Smoking induces substantial diseases burden on both individual and the whole society. To identify the true smoking-attributable economic loss, we introduce medical cost accounting as a means to calculate disease-speciflc medical cost, including inpatient and outpatient cost of those diseases caused by smoking. Medical cost is defined as health fesource consumption in terms of money. Cost is allocated to departrpent and services according to coeffcient of benefit and operation time. The study in 1988 indicates that total smoking-attributable medical cost is 2.32 billion RMB Yuan in China, 1.70 billion RMB Yuan for outpatient, 0.62 billion RMB Yuan for inpatient. If indirect cost is included, the cost will be greater. Chronic obstructive emphysema has the highest proportion (55.41%) in smoking attributable medical cost 展开更多

Quality evaluation is a bottleneck restricting the modernization and internationalization of Chinese materia medica (CMM). Due to characteristics of multi-component, multi-efficacy, multi-target, the existing quality evaluation system still cannot fully meet quality control needs of CMM. Hence, the author put forward “Bio-characteristic profiling related to clinic, BPRC” academic concept, meanwhile, fully take the advantage of analysis method with clinical monitoring superiority or profiling characteristics, build up “BPRC” new technology platform, in order to realize “real-time, dynamic and full-range monitoring” new technologies of CMM quality evaluation system and promote the further development of CMM industry. 展开更多

Objective:This study aimed to design an evaluation scale of traditional Chinese medicine(TCM)confidence and then evaluate its validity and reliability. Methods:Three parallel samples with TCM learning experience were included in this current study.Randomly selected 500 nursing students were included factor analysis and reliability and validity analyses. After 4 weeks,200 nursing students were recruited to retest the reliability. The evaluation tool of TCM confidence included 6 dimensions and 25 entries. Results:Cronbach's a coefficient of the evaluation scale was 0.95 and that of the 6 observed aspects were higher than 0.90,and splithalf reliability was 0.81.For the retest reliability,Cronbach's a coefficient of the evaluation scale was 0.93 and that of the 6 examined aspects was all over 0.80.The index of content validity Scale Content Validity Index(S-CVI)was 0.92 and Item Content Validity Index(I-CVI)was 0.83–1.00.Conclusions:Observed results may indicate that the designed tool can be used to assess the TCM confidence level of students with TCM studied experience and also can evaluate results of TCM education during studying TCM lectures. 展开更多

Chinese medicine (CM) has been an experience based practice over the past three thousand years. The safety and efficacy of CM has been practised through experience, rather than nowadays’ modern scientific measurements. In this way, the development of CM has not been keeping pace with the advance in science and technology and the progress in orthodox medicine. In order to meet the requirements of modern science and technology, an evidence based approach must be adopted. Reassurance is needed to convince people for safety and efficacy of CM and give confidence to the evidence based demands ——“Tradition is confirmed by Modern Science” (1) . 展开更多

Objective:To evaluate the objectivity and comprehensiveness of Response Evaluation Criteria of Traditional Chinese Medicine for Solid Tumor(Draft,REC-TCM-ST) in application of Chinese medicine therapeutic effect in patients with advanced non-small cell lung cancer(NSCLC).Methods:A retrospective clinical research was used in 104 NSCLC patients in stages of Ⅲ-Ⅳ,53 cases were in Chinese medicine(CM) group and 51 cases were in Western medicine(WM) group. The therapeutic effect of the two groups was evaluated with both REC-TCM-ST and Response Evaluation Criteria in Solid Tumor(RECIST).Kaplan-Meier method was used to analyze the survival time.Kappa test method was used to test the consistency of the two kinds of evaluation results. Results:According to REC-TCM-ST,the effective rate on relieving tumor mass in the CM group was significantly lower than that in the WM group(P〈0.05),but there was no significant difference in tumor-mass stable rate(P〉0.05);the symptom of weakness in the CM group was improved significantly, indicating better therapeutic effect than that in the WM group(P〈0.01).Karnofsky score in the CM group was significantly better than that in the WM group(P〈0.01).In terms of survival conditions, the median survival time and the survival rate of 6 months,1 year and 2 years of the CM group were higher than the WM group. The total effective rate was 9.62%,and the total stable rate was 72.12%for 104 cases according to RECIST;while the total effective rate was 34.62%,and the total stable rate was 84.62%according to REC-TCM-ST,thus there were significant differences between the results of the two criteria(P〈0.01),and there was also some consistency between them, but not satisfactory.Conclusions:REC-TCM-ST was used to evaluate the therapeutic effect of CM in the treatment of advanced NSCLC,which shows that its evaluation results can better reflect the advantages and disadvantages of CM,and the effectiveness of CM is more objective and comprehensive than RECIST,so REC-TCM-ST is worthy of further improvement and clinical expansion. 展开更多

The randomized controlled trial (RCT) as applied to medicine started in the mid last century(1) and is now deemed as the most rigorous scientific method for evaluating the clinical effectiveness of medical interventions. The earliest RCTs in Chinese medicine (CM) appeared in the early 1980s. In the past 30 years, RCT was seen to have gradually infiltrated into and have been accepted reluctantly by CM. 展开更多

Although tumor node metastasis (TNM) staging in WM has been accepted and used with no reserve in the criteria of CM diagnosis and treatment of cancer, some differences still exist between CM and WM in the evaluation criterion. The reason for this focuses on evaluation criterion of effects on solid tumor made by the World Health Organization (WHO), which does not reflect some aspects such as survival time of tumor patients or improvement in quality of life (QOL). Therefore, forerunners in the CM field have been making efforts to try to find an evaluation criteria that would integrate CM and WM, and thus could be approved by CM and WM specialists. 展开更多

In China, CM has been applied to treat almost every cancer patient, especially during the observation period after surgery, routine radiotherapy and chemotherapy. Several decades of CM treatment results have shown that CM treatment could be helpful in improving patients' physical rehabilitation, relieving symptoms and strengthening immunity. CM also could aid in the decline of rates of recurrence and metastasis, prolong the survival time, and especially, have significant effects on relieving symptoms such as relieving fatigue, improving syndromes caused by conventional therapies (surgery, radiotherapy, chemotherapy and biological treatment), and indescribable discomforts caused by the unknown reason. It has been found in several studies that conventional medical treatment combined with CM could significantly improve QOL, and reduce treatment-related and disease-related symptoms. Patients treated with CM and Western medicine have higher scores in the fields of psychology, society, 展开更多
