We report the case of a 29 years old male patient with Gardner’s syndrome who presented with no intestinal polyps but instead had an osteoma on the left side of the mandible and multiple impacted teeth. The case showed the possibility to manage the multiple impacted teeth without jeopardizing the health of the teeth in terms of mobility or significant root resorption taking into consideration the increased bone density in such patients.展开更多

The suspicion of Gardner's syndrome was raised because extirpation of an osteoma of the left temporo-occipital region was made 10 years ago. Restorative procto-colectomy and ileal pouch anal anastomosis was made but histology delineated adenocarcinoma of the rectum (Dukes C stage). We conclude that cranial osteomas often precede gastrointestinal manifestations of familial adenomatous polyposis or Gardner's syndrome and such patients should be evaluated with genetic testing followed by colonoscopy if results are positive to prevent the development of colorectal carcinoma. If the diagnosis is positive all family members should be evaluated for familial adenomatous polyposis.展开更多

All gastrointestinal s...Gardner's syndrome(GS) is a rare syndrome with autosomal dominant inheritance, which is characterized by multiple intestinal polyps, dental anomalies, desmoid tumors, and soft tissue tumors. All gastrointestinal symptoms seen in GS are associated with the underlying familial adenomatosis polyposis and abdominal desmoid tumors, with the most common symptoms being anemia, lower gastrointestinal bleeding, abdominal pain, diarrhea, obstruction, and mucous defecation. To our best knowledge, no case of GS that has presented with gastrointestinal perforation and bleeding has ever been reported in the English language medical literature. A 37-year-old male who had been diagnosed with GS five years earlier was referred to our clinic for lower gastrointestinal bleeding. Despite the absence of a bleeding focus on conventional angiography, the patient was operated on with laparotomy, due to the persistence of both signs and symptoms of mild peritonitis. On the laparotomy, the patient was noted to have areas of perforation in the duodenum, splenic flexura, and midrectum. The third and fourth part of the duodenum, the proximal 15 cm segment of the jejunum, a 10 cm segment of the terminal ileum, the whole colon, and the upper and middle rectum were resected, and duodenojejunal side-to-side anastomosis and terminal ileostomy were performed. The histopathological analysis of the large mass measuring 30 cm × 20 cm was reported as a desmoid tumor. The pathological examination of the tumor foci detected in the colonic specimen revealed poorly differentiated adenosquamous carcinoma.展开更多

This study is based on S-wave velocity analysis of density estimation of Meyal Oil Field(Potwar Plateau, Pakistan), which reveals a very high correlation(R=0.87) between predicted and actual density. Since the availability of S-wave velocity(Vs) data increases, it’s highly appreciable to take into account the contribution of Vs in improvement of density estimates. This investigation only focuses on driving new constants(a = 1.92 and b = 0.186) for Gardner’s relationship to estimate density of pure carbonates(Vsh< 10%) in the study area, where the traditional model gave poorer density predictions. The proposed empirical expression with locally extracted unknowns from well log data proved its ability in terms of density prediction(showing very close agreement with measured densities) and best fitting the datasets at low as well as high density values when compared with Gardner’s equation. Based on our analysis, the exploratory study neither intends to replace the well established density estimation models nor to extend information over what information can possibly be extractable from Vp. However, our research does stimulate the growing use of Vs and importance of local unknown constants especially in carbonates reservoir interval to avoid misuse of most widely used Gardner relation in all geological settings.展开更多

It has features: 1, it belongs to certain type;2, it emphases the effect of buoyancy in equilibrium and potential energy, and the Archimedes principle of buoyancy is firstly extended to lateral directions based on logical deduction, the phenomenon of stirring a glass of oil-water mixture and the recorded of Hurricane Isabel (2003) from space station. It is characterized by a strictly local evolution with an infiltration and recidivism tendancy without giving metastasis. The treatment is basically surgical. We report here-with an observed case of 13-year-old girl, presenting with a huge tumor located in the left breast with ulceration of the nipple-areolar plate. The diagnosis is confirmed by the histological study. The treatment consisted of a mastectomy. Suites are simple. In view of the clinical evidence of the literature, we call back the difficulties of management of this tumor.展开更多

Histologically, it corresponds to a benign fibroblastic proliferation of a local evolution, with a high recurrence potential. We report the first case of elderly woman presented with desmoid-tumor of the breast as a manifestation of Gardner’s syndrome. A brief literature review was provided. We report the case of a 72-year-old woman who presented with five months history of painful and an exophytic mass of her left breast. She is known to have familial polyposis and had a total colectomy with Gardner’s syndrome. She had a history of osteomas of the maxilla. On clinical examination, there was an exophytic painless mass, on the upper medial quadrant of the breast. She had also lipoma in the left leg and pigmented skin lesions in legs. Her mammograms showed a suspicious stellar image. A computed tomography scan showed an ovoid lesion. Excision of the lesion was performed. Histopathology confirms a desmoid-tumor of the breast. Evolution was marked by lesion recurrence. In conclusion, desmoid tumor of the breast is rare, non-metastatic but locally aggressive. Clinical expression is often nonspecific. Treatment remains controversial;surgical excision is the treatment of choice.展开更多
