Intervention effect and mechanism of curcumin in chronic urinary tract infection in rats 被引量:4

Objective: To analyze the invention effect of curcumin on chronic urinary tract infection in rats and explore its possible mechanism of action. Methods: The experimental animals were randomly divided into three groups, normal, model and curcumin group. Chronic urinary tract infection models were built for model group and curcumin group by injecting coliform fluid into the cavity of bladder. From the first day of modeling, rats in the curcumin group were injected with 150 mg/kg curcumin, while rats in normal group and model group were given no other treatment. The treatment lasted for 14 d. The white blood cell counts in blood and urine, bacterial colony count in urine and renal tubular functional indexes of rats in all groups at day I, 7, and 14 after treatment were detected. Urine beta 2-microglobulin (beta 2-MG), urinary N-acetyl-D glucosaminidase (NAG) levels were used to detected the inflammatory cytokines in serum after treatment including the contents of IL-6, IL-8, IL-10 and monocyte chemoattractant protein-I(MCP-1), and real-time PCR was employed to determine the expression of mRNA of toll-like receptor 2(TLR-2) and TLR-4 in renal tissues and bladder tissues of all groups after treatment. Results: The white blood cell counts at day I and 7 after treatment in rats of model group and curcumin group were significantly higher than those of normal group at the same time points, while the white blood cell counts of the curcumin group were significantly lower than those of model group (P < 0.05). The urine white blood cell counts in rats of model group at day 1, 7 and 14 were all significantly higher than those of normal group at the same time points: those in the curcumin group were significantly lower than those of the model group at day I, 7 and 14 at the same time points (p < 0.05). The bacterial colony counts of urine in rats of model group and curcumin group at day I. 7 and 14 were all significantly higher than those of normal group at the same time points, while the counts of curcumin group were significantly lower than those of model group at the same time points (P < 0.05). Levels of urine beta 2-MG, NAG, IL-6, IL-8, IL-10, MCP-1 and expression of TLR2 mRNA and TLR4 mRNA in renal and bladder tissues in rats of model group were significantly higher than those of the normal group, while these variables of the cercumin group were significantly higher than those of the normal group but lower than those of model group (P < 0.05). Conclusions: Curcumin can significantly improve the symptoms of chronic urinary tract infections. protect renal tubular function, and also decline inflammatory responses by influencing the expressions of TLR2 mRNA and TLR4 mRNA so as to exert its curative effect on chronic urinary tract infections. 展开更多

Urinary retention in women is rare and is more frequently described as case reports or small case series. The female/male ratio is 1:13 with about 3 cases per 100,000 women every year We report a case of a 12-year old female student. She presented with progressive weight loss, worsening lower urinary tract symptoms with distended lower abdomen of 10 weeks duration. Physical examination revealed a mobile tender firm pelvic mass, 18 centimeters (cm) × 16 cm in size. Laboratory and imaging studies showed obstructive nephropathy and uropathy respectively. She was worked up and had uneventful exploratory laparotomy with right salpingo-oophorectomy, urinary bladder diverticulectomy and pelvic lymphadenectomy. Histopathology of the pelvic mass showed ovarian dysgerminoma with lymph node metastasis. She responded very well to chemotherapy and resumed her school activities. Bladder outlet obstruction is relatively rare in females and in the index patient, ovarian dysgerminoma is the cause leading to obstructive nephropathy and uropathy. 展开更多

The association between postvoid residual and response to standard therapy in male and female patients with non-muscle-invasive bladder cancer

Background:The urinary retention is recognized as a promoting factor for bladder cancer,but its role as prognostic factor of therapeutic response has not yet been widely considered.To correlate bladder outlet characteristics with short-term response to treatment in non-muscle-invasive bladder cancer.Materials and methods:We carried out a case-control study on 600 consecutive patients with a first diagnosis of non-muscle-invasive bladder cancer,recruited at the first endoscopic follow-up visit after standard treatment:200 patients were not tumor-free(cases)and 400 were tumor-free(controls).Patients were compared based on baseline and bladder-outlet characteristics("functional param-eters":postvoid residual volume[PVR],International Prostatic Symptoms Score,perceived quality of bladder outlet).t Test,x^(2)test,receiver operating characteristic curves,logistic correlations,and multivariate analysis were applied.Results:The cases had higher statistically significant PVR values compared with controls.We reported a linear correlation of no-tumor-free status with PVR(R^(2)=0.087,p<0.005);the receiver operating characteristic curves revealed an area under the curve of 0.824(95%confidence interval,0.783-0.865;optimal PVR cutoff,50 mL).In the multivariate analysis,age,American Society of Anesthesiologists score≥2,risk category≥intermediate,and all functional parameters represented independent factors for no-tumor-free status.Conclusions:Urinary retention could represent a prognostic factor of treatment response,and its active treatment should be considered as an important therapeutic step into the clinical management of bladder cancer patients. 展开更多
