Background:Rheumatoid arthritis(RA),a chronic autoimmune disease, has a high incidence and disability rate, causing patients significant discomfort.Although several medicines can be effective, they are also associated with significant adverse effects. In contrast, fumigation as one of the most often used traditional Chinese medicine(TCM)external therapy has shown both efficacy and safety with less side effect. In light of this,we comple-ment western medicine treatment with TCM fumigation therapy to improve patients’clinical efficacy, alleviate symptoms, and improve prognosis. Objective:To investigate the therapeutic effect of TCM fumigation and modern medicine combined therapy in treating RA patients with cold-dampness Bi syndrome. Methods:A single-center,randomized,controlled study was designed. From January 2022 to December 2022,a total of 60 RA patients with cold-dampness Bi syndrome were enrolled in the study. The control group(30 cases)received conventional modern medicine treatment with methotrexate for 4 weeks, while the observation group(30 cases)for 4 weeks received a combination of TCM fumigation treatment and conventional modern medicine. The effects of the two groups were comprehensively compared, including the changes in TCM symptom scores and laboratory indicators,as well as the use of visual analogue scale(VAS)and health status rating scale(HAQ)before and after treatment. Results:The total effective rate of the observation group was 93.3%,which was significantly higher than that of the control group, which is 70%(P<0.05).Before treatment, there were no significant differences in joint functional activity, pain visual analogue scale(VAS),health assessment questionnaire(HAQ-DI)and laboratory indexes between the two groups(P>0.05).After treatment, each group compared to their initial condition, respectively, showed major improvement of joint functional activity and significantly decreased VAS,HAQ-DI,and RA-related biomarkers(P<0.05);inter-group comparison of these indicators showed significanty further enhanced effect of TCM fumigation combined therapy on the RA-related biomarkers and joint functional activity of patients(P<0.05),yet the control group receiving only modern medicine showed better results of the VAS and HAQ-DI.Conclusion:The TCM fumigation combined therapy is effective to treat RA patients with cold-dampness Bi syn-drome,and to enhance their life quality, improving the joint function, and reducing inflammation. 展开更多

OBJECTIVE: To explore the relationships between different lifestyle-behavioral factors and phlegm wetness type of Traditional Chinese Medicine constitution,so as to provide health management strategies for phlegm-wetness constitution.METHODS: A case-control study was conducted with the cases selected from the database of Chinese constitution survey in 9 provinces or municipalities of China. 1380 cases met the diagnostic criteria of phlegm-wetness type were taken as the case group, and 1380 cases were randomly selected from gentleness type as the control group. Using Chi-square test to compare the differences of lifestyle-behavior composition in each group; single factor and multiple logistic regression analysis were used to compare the relationships of life-style-behavioral factors and phlegm-wetness type.RESULTS: There were statistically significant differences between phlegm-wetness type group and gentleness type group in lifestyle behaviors(dietary habits, tobacco and liquor consumptions, exercise habits, sleeping habits). The results of single factor logistic regression analysis demonstrated that the risk of phlegm-wetness constitution decreased significantly in light diet(odds ratio, OR=0.68);The risk factors of phlegm-wetness type were fatty food intake(OR=2.36), sleeping early and getting up late(OR=1.87), tobacco smoking(OR=1.83),barbecued food intake(OR=1.68), alcohol drinking(OR=1.63), salty food intake(OR=1.44), sleeping erratically(OR=1.43), less physical activities(OR=1.42), sweet food intake(OR=1.29), sleeping and getting up late(OR=1.26), and pungent food intake(OR=1.21), respectively. Regardless of the interaction among lifestyle-behavioral factors, the results of the multiple logistic regression analysis revealed that the risk factors of phlegm-wetness type were sleeping early and getting up late(OR=1.94), fatty food intake(OR=1.80), tobacco smoking(OR=1.50),sleeping erratically(OR=1.50), barbecued food intake(OR=1.40), sleeping and getting up late(OR=1.40), less physical activities(OR=1.31), sleeping late and getting up early(OR=1.27),and sweet food intake(OR=1.27), respectively, and the risk ofphlegm-wetness type still decreased significantly inlightfoodintake(OR=0.79).CONCLUSION: Light diet can decrease the risk of being phlegm-wetness constitution, and bad lifestyle behaviors such as sleeping early and getting up late, sleeping erratically, fatty food, barbecued food or sweet food intake, tobacco and liquor consumptions, and less physical activities can increase the risks of becoming phlegm-wetness constitution. 展开更多

OBJECTIVE: To investigate the influence of the quickness and duration of De Qi(or Qi arrival) on the analgesic effect of acupuncture in primary dysmenorrhea patients with a cold and dampness stagnation pattern.METHODS: Sixty-eight patients were randomly assigned to the De Qi group(deep needling with thick needles and manipulation, n = 17) or the non-De Qi group(shallow needling with thin needles and no manipulation, n = 51). Both groups underwent needling at Sanyinjiao(SP 6) for 30 min.The visual analogue scale was used to measure the degree of menstrual pain, and the Acupuncture De Qi Clinical Assessment Scale was used to assess De Qi. Only data from patients who experienced actual De Qi were included in the analysis.RESULTS: Thirty-nine patients experienced actual De Qi. Patients who experienced actual De Qi in the De Qi group(n = 14) felt De Qi more rapidly(P =0.028) and for a longer duration(P = 0.04) than patients who experienced actual De Qi in the non-De Qi group(n = 25). Both groups showed a reduction in the visual analogue scale score for pain after treatment. The analgesic effect did not significantly differ between the two groups. The occurrence time of De Qi showed a significant negative correlation with pain reduction at 20 and 30 min after needle removal(P < 0.05). There was no correlation between the duration of De Qi and the therapeutic effect.CONCLUSION: In primary dysmenorrhea patients with a cold and dampness stagnation pattern, quicker onset of De Qi when needling Sanyinjiao(SP 6) achieves a better analgesic outcome. However, a longer duration of De Qi does not affect the degree of analgesia. Compared with minimal acupuncture, active acupuncture stimulation achieves a more rapid onset and longer duration of De Qi. 展开更多

Objective: To investigate a method for quantitative differential diagnosis of damp-heat and cold-damp impeding syndrome of rheumatoid arthritis (RA) in Chinese medicine (CM). Methods: Laboratory parameters were collected from 306 patients with RA. The clinical symptoms and laboratory parameters were compared between patients with these two syndromes (158 with RA of damp-heat impeding syndrome, and 148 with RA of cold-damp impeding syndrome), and a regression equation was established to facilitate discrimination of the two RA syndromes. Results: There were significant differences in disease activity score in 28 joints [DAS28 (4)], erythrocyte sedimentation rate (ESR), white blood cell count (WBC), C-reactive protein (CRP), platelet count (PLT), albumin (ALB) and globulin (GLB) between the two syndrome of RA (P〈0.05). Logistic regression analysis showed that the parameters ESR, WBC, CRP, joint pyrexia, joint cold, thirst, sweating, aversion to wind and cold, and cold extremities were statistically useful to discriminate damp-heat from cold-damp impeding syndrome. The regression equation was as follows: P=1/{1+exp[-(3.0-0.021X1-0.196X2-0.163X3- 1.559X4+1.504X5-0.927X6-1.039X7+1.070X8+1.330X9)]}. The independent variables X1-X9 were ESR, WBC, CRP, hot joint, cold joint, thirst, sweating, aversion to wind and cold, and cold limbs. A P value 〉 0.5 signified cold-damp impeding syndrome, and a P value 〈 0.5 signified damp-heat impeding syndrome. The accuracy was 90.2%. Conclusion: The regression equation may be useful for discriminating damp-heat from cold-damp impeding syndrome of RA. 展开更多
