BACKGROUND Vertebral artery dissection(VAD)is a rare but life-threatening condition characterized by tearing of the intimal layer of the vertebral artery,leading to stenosis,occlusion or rupture.The clinical presentat...BACKGROUND Vertebral artery dissection(VAD)is a rare but life-threatening condition characterized by tearing of the intimal layer of the vertebral artery,leading to stenosis,occlusion or rupture.The clinical presentation of VAD can be heterogeneous,with common symptoms including headache,dizziness and balance problems.Timely diagnosis and treatment are crucial for favorable outcomes;however,VAD is often missed due to its variable clinical presentation and lack of robust diagnostic guidelines.High-resolution magnetic resonance imaging(HRMRI)has emerged as a reliable diagnostic tool for VAD,providing detailed visualization of vessel wall abnormalities.CASE SUMMARY A young male patient presented with an acute onset of severe headache,vomiting,and seizures,followed by altered consciousness.Imaging studies revealed bilateral VAD,basilar artery thrombosis,multiple brainstem and cerebellar infarcts,and subarachnoid hemorrhage.Digital subtraction angiography(DSA)revealed vertebral artery stenosis but failed to detect the dissection,potentially because intramural thrombosis obscured the VAD.In contrast,HRMRI confirmed the diagnosis by revealing specific signs of dissection.The patient was managed conservatively with antiplatelet therapy and other supportive measures,such as blood pressure control and pain management.After 5 mo of rehabilitation,the patient showed significant improvement in swallowing and limb strength.CONCLUSION HR-MRI can provide precise evidence for the identification of VAD.展开更多

As radiation-free al...Inflammatory bowel diseases(IBD) represent challenges, both from a diagnostic, and therapeutic point of view. Deep-seated anatomic structures are difficult to assess by ultrasound technique alone. As radiation-free alternative cross-sectional imaging method, magnetic resonance imaging of the intestinal structures is costly and time-consuming. Examination of pediatric patients imply additional considerations: reduction of body motions in younger children and consideration of the most appropriate preparation, and examination technique. The demanding Sellink technique is the only means for appropriately distending the lesser intestine in order to detect small bowel strictures. Oral intake of contrast medium(CM) alone shows its limitations regarding distensibility. The need for intravenous contrast media application needs to be considered, too. Active inflammation of both intestinal wall, and mesentery can be demonstrated accurately. Nevertheless, viable alternatives to CM application is desirable, considering non-negligible adverse reactions. Recent data suggest diffusion weighted imaging might fill this diagnostic gap. Irrespective of sequence technique chosen, bowel movement remains a major obstacle. Antispasmolytics in their function as smooth muscle relaxants help in improving image quality, however, their use in children might be off-label. Optimal preparation for the examination and appropriate imaging technique allow for diagnosing typical patterns of changes in IBD, such as bowel wall thickening, ulcers, mural stratification, strictures, creeping fat, and comb sign, and lymphadenopathy. The article gives a detailed overview of current significance of magnetic resonance imaging pediatric patients suffering from IBD, considering indications, limitations, and safety aspects.展开更多

Axial MRI plain scans cent...The present study examined 24 children with acute Guillain-Barre syndrome using magnetic resonance imaging (MRI) plain scans and fat-suppressed enhanced Tl-weighted imaging (T1WI) scans. Axial MRI plain scans centering on the medullary conus were positive in nine patients (38%). These displayed variable thickening involving the cauda equina with isointensity on T1WI and isointensity or slight hyperintensity on T2WI. False negatives were obtained in patients with cervical and cranial nerve symptoms. Contrast enhancement of T1WI with fat suppression was positive in all patients in the cauda equina with varied thickening and enhancement centering on the medullary conus. Five patients (36%) were positive in the cervical nerves and 3 patients (50%) were positive in the cranial nerves. These patients had corresponding cervical and cranial nerve symptoms, respectively. Patients with serious clinical symptoms in the lower limbs exhibited obvious involvement of the cauda equina by MRI. Statistical analysis revealed a positive correlation between the extent of enlargement of the cauda equina, centering on the medullary conus, and cerebrospinal fluid protein concentration.展开更多

METHODS: Spin-echo sequence(SE),fast spin-echo seque...AIM: To describe the imaging features of hepatic mesenchymal hamartoma with emphasis on magnetic resonance imaging (MRI) compared to histopathologic results. METHODS: Spin-echo sequence(SE),fast spin-echo sequence(FSE) were detected in 12 children (7 males,5 females) with mesenchymal hamartoma of the liver (IVlHL), aged 1.2 months to 12 years,( mean age, 6.3 years) by axial, saggital, coronary plain imaging with an Elscint 2.0T MR equipment. Their main symptoms were abdominal mass (5 cases), enlarged liver (8 cases), abdominal pain (1 case) and anemia (2 cases), and negative alpha-fetoprotein. Dynamic enhancement examination was added in 2 cases. RESULTS: Six cases had single mass type of MHL, in which 3 cases had solid masses showing slight low- signal-intensity in TlWI, and irregular high-signal- intensity in T2WI, 1 case had a cystic-solid mixed mass showing several border-clear cysts in a solid mass, 2 cases had cystic masses with multi-septa. Five cases had diffuse and multifocal lesions type of MHL with its signal intensity being similar to that of the solid mass. One case had a combined diffuse and single cystic mass. In the early dynamic enhancement examination, the lesions were slightly circum-enhanced , and the center was enhanced in the later scan images. Inner hepatic vessels were compressed in 5 cases, vena cava and abdominal aortae were compressed in 3 cases. Pathological findings included fiber hyperplasia, hyaline degeneration, biliary duct hyperplasia, Iobule-like array. CONCLUSION: MR imaging is a better way to differentiate and diagnose MHL. MHL may be recognized by its characteristic occurrence in infancy and MR imaging features.展开更多
Intracranial incidental findings on magnetic resonance imaging(MRI) of the brain continue to generate interest in healthy control,research,and clinical subjects.However,in clinical practice,the discovery of incidental...Intracranial incidental findings on magnetic resonance imaging(MRI) of the brain continue to generate interest in healthy control,research,and clinical subjects.However,in clinical practice,the discovery of incidental findings acts as a "distractor".This review is based on existing heterogeneous reports,their clinical implications,and how the results of incidental findings influence clinical management.This draws attention to the followings:(1) the prevalence of clinically significant incidental findings is low;(2) there is a lack of a systematic approach to classification;and discusses(3) how to deal with the detected incidental findings based a proposed common clinical profile.Individualized neurological care requires an active discussion regarding the need for neuroimaging.Clinical significance of incidental findings should be decided based on lesion's neuroradiologic characteristics in the given clinical context.Available evidence suggests that the outcome of an incidentally found "serious lesion in children" is excellent.Future studies of intracranial incidental findings on pediatric brain MRI should be focused on a homogeneous population.The study should address this clinical knowledge based review powered by the statistical analyses.展开更多
BACKGROUND Intracranial atherosclerosis,a leading cause of stroke,involves arterial plaque formation.This study explores the link between plaque remodelling patterns and diabetes using high-resolution vessel wall imag...BACKGROUND Intracranial atherosclerosis,a leading cause of stroke,involves arterial plaque formation.This study explores the link between plaque remodelling patterns and diabetes using high-resolution vessel wall imaging(HR-VWI).AIM To investigate the factors of intracranial atherosclerotic remodelling patterns and the relationship between intracranial atherosclerotic remodelling and diabetes mellitus using HR-VWI.METHODS Ninety-four patients diagnosed with middle cerebral artery or basilar artery INTRODUCTION Intracranial atherosclerotic disease is one of the main causes of ischaemic stroke in the world,accounting for approx-imately 10%of transient ischaemic attacks and 30%-50%of ischaemic strokes[1].It is the most common factor among Asian people[2].The adaptive changes in the structure and function of blood vessels that can adapt to changes in the internal and external environment are called vascular remodelling,which is a common and important pathological mechanism in atherosclerotic diseases,and the remodelling mode of atherosclerotic plaques is closely related to the occurrence of stroke.Positive remodelling(PR)is an outwards compensatory remodelling where the arterial wall grows outwards in an attempt to maintain a constant lumen diameter.For a long time,it was believed that the degree of stenosis can accurately reflect the risk of ischaemic stroke[3-5].Previous studies have revealed that lesions without significant luminal stenosis can also lead to acute events[6,7],as summarized in a recent meta-analysis study in which approximately 50%of acute/subacute ischaemic events were due to this type of lesion[6].Research[8,9]has pointed out that the PR of plaques is more dangerous and more likely to cause acute ischaemic stroke.Previous studies[10-13]have found that there are specific vascular remodelling phenomena in the coronary and carotid arteries of diabetic patients.However,due to the deep location and small lumen of intracranial arteries and limitations of imaging techniques,the relationship between intracranial arterial remodelling and diabetes is still unclear.In recent years,with the development of magnetic resonance technology and the emergence of high-resolution(HR)vascular wall imaging,a clear and multidimensional display of the intracranial vascular wall has been achieved.Therefore,in this study,HR wall imaging(HR-VWI)was used to display the remodelling characteristics of bilateral middle cerebral arteries and basilar arteries and to explore the factors of intracranial vascular remodelling and its relationship with diabetes.展开更多

Data Sources: This review was based on the data in articles published between 2005 and 2015, which were obtained from PubMed. The keywords included HR-MRI, MMD, ICAD, and intracranial artery diseases. Study Selection: Articles related to HR-MRI for MMD or other intracranial artery diseases were selected for review. Results: There are differences between the characteristic patterns of HR-MRI in MMD and ICAD. MMD is associated with inward remodeling, smaller outer diameters, concentric occlusive lesions and homogeneous signal intensity, while ICAD is more likely to be associated with outward remodeling, normal outer diameters, eccentric occlusive lesions, and heterogeneous signal intensity. Other intracranial artery diseases, such as dissection and vasculitis, also have distinctive characteristics in HR-MRI. HR-MRI may become a useful tool for the differential diagnosis of MMD in the future. Conclusions: HR-MRI of MMD provides a more in-depth understanding of MMD, and it is helpful in evaluating pathological changes in the vessel wall and in differentiating MMD from other intracranial artery steno-occlusive diseases, particularly ICAD.以展开更多
In vivo studies on association between wall shear stress(WSS)and intracranial plaque are deficient.Based on the three-dimensional T1-weighted high-resolution magnetic resonance imaging(3DT1 HR-MRI)data of patients wit...In vivo studies on association between wall shear stress(WSS)and intracranial plaque are deficient.Based on the three-dimensional T1-weighted high-resolution magnetic resonance imaging(3DT1 HR-MRI)data of patients with low-grade stenotic(<50%)atherosclerotic middle cerebral artery(MCA)and subjects with normal MCA,we built a three-dimensional reconstructed WSS model by computational fluid dynamics(CFD)technique.Three-dimensional registration of the CFD model to the HR-MRI was performed with projections based on the resolution and thickness of the images.The relationships between the Wss at each side of the vessel wall and plaque location were analyzed.A total of 94 MCA plaques from 43 patients and 50 normal MCAs were analyzed.In the normal MCAs,WSS was lower at the ventral-inferior wall than at the dorsal-superior wall(proximal segment,p<0.001;middle segment,p<0.001)and lower at the inner wall than at the outer wall of the MCA curve(p<0.001).In atherosclerotic MCAs,similar low Wss regions were observed where plaques developed.The WSS ratio of the ventral-inferior wall to the dorsal-superior wall in atherosclerotic MCAs was lower than that in normal MCAs(p=0.002).The WSS_(inmer-outer)ratio in atherosclerotic MCAs was lower than that in normal MCAs(p=0.002).Low WSS was associated with MCA atherosclerosis formation and occurred mainly at the ventral inferior wall,which was anatomically opposite the orifices of penetrating arteries,and at the inner wall of the MCA curve.Overall,the results were well consistent with the low WSS theory in atherosclerosis formation.The reconstructed WSS model is a promising novel method for assessing an individualized vascular profile once validated by further studies.展开更多
Background Myocarditis is a common,potentially life-threatening disease that presents a wide rang of symptoms in children,as an important underlying etiology of other myocardial diseases such as dilated and arrhythmog...Background Myocarditis is a common,potentially life-threatening disease that presents a wide rang of symptoms in children,as an important underlying etiology of other myocardial diseases such as dilated and arrhythmogenic right ventricular cardiomyopathy.The incidence of nonfatal myocarditis is probably greater than that of the one actually diagnosed,which is the result of the challenges of establishing the diagnosis in standard clinical settings.Currently,no single clinical or imaging finding confirms the diagnosis of myocarditis with absolute certainty.Historically,clinical exam,electrocardiogram (ECG),serology and echocardiography had an unsatisfactory diagnostic accuracy in myocarditis.Endomyocardial biopsy remains as a widely accepted standard,but may not be suitable for every patient,especially for those with less severe disease.Our aim was to find the changes in cardiovascular magnetic resonance (CMR) imaging of children with myocarditis diagnosed by clinical criteria.Methods We studied 25 children (18 male,7 female; aged from 5-17 years) with diagnosed myocarditis by clinical criteria.CMR included function analyses,T2-weighted imaging,T1-weighted imaging before and after i.v.gadolinium injection (early gadolinium enhancement (EGE) and late gadolinium enhancement (LGE)).Results The T2 ratio was elevated in 21 children (84%,11 in anterolateral (44%),5 in inferolateral (20%),and 5 in septum (20%)),EGE was present in 9 children (36%,3 in anterolateral (12%),4 in inferolateral (20%),and 2 in septum (8%)),and LGE was present in 5 children (20%,2 in anterolateral (8%),1 in inferolateral (4%),1 in septum (4%),and 1 in midwall of left ventricular (LV) wall).In 9 children (36%),two (or more) out of three sequences (T2,EGE,LGE) were abnormal.Conclusions The CMR findings in children with clinically diagnosed myocarditis vary within the groups,including regional or global myocardial signal increase in T2-weighted images,EGE and LGE in T1-weighted images.The T2 ratio elevation is the most common CMR finding.Children with mild cardiac symptoms may also appear serious myocardial injuries.展开更多

Date Sources: We retrieved information from PubMed database up to December 2015, using various search terms including vessel wall imaging (VWI), high-resolution magnetic resonance imaging, intracranial arterial stenosis, black blood, and intracranial atherosclerosis. Study Selection: We reviewed peer-reviewed articles printed in English on imaging technique of VWI and characteristic findings of various intracranial vasculopathies on VWI. We organized this data to explain the value of VWI in clinical application. Results: VWI with black blood technique could provide high-quality images with submillimeter voxel size, and display both the vessel wall and lumen of intracranial artery simultaneously. Various intracranial vasculopathies (atherosclerotic or nonatherosclerotic) had differentiating features including pattern of wall thickening, enhancement, and vessel remodeling on VWI. This technique could be used for determining causes of stenosis, identification of stroke mechanism, risk-stratifying patients, and directing therapeutic management in clinical practice. In addition, a new morphological classification based on VWI could be established for predicting the efficacy of endovascular therapy. Conclusions: This review highlights the value of HRMR VWI for discrimination of different intracranial vasculopathies and directing therapeutic management.展开更多

Liver biopsy is the reference standard to diagnose NAFLD but invasive, thus it is not the best choice in clinical diagnosis and follow-up.Magnetic resonance(MR) is widely used in clinical trials to noninvasively quantify liver fat content in adults and children in foreign countries. While currently, it is rarely used in Chinese children and adolescents. We postulated that quantifying hepatic steatosis by MR could be extended to children and adolescents in China.AIM To investigate the accuracy of MR imaging(MRI) in quantifying liver fat with MR spectroscopy(MRS) as a reference. A secondary goal was to assess the prevalence of NAFLD in overweight and obese Chinese children and adolescents.METHODSThere were 86 children and adolescents enrolled in this study, including 65 overweight and obese children and 21 healthy children. The participants underwent MRI and MRS. MRI and MRS were performed using multi-echo Dixon and HISTO sequences, respectively, to calculate hepatic proton density fat fraction(PDFF). Hepatic steatosis was diagnosed using MRS-PDFF > 5% as the threshold. Spearman's analysis was used to evaluate the correlation between MRI and MRS. The agreement between these two methods was assessed by BlandAltman analysis.RESULTS The MRI-PDFF in the MRS region of interest and the entire liver was 9.9% ±10.3% with a range of 0.3%-39.9%, and 10.6% ± 9.4% with a range of 1.9%-38.9%,respectively. The MRS-PDFF was 9.1% ± 10.0%, with a range of 0.5%-37.8%. The incidence of hepatic steatosis detected by MRS-PDFF was 46.5%(40/86) of all participants, all of whom belonged to the overweight and obese group.Spearman's analysis indicated an excellent correlation between multi-echo Dixon and MRS(r > 0.9, P < 0.01). Bland-Altman analysis also demonstrated a good agreement between these two methods.CONCLUSION Multi-echo Dixon shows an excellent correlation and agreement with MRS in quantifying liver fat content and could be a potential tool to detect hepatic steatosis in Chinese children and adolescents.展开更多

Methods: W...Objective: Our study objective was to compare the anteroposterior (AP) oropharyngeal airway diameter measured using static MRI images in children with and without developmental disabilities during sedation. Methods: We performed a secondary analysis on a prospective, observational age-matched, 1:2 case-control study of children aged 3 - 10 years who were sedated for brain MRI. AP airway diameters were determined at the level of soft palate (airway_SP) and tongue (airway_T) from midline sagittal images by a single blinded radiologist. Results: Airway diameter was evaluated in 134 controls (C) and 68 cases (D). There was no difference in the overall adverse events (D: 30% C: 32.9%;p = 0.7) as well as hypoxia (D: 10%;C: 9.3%, p = 0.9). There was no significant difference in mean airway_T (D: 10.62 ± 3.48 vs. C: 10.38 ± 3.47;p = 0.64) and airway_SP diameter (D: 4.59 ± 2.36 vs. C: 5.17 ± 3.9;p = 0.14) between the two groups. There was no significant difference in airway_T and airway_SP diameters amongst those who experienced and those who did not experience hypoxia in cases. Amongst controls however, the airway_SP was smaller in those who experienced hypoxia compared to those who did not experience hypoxia. Conclusions: There was no difference in the AP airway_T and airway_SP diameter during sedation using static MRI images in children with and without developmental disabilities. Changes in airway_SP was associated with hypoxia amongst controls in our study cohort.展开更多
BACKGROUND Congenital heart disease(CHD)is a cardiovascular malformation caused by abnormal heart and/or vascular development in the fetus.In children with CHD,abnormalities in the development and function of the nerv...BACKGROUND Congenital heart disease(CHD)is a cardiovascular malformation caused by abnormal heart and/or vascular development in the fetus.In children with CHD,abnormalities in the development and function of the nervous system are common.At present,there is a lack of research on the preoperative neurological development and injury in young children with non-cyanotic CHD.AIM To determine the changes in white matter,gray matter,and cerebrospinal fluid(CSF)by magnetic resonance imaging(MRI)in children with non-cyanotic CHD as compared with healthy controls.METHODS Children diagnosed with non-cyanotic CHD on ultrasonography(n=54)and healthy control subjects(n=35)were included in the study.All the subjects were aged 1-3 years.Brain MRI was performed prior to surgery for CHD.The SPM v12 software was used to calculate the volumes of the gray matter,white matter,CSF,and the whole brain(sum of the gray matter,white matter,and CSF volumes).Volume differences between the two groups were analyzed.Voxel-based morphometry was used to compare specific brain regions with statistically significant atrophy.RESULTS Compared with the control group,the study group had significantly reduced whole-brain white matter volume(P<0.05),but similar whole-brain gray matter,CSF,and whole-brain volumes(P>0.05).As compared with the healthy controls,children with non-cyanotic CHD had mild underdevelopment in the white matter of the anterior central gyrus,the posterior central gyrus,and the pulvinar.CONCLUSION Children with non-cyanotic CHD show decreased white matter volume before surgery,and this volume reduction is mainly concentrated in the somatosensory and somatic motor nerve regions.展开更多

目的探讨应用磁共振质子密度脂肪分数(MRI-PDFF)诊断超重和肥胖儿童非酒精性脂肪性肝炎(NASH)的效能。方法2022年12月~2023年12月江油市903医院诊治的超重/肥胖的非酒精性脂肪性肝病(NAFLD)儿童102例,其中超重儿童74例和肥胖儿童28例,使...目的探讨应用磁共振质子密度脂肪分数(MRI-PDFF)诊断超重和肥胖儿童非酒精性脂肪性肝炎(NASH)的效能。方法2022年12月~2023年12月江油市903医院诊治的超重/肥胖的非酒精性脂肪性肝病(NAFLD)儿童102例,其中超重儿童74例和肥胖儿童28例,使用3.0 T MRI扫描仪行肝脏检查,获取MRI-PDFF,使用FibroScan诊断仪检查获得肝脏硬度检测(LSM)和受控衰减参数(CAP),行肝穿刺诊断NASH,绘制受试者工作特征曲线(ROC),计算曲线下面积(AUC),评估诊断效能。结果在102例超重/肥胖的NAFLD儿童中,经组织学检查诊断单纯性非酒精性脂肪肝(SNFL)75例和NASH 27例;NASH组BMI、血清ALT、AST、TG、MRI-PDFF、LSM和CAP分别为(32.2±3.4)kg/m^(2)、(92.7±31.3)U/L、(96.8±34.1)U/L、(3.1±1.2)mmol/L、(12.5±4.7)%、(11.7±1.4)kPa和(370.4±146.9)dB/m,与SNFL组【分别为(26.7±2.2)kg/m^(2)、(38.2±4.4)U/L、(33.0±3.4)U/L、(2.3±0.7)mmol/L、(7.7±2.0)%、(6.2±1.0)kPa和(262.3±61.2)dB/m】比,差异具有统计学意义(P<0.05),NASH组超重和肥胖占比分别37.0%和63.0%,与SNFL组的85.3%和14.7%比,差异具有统计学意义(P<0.05);经ROC曲线分析显示,以MRI-PDFF为11.9%为截断点,其诊断NASH的AUC为0.87(95%CI为0.80~0.93),敏感性为81.5%,特异性为85.3%,而分别以LSM为10.8 kPa和CAP为310.5dB/m为截断点,其诊断的AUC为0.81(95%CI为0.78~0.89),敏感性为85.2%,特异性为58.7%,后者的特异性较差。结论应用MRI-PDFF诊断儿童NASH的效能较优,但考虑到临床的便捷性,使用弹性成像检测诊断也可获得初步结果,以便必要时进一步检查。展开更多

文摘BACKGROUND Vertebral artery dissection(VAD)is a rare but life-threatening condition characterized by tearing of the intimal layer of the vertebral artery,leading to stenosis,occlusion or rupture.The clinical presentation of VAD can be heterogeneous,with common symptoms including headache,dizziness and balance problems.Timely diagnosis and treatment are crucial for favorable outcomes;however,VAD is often missed due to its variable clinical presentation and lack of robust diagnostic guidelines.High-resolution magnetic resonance imaging(HRMRI)has emerged as a reliable diagnostic tool for VAD,providing detailed visualization of vessel wall abnormalities.CASE SUMMARY A young male patient presented with an acute onset of severe headache,vomiting,and seizures,followed by altered consciousness.Imaging studies revealed bilateral VAD,basilar artery thrombosis,multiple brainstem and cerebellar infarcts,and subarachnoid hemorrhage.Digital subtraction angiography(DSA)revealed vertebral artery stenosis but failed to detect the dissection,potentially because intramural thrombosis obscured the VAD.In contrast,HRMRI confirmed the diagnosis by revealing specific signs of dissection.The patient was managed conservatively with antiplatelet therapy and other supportive measures,such as blood pressure control and pain management.After 5 mo of rehabilitation,the patient showed significant improvement in swallowing and limb strength.CONCLUSION HR-MRI can provide precise evidence for the identification of VAD.
文摘Inflammatory bowel diseases(IBD) represent challenges, both from a diagnostic, and therapeutic point of view. Deep-seated anatomic structures are difficult to assess by ultrasound technique alone. As radiation-free alternative cross-sectional imaging method, magnetic resonance imaging of the intestinal structures is costly and time-consuming. Examination of pediatric patients imply additional considerations: reduction of body motions in younger children and consideration of the most appropriate preparation, and examination technique. The demanding Sellink technique is the only means for appropriately distending the lesser intestine in order to detect small bowel strictures. Oral intake of contrast medium(CM) alone shows its limitations regarding distensibility. The need for intravenous contrast media application needs to be considered, too. Active inflammation of both intestinal wall, and mesentery can be demonstrated accurately. Nevertheless, viable alternatives to CM application is desirable, considering non-negligible adverse reactions. Recent data suggest diffusion weighted imaging might fill this diagnostic gap. Irrespective of sequence technique chosen, bowel movement remains a major obstacle. Antispasmolytics in their function as smooth muscle relaxants help in improving image quality, however, their use in children might be off-label. Optimal preparation for the examination and appropriate imaging technique allow for diagnosing typical patterns of changes in IBD, such as bowel wall thickening, ulcers, mural stratification, strictures, creeping fat, and comb sign, and lymphadenopathy. The article gives a detailed overview of current significance of magnetic resonance imaging pediatric patients suffering from IBD, considering indications, limitations, and safety aspects.
文摘The present study examined 24 children with acute Guillain-Barre syndrome using magnetic resonance imaging (MRI) plain scans and fat-suppressed enhanced Tl-weighted imaging (T1WI) scans. Axial MRI plain scans centering on the medullary conus were positive in nine patients (38%). These displayed variable thickening involving the cauda equina with isointensity on T1WI and isointensity or slight hyperintensity on T2WI. False negatives were obtained in patients with cervical and cranial nerve symptoms. Contrast enhancement of T1WI with fat suppression was positive in all patients in the cauda equina with varied thickening and enhancement centering on the medullary conus. Five patients (36%) were positive in the cervical nerves and 3 patients (50%) were positive in the cranial nerves. These patients had corresponding cervical and cranial nerve symptoms, respectively. Patients with serious clinical symptoms in the lower limbs exhibited obvious involvement of the cauda equina by MRI. Statistical analysis revealed a positive correlation between the extent of enlargement of the cauda equina, centering on the medullary conus, and cerebrospinal fluid protein concentration.
文摘AIM: To describe the imaging features of hepatic mesenchymal hamartoma with emphasis on magnetic resonance imaging (MRI) compared to histopathologic results. METHODS: Spin-echo sequence(SE),fast spin-echo sequence(FSE) were detected in 12 children (7 males,5 females) with mesenchymal hamartoma of the liver (IVlHL), aged 1.2 months to 12 years,( mean age, 6.3 years) by axial, saggital, coronary plain imaging with an Elscint 2.0T MR equipment. Their main symptoms were abdominal mass (5 cases), enlarged liver (8 cases), abdominal pain (1 case) and anemia (2 cases), and negative alpha-fetoprotein. Dynamic enhancement examination was added in 2 cases. RESULTS: Six cases had single mass type of MHL, in which 3 cases had solid masses showing slight low- signal-intensity in TlWI, and irregular high-signal- intensity in T2WI, 1 case had a cystic-solid mixed mass showing several border-clear cysts in a solid mass, 2 cases had cystic masses with multi-septa. Five cases had diffuse and multifocal lesions type of MHL with its signal intensity being similar to that of the solid mass. One case had a combined diffuse and single cystic mass. In the early dynamic enhancement examination, the lesions were slightly circum-enhanced , and the center was enhanced in the later scan images. Inner hepatic vessels were compressed in 5 cases, vena cava and abdominal aortae were compressed in 3 cases. Pathological findings included fiber hyperplasia, hyaline degeneration, biliary duct hyperplasia, Iobule-like array. CONCLUSION: MR imaging is a better way to differentiate and diagnose MHL. MHL may be recognized by its characteristic occurrence in infancy and MR imaging features.
文摘Intracranial incidental findings on magnetic resonance imaging(MRI) of the brain continue to generate interest in healthy control,research,and clinical subjects.However,in clinical practice,the discovery of incidental findings acts as a "distractor".This review is based on existing heterogeneous reports,their clinical implications,and how the results of incidental findings influence clinical management.This draws attention to the followings:(1) the prevalence of clinically significant incidental findings is low;(2) there is a lack of a systematic approach to classification;and discusses(3) how to deal with the detected incidental findings based a proposed common clinical profile.Individualized neurological care requires an active discussion regarding the need for neuroimaging.Clinical significance of incidental findings should be decided based on lesion's neuroradiologic characteristics in the given clinical context.Available evidence suggests that the outcome of an incidentally found "serious lesion in children" is excellent.Future studies of intracranial incidental findings on pediatric brain MRI should be focused on a homogeneous population.The study should address this clinical knowledge based review powered by the statistical analyses.

文摘BACKGROUND Intracranial atherosclerosis,a leading cause of stroke,involves arterial plaque formation.This study explores the link between plaque remodelling patterns and diabetes using high-resolution vessel wall imaging(HR-VWI).AIM To investigate the factors of intracranial atherosclerotic remodelling patterns and the relationship between intracranial atherosclerotic remodelling and diabetes mellitus using HR-VWI.METHODS Ninety-four patients diagnosed with middle cerebral artery or basilar artery INTRODUCTION Intracranial atherosclerotic disease is one of the main causes of ischaemic stroke in the world,accounting for approx-imately 10%of transient ischaemic attacks and 30%-50%of ischaemic strokes[1].It is the most common factor among Asian people[2].The adaptive changes in the structure and function of blood vessels that can adapt to changes in the internal and external environment are called vascular remodelling,which is a common and important pathological mechanism in atherosclerotic diseases,and the remodelling mode of atherosclerotic plaques is closely related to the occurrence of stroke.Positive remodelling(PR)is an outwards compensatory remodelling where the arterial wall grows outwards in an attempt to maintain a constant lumen diameter.For a long time,it was believed that the degree of stenosis can accurately reflect the risk of ischaemic stroke[3-5].Previous studies have revealed that lesions without significant luminal stenosis can also lead to acute events[6,7],as summarized in a recent meta-analysis study in which approximately 50%of acute/subacute ischaemic events were due to this type of lesion[6].Research[8,9]has pointed out that the PR of plaques is more dangerous and more likely to cause acute ischaemic stroke.Previous studies[10-13]have found that there are specific vascular remodelling phenomena in the coronary and carotid arteries of diabetic patients.However,due to the deep location and small lumen of intracranial arteries and limitations of imaging techniques,the relationship between intracranial arterial remodelling and diabetes is still unclear.In recent years,with the development of magnetic resonance technology and the emergence of high-resolution(HR)vascular wall imaging,a clear and multidimensional display of the intracranial vascular wall has been achieved.Therefore,in this study,HR wall imaging(HR-VWI)was used to display the remodelling characteristics of bilateral middle cerebral arteries and basilar arteries and to explore the factors of intracranial vascular remodelling and its relationship with diabetes.
文摘Objective: To introduce the imaging characteristics of moyamoya disease (MMD) using high-resolution magnetic resonance imaging (HR-MRI) and to discuss the role of HR-MRI in differentiating MMD from other intracranial artery diseases, especially intracranial atherosclerotic disease (ICAD). Data Sources: This review was based on the data in articles published between 2005 and 2015, which were obtained from PubMed. The keywords included HR-MRI, MMD, ICAD, and intracranial artery diseases. Study Selection: Articles related to HR-MRI for MMD or other intracranial artery diseases were selected for review. Results: There are differences between the characteristic patterns of HR-MRI in MMD and ICAD. MMD is associated with inward remodeling, smaller outer diameters, concentric occlusive lesions and homogeneous signal intensity, while ICAD is more likely to be associated with outward remodeling, normal outer diameters, eccentric occlusive lesions, and heterogeneous signal intensity. Other intracranial artery diseases, such as dissection and vasculitis, also have distinctive characteristics in HR-MRI. HR-MRI may become a useful tool for the differential diagnosis of MMD in the future. Conclusions: HR-MRI of MMD provides a more in-depth understanding of MMD, and it is helpful in evaluating pathological changes in the vessel wall and in differentiating MMD from other intracranial artery steno-occlusive diseases, particularly ICAD.以

文摘In vivo studies on association between wall shear stress(WSS)and intracranial plaque are deficient.Based on the three-dimensional T1-weighted high-resolution magnetic resonance imaging(3DT1 HR-MRI)data of patients with low-grade stenotic(<50%)atherosclerotic middle cerebral artery(MCA)and subjects with normal MCA,we built a three-dimensional reconstructed WSS model by computational fluid dynamics(CFD)technique.Three-dimensional registration of the CFD model to the HR-MRI was performed with projections based on the resolution and thickness of the images.The relationships between the Wss at each side of the vessel wall and plaque location were analyzed.A total of 94 MCA plaques from 43 patients and 50 normal MCAs were analyzed.In the normal MCAs,WSS was lower at the ventral-inferior wall than at the dorsal-superior wall(proximal segment,p<0.001;middle segment,p<0.001)and lower at the inner wall than at the outer wall of the MCA curve(p<0.001).In atherosclerotic MCAs,similar low Wss regions were observed where plaques developed.The WSS ratio of the ventral-inferior wall to the dorsal-superior wall in atherosclerotic MCAs was lower than that in normal MCAs(p=0.002).The WSS_(inmer-outer)ratio in atherosclerotic MCAs was lower than that in normal MCAs(p=0.002).Low WSS was associated with MCA atherosclerosis formation and occurred mainly at the ventral inferior wall,which was anatomically opposite the orifices of penetrating arteries,and at the inner wall of the MCA curve.Overall,the results were well consistent with the low WSS theory in atherosclerosis formation.The reconstructed WSS model is a promising novel method for assessing an individualized vascular profile once validated by further studies.
文摘Background Myocarditis is a common,potentially life-threatening disease that presents a wide rang of symptoms in children,as an important underlying etiology of other myocardial diseases such as dilated and arrhythmogenic right ventricular cardiomyopathy.The incidence of nonfatal myocarditis is probably greater than that of the one actually diagnosed,which is the result of the challenges of establishing the diagnosis in standard clinical settings.Currently,no single clinical or imaging finding confirms the diagnosis of myocarditis with absolute certainty.Historically,clinical exam,electrocardiogram (ECG),serology and echocardiography had an unsatisfactory diagnostic accuracy in myocarditis.Endomyocardial biopsy remains as a widely accepted standard,but may not be suitable for every patient,especially for those with less severe disease.Our aim was to find the changes in cardiovascular magnetic resonance (CMR) imaging of children with myocarditis diagnosed by clinical criteria.Methods We studied 25 children (18 male,7 female; aged from 5-17 years) with diagnosed myocarditis by clinical criteria.CMR included function analyses,T2-weighted imaging,T1-weighted imaging before and after i.v.gadolinium injection (early gadolinium enhancement (EGE) and late gadolinium enhancement (LGE)).Results The T2 ratio was elevated in 21 children (84%,11 in anterolateral (44%),5 in inferolateral (20%),and 5 in septum (20%)),EGE was present in 9 children (36%,3 in anterolateral (12%),4 in inferolateral (20%),and 2 in septum (8%)),and LGE was present in 5 children (20%,2 in anterolateral (8%),1 in inferolateral (4%),1 in septum (4%),and 1 in midwall of left ventricular (LV) wall).In 9 children (36%),two (or more) out of three sequences (T2,EGE,LGE) were abnormal.Conclusions The CMR findings in children with clinically diagnosed myocarditis vary within the groups,including regional or global myocardial signal increase in T2-weighted images,EGE and LGE in T1-weighted images.The T2 ratio elevation is the most common CMR finding.Children with mild cardiac symptoms may also appear serious myocardial injuries.
文摘Objective: To discuss the feasibility and clinical value of high-resolution magnetic resonance vessel wall imaging (HRMR VWI) for intracranial arterial stenosis. Date Sources: We retrieved information from PubMed database up to December 2015, using various search terms including vessel wall imaging (VWI), high-resolution magnetic resonance imaging, intracranial arterial stenosis, black blood, and intracranial atherosclerosis. Study Selection: We reviewed peer-reviewed articles printed in English on imaging technique of VWI and characteristic findings of various intracranial vasculopathies on VWI. We organized this data to explain the value of VWI in clinical application. Results: VWI with black blood technique could provide high-quality images with submillimeter voxel size, and display both the vessel wall and lumen of intracranial artery simultaneously. Various intracranial vasculopathies (atherosclerotic or nonatherosclerotic) had differentiating features including pattern of wall thickening, enhancement, and vessel remodeling on VWI. This technique could be used for determining causes of stenosis, identification of stroke mechanism, risk-stratifying patients, and directing therapeutic management in clinical practice. In addition, a new morphological classification based on VWI could be established for predicting the efficacy of endovascular therapy. Conclusions: This review highlights the value of HRMR VWI for discrimination of different intracranial vasculopathies and directing therapeutic management.
文摘The introduction of coronavirus disease-2019(COVID-19)as a global pandemic has contributed to overall morbidity and mortality.With a focus on understanding the immunology and pathophysiology of the disease,these features can be linked with the respective findings of imaging studies.Thus,the constellation between clinical presentation,histological,laboratory,immunological,and imaging results is crucial for the proper management of patients.The purpose of this article is to examine the role of imaging during the particular stages of severe acute respiratory syndrome coronavirus 2 infection–asymptomatic stage,typical and atypical COVID-19 pneumonia,acute respiratory distress syndrome,multiorgan failure,and thrombosis.The use of imaging methods to assess the severity and duration of changes is crucial in patients with COVID-19.Radiography and computed tomography are among the methods that allow accurate characterization of changes.

文摘BACKGROUND Nonalcoholic fatty liver disease(NAFLD) is currently the outstanding cause of chronic liver disease in children and adolescents, especially in overweight and obese groups. Liver biopsy is the reference standard to diagnose NAFLD but invasive, thus it is not the best choice in clinical diagnosis and follow-up.Magnetic resonance(MR) is widely used in clinical trials to noninvasively quantify liver fat content in adults and children in foreign countries. While currently, it is rarely used in Chinese children and adolescents. We postulated that quantifying hepatic steatosis by MR could be extended to children and adolescents in China.AIM To investigate the accuracy of MR imaging(MRI) in quantifying liver fat with MR spectroscopy(MRS) as a reference. A secondary goal was to assess the prevalence of NAFLD in overweight and obese Chinese children and adolescents.METHODSThere were 86 children and adolescents enrolled in this study, including 65 overweight and obese children and 21 healthy children. The participants underwent MRI and MRS. MRI and MRS were performed using multi-echo Dixon and HISTO sequences, respectively, to calculate hepatic proton density fat fraction(PDFF). Hepatic steatosis was diagnosed using MRS-PDFF > 5% as the threshold. Spearman's analysis was used to evaluate the correlation between MRI and MRS. The agreement between these two methods was assessed by BlandAltman analysis.RESULTS The MRI-PDFF in the MRS region of interest and the entire liver was 9.9% ±10.3% with a range of 0.3%-39.9%, and 10.6% ± 9.4% with a range of 1.9%-38.9%,respectively. The MRS-PDFF was 9.1% ± 10.0%, with a range of 0.5%-37.8%. The incidence of hepatic steatosis detected by MRS-PDFF was 46.5%(40/86) of all participants, all of whom belonged to the overweight and obese group.Spearman's analysis indicated an excellent correlation between multi-echo Dixon and MRS(r > 0.9, P < 0.01). Bland-Altman analysis also demonstrated a good agreement between these two methods.CONCLUSION Multi-echo Dixon shows an excellent correlation and agreement with MRS in quantifying liver fat content and could be a potential tool to detect hepatic steatosis in Chinese children and adolescents.
文摘Objective: Our study objective was to compare the anteroposterior (AP) oropharyngeal airway diameter measured using static MRI images in children with and without developmental disabilities during sedation. Methods: We performed a secondary analysis on a prospective, observational age-matched, 1:2 case-control study of children aged 3 - 10 years who were sedated for brain MRI. AP airway diameters were determined at the level of soft palate (airway_SP) and tongue (airway_T) from midline sagittal images by a single blinded radiologist. Results: Airway diameter was evaluated in 134 controls (C) and 68 cases (D). There was no difference in the overall adverse events (D: 30% C: 32.9%;p = 0.7) as well as hypoxia (D: 10%;C: 9.3%, p = 0.9). There was no significant difference in mean airway_T (D: 10.62 ± 3.48 vs. C: 10.38 ± 3.47;p = 0.64) and airway_SP diameter (D: 4.59 ± 2.36 vs. C: 5.17 ± 3.9;p = 0.14) between the two groups. There was no significant difference in airway_T and airway_SP diameters amongst those who experienced and those who did not experience hypoxia in cases. Amongst controls however, the airway_SP was smaller in those who experienced hypoxia compared to those who did not experience hypoxia. Conclusions: There was no difference in the AP airway_T and airway_SP diameter during sedation using static MRI images in children with and without developmental disabilities. Changes in airway_SP was associated with hypoxia amongst controls in our study cohort.
文摘BACKGROUND Congenital heart disease(CHD)is a cardiovascular malformation caused by abnormal heart and/or vascular development in the fetus.In children with CHD,abnormalities in the development and function of the nervous system are common.At present,there is a lack of research on the preoperative neurological development and injury in young children with non-cyanotic CHD.AIM To determine the changes in white matter,gray matter,and cerebrospinal fluid(CSF)by magnetic resonance imaging(MRI)in children with non-cyanotic CHD as compared with healthy controls.METHODS Children diagnosed with non-cyanotic CHD on ultrasonography(n=54)and healthy control subjects(n=35)were included in the study.All the subjects were aged 1-3 years.Brain MRI was performed prior to surgery for CHD.The SPM v12 software was used to calculate the volumes of the gray matter,white matter,CSF,and the whole brain(sum of the gray matter,white matter,and CSF volumes).Volume differences between the two groups were analyzed.Voxel-based morphometry was used to compare specific brain regions with statistically significant atrophy.RESULTS Compared with the control group,the study group had significantly reduced whole-brain white matter volume(P<0.05),but similar whole-brain gray matter,CSF,and whole-brain volumes(P>0.05).As compared with the healthy controls,children with non-cyanotic CHD had mild underdevelopment in the white matter of the anterior central gyrus,the posterior central gyrus,and the pulvinar.CONCLUSION Children with non-cyanotic CHD show decreased white matter volume before surgery,and this volume reduction is mainly concentrated in the somatosensory and somatic motor nerve regions.
文摘目的:分析儿童肝脏未分化胚胎性肉瘤(undifferentiated embryonal sarcoma of the liver,UESL)的超声、CT及MRI影像学表现,以提高对该病的认识水平。方法:回顾性收集2009年12月至2021年12月首都儿科研究所附属儿童医院经手术及病理证实的11例儿童UESL,分析所有患儿的超声、CT及MRI影像表现,总结其影像学特点。结果:病灶均为单发,肿块最大径11.5~19.8 cm。CT平扫显示所有病灶边缘较清楚、其内呈不均匀低密度,内可见厚薄不均的分隔;囊性密度区边缘或分隔周围可见少许片絮样软组织密度影。6例超声图像均提示实性占位,表现为混合回声的实性包块内可见大小不等的无回声区。3例MRI表现为边界清楚肿块影,T1WI呈不均匀混杂低信号,病灶内可见高信号区;T2WI呈混合高信号区和条状低信号区。9例病灶增强扫描动脉期均以边缘强化为主,8例可见迂曲增粗血管影;延迟期病灶呈持续不均匀强化,边缘持续强化,且从外围向中心充盈,强化范围增大,8例延迟期可见假包膜。结论:儿童UESL影像学表现有一定特征性,了解其影像特点并结合临床特征,有助于早期诊断。
文摘目的探讨应用磁共振质子密度脂肪分数(MRI-PDFF)诊断超重和肥胖儿童非酒精性脂肪性肝炎(NASH)的效能。方法2022年12月~2023年12月江油市903医院诊治的超重/肥胖的非酒精性脂肪性肝病(NAFLD)儿童102例,其中超重儿童74例和肥胖儿童28例,使用3.0 T MRI扫描仪行肝脏检查,获取MRI-PDFF,使用FibroScan诊断仪检查获得肝脏硬度检测(LSM)和受控衰减参数(CAP),行肝穿刺诊断NASH,绘制受试者工作特征曲线(ROC),计算曲线下面积(AUC),评估诊断效能。结果在102例超重/肥胖的NAFLD儿童中,经组织学检查诊断单纯性非酒精性脂肪肝(SNFL)75例和NASH 27例;NASH组BMI、血清ALT、AST、TG、MRI-PDFF、LSM和CAP分别为(32.2±3.4)kg/m^(2)、(92.7±31.3)U/L、(96.8±34.1)U/L、(3.1±1.2)mmol/L、(12.5±4.7)%、(11.7±1.4)kPa和(370.4±146.9)dB/m,与SNFL组【分别为(26.7±2.2)kg/m^(2)、(38.2±4.4)U/L、(33.0±3.4)U/L、(2.3±0.7)mmol/L、(7.7±2.0)%、(6.2±1.0)kPa和(262.3±61.2)dB/m】比,差异具有统计学意义(P<0.05),NASH组超重和肥胖占比分别37.0%和63.0%,与SNFL组的85.3%和14.7%比,差异具有统计学意义(P<0.05);经ROC曲线分析显示,以MRI-PDFF为11.9%为截断点,其诊断NASH的AUC为0.87(95%CI为0.80~0.93),敏感性为81.5%,特异性为85.3%,而分别以LSM为10.8 kPa和CAP为310.5dB/m为截断点,其诊断的AUC为0.81(95%CI为0.78~0.89),敏感性为85.2%,特异性为58.7%,后者的特异性较差。结论应用MRI-PDFF诊断儿童NASH的效能较优,但考虑到临床的便捷性,使用弹性成像检测诊断也可获得初步结果,以便必要时进一步检查。
