The willingness to comply with the law or rule is a litmus test of the quality of legislation.Without compliance, there is no rule of law,no matter how well the laws or regulations are designed.Regulatory compliance continues to be one of the greatest challenges for energy industry due to factors such as central protectionism, inadequate legal system and weak law enforcement. To meet the challenges of achieving environmental compliance,it is proposed that the environmental compliance system should incorporate three subsystems:a primary environmental compliance system,a primary environmental compliance system, and an environmental noncompliance response system. 展开更多

BACKGROUND Non-alcoholic fatty liver disease(NAFLD)is the most common liver disease worldwide, affecting about 1/4th of the global population and causing a huge global economic burden. To date,no drugs have been approved for the treatment of NAFLD,making the correction of unhealthy lifestyles the principle method of treatment.Identifying patients with poor adherence to lifestyle correction and attempting to improve their adherence are therefore very important.AIM To develop and validate a scale that can rapidly assess the adherence of patients with NAFLD to lifestyle interventions.METHODS The Exercise and Diet Adherence Scale(EDAS)was designed based on com-pilation using the Delphi method, and its reliability was subsequently evaluated.Demographic and laboratory indicators were measured, and patients completed the EDAS questionnaire at baseline and after 6 months. The efficacy of the EDAS was evaluated in the initial cohort.Subsequently, the efficacy of the EDAS was internally verified in a validation cohort.RESULTS The EDAS consisted of 33 items in six dimensions, with a total of 165 points.Total EDAS score correlated significantly with daily number of exercise and daily reduction in calorie intake(P<0.05 each),but not with overall weight loss.A total score of 116 was excellent in predicting adherence to daily reduction in calorie intake(>500 kacl/d),(sensitivity/specificity was 100.0%/75.8%),while patients score below 97 could nearly rule out the possibility of daily exercise(sensitivity/specificity was 89.5%/44.4%).Total EDAS scores≥116,97-115,and<97 points were indicative of good, average, and poor adherence, respectively,to diet and exercise recommendations.CONCLUSION The EDAS can reliably assess the adherence of patients with NAFLD to lifestyle interventions and have clinical application in this population. 展开更多

Non-descriptive and convenient labels are uninformative and unfairly project blame onto patients. The language clinicians use in the Electronic Medical Record, research, and clinical settings shapes biases and subsequent behaviors of all providers involved in the enterprise of transplantation.Terminology such as noncompliant and nonadherent serve as a reason for waitlist inactivation and limit access to life-saving transplantation. These labels fail to capture all the circum-stances surrounding a patient’s inability to follow their care regimen, trivialize social determinants of health variables, and bring unsubstantiated subjectivity into decisions regarding organ allocation.Furthermore, insufficient Medicare coverage has forced patients to ration or stop taking medication, leading to allograft failure and their subsequent diagnosis of noncompliant. We argue that perpetuating non-descriptive language adds little substantive information,in-creases subjectivity to the organ allocation process, and plays a major role in reduced access to transplantation. For patients with existing barriers to care, such as racial/ethnic minorities, these effects may be even more drastic.Transplant committees must ensure thorough documentation to correctly encapsulate the entirety of a patient’s position and give voice to an already vulnerable population. 展开更多

Air flow in an avian lung was studied numerically to determine the effects of air sac compliance on flow in the parabronchi. In this preliminary study, the geometry of the avian respiratory system was simplified to capture the characteristics of respiratory flow. The pressure fluctuation within air sacs caused by inflation and deflation was expressed by a lumped parameter model. The results demonstrate that the flow direction in the parabronchi varied, depending upon the compliance of the air sacs. A unidirectional flow in the parabronchi was achieved for compliances where pressure fluctuations in all air sacs were in phase. Air sac compliance significantly affected the pressures in the anterior and posterior air sacs and thus the pressure difference over the parabronchi that drove the flow in the parabronchi. These results address the importance of air sac compliance in the avian respiratory system and suggest that the compliance of air sacs would be optimized to accomplish unidirectional flow in the parabronchi. 展开更多

Objective:To investigate the effect of sevoflurane pretreatment on cerebral oxygen saturation, pulmonary compliance and systemic stress response in patients with one-lung ventilation. Methods:A total of 70 patients with lung cancer who accepted the selective pulmonary lobectomy in our hospital between January 2012 and December 2015 were collected and divided into observation group and control group (n=35) according to the single-blind randomized control method. After general anesthesia induction, the control group received air/oxygen mixed ventilation, and the observation group received sevoflurane pretreatment for 30 min and received one-lung ventilation after that. Immediately after anesthesia induction (T0), 30 min after sevoflurane pretreatment (T1), 60 min after sevoflurane pretreatment (T2) and at chest wall suture after operation (T3), the cerebral oxygen saturation monitor was used to determine the left and right regional cerebral oxygen saturation (rSO2);end-expiratory airway blocking method and related indexes were used to calculate the contralateral pulmonary static compliance (Cst) and pulmonary dynamic compliance (Cdyn);the RIA method was used to determine serum stress hormone levels. Results:At T0, differences in cerebral oxygen saturation, pulmonary compliance and systemic stress response were not statistically significant between two groups of patients;at T1, T2 and T3, left and right rSO2 levels of observation group were higher than those of control group, Cst and Cdyn levels were significantly higher than those of control group, and serum epinephrine (E), norepinephrine (NE), cortisol (Cor) and angiotensinⅡ (AngⅡ) levels were lower than those of control group. Conclusion:Sevoflurane pretreatment can promote the intraoperative cerebral oxygen saturation and pulmonary compliance, and reduce systemic stress response in patients with one-lung ventilation. 展开更多

Introduction: The management of hypertension is mostly based on pharmacotherapy and hygienic and dietary measures (HDMs) for which little data is available in Cameroon. The concern to improve the quality of life of hypertensive patients led us to study the knowledge, compliance and effect of HDMs among Cameroonian hypertensive patients. Methods: This was a cross-sectional study carried out at the Douala General Hospital;the census of patients was carried out from 05 March to 10 May 2018. The data evaluated were knowledge and compliance with HDMs with an inference of their effect on blood pressure control. Results: We recruited 330 participants at mean age of 60 ± 11 years, 37.9% men;57.3% with blood pressure (BP) controlled. Out of 330 subjects, 308 (93.3%) who had been educated about dietary health measures for hypertension were assessed on knowledge, compliance and effect of these measures against 22 (6.7%) who had never heard about them. Around 85.7% of participants had good knowledge of HDMs and 78.9% had good compliance with them. There was no statistically significant influence of knowledge and compliance with HDMs on blood pressure control. Conclusion: The level of knowledge and adherence to HDMs of hypertensive patients at the Douala General Hospital was appreciable. It is however appropriate for physicians to intensify patient education on HDMs and BP control. 展开更多

Introduction: MRI is a rapidly growing technique with more and more indica-tions and requests in the Republic of Guinea. Its correct prescription is a guar-antee for the satisfaction of the actors, both prescribers, radiologists and pa-tients. The main objective of this study was to evaluate the compliance of MRI examination requests at the Diagnostic Center of the National Social Se-curity Fund (CNSS) in Conakry. Material and Methods: This was a descriptive cross-sectional study of MRI prescription forms sent to the MRI unit of the CNSS Diagnostic Center from February 1 to May 1, 2021. The 8 compliance criteria established by the French High Authority for Health were used to evaluate the compliance of the examination requests. Results: A total of 7003 examination forms were sent to the facility, including 7% (n = 468) of MRIs. 56.2% of MRI requests were performed by specialists. We observed an overall compliance of 10%. Administrative and clinical compliance were missing in 24% and 38%, respectively. More specifically, the purpose of the examination was not mentioned in 60%, followed by the requesting department in 48.1% and the patient’s age in 35.1%. Conclusion: This study allowed us to highlight the gaps in establishing MRI requests. It would be important to organize an awareness campaign for prescribers on the usefulness of correctly filling an MRI request and to design templates to be filled out by prescribers. 展开更多

In the current digital era, it is difficult to preserve the confidentiality, integrity, and availability of an organization’s information and technology assets against cyber attacks. Organizations cannot rely solely on technical solutions for defense, since many cyber attacks attempt to exploit non-technical vulnerabilities such as how well employees comply with the organization’s cybersecurity policies. This study surveyed 245 randomly selected employees of government organizations in the Kingdom of Saudi Arabia with an electronically distributed questionnaire about factors that influence employees’ compliance with cybersecurity policies. The study found that ethical factors had the most influence on employee compliance with cybersecurity policies, followed in decreasing order of influence by legislative factors, technical factors, and administrative factors. 展开更多

Compliance to local energy efficiency strategies can significantly reduce energy consumption in buildings and promote sustainability. However, Nigeria Building Energy Efficiency Code (BEEC) seems to be just paper work without enforcement or compliance. Assessment of Compliance to Green Building Energy Efficiency Strategies in the Development of Housing Estates in FCC, Abuja Nigeria was carried out. The compliance of estate buildings to BEEC in the study area was assessed;and the aspects of the building energy efficiency strategies with more compliance were determined. Data were mainly from primary sources such as questionnaire survey, interview with key informants and observation. The study population included all estate developers and building construction professionals. Data were gathered from one hundred and fifty (150) building professionals through questionnaire administration. Data collected were analysed using Z score and ANOVA and presented in tables. Results show that only 18% of respondents agreed that building complied with BEEC, and the Z score for positive affirmation is below average score. There is a significant difference in the integration of different energy efficient management strategies in the estates’ construction at 95% confidence level. The integration of measures such as window to wall ratio and window shading’ were significantly higher than other BEEC strategies such as roof insulation and minimal air condition unit (≤EER/COP of 2.8). Thus, the development of estates in FCC Abuja did not achieve energy efficiency. 展开更多
