OBJECTIVE To investigate the effects of Tongmai Yangxin Pill(TMYXP)combined with metoprolol tartrate or metoprolol alone for the treatment of premature ventricular complex(PVC)in patients with symptomatic frequent PVC...OBJECTIVE To investigate the effects of Tongmai Yangxin Pill(TMYXP)combined with metoprolol tartrate or metoprolol alone for the treatment of premature ventricular complex(PVC)in patients with symptomatic frequent PVC.METHODS A total of 584 patients with symptomatic frequent PVC were randomly assigned(in a 1:1 ratio)into two groups:study group[n=292,TMYXP(40 pills twice/day,orally)combined with metoprolol tartrate(25 mg twice/day,orally)]and control group[n=292,metoprolol tartrate(25 mg twice/day,orally)plus placebo pill(40 pills twice/day,orally)].The total treatment period was eight weeks.RESULTS After eight weeks of treatment,the total effective rate of reduction of PVC in the study group and the control group were 76.4%and 51.4%,respectively(P<0.001).TMYXP combined with metoprolol tartrate demonstrated a significantly greater reduction of the frequency of PVCs compared with the metoprolol tartrate alone(−4537 times/24 h vs.−3013 times/24 h,P<0.001).The study group also showed a better result compared with the control group with respect to PVC related symptoms.In terms of New York Heart Association classification improvement,the total effective rates were 21.9%in the study group and 12.4%in the control group(P<0.05).Both the study group and the control group exhibited improvements in echocardiographic indexes.Left ventricular ejection fraction was significantly improved in the study group compared with the control group(P<0.05).There was no significant difference in the incidence of adverse events between the two groups.CONCLUSIONS Compared with metoprolol tartrate alone,TMYXP combined with metoprolol tartrate could more effectively reduce the frequency of PVC and alleviated PVC related symptoms,and improve cardiac function in patients with symptomatic PVC.展开更多

Every effort has been made to develop the physical and mechanical properties of the materials to mimic the natural tooth substances. However, that has not yet been achieved, making it necessary to apply conservative dental treatments. Conservative treatments are procedures which minimize the amount of defective tooth structure removed while maximizing the remaining amount of intact tooth structure, and defining the margin of the restoration at the supragingival level. In a substantially destructed tooth, the direct restoration, which is the most conservative restoration, has the limitations on the retention or creation of the proper contour of the restoration. The indirectly partial coverage restoration has recently gained in popularity. This treatment preserves the tooth structure while allowing reconstruction of the destructed structure to enable correct and proper tooth functioning and also provides a satisfactory cosmetic appearance. The purpose of this paper is to review and compare the various conservative techniques of partial coverage restorations and the full coverage treatment with crowns.展开更多

I write to present a case report of the treatment of complex traumatic retina detachment, which has three huge breaks. A 35-year-old man, who was knocked down and hurt his right eye while playing football, complained of blur vision for a day. The visual acuity were 4/20 OD and 10/20 OS. The corrected visual acuity were 4/20 OD and 10/20 OS. The intraocular pressure were 12mmHg OD and 13mmHg OS. There was blood in the central vitreous of the right eye. One giant break and three nail type breaks were found in the temple retina. The temple retina detached, yet the macular area was not involved (Figure 1). And the fundus of the left eye was normal. B ultrasonic scan showed the right eye temple retina detachment (Figure 2). VEP and ERG of the eyes were normal.展开更多
BACKGROUND Complicated cataract surgery is challenging,especially in cases of hard nuclear cataract with severe anterior capsule organization.It is important to avoid the risk of surgery and improve the surgical skill...BACKGROUND Complicated cataract surgery is challenging,especially in cases of hard nuclear cataract with severe anterior capsule organization.It is important to avoid the risk of surgery and improve the surgical skills of surgeons.CASE SUMMARY A 60-year-old man presented with severe cataract and visual impairment.The anterior capsule of the lens was irregularly organized and pulled to the surrounding capsule,and white porcelain organized cord and brown-black lens nucleus were clearly visible.In phacoemulsification,maintaining the anterior capsule round and intact plays a key role in a successful surgery.In this case,if the conventional capsule treatment method was used,the anterior capsule would be torn.Therefore,we adopted a segmented anterior capsule treatment method,and a blasting method to release energy when dealing with the lens nucleus,and achieved good surgical results.CONCLUSION Complicated cataract surgery is challenging and requires precise skills.Operation plans should be made reasonably to predict the risk of surgery,and improve the visual quality of the patients.展开更多

AIM To systematically review the literature on epidemiology,disease burden, and treatment outcomes for Crohn's disease(CD) patients with complex perianal fistulas.METHODS PubMed, Embase, and Cochrane were searched...AIM To systematically review the literature on epidemiology,disease burden, and treatment outcomes for Crohn's disease(CD) patients with complex perianal fistulas.METHODS PubMed, Embase, and Cochrane were searched for relevant articles(published 2000-November 2016) and congress abstracts(published 2011-November 2016).RESULTS Of 535 records reviewed, 62 relevant sources were identified(mostly small observational studies). The cumulative incidence of complex perianal fistulas in CD from two referral-centre studies was 12%-14%(follow-up time, 12 years in one study; not reported in the second study). Complex perianal fistulas result in greatly diminished quality of life; up to 59% of patients are at risk of faecal incontinence. Treatments include combinations of medical and surgical interventions and expanded allogeneic adipose-derived stem cells. High proportions of patients experience lack of or inadequate response to treatment(failure and relapse rates,respectively: medical, 12%-73% and 0%-41%; surgical:0%-100% and 11%.20%; combined medical/surgical:0%-80% and 0%-50%; stem cells: 29%-47% and not reported). Few studies(1 of infliximab; 3 of surgical interventions)have been conducted in treatment-refractory patients, a population with high unmet needs. Limited data exist on the clinical value of anti-tumour necrosis factor-α dose escalation in patients with complex perianal fistulas in CD.CONCLUSION Complex perianal fistulas in CD pose substantial clinical and humanistic burden. There is a need for effective treatments, especially for patients refractory to antitumour necrosis factor-α agents, as evidenced by high failure and relapse rates.展开更多
