Efficacy of mucosa-submucosa clip closure method after gastric endoscopic submucosal dissection

BACKGROUND We recently developed a new endoscopic closure technique using only conventional endo-clips for colorectal lesions.Little is known about the feasibility of the endoscopic mucosa-submucosa clip closure method for gastric lesions.AIM To elucidate the efficacy of the endoscopic mucosa-submucosa clip closure method after gastric endoscopic submucosal dissection(ESD).METHODS Twenty-two patients who underwent gastric ESD and mucosa-submucosa clip closure were included in this study.In this method,endo-clips are placed at the edges of a mucosal defect.Additional endo-clips are then applied in the same way to facilitate reduction of the defect size.Additional endo-clips are applied to both sides of the mucosal defect.Complete closure can be achieved.We have also developed a“location score”and“closure difficulty index”for assessment purposes.RESULTS Complete closure was achieved in 68.2%of the patients(15/22).The location score in the failure group was significantly larger than that in the complete closure group(P=0.023).The closure difficulty index in the failure group was significantly higher than that in the complete closure group(P=0.007).When the cutoff value of the closure difficulty index was set at 99,the high closure difficulty index predicted failure with a sensitivity of 57.1%,specificity of 100%,and accuracy of 86.3%.CONCLUSION The endoscopic mucosa-submucosa clip closure method was unreliable after gastric ESD,especially in cases with a high closure difficulty index. 展开更多

Based on Edong Yangtze River Bridge, which is the second longest hybrid girder cable stayed bridge with 926 m long main span, the influencing factors and crucial techniques of the main span closure method for long span hybrid girder cable stayed bridge are studied. After theoretical analysis, numerical evaluation and practical test, the loading assistant closure method is employed in Edong Yangtze River Bridge. The loading assistant closure method, with better thermal adaptability and less influence on bridge line and the forced status, can meet the requirements of the unstressed state control method. Based on the mentioned advantages, the loading assistant closure method is applicable to long span hybrid girder cable stayed bridges. The conclusion can provide a reference for the further design of the similar brid^es. 展开更多

Water quality assessment of lakes is important to determine functional zones of water use.Considering the fuzziness during the partitioning process for lake water quality in an arid area,a multiplex model of fuzzy clustering with pattern recognition was developed by integrating transitive closure method,ISODATA algorithm in fuzzy clustering and fuzzy pattern recognition.The model was applied to partition the Ulansuhai Lake,a typical shallow lake in arid climate zone in the west part of Inner Mongolia,China and grade the condition of water quality divisions.The results showed that the partition well matched the real conditions of the lake,and the method has been proved accurate in the application. 展开更多

An evaluation index is a prerequisite for the scientific evaluation of a public meteorological service.This paper aims to explore a technical method for determining and screening evaluation indicators.Based on public satisfaction survey data obtained in Wafangdian,China in 2010,this study investigates the suitability of fuzzy clustering analysis method in establishing an evaluation index.Through quantitative analysis of multilayer fuzzy clustering of various evaluation indicators,correlation analysis indicates that if the results of clustering were identical for two evaluation indicators in the same sub-evaluation layer,then one indicator could be removed,or the two indicators merged.For evaluation indicators in different sub-evaluation layers,although clustering reveals attribute correlations,these indicators may not be substituted for one another.Analysis of the applicability of the fuzzy clustering method shows that it plays a certain role in the establishment and correction of an evaluation index. 展开更多

In this study,flowback-assisted minifrac tests were conducted in low-permeability shale and salt formations to measure the in situ stress.An injection/flowback testing protocol was implemented in each test to achieve accuracy and efficiency.Accurate and efficient injection/flowback testing is very important,given the impermeable nature of these formations and the need to complete each test as quickly as possible.Each flowback cycle yields a distinct and repeatable fracture closure signature,simplifying the interpretation of the fracture closure pressure.The objective of this paper is to share our field experience and to present a numerical analysis of the flowback test pressure responses,fracture closure behaviors,and fracture closure diagnostic methods.Examples from open-hole and casedhole minifrac tests are used to demonstrate site operation procedures.Then,two numerical models are presented for simulating the fracture closure behavior during a flowback test.Field evidence is provided to demonstrate that the fracture closure pressures from the flowback tests are identical to those from tests without flowback.The fracture closure diagnostic methods for flowback tests are discussed,and it is found that the G-function diagnostic method yields a distinct fracture closure signal during the flowback tests.This study is intended to provide additional insights regarding flowback tests by sharing our successes,experience,and knowledge,thereby benefiting the industry. 展开更多
