Clinical experience in treating 78 cases of upper limb edema after breast cancer operation by WenYang HuoXue Washing Prescription

Objective To observe the clinical efficacy and safety of WenYang HuoXue Washing Prescription （WYHX） in the treatment of upper limb edema after breast cancer surgery. Methods： Seventy-eight patients with upper extremity edema after operation of breast cancer with Yin Syndrome were given wet compress with prescription and nursing guidance. The symptom score of affected extremity was observed before and 14 days after treatment, and the total effective rate was evaluated. Results： Twenty-three cases were markedly effective, 41 cases were effective and 14 cases were Invalid. The total effective rate was 82.1%. There was no significant difference between the two groups according to stage I and stage II of edema （P 〉 0.05）. Conclusion： WYHX can effectively improve the discomfort symptoms of upper limb edema and cold after breast cancer surgery, and is worthy of clinical application.

Clinical experience of 2 086 cases of kidney preservation

Objective To summarize the experience of 20 years in using hypertonic citrate adenine(HC-A) for clinical kidney preservation. Methods A total of 2 086 cases of kidneys for transplantation from Angust 1980 to December 2000 were reported. The donors’ age ranged from 18 to 49 years. The cadaveric kidneys were harvested with en-bloc method, and were perfused via renal arteries with 250 ml HC- A at 2 - 4 ℃ and 10 cm H2O, and then stored in HC- A at 2 - 4 ℃ throughout preservation. Serum creatinine was measured at 1 week and 1 month after transplantation. Graft survival rates at 1 year and 3,5,10 years and mean graft survival periods were followed up. According to the cold ischemic time, the cases were divided into 3 groups; Group 1, n = 573, with cold ischemic time less than 12 h; Group 2, n = 779, with 12 to 24 h; and Group 3, n = 734, with more than 24 h. Graduating Nurses in the Time of COVID-19

Nursing has been a leader in online education for more than two decades. Nursing students have taken advantage of the flexible scheduling and variety of online nursing programs to pursue their education due to distance, family, or work demands;however, since the outbreak of COVID-19, nursing schools in the United States have adapted by implementing an online alternative to assess students’ clinical skills required to graduate. Nursing faculty have been forced into this new learning culture, navigating the online landscape, learning new technologies and teaching methods, and adapting to clinical social distance education. Nursing is a performance-based profession in which the clinical learning environment plays an important role in developing skills and professional abilities. The purpose of this evidence-based teaching project is to, based on National League for Nursing (NLN), not only contribute to improvement but ensure educational quality for undergraduate nursing students by examining how online simulation compared to clinical experience with an actual patient, affects nurses’ clinical competence. Therefore, after reviewing 14 peer-reviewed articles, evidence shows benefits for undergraduate nursing students to retain better clinical competence when virtual simulation and clinical sites are combined;however, further research is recommended. Articles were selected as the evidence base for this project, following specific inclusion criteria. Inclusion criteria for evidence were articles within six years, including undergraduate nurse students with online clinical education;the exclusion criteria were studies including graduate nurse students and articles older than six years. Based on the project’s outcome, it is suggested that a nurse residency should follow online simulations to complete clinical hours. .

Transanal surgery for obstructed defecation syndrome: Literature review and a single-center experience

Obstructed defecation syndrome(ODS) is a functional disorder commonly encountered by colorectal surgeons and gastroenterologists, and greatly affects the quality of life of patients from both societal and psychological aspects. The underlying anatomical and pathophysiological changes of ODS are complex. However, intra-rectal intussusception and rectocele are frequently found in patients with ODS and both are thought to play an important role in the pathogenesis of ODS. With the development of evaluation methods in anorectal physiology laboratories and radiology studies, a great variety of new operative procedures, especially transanal procedures, have been invented to treat ODS. However, no procedure has been proved to be superior to others at present. Each operation has its own merits and defects. Thus, choosing appropriate transanal surgical procedures for the treatment of ODS remains a challenge for all surgeons. This review provides an introduction of the current problems and options for treatment of ODS and a detailed summary of the essential assessments needed for patient evaluation before carrying out transanal surgery. Besides, an overview of the benefits and problems of current transanal surgical procedures for treatment of ODS is summarized in this review. A report of clinical experience of some transanal surgical techniques used in the authors' center is also presented.

Stem cells:a promising candidate to treat neurological disorders

Neurologic impairments are usually irreversible as a result of limited regeneration in the central nervous system.Therefore,based on the regenerative capacity of stem cells,transplantation therapies of various stem cells have been tested in basic research and preclinical trials,and some have shown great prospects.This manuscript overviews the cellular and molecular characteristics of embryonic stem cells,induced pluripotent stem cells,neural stem cells,retinal stem/progenitor cells,mesenchymal stem/stromal cells,and their derivatives in vivo and in vitro as sources for regenerative therapy.These cells have all been considered as candidates to treat several major neurological disorders and diseases,owing to their self-renewal capacity,multi-directional differentiation,neurotrophic properties,and immune modulation effects.We also review representative basic research and recent clinical trials using stem cells for neurodegenerative diseases,including Parkinson＇s disease,Alzheimer＇s disease,and age-related macular degeneration,as well as traumatic brain injury and glioblastoma.In spite of a few unsuccessful cases,risks of tumorigenicity,and ethical concerns,most results of animal experiments and clinical trials demonstrate efficacious therapeutic effects of stem cells in the treatment of nervous system disease.In summary,these emerging findings in regenerative medicine are likely to contribute to breakthroughs in the treatment of neurological disorders.Thus,stem cells are a promising candidate for the treatment of nervous system diseases.
