Early diagnostic value of carotid artery ultrasound parameters combined with epicardial adipose layer thickness in coronary heart disease

BACKGROUND Coronary heart disease is associated with coronary atherosclerosis indicated by carotid intima-media thickness(CIMT)thickening and altered vascular elasticity.The epicardial adipose layer can secrete proinflammatory factors that promote the formation of coronary atherosclerosis.Thus,the epicardial fat layer thickness(EAT)may also predict coronary heart disease.AIM To determine the role of common carotid artery ultrasound parameters and EAT in the early diagnosis of coronary artery disease.METHODS Based on coronary angiography,patients with newly suspected coronary heart disease were divided into case(n=107)and control(n=41)groups.The carotid ultrasound parameters,including vascular stiffness(β),elastic coefficient(EP),pulse wave conduction velocity(PWV-β),CIMT,and EAT were compared between the case and control groups and among patients with different lesion numbers in the case group.Pearson correlation was used to evaluate the early diagnostic value of EAT,common carotid artery elasticity,and CIMT for coronary heart disease.RESULTS EP,β,PWV-β,CIMT,and EAT were significantly higher in the case group compared with the levels in the control group(all P<0.001).In the case group,lesions were detected in one vessel in 34 patients,two vessels in 38 patients,and three vessels in 35 patients.Within the case group,β,EP,PWV-β,CIMT,and EAT levels significantly increased with an increased number of lesions(all P<0.001).EAT positively correlated withβ,EP,PWV-β,and CIMT(all P<0.01).The area under the curve for diagnosing coronary heart disease using EAT combined with CIMT and carotid elasticity was 0.893,and the sensitivity and specificity were 0.890 and 0.837.CONCLUSION EAT correlated well with changes in carotid artery elasticity and CIMT in patients with coronary heart disease.The combination of EAT,carotid artery elasticity,and CIMT facilitates the early diagnosis of coronary heart disease. 展开更多

Background: Aortic dissection (AD) is a relatively rare but dreadful illness, often accompanied by severe, sharp (or tearing) back pain or anterior chest pain, as well as acute hemodynamic compromise. Painless dissection has also been reported in rare cases and might be misdiagnosed due to its atypical symptoms leading to catastrophic outcomes. Case presentation: The patient was admitted to the hospital due to right limb weakness with speech inability for more than 10 hours. In the routine cardiac ultrasound examination, the avulsion intimal echo was found in the initial segment of the descending aorta. The rupture range was about 11 mm, and the lumen was separated into real and false lumen. Further computed tomography angiography (CTA) examination confirmed the major arterial dissection (De Bakey Type I). Conclusion: We report a case of painless aortic dissection with active carotid artery thrombosis diagnosed by ultrasound and CTA, and to improve the understanding of painless aortic dissection by reviewing relevant domestic and foreign literature. 展开更多

Correlation of Enhancement Degree on Contrast-enhanced Ultrasound with Histopathology of Carotid Plaques and Serum High Sensitive C-Reactive Protein Levels in Patients Undergoing Carotid Endarterectomy 被引量:3

This study was undertaken to investigate the correlation of the enhancement degree on contrast-enhanced ultrasound(CEUS) with the histopathology of carotid plaques and the serum high sensitive C-reactive protein(hs-CRP) levels in patients undergoing carotid endarterectomy(CEA). Carotid CEUS was performed preoperatively in 115 patients who would undergo CEA, and the enhancement degree of the carotid plaques was evaluated by both the visual semiquantitative analysis and the quantitative time-intensity curve analysis. Serum hs-CRP levels were detected using the particle-enhanced immunoturbidimetric assay also before the operation. Additionally, the carotid plaque samples were subjected to histopathological examination postoperatively. The density of neovessels and the number of macrophages in the plaques were assessed by immunohistochemistry. The results showed that among the 115 patients, grade 0 plaque contrast enhancement was noted in 35 patients, grade 1 in 48 patients and grade 2 in 32 patients. The degree of plaque enhancement, the density of neovessels, the number of macrophages, and the hs-CRP levels were highest in the grade 2 patients. Correlation analysis showed that the enhancement degree of the carotid plaques was closely related to the immunohistochemical parameters of the plaques and the serum hs-CRP levels. It was suggested that the carotid plaque enhancement on CEUS can be used to evaluate the vulnerability of carotid plaques. 展开更多

BACKGROUND: The purpose of this study was to use point-of-care ultrasound(POCUS) to investigate the relationship between tobacco smoke exposure and the characteristics of the common carotid artery(CCA). The effect of both primary and secondary smoking on CCA properties was evaluated.METHODS: We performed a prospective cross-sectional study across 20 primary care clinics in Bandung, West Java, Indonesia in July 2016. Point of care ultrasound was performed on a convenience sample of Indonesian patients presenting to clinic. The CCA wall stiffness and carotid intima-media thickness(CIMT) were measured during diastole and systole. These measurements were correlated with smoke exposure and cardiovascular disease.RESULTS: We enrolled 663 patients in the study, with 426 patients enrolled in the smoking category and 237 patients enrolled in the second-hand smoke category. There was an overall positive correlation with the measured lifestyle factors and the ultrasound-measured variables in the group of individuals who smoked. For all variables, age seemed to contribute the most out of all of the lifestyle factors for the positive changes in CIMT and CCA wall stiffness.CONCLUSION: Our data yielded correlations between CCA properties and cardiovascular risk, as well as between CIMT and arterial stiffness. We were also able to demonstrate an increase in thickness of the CIMT in patients who have been exposed by tobacco through the use of ultrasound. Further large scale studies comparing patients with multiple cardiac risk factors need to be performed to confi rm the utility of ultrasound fi ndings of cardiovascular disease and stroke. 展开更多

BACKGROUND Spontaneous visceral artery dissection(SVAD)is a rare condition that affects the visceral arteries,such as the celiac,superior mesenteric,and inferior mesenteric arteries,without involving the aorta.Organ ischemia or hemorrhage from vessel rupture can occur in SVAD;therefore,prompt detection and management is essential.Contrast-enhanced computed tomography(CECT)has been used to diagnose most of the previous cases,but few studies have explored the potential of contrast-enhanced ultrasound(CEUS)for early detection of this disease.CASE SUMMARY A 53-year-old male presented with complaints of poor appetite and abnormal liver function for the past 6 months.He had previously undergone transabdominal splenectomy,esophagogastric devascularization,and cholecystectomy for gallstones and severe portal hypertension.Liver ultrasound was performed in our department to assess liver status.An abnormal hepatic artery spectrum was observed,and dissection involving both the celiac artery and the common hepatic artery was observed.A CEUS was then performed and clearly showed the entry site of the intimal tear and the false lumen,and dissection was subsequently confirmed by CECT.The patient was asymptomatic;therefore,treatment to control the blood pressure was provided,and follow-up was recommended.After 6 months of follow-up,the celiac artery was found to be dilated with an adherent thrombus visible in the wall,and the common hepatic artery was occluded with the presence of collateralization.Despite these findings,no significant changes in liver function were observed.CONCLUSION Multi-modal imaging is effective in diagnosing SVAD,and conservative treatment is a choice for asymptomatic patients. 展开更多

Stroke and heart attack,which could be led by a kind of cerebrovascular and cardiovascular disease named as atherosclerosis,would seriously cause human morbidity and mortality.It is important for the early stage diagnosis and monitoring medical intervention of the atherosclerosis.Carotid stenosis is a classical atherosclerotic lesion with vessel wall narrowing down and accumulating plaques burden.The carotid artery of intima-media thickness(IMT)is a key indicator to the disease.With the development of computer assisted diagnosis technology,the imaging techniques,segmentation algorithms,measurement methods,and evaluation tools have made considerable progress.Ultrasound imaging,being real-time,economic,reliable,and safe,now seems to become a standard in vascular assessment methodology especially for the measurement of IMT.This review firstly attempts to discuss the clinical relevance of measurements in clinical practice at first,and then followed by the challenges that one has to face when approaching the segmentation of ultrasound images.Secondly,the commonly used methods for the IMT segmentation and measurement are presented.Thirdly,discussion and evaluation of different segmentation techniques are performed.An overview of summary and future perspectives is given finally. 展开更多

Hepatocellular carcinoma(HCC)is one of the few cancers for which locoregional treatments(LRTs)are included in international guidelines and are considered as a valid alternative to conventional surgery.According to Barcelona Clinic Liver Cancer classification,percutaneous treatments such as percutaneous ethanol injection,radiofrequency ablation and microwave ablation are the therapy of choice among curative treatments in patients categorized as very early and early stage,while transcatheter arterial chemoembolization is considered the better option for intermediate stage HCC.A precise assessment of treatment efficacy and surveillance is essential to optimize survival rate,whereas residual tumor requires additional treatment.Imaging modalities play a key role in this task.Currently,contrast-enhanced computed tomography/magnetic resonance imaging are considered the standard imaging modalities for this purpose.Contrast enhanced ultrasound(CEUS),using second generation contrast agents,plays an increasingly important role in detecting residual disease after LRTs.CEUS is a straightforward to perform,repeatable and cost-effective imaging modality for patients with renal failure or iodine allergies.Due to the ability to focus on single regions,CEUS can also provide high temporal resolution.Moreover,several studies have reported the same or better diagnostic accuracy as contrast-enhanced computed tomography for assessing tumor vascularity 1 mo after LRTs,and recently three-dimensional(3D)-CEUS has been reported as a promising technique to improve the evaluation of tumor response to therapy.Furthermore,CEUS could be used early after procedures in monitoring HCC treatments,but nowadays this indication is still debated,and data from literature are conflicting,especially after transcatheter arterial chemoembolization procedure. 展开更多

Endothelial Function and Carotid Artery Wall Thickening in Patients with Early Essential Hypertension

To investigate the relationship between endothelial function and carotid artery wall thickening in patients with early mild essential hypertension, the percent dilatation of humerus diameter and intimal-medial thickening (IMT) of common carotid artery was measured by using high-resolution ultrasound in 20 patients with early mild essential hypertension and 18 patients with normotension.The patients with hypertension had not been treated and their history of increased blood pressure was less than 12 months. In essential hypertension group, the percent dilatation of humerus diameter decreased significantly (2. 65 ± 0. 98 % vs6. 38 ± 1. 61 % ) ; IMT of carotid artery increased (0. 88 ± 0. 16mm vs 0. 58 ±0. 08). There was significant negative correlation between IMT and the percentdilatation of humerus diameter (γ= - 0. 82, P<0. 05), and no correlation be-tween IMT and 24 h mean systolic, diastolic pressure (γ= 0. 12 and γ= 0.07,respectively ; P> 0. 05). Our results suggested that there was endothelial dys-function in early mild essential hypertension. Endothelial dysfunction may not on-ly contribute to the pathogenesis of hypertension but also serve as the most impor-tant inducing factor leading trans formation from hypertension to atherosclerosis. 展开更多

目的探究脑梗死患者颈动脉超声造影的特征及与不同梗死部位、面积等梗死特征的相关性。方法回顾性分析郑州大学第二附属医院2018-02—2021-03诊治的135例脑梗死患者,所有患者均接受超声造影检查,另选取同期因颈动脉斑块行超声造影检查... 目的探究脑梗死患者颈动脉超声造影的特征及与不同梗死部位、面积等梗死特征的相关性。方法回顾性分析郑州大学第二附属医院2018-02—2021-03诊治的135例脑梗死患者,所有患者均接受超声造影检查,另选取同期因颈动脉斑块行超声造影检查的无症状中老年人112例为对照组,比较2组患者超声造影参数,包括峰值强度比值(P)、曲线下面积(AUC)、达峰时间(Tp)。将脑梗死患者按梗死部位、梗死面积分成不同亚组,分析不同亚组颈动脉斑块超声造影相关参数。结果脑梗死患者P和AUC值均明显高于对照组,Tp值明显低于对照组(P<0.05)。随着颈动脉狭窄程度的加重,P和AUC值明显升高,Tp值明显降低(P<0.05)。Spearman相关性分析显示,P、AUC值与颈动脉狭窄程度呈正相关,Tp值与狭窄程度呈负相关(P<0.05)。不同梗死部位间颈动脉狭窄程度比较有统计学差异(P<0.05),其中皮质梗死组患者颈动脉完全闭塞发生率最高;不同梗死面积患者颈动脉狭窄程度比较差异有统计学意义(P<0.05),其中大面积梗死组完全闭塞发生率最高。结论脑梗死患者颈动脉斑块超声造影参数水平明显异于无症状有斑块者,且皮质部位脑梗死患者颈动脉完全闭塞发生率最高,大面积脑梗死患者更易发生颈动脉完全闭塞。 展开更多

目的探讨前交通动脉(ACoA)的代偿功能对双侧颈动脉粥样硬化性狭窄患者非术侧颈动脉血流速度测定与狭窄程度评估的影响。方法回顾性连续纳入2019年1月至2021年1月首都医科大学宣武医院神经外科双侧颈动脉粥样硬化性狭窄、接受单侧颈动脉... 目的探讨前交通动脉(ACoA)的代偿功能对双侧颈动脉粥样硬化性狭窄患者非术侧颈动脉血流速度测定与狭窄程度评估的影响。方法回顾性连续纳入2019年1月至2021年1月首都医科大学宣武医院神经外科双侧颈动脉粥样硬化性狭窄、接受单侧颈动脉内膜切除术(CEA)且ACoA开放的患者161例。所有患者采用经颅彩色多普勒超声(TCCD)和(或)经颅多普勒超声(TCD)检查,并经DSA和(或)CT血管成像(CTA)证实。依据ACoA开放代偿是否完全,将所有患者分为ACoA开放代偿完全组(69例)和开放代偿不完全组(92例)。比较CEA前后ACoA开放代偿完全组和开放代偿不完全组颈动脉超声(CDU)测得的收缩期峰值流速(PSV)和舒张期末流速(EDV),并将超声与DSA或CTA评估非术侧颈动脉狭窄程度(轻度、中度、重度狭窄)的结果进行一致性分析。结果两组患者年龄、性别及危险因素的差异均无统计学意义(均P>0.05)。CEA后1周内,ACoA开放代偿完全组和开放代偿不完全组的非术侧颈动脉的PSV[分别为(107±35)、(122±40)cm/s]和EDV[分别为(37±12)、(47±16)cm/s]与术前PSV[分别为(177±58)、(163±54)cm/s]和EDV[分别为(64±21)、(76±25)cm/s]比较均下降,差异均有统计学意义(均P<0.01)。在ACoA开放代偿完全组的非术侧颈动脉中,CDU评估颈动脉的狭窄程度与DSA或CTA评估结果存在相对不一致性(Kappa=0.074);DSA或CTA评估为轻度狭窄的58例患者中,55例(94.8%)超声高估为中度狭窄,2例(3.4%)高估为重度狭窄;DSA或CTA评估为中度狭窄的9例患者中,8例超声高估为重度狭窄。ACoA开放代偿不完全组的非术侧颈动脉狭窄程度与DSA或CTA评估结果具有一致性(Kappa=0.920);DSA或CTA评估为轻度狭窄的70例患者中,2例(2.9%)超声高估为中度狭窄;DSA或CTA评估为中度狭窄的9例患者中,1例超声高估为重度狭窄。结论ACoA开放且代偿功能完全的双侧颈动脉粥样硬化性狭窄患者术前非术侧颈动脉PSV和EDV明显升高,进而可导致高估非术侧颈动脉的狭窄程度。 展开更多

目的探讨颈动脉超声、高分辨MRI参数联合血清miR-27a预测颈动脉狭窄患者经颈动脉支架植入术后抗血小板治疗预后的价值。方法纳入2021-01—2022-12于大庆龙南医院接受颈动脉支架植入术及抗血小板治疗的110例颈动脉狭窄患者。术前采用彩... 目的探讨颈动脉超声、高分辨MRI参数联合血清miR-27a预测颈动脉狭窄患者经颈动脉支架植入术后抗血小板治疗预后的价值。方法纳入2021-01—2022-12于大庆龙南医院接受颈动脉支架植入术及抗血小板治疗的110例颈动脉狭窄患者。术前采用彩色多普勒超声诊断仪对所有研究对象进行检查,测定并记录平均血流速度(MFV)、舒张末期最低血流速度(EDV)、最大血流速度(PSV)及颈动脉狭窄率,采用高分辨MRI检查并记录患者的斑块偏心指数和斑块强化程度,应用荧光定量PCR检测血清miR-27a水平。患者出院后随访1 a,观察预后情况,分为预后良好组和预后不良组。结果110例患者中预后良好者83例,预后不良者27例。预后良好组和预后不良组患者性别、年龄、身高、体质量、吸烟史、饮酒史情况比较均无统计学差异(P>0.05)。预后良好组患者的PSV、斑块强化程度和血清miR-27a水平与预后不良组相比显著降低(P<0.05)。颈动脉狭窄患者MFV和PSV与血清miR-27a水平呈显著正相关(P<0.05),颈动脉狭窄患者的MFV、EDV、PSV、斑块偏心指数、斑块强化程度和血清miR-27a水平均与颈动脉狭窄率呈显著正相关(P<0.05)。PSV、斑块强化程度和血清miR-27a水平预测颈动脉狭窄患者预后的AUC分别为0.764(95%CI:0.662~0.866)、0.649(95%CI:0.519~0.779)和0.832(95%CI:0.748~0.916)。PSV、斑块强化程度联合血清miR-27a预测颈动脉狭窄患者预后的AUC为0.895(95%CI:0.830~0.960)。结论颈动脉超声、高分辨MRI参数联合血清miR-27a对颈动脉狭窄术后患者的预后预测具有一定价值。 展开更多

颈总动脉流速对创伤合并休克患者容量反应性的评价

目的探讨颈总动脉呼气末及吸气末流速对评估创伤合并休克患者容量反应性的意义。方法前瞻性研究2017年7月—2019年1月上海市浦东新区人民医院重症监护室收治需要机械通气控制呼吸的创伤合并休克患者60例,根据液体复苏后是否具有容量反应... 目的探讨颈总动脉呼气末及吸气末流速对评估创伤合并休克患者容量反应性的意义。方法前瞻性研究2017年7月—2019年1月上海市浦东新区人民医院重症监护室收治需要机械通气控制呼吸的创伤合并休克患者60例,根据液体复苏后是否具有容量反应性(VR)且扩容前后心输出量(CO)变化是否≥15%将患者分为有反应(VR+组)及无反应(VR-组),各30例。VR+组男性23例,女性7例;年龄18~86岁,平均46.9岁。VR-组男性21例,女性9例;年龄18~90岁,平均46.1岁;采用迈瑞M9便携式多普勒超声取右颈总动脉长轴方向上测量两组患者呼气末最大血流速度(ex-Vmax)、吸气末最大血流速度(in-Vmax)与呼气末平均血流流速(ex-TAmax)、吸气末平均血流流速(in-TAmax),并计算出各自的变异度值:颈总动脉吸呼气末最大血流速度变异度(△Vmax)、吸呼气末平均血流速度变异度(△TAmax),分析不同组别上述数值的变化;绘制受试者工作特征(ROC)曲线,分析以上各项指标在容量反应性评估方面的价值。结果VR+组与VR-组in-Vmax[(71.72±18.67)cm/s vs.(79.47±24.04)cm/s]、in-TAmax[(40.67±11.44)cm/svs.(46.44±15.84)cm/s]比较无明显差异,但ex-Vmax[(61.00±17.84)cm/s vs.(76.86±25.28)cm/s]、ex-TAmax[(34.83±11.01)cm/s vs.(44.66±15.43)cm/s]比较差异有统计学意义(P<0.05)。VR+组与VR-组△Vmax(19.42%±14.18%vs.5.09%±13.59%)、△TAmax(18.19%±11.63%vs.4.29%±7.57%)比较差异有统计学意义(P<0.001)。ex-Vmax与ex-TAmax对创伤合并休克患者容量评估的ROC曲线下面积分别为0.695和0.678,而△Vmax与△TAmax的ROC曲线下面积均>0.800,分别是0.803和0.863,提示△Vmax、△TAmax对于创伤合并休克患者容量的评估更敏感。结论呼气末颈总动脉最大血流流速及平均血流流速、吸呼气末最大血流流速差异度及平均血流流速变异度均可用来评估机械通气创伤合并休克患者的容量反应性,且吸呼气末最大血流流速变异度及平均血流流速变异度对容量反应性的评估效果更佳。 展开更多

目的研究急性脑梗死(ACI)患者颈动脉超声参数,分析其对患者动脉斑块性质及预后不良的预测价值。方法采取回顾性研究,选择2020年10月至2023年10月中国人民解放军陆军第八十一集团军医院和张家口市第一医院收治的106例完成3个月随访的ACI... 目的研究急性脑梗死(ACI)患者颈动脉超声参数,分析其对患者动脉斑块性质及预后不良的预测价值。方法采取回顾性研究,选择2020年10月至2023年10月中国人民解放军陆军第八十一集团军医院和张家口市第一医院收治的106例完成3个月随访的ACI患者临床资料作为研究对象。根据患者术后斑块病理检查结果进行分组,将易损斑块患者临床资料纳入易损斑块组(n=42),将稳定斑块患者临床资料纳入稳定斑块组(n=64);根据患者术后3个月时改良Rankin量表(mRS)评分评估患者预后,将评分≤2分患者临床资料纳入预后良好组(n=76),将评分>2分患者临床资料纳入预后不良组(n=30);统计患者颈动脉超声参数[斑块体积、斑块厚度、标准化管壁指数(NWI)、灰阶中位数(GSM)]及基线资料。采用受试者工作特征(ROC)曲线分析颈动脉超声参数对ACI患者颈动脉斑块性质及预后的预测价值;采用二元logistic回归分析ACI患者发生预后不良的危险因素。结果易损斑块组NWI水平高于稳定斑块组,GSM低于稳定斑块组(P<0.05),2组斑块体积、斑块厚度比较差异无统计学意义(P>0.05)。绘制ROC曲线,结果显示,NWI、GSM水平及联合检测评估ACI患者斑块性质的AUC均>0.70,有一定预测价值,其中联合检测最高。预后不良组NIHSS评分、NWI高于预后良好组,GSM水平低于预后良好组,合并高脂血症、糖尿病患者占比高于预后良好组(P<0.05);二元logistic回归分析结果显示,NIHSS评分高、合并高脂血症、合并糖尿病及NWI高水平是ACI患者预后不良的危险因素(OR>1,P<0.05),GSM高水平是ACI患者预后不良的保护因素(OR<1,P<0.05);绘制ROC曲线,结果显示,NWI、GSM水平及联合检测评估ACI患者预后不良的AUC均>0.70,有一定预测价值,其中联合检测最高。结论ACI患者颈动脉超声参数能够有效评估动脉斑块性质,预测预后不良风险,临床可针对预后不良风险较高者采取针对性干预,以改善患者预后。 展开更多

目的:探讨彩色多普勒血流成像(CDFI)在缺血性脑血管病患者颈动脉狭窄评估中的应用价值。方法:回顾性分析2021年1月—2023年11月寿光市人民医院收治的125例缺血性脑血管病患者颈动脉数字减影血管造影(DSA)及CDFI检查结果,以DSA结果为金... 目的:探讨彩色多普勒血流成像(CDFI)在缺血性脑血管病患者颈动脉狭窄评估中的应用价值。方法:回顾性分析2021年1月—2023年11月寿光市人民医院收治的125例缺血性脑血管病患者颈动脉数字减影血管造影(DSA)及CDFI检查结果,以DSA结果为金标准分析CDFI诊断颈动脉狭窄的灵敏度、特异度、准确度、阳性预测值、阴性预测值,并用Kappa检验验证CDFI与DSA诊断颈动脉狭窄的一致性;根据DSA结果显示的颈动脉狭窄情况将患者分为颈动脉狭窄组与正常组,比较两组患者颈动脉血搏动指数(PI)、阻力指数(RI)、舒张期血流速度(EDV)、收缩期峰值流速(PSV)。结果:DSA结果显示,125例患者中74例存在颈动脉狭窄,设为颈动脉狭窄组;51患者无颈动脉狭窄,设为正常组。CDFI检查结果显示,125例患者中73例存在颈动脉狭窄,以DSA结果为金标准,CDFI诊断颈动脉狭窄的灵敏度为97.30%、特异度为98.04%、准确率为97.60%、阳性预测值为98.63%、阴性预测值为96.15%,且Kappa检验结果显示,CDFI与DSA诊断颈动脉狭窄的一致性极好(Kappa值=0.950,P<0.05)。颈动脉狭窄组患者PI、RI高于对照组,EDV、PSV低于对照组,差异有统计学意义(P<0.05)。结论:CDFI在缺血性脑血管病患者颈动脉狭窄评估中的应用价值较高,便于临床更好地对患者进行治疗,促进患者病情好转。 展开更多

目的研究分析颈动脉超声评估颈动脉硬化程度与MSCTA评估冠脉狭窄程度的相关性与联合预测价值。方法选取我院心内科在2017年1月-2019年12月收治的确诊及疑似冠心病的300例患者,所有患者均进行多层螺旋CT血管造影(MSCTA)、颈动脉彩超和冠... 目的研究分析颈动脉超声评估颈动脉硬化程度与MSCTA评估冠脉狭窄程度的相关性与联合预测价值。方法选取我院心内科在2017年1月-2019年12月收治的确诊及疑似冠心病的300例患者,所有患者均进行多层螺旋CT血管造影(MSCTA)、颈动脉彩超和冠状动脉造影(CAG)检查,以CAG检查为金标准,计算上述两种诊断方法对冠心病检出率、阳性率、敏感性、特异性、预测准确性等诊断学指标,并研究分析超声评估与MSCTA评估之间的相关性。结果超声与CAG检查结果一致性为Kappa=0.588,MSCTA与CAG检查结果一致性为Kappa=0.503(P<0.05);超声评估颈动脉硬化程度准确度、敏感度、特异度、阳性预测值及阴性预测值分别为80.33%、80.95%、79.28%、86.93%、70.97%;MSCTA评估冠脉狭窄程度的准确度为81.07%,其中轻度狭窄的准确度、敏感度、特异度、阳性预测值及阴性预测值分别为83.54%、83.72%、83.33%、85.04%、81.90%;中度狭窄的分别为84.36%、65.28%、92.40%、78.33%、86.34%;重度狭窄的分别为86.83%、78.57%、88.56%、58.93%、95.19%;颈动脉超声评估颈动脉硬化程度与MSCTA评估冠脉狭窄程度呈正相关性(r=0.852,P<0.05)。结论颈动脉超声评估与MSCTA评估在诊断冠心病冠脉硬化程度与狭窄程度时与CAG金标准有较好的一致性,有助于提高冠心病的诊断价值,且其具有无创、简单便捷、重复性高等优点,值得临床推广使用。 展开更多

目的探讨经颅多普勒超声(TCD)对颈内动脉重度狭窄引起大脑中动脉血流动力学变化及侧支循环的评估价值。方法选取2020年10月至2022年10月在洛阳市中心医院就诊的96例颈内动脉狭窄患者作为研究对象,根据数字减影血管造影(DSA)结果分为轻... 目的探讨经颅多普勒超声(TCD)对颈内动脉重度狭窄引起大脑中动脉血流动力学变化及侧支循环的评估价值。方法选取2020年10月至2022年10月在洛阳市中心医院就诊的96例颈内动脉狭窄患者作为研究对象,根据数字减影血管造影(DSA)结果分为轻度狭窄、中度狭窄、重度狭窄,采用TCD测量大脑中动脉血流动力学参数[收缩期峰值血流速度(Vs)、搏动指数(PI)、脑血流储备能力(CVR)],分析大脑中动脉血流动力学参数与颈内动脉狭窄程度相关性及评估价值,并比较有无侧支循环开放患者大脑中动脉血流动力学参数及DSA、TCD对侧支循环开放检出率。结果不同颈内动脉狭窄程度患者Vs、PI、CVR比较:重度狭窄<中度狭窄<轻度狭窄(P<0.05);经相关性分析显示,大脑中动脉血流动力学参数Vs、PI、CVR与颈内动脉狭窄程度均呈负相关(P<0.05);Vs、PI、CVR对轻度与中度颈内动脉狭窄诊断曲线下面积(AUC)分别为0.798、0.763、0.854,联合诊断AUC为0.893,对轻度、中度与重度颈内动脉狭窄诊断AUC分别为0.832、0.795、0.856,联合诊断AUC为0.927;与无侧支循环患者比较,有侧支循环患者Vs、PI、CVR较高(P<0.05);TCD、DSA对于侧支循环开放检出率比较,差异无统计学意义(P>0.05)。结论TCD可评估颈内动脉重度狭窄引起的大脑中动脉血流动力学变化,为临床早期诊断颈内动脉狭窄程度、侧支循环建立提供参考,以针对性制定治疗方案,改善预后。 展开更多
