge standards for patients with COVID-19. 展开更多

BACKGROUND The global outbreak of human severe acute respiratory syndrome coronavirus(SARS-CoV)-2 infection represents an urgent need for readily available,accurate and rapid diagnostic tests.Nucleic acid testing of respiratory tract specimens for SARS-CoV-2 is the current gold standard for diagnosis of coronavirus disease 2019(COVID-19).However,the diagnostic accuracy of reverse transcription polymerase chain reaction(RT-PCR)tests for detecting SARS-CoV-2 nucleic acid may be lower than optimal.The detection of SARS-CoV-2-specific antibodies should be used as a serological non-invasive tool for the diagnosis and management of SARS-CoV-2 infection.AIM To investigate the diagnostic value of SARS-CoV-2 IgM/IgG and nucleic acid detection in COVID-19.METHODS We retrospectively analyzed 652 suspected COVID-19 patients,and 206 non-COVID-19 patients in Wuhan Integrated TCM and Western Medicine Hospital.Data on SARS-CoV-2 nucleic acid tests and serum antibody tests were collected to investigate the diagnostic value of nucleic acid RT-PCR test kits and immunoglobulin(Ig)M/IgG antibody test kits.The j2 test was used to compare differences between categorical variables.A 95%confidence interval(CI)was provided by the Wilson score method.All analyses were performed with IBM SPSS Statistics version 22.0(IBM Corp.,Armonk,NY,United States).RESULTS Of the 652 suspected COVID-19 patients,237(36.3%)had positive nucleic acid tests,311(47.7%)were positive for IgM,and 592(90.8%)were positive for IgG.There was a significant difference in the positive detection rate between the IgM and IgG test groups(P<0.001).Using the RT-PCR results as a reference,the specificity,sensitivity,and accuracy of IgM/IgG combined tests for SARS-CoV-2 infection were 98.5%,95.8%,and 97.1%,respectively.Of the 415 suspected COVID-19 patients with negative nucleic acid test results,366 had positive IgM/IgG tests with a positive detection rate of 88.2%.CONCLUSION Our data indicate that serological IgM/IgG antibody combined test had high sensitivity and specificity for the diagnosis of SARS-CoV-2 infection,and can be used in combination with RT-PCR for the diagnosis of SARS-CoV-2 infection. 展开更多

目的探讨COVID-19在不同时期的胸部CT表现及出院后核酸复阳肺内病灶变化特点。方法回顾性分析54例COVID-19患者的临床资料及影像学表现,通过3次以上胸部CT检查动态观察病灶变化,对比住院期间及出院后核酸检测复阳患者的胸部CT改变。结果... 目的探讨COVID-19在不同时期的胸部CT表现及出院后核酸复阳肺内病灶变化特点。方法回顾性分析54例COVID-19患者的临床资料及影像学表现,通过3次以上胸部CT检查动态观察病灶变化,对比住院期间及出院后核酸检测复阳患者的胸部CT改变。结果78%的病例近14 d去过武汉市或湖北省,22%的病例无去武汉市或湖北省史;普通型51例(94%),重型3例(6%);单叶单灶2例(3.7%),单叶多灶3例(5.6%),多叶多灶例49例(90.7%);沿胸膜下分布为主46例,沿血管束分布为主6例,沿肺叶分布2例;磨玻璃(GGO)54例,GGO伴局灶实变28例,GGO伴细网格征34例,GGO伴晕征23例,含气囊腔征3例,蜂窝征1例;血管束增粗14例,支气管增粗或扩张18例;1例伴双侧少量胸腔积液。早期表现单发或多发斑片磨玻璃灶;进展期病灶数量增多、范围增大、密度增高、局部实变或大片实变;重症期双肺多发或弥漫磨玻璃密度背景下节段或肺叶实变;转归期病灶缩小、密度不均匀减低,大部分病灶完全消失,少量形成纤维条索;6例(11.5%)在转归期出现“此消彼长”表现。47例(87%)在7~14 d病灶开始吸收,7例(13%)病灶开始吸收时间在14 d后。46例符合临床出院标准时,出院前胸部CT仍见多发磨玻璃病灶;9例出院后7~15 d出现核酸检测复阳,2例核酸复阳出现在15 d后,但其肺内病灶较上一次CT(出院前)吸收减少,未见新增病灶。结论COVID-19不同时期影像学表现不同,其转归过程有一定的特征及规律,正确认识及掌握其影像学变化对其早期诊断、排查及疗效评估有重要价值。 展开更多

Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)has emerged as one of the most dreadful viruses the mankind has witnessed.It has caused worldwide havoc and wrecked human life.In our quest to find therapeutic options to counter this threat,several drugs have been tried,with varying success.Certain agents like corticosteroids,some anti-virals and immunosuppressive drugs have been found useful in improving clinical outcomes.Vitamin C,a water-soluble vitamin with good safety profile,has been tried to reduce progression and improve outcomes of patients with coronavirus disease 2019(COVID-19).Because of its anti-oxidant and immunomodulatory properties,the role of vitamin C has expanded well beyond the management of scurvy and it is increasingly been employed in the treatment of critically ill patients with sepsis,septic shock,acute pancreatitis and even cancer.However,in spite of many case series,observational studies and even randomised control trials,the role of vitamin C remains ambiguous.In this review,we will be discussing the scientific rationale and the current clinical evidence for using high dose vitamin C in the management of COVID-19 patients. 展开更多

Background:The emergence of Coronavirus SARS-CoV-2 evoked an unprecedented threat globally.Ever since the spread of this pandemic research and clinical trials have concentrated on the repurposing of already exciting FDA drugs to find a successful candidate to combat Covid-19.Objective:The objective of this study is to propose a therapeutic protocol that may have a potential solution to treat the severe infections associated with coronavirus.The clinical application of this protocol(Al-Akidi Therapeutic Protocol,A-TP)is highly recommended,as there are several scientific evidences that support this trusted protocol to be of great potential.Methods and Materials:This A-TP includes the use of one of the respiratory Fluoroquinolones(Levofloxacin or Moxifloxacin)in doses of 500 mg twice daily for 7-10 days,together with high doses of Vitamin D3(10000 IU/day)and Zinc(50 mg daily)for few weeks.This protocol is based on the previous antiviral activity of those Fluoroquinolones towards few viruses,the potent antibacterial activity on respiratory infections and high available concentrations in the lungs.It is also based on molecular docking of Levofloxacin and Moxifloxacin on various viral enzymes.Results:Molecular docking showed encouraging and very interesting docking scores and binding affinity of Levofloxacin and Moxifloxacin to certain viral enzymes,such as,RNA dependent RNA polymerase(RdRp),3-Cysteine-Like protease,Neuraminidase,Replicase polyproteins and Trans-Membrane Protease Serine 2 inhibitor(TMPRSS2).The highly expected clinical results of using this protocol are:reduce infection,control of temperature,improve breathing with less dependent on supplemented oxygen,and remarkable reduction of the pro-inflammatory cytokine storm,and hence,reduce hospitalization and mortality.Conclusion:Levofloxacin is highly recommended in managing the severe infections associated with Corona virus and has a remarkable reduction of pro-inflammatory cytokine storm as an immuomodulating agent.Levofloxacin is superior in this protocol over Moxifloxacin,due to its high excretion(≤83%)as unchanged through the kidneys,while Moxifloxacin is only 20%is excreted unchanged.It is an extra advantage of Levofloxacin to manage coronavirus in the kidneys.High doses of Vitamin D3 and Zinc are very useful to provide additional effective measures to combat Corona virus.Therefore,the use of this A-TP is highly and strongly recommended,as it serves the full requirements for excellent and potential therapy for the severe infections associated with Covid-19. 展开更多
