Active Learning Improves Nursing Student Clinical Performance in an Academic Institution in Macao

Objective： To assess the outcome of the application of active learning during practicum among nursing students using clinical assessment and evaluation scores as a measurement. Methods： Nursing students were instructed on the basics of active learning prior to the initiation of their clinical experience. The participants were divided into 5groups of nursing students （ n = 56） across three levels （years 2-4） in a public academic institute of a bachelor degree program in Macao. Final clinical evaluation was averaged and compared between groups with and without intervention. Results： These nursing students were given higher appraisals in verbal and written comments than previous students without interventian. The groups with the invention achieved higher clinical assessment and evaluation scores on average than comparable groups without the active learning intervention. One group of sophomore nursing students （year 2） did not receive as high of evaluations as the other groups, receiving an average score of above 80. Conclusions＂ Nursing students must engage in active learning to demonstrate that they are willing to gain knowledge of theory, nursing skills and communication skills during the clinical practicum.

Immune checkpoint inhibitor-associated arthritis in advanced pulmonary adenocarcinoma:A case report

BACKGROUND With the wide application of immune checkpoint inhibitors(ICIs)in cancer treatment,immune-related adverse events occur frequently,involving almost all organs and systems.The incidence of ICI-associated arthritis(IA)is unknown.In most cases,IA is not serious and non-lethal.Higher checkpoint inhibitor arthritis disease activity may be associated with cancer progression.Here,we report a severe case of IA with high arthritis disease activity in advanced pulmonary adenocarcinoma,causing permanent withdrawal of pembrolizumab,but the patient remained in complete remission(CR)20 mo after the development of IA.CASE SUMMARY An 81-year-old smoking man was admitted to our hospital because of left chest pain for 9 mo.He was finally diagnosed with advanced pulmonary adenocarcinoma,with programmed cell death 1 ligand 1 expression of 70%.The patient responded to pembrolizumab treatment and achieved CR,but IA occurred after the 5th cycle of pembrolizumab administration.Although non-steroidal antiinflammatory drugs and disease-modifying anti-rheumatic drugs were prescribed,arthralgia and joint swelling occurred.The symptoms of arthritis were further aggravated when immunotherapy was given again after short-term withdrawal.Clinical Disease Activity Index(CDAI)score,a traditional measure of arthritis activity,was 43.Intravenous methylprednisolone was prescribed at 20 mg/d and then tapered over the subsequent 4 wk.The symptoms of arthritis steadily improved and completely resolved 4 mo after withdrawal of pembrolizumab.A recent follow-up in June 2022 revealed satisfactory clinical recovery of arthritis and the patient remained in CR.CONCLUSION This case report highlights that early recognition of IA and appropriate treatment are critical to improving the outcome of both ICI-arthritis and lung cancer.

A NEW EXPERIMENTAL AND CLINICAL APPROACH OF COMBINING USAGE OF HIGHLY ACTIVE TUMOR-INFILTRATING LYMPHOCYTES AND HIGHLY SENSITIVE ANTITUMOR DRUGS FOR THE ADVANCED MALIGNANT TUMOR

In recent years, tumor-nfiltrating lymphocytes (TILs) have been reported to be effective for tumors in experimental and clinical research. In order to increase the therapeutical effect, we modified some steps of Rosenberg's approach a. cold digestion with collagenase at 4C for 24 hours; b. sedimentation instead of centrifugation; c. elimination of tumor cells before the cultivation procedure. Compared with the original approach, the proliferation, activity and cytotoxicity of TILs obtained by the modified procedure were much improved. TILs' expansion-old was greater than that with the original approach. Cytotoxicity against rumor cells was more potent. Increased TILs' subsets were CD3 and CD8 cells. Meanwhile, we took tumor cells from tumor tissues to test their in vitro chemosensitivities to different drugs in order to select highly sensitive antitumor drugs for treatment of cases with advanced tumors. According to the design of using highly active TILs and highly sensitive drugs (H & H therapy), preliminary clinical results of 50 cases showed higher response rates than those in treatment with TIL / IL2, LAK / 1L2 and TIL+IL2+CTX. Less toxic side effects were observed in 14 patients.
