Most often, occurring form of arterial hypertension among geriatric patients is isolated systolic arterial hypertension (ISAH). The comparison of two ethnic groups of the population has showed higher levels of systolic and diastolic blood pressure (SBP and DBP) in the group of patients of non-indigenous nationality, rather than Yakut patients. Correlation was recorded in the group of non-indigenous patients as they age. Significant increase in the level of SBP was identified in the group of senile and long-livers than in the elderly. Detection of ECG signs of LVH showed the lowest specificity of Cornell voltage criterion comparing to Sokolow-Lyon criterion with its more often occurrence. ECG-signs of left ventricle hypertrophy are significantly more often established by Sokolow-Lyon criterion for men, Cornell voltage—for women. Correlation has been found between the presence of LVH and combination of Cornell voltage criterion with both Gubner-Ungerleider and Sokolow-Lyon criteria.展开更多

Objective:To explore the characteristics and rules of Danshen Ligustrazine Injection in the treatment of coronary heart disease with hypertension.Methods:From the information systems of 12 tertiary tier-one hospitals ...Objective:To explore the characteristics and rules of Danshen Ligustrazine Injection in the treatment of coronary heart disease with hypertension.Methods:From the information systems of 12 tertiary tier-one hospitals across the country,we extracted the medical data of the application of Danshen Ligustrazine Injection in the treatment of patients with coronary heart disease and hypertension.After normalization,the model was established by Apriori algorithm,and the association rules were analyzed by Clementine 12.0 software.Results:Most of the 1928 patients were between 75 and 90 years old(54.26%).There were more males than females,most with type 2 diabetes,cerebral infarction,etc.Each dose was more than 10 mL(52.78%).Aspirin enteric-coated tablets(67.63%),L-carnitine injection(58.77%),and atorvastatin calcium capsules(50.93%)were often used in combination with safflower yellow pigment(22.20%),Shexiang Baoxin Pill(16.55%),Suxiao Jiuxin Pill(15.09%);the most commonly used combination of western medicine was anticoagulant thrombolytic and antiplatelet drugs(85.84%),and the type of Chinese medicine was Huoxuehuayu(72.98%);The most common combination of two western medicines was L-carnitine injection+aspirin enteric-coated tablets,with a support of 41.9%;The most common combination of two Chinese and western medicines is western medicine·anti-anginal medicine+western medicine·anticoagulant thrombolytic and antiplatelet drugs with a support of 67.6%.Conclusion:Danshen Ligustrazine injection is mainly used in elderly patients with coronary heart disease and hypertension,with many comorbidities.The dosage standard needs to be optimized.The combination of drugs and guidelines should coordinate with each other,which provide clues for clinical diagnosis and treatment and optimization of medication.展开更多

Methods: 48 patients with H-type hypertension and coronary heart disease were selected as observation group, and 57 patients with normal hypertension and coronary heart disease were selected as control group. Echocardiography was used to determine coronary lesion parameters, enzyme-linked immunosorbent assay (ELISA) was used to determine serum levels of homocysteine (Hcy) and coronary heart disease-related indexes, and the correlation between Hcy levels and coronary heart disease was further analyzed. Results:Serum Hcy level of observation group was higher than that of control group (P<0.05), absolute GLPSS value and E/A value under echocardiography were less than those of control group while E-DT and E/e value were higher than those of control group (P<0.05);serum adiponectin (APN) level was lower than that of control group while P-selectin, asymmetric dimethylarginine (ADMA), oxidized high-density lipoprotein (OX-HDL), MMP-2, MMP-9, lipoprotein-associated phospholipase A2 (Lp-PLA2) and Resistin levels were higher than those of control group (P<0.05);Hcy was negatively correlated with absolute GLPSS value, E/A value and APN level, and was positively correlated with E-DT value, E/e value as well as P-selectin, ADMA, OX-HDL, MMP-2, MMP-9, Lp-PLA2 and Resistin levels (P<0.05). Conclusions:There is direct correlation between serum Hcy levels and the severity of coronary heart disease in patients with H-type hypertension and coronary heart disease, it can be a reliable way to early screen for coronary heart disease and evaluate the illness, and it is also a new target of coronary heart disease intervention.展开更多

Methods:A total of 92 cases of patients with coronary heart disease with mild to moderate hypertension were selected as the object of observation, according to the random data table, they were divided into the control group (n=46) and observation group (n=46), and patients in the control group were treated with Nifedipine Controlled Release Table therapy, on this basis, the observation group patients were given telmisartan treatment, two groups were treated for 6 months. The levels of the blood pressure, inflammatory factors, vascular endothelial function and left ventricular function compared between the two groups before and after treatment. Results: There were no significant differences in the levels of SBP, DBP, hs-CRP, TNF-α, NO, ET-1, LVEF, LVEDD and LVESD in the two groups before treatment. After treatment, two groups of SBP, DBP, hs-CRP, TNF-α, ET-1, LVEDD and LVESD levels were significantly lower than those in the same group before treatment, and after treatment, the levels of SBP, DBP, hs-CRP, TNF-α, ET-1 and LVESD in the observation group were significantly lower than those in the control group, while there were no significant difference in the level of LVEDD between the two groups after treatment;Compared with level in the group before treatment, the levels of NO and LVEF in the two groups were significantly increased, and the observation group [(82.13±19.01) μmol/L, (52.83±7.45)%] was significantly higher than the control group ((67.37±13.08) μmol/L, (49.47±6.96)%)Conclusion: Telmisartan combined with Nifedipine Controlled Release Table in treating coronary heart disease with mild to moderate hypertension, can effectively control blood pressure, reduce the inflammatory stress, improve vascular endothelial function and left ventricular function of patients, has an important clinical value.展开更多
Objective To evaluate the capability of wrist pulse analysis in distinguishing three physiolog-ical and pathological conditions:healthy individuals,coronary heart disease(CHD)patients without a history of ischemic str...Objective To evaluate the capability of wrist pulse analysis in distinguishing three physiolog-ical and pathological conditions:healthy individuals,coronary heart disease(CHD)patients without a history of ischemic stroke,and CHD patients with a history of ischemic stroke.Methods Study participants were recruited from Shuguang East Hospital,Yueyang Hospital of Integrated Traditional Chinese and Western Medicine,and Shanghai Municipal Hospital of Traditional Chinese Medicine,affiliated with Shanghai University of Traditional Chinese Medicine,from April 15 to September 15,2021.They were categorized into three groups:healthy controls(Group 1),CHD patients without a history of ischemic stroke(Group 2),and CHD patients with a history of ischemic stroke(Group 3).The wrist pulse signals of the study participants were non-invasively collected using a pulse diagnosis instrument.The linear time-domain features and nonlinear time-series multiscale entropy(MSE)features of the pulse signals were extracted using time-domain analysis and the MSE methods,which were subsequently compared between groups.Based on these extracted features,a recognition model was developed using a random forest(RF)algorithm.The classification performance of the models was evaluated using metrics,including accuracy,precision,recall,and F1-score derived from confusion matrix as well as the area under the receiver operating characteristics(ROC)curve(AUC).Results A total of 189 participants were enrolled,with 63 in Group 1,61 in Group 2,and 65 in Group 3.Compared with Group 1,Group 2 showed significant increases in pulse features H2/H1,H3/H1,W1,W2,and W2/T,and decreased in MSE_(1)-MSE7(P<0.05),while Group 3 showed significant increases in pulse features T5/T4,T,H1/T1,W1,W2,AS,and Ad,and de-creased in MSE_(1)-MSE_(20)(P<0.05).Compared with Group 2,Group 3 demonstrated notable increases in H1/T1 and As(P<0.05).The RF model achieved precision of 80.00%,61.54%,and 61.54%,recall of 74.29%,60.00%,and 68.97%,F1-scores of 70.04%,60.76%,and 65.04%,and AUC values of 0.92,0.74,and 0.81 for Groups 1,2,and 3,respectively.The overall accuracy was 67.69%,with micro-average AUC of 0.83 and macro-average AUC of 0.82.Conclusion Differences in pulse features reflect variations in arterial compliance,peripheral resistance,cardiac afterload,and pulse signal complexity among healthy individuals,CHD patients without a history of ischemic stroke,and those with such a history.The developed pulse-based recognition model holds the potential in distinguishing between these three groups,offering a novel diagnostic reference for clinical practice.展开更多

RT3DE data of ...Left ventricular remodeling index (LVRI) was assessed in patients with hypertensive heart disease (HHD) and coronary artery disease (CAD) by real-time three-dimensional echocardiography (RT3DE). RT3DE data of 18 patients with HHD, 20 patients with CAD and 22 normal controis (NC) were acquired. Left ventricular end-diastolic volume (EDV) and left ventricular end-diastolic epicardial volume (EDVepi) were detected by RT3DE and two-dimensional echocardiography Simpson biplane method (2DE). LVRI (left ventricular mass/EDV) was calculated and compared. The results showed that LVRI measurements detected by RT3DE and 2DE showed significant differences inter-groups (P〈0.01). There was no significant difference in NC group (P〉0.05), but significant difference in HHD and CAD intra-group (P〈0.05). There was good positive correlations between LVRI detected by RT3DE and 2DE in NC and HHD groups (t=0.69, P〈0.01; r=0.68, P〈0.01), but no significant correlation in CAD group (r=0.30, P〉0.05). It was concluded that LVRI derived from RT3DE as a new index for evaluating left ventricular remodeling can provide more superiority to LVRI derived from 2DE.展开更多
AIM:To investigate whether serum adiponectin protects against cardiometabolic risk in a population sample with prevailing metabolic syndrome.METHODS:Middle-aged adults representative of a general population with basel...AIM:To investigate whether serum adiponectin protects against cardiometabolic risk in a population sample with prevailing metabolic syndrome.METHODS:Middle-aged adults representative of a general population with baseline circulating adiponectin measurements(n=1224)were analyzed prospectively at a mean of 3.8 years’follow-up,using continuous values or sex-specific tertiles.Total adiponectin was assayed by an ELISA kit.Type-2 diabetes was identified by criteria of the American Diabetes Association.Hypertension was defined as a blood pressure≥140 mmHg and/or≥90 mmHg and/or use of antihypertensive medication.Outcomes were predicted using Cox proportional hazards regression analysis in models that were controlled for potential confounders.RESULTS:In models of multiple linear regression,sex hormone-binding globulin,fasting insulin(inverse)and,in men,age were significant independent covariates of serum adiponectin which further tended in women to be positively associated with serum creatinine.Cox regression analyses for incident coronary heart disease(CHD),adjusted for sex,age,non-HDL cholesterol,waist circumference and C-reactive protein,revealed significant inverse association with adiponectin tertiles in men but not women(HR=0.66;95%CI:0.32-1.38 for highest tertile).Cox regression for type-2 diabetes in a similar model(wherein glucose replaced non-HDL cholesterol),adiponectin tertiles appeared to protect in each gender.HR for incident hypertension roughly displayed unity in each of the adiponectin tertiles(P-trend=0.67).CONCLUSION:High adiponectin levels failed to protect against the development of hypertension and,in women,against CHD,presumably paralleling impairment in renal function as well.Involvement of adiponectin in autoimmune complex with loss of antioxidative-antiatherogenic properties may be underlying.展开更多

Methods Twent...Objective To explore the correlation between endothelia cells activation and cytokines (ET-1, NO) levels in patients with pulmonary hypertension (PH), and to discuss their roles in the development of PH. Methods Twenty patients with simple ventricular septal defect (VSD) were chosen as controls, and 30 patients with PH were studied. Plasma levels of ET-1 and NO were measured by radioimmunoassay or colorimetric method. Before cardiopulmonary bypass was established, the specimens from right lung were fixed with formaldehyde solution, embedded with paraffin and stained by SP immunohistochemistry. Intercellular adhesion molecule-1 (ICAM-1) expression was measured through the determination of the light density with computer imaging technology. Results Compared with that of the patients with simple VSD, the light density of ICAM-1 and plasma level of ET-1 increased in patients with PH; but plasma level of NO decreased (P<0.05). Positive correlation was observed between ICAM-1 and ET-1/NO (P<0.05). Conclusion Endothelia cells activation and imbalance of ET-1/NO might play an important role in the development of PH.展开更多

Aim: The aim of the present study was to estimate the prevalence of coronary heart diseases (CHD) risk factors among Ghaza (Palastine) university students, to assess the CHD risk among them using a suitable scoring sy...Aim: The aim of the present study was to estimate the prevalence of coronary heart diseases (CHD) risk factors among Ghaza (Palastine) university students, to assess the CHD risk among them using a suitable scoring system and to identify how they perceive their risk of CHD. Methods: During the period from May 2008 to May 2009, 501 students were involved. Participants were subjected to the following activities;self administered questionnaire: including socio-demographic data, medical history of chronic diseases, family history about CHD, knowledge and perception of risk factors, anthropometric measurements in addition to laboratory testing. Results: The mean age was 20.8 ± 2.07 years;54% were females. The prevalence of hypertension and DM was 3.6% and 0.4% while it was 2.6% for hypercholesterolemia. The mean levels of LDL-C (88 mg/dl vs 85.5 mg/dl) and HDL-C (52.4 mg/dl vs 42.6 mg/dl) were higher among females than among males. Smoking was more prevalent among males than among females (33.1% vs 1.7%) with a total prevalence of 19%. Overweight and obesity were more prevalent among males (30.7% vs 22.5% and 9.6% vs 5.6%). In contrast to risk perception female students tend to be more knowledgeable than males with regards to different aspects of CHD. The overall level of perceived risk was moderate. Logistic regression analysis revealed that age and sex were associated significantly by higher level of total perception (p < 0.05). Conclusion: Health education and health promotion programs should be implemented and integrated within the primary health care sectors and directed to university students before admission. Further research needed to be implemented on larger sample to test knowledge and perception of the public in regard to their risk for heart disease including school children and community so that education could be provided in a more focused manner.展开更多

Objective: To study the characteristics of pulse tracings in CHD, and objectively evaluate the significance of pulse signal in diagnosis and appreciation of therapeutic effect in patients with coronary heart disease(C...Objective: To study the characteristics of pulse tracings in CHD, and objectively evaluate the significance of pulse signal in diagnosis and appreciation of therapeutic effect in patients with coronary heart disease(CHD), and accordingly provide with theoretic proofs for developing non-invasive technique of pulse diagnosis. Methods: By using the pulse detection system, pulse graphs in CHD patients, patients without CHD and "health" adults were collected and compared. Then characters of the pulse signal were analyzed with Hilbert-Huang transformation routine (HHT) and time-domain method respectively. Results: There existed characteristic change in pulse graph in CHD. ① h1,h3,h4,h3/h1,t,t5/t4 in time domain parameters of pulse graph increased and w1 was widened. ② 44% of C2 wave in HHT display showed chaotic and disorderly wave and irregularly wave amplitude in CHD. And 72% of C5 Wave exhibited in irregular wave with average wave amplitude over 10 gram-forces. These changes were significantly different from health adults. Conclusion: Characteristic wave of pulse graph extracted with methods of time domain or HHT routine might be considered as proofs for diagnosis and differentiation in CHD. Our researches prognosticate that pulse diagnosis can be used as an ancillary determination in occurrence of CHD for reasons of the advantage of convenient operation and non-invasion.展开更多

Objective:To investigate the effect of psychological education combined with progressive muscle relaxation training on the symptom cluster and rehospitalization of elderly patients with coronary heart disease.Methods:...Objective:To investigate the effect of psychological education combined with progressive muscle relaxation training on the symptom cluster and rehospitalization of elderly patients with coronary heart disease.Methods:This study is a longitudinal randomized controlled study involving 140 elderly patients with coronary heart disease.The patients were divided into two groups:an intervention group and a control group,with 70 cases in each group,via random number table.The patients in the control group received routine nursing,whereas those in the intervention group received psychological education combined with progressive muscle relaxation training for 4 weeks on the basis of routine nursing.The effect of the intervention was evaluated before intervention,at the end of 1 month,3 months,and 6 months after intervention.Results:At the end of 1 month,3 months,and 6 months,the sleep,fatigue,anxiety,and functional status of the patients in the intervention group were significantly better than those of the control group,with statistical significance difference(P<0.05).The rehospitalization rate of the intervention group was lower than that of the control group,and the difference was statistically significant(X2=10.685,P=0.001).Conclusion:Psychological education combined with progressive muscle relaxation training is effective in alleviating the symptom cluster of elderly patients with coronary heart disease and reducing their rehospitalization rate;thus,it should be popularized.展开更多
Objective:To analyze the independent risk factors and establish a risk prediction model by investigating the readmission of elderly patients with coronary heart disease(CHD)within 1 year after discharge.Methods:A tota...Objective:To analyze the independent risk factors and establish a risk prediction model by investigating the readmission of elderly patients with coronary heart disease(CHD)within 1 year after discharge.Methods:A total of 480 CHD patients,who were hospitalized in the Affiliated Hospital of Hebei University from October 2019 to December 2020,were included in this study.A general data scale,mental health status scale,the Clinical Frailty Scale,Pittsburgh Sleep Quality Index,as well as the Family Adaptability and Cohesion Evaluation Scale were used to collect data.According to the number of readmissions due to CHD within 1 year after discharge,the patients were divided into two groups:the readmission group(n=212)and the no readmission group(n=268).General data,laboratory examination indicators,frailty,mental health status,sleep status,as well as family intimacy and adaptability were compared between the two groups.Logistic regression was used to analyze the independent risk factors for the readmission of these patients,and R software was used to construct a line diagram model for predicting readmission of elderly patients with CHD.Results:Five factors including body mass index(OR=1.045),low density lipoprotein(OR=1.123),frailty(OR=1.946),mental health(OR=1.099),as well as family intimacy and adaptability(OR=0.928)were included to construct the risk prediction model for the readmission of elderly patients with CHD within 1 year after discharge.The ROC curve showed that the area under the curve for predicting readmission of elderly patients with CHD was 0.816;Hosmer-Lemeshow goodness of fit test showed X2=1.456 and P=0.989;the maximum Youden index corresponding to the predicted value of risk was 0.526.The results showed that the model could accurately predict the risk of readmission in elderly patients with CHD within 1 year after discharge.Conclusion:This study constructed a line diagram model based on five independent risk factors of the readmission of elderly patients with CHD:body mass index,low density lipoprotein,frailty,mental health status,as well as family intimacy and adaptability.This model has good discrimination,accuracy,and predictive efficiency,providing reference for the early prevention and intervention of readmission in elderly patients with CHD recurrence.展开更多

[Methods]One hundred an...[Objectives] To explore the efficacy of Danlou Tablet( DLT) in the treatment of coronary heart disease( CHD) with phlegm and blood stasis syndrome and its effects on serum inflammatory factors. [Methods]One hundred and ninety-seven patients with CHD and phlegm and blood stasis syndrome in our hospital from January 2016 to January 2018 were selected and randomly divided into two groups: control group( n =98) treated with aspirin plus atorvastatin,and research group( n =99) treated with DLT and aspirin plus atorvastatin for one month. The clinical efficacy and incidence of adverse reactions were observed. Serum secretory phospholipase A2( s PLA2),lipoprotein-associated phospholipase A2( LP-PLA2),oxidized low-density lipoprotein( ox-LDL),monocyte chemoattractant protein-1( MCP-1) and World Health Organization Quality of Life( WHOQOL-100) scores were compared before and after one month of treatment. [Results] The total effective rate was93. 94% in the research group,which was higher than that in the control group( 79. 59%,P < 0. 05);the levels of serum s PLA2,LP-PLA2,ox-LDL and MCP-1 in the research group were lower than those in the control group after one month of treatment( P < 0. 05). There was no statistical significance of the difference in the total incidence of adverse reactions between the research group and the control group( P > 0. 05).After one month of treatment,WHOQOL-100 scores were higher in two groups,which were higher in the research group than that in the control group( P < 0. 05). PubMed and Science Citation Index were searched for relevant papers. Forty eight papers satisfying inclusion criteria and reporting an association between a measure of depression and a coronary disease outcome were compared in terms of baseline assessment, exposure and endpoint definition, covariates measured and whether changes in, or treatment of, depression was assessed during follow-up. There was considerable variation in the definition of depression and coronary heart disease and contradictory findings are reported. Conventional risk factors for coronary heart disease were not assessed consistently or adequately. Only three of the forty-eight papers gave consideration to the time course of depression during follow-up and prior to study entry. Potentially confounding variables such as anxiety, personality traits and other psychiatric disorders were not taken into consideration in the majority of papers. Treatment of depression during the follow-up period was not mentioned in any of the papers. In light of identified methodological shortcomings and the inconsistent findings reported we suggest that there is as yet no convincing evidence that depression is an independent causal risk factor for coronary heart disease.展开更多
文摘As a result of research, high prevalence of arterial hypertension has been identified in the group of patients with coronary heart disease aged 60 and older. Most often, occurring form of arterial hypertension among geriatric patients is isolated systolic arterial hypertension (ISAH). The comparison of two ethnic groups of the population has showed higher levels of systolic and diastolic blood pressure (SBP and DBP) in the group of patients of non-indigenous nationality, rather than Yakut patients. Correlation was recorded in the group of non-indigenous patients as they age. Significant increase in the level of SBP was identified in the group of senile and long-livers than in the elderly. Detection of ECG signs of LVH showed the lowest specificity of Cornell voltage criterion comparing to Sokolow-Lyon criterion with its more often occurrence. ECG-signs of left ventricle hypertrophy are significantly more often established by Sokolow-Lyon criterion for men, Cornell voltage—for women. Correlation has been found between the presence of LVH and combination of Cornell voltage criterion with both Gubner-Ungerleider and Sokolow-Lyon criteria.

文摘Objective:To explore the characteristics and rules of Danshen Ligustrazine Injection in the treatment of coronary heart disease with hypertension.Methods:From the information systems of 12 tertiary tier-one hospitals across the country,we extracted the medical data of the application of Danshen Ligustrazine Injection in the treatment of patients with coronary heart disease and hypertension.After normalization,the model was established by Apriori algorithm,and the association rules were analyzed by Clementine 12.0 software.Results:Most of the 1928 patients were between 75 and 90 years old(54.26%).There were more males than females,most with type 2 diabetes,cerebral infarction,etc.Each dose was more than 10 mL(52.78%).Aspirin enteric-coated tablets(67.63%),L-carnitine injection(58.77%),and atorvastatin calcium capsules(50.93%)were often used in combination with safflower yellow pigment(22.20%),Shexiang Baoxin Pill(16.55%),Suxiao Jiuxin Pill(15.09%);the most commonly used combination of western medicine was anticoagulant thrombolytic and antiplatelet drugs(85.84%),and the type of Chinese medicine was Huoxuehuayu(72.98%);The most common combination of two western medicines was L-carnitine injection+aspirin enteric-coated tablets,with a support of 41.9%;The most common combination of two Chinese and western medicines is western medicine·anti-anginal medicine+western medicine·anticoagulant thrombolytic and antiplatelet drugs with a support of 67.6%.Conclusion:Danshen Ligustrazine injection is mainly used in elderly patients with coronary heart disease and hypertension,with many comorbidities.The dosage standard needs to be optimized.The combination of drugs and guidelines should coordinate with each other,which provide clues for clinical diagnosis and treatment and optimization of medication.
文摘The objective of this study was to analyze the efficacy of combined drug therapy for elderly patients with coronary heart disease and hypertension.66 elderly patients with coronary heart disease and hypertension were enrolled from December 2016 to November 2017.They were randomly divided into two groups,33 patients in each group.Patients in the experimental group received nifedipine.In combination with enalapril,patients enrolled in the control group received nifedipine monotherapy.Compared with the control group,the total effective rate,serum nitric oxide(NO)after treatment,C-reactive protein(CRP)after treatment,homocysteine(HCY)after treatment,and blood pressure after treatment were significantly improved(P<0.05).There were no significant differences in serum NO,pre-treatment CRP,pre-treatment HCY,pre-treatment blood pressure,and adverse reactions during treatment between the two groups(P>0.05).The elderly patients with coronary heart disease and hypertension are treated with nifedipine and enalapril.
文摘Objective:To analyze the correlation between serum Hcy content and coronary atherosclerosis severity in patients with H-type hypertension and coronary heart disease. Methods: 48 patients with H-type hypertension and coronary heart disease were selected as observation group, and 57 patients with normal hypertension and coronary heart disease were selected as control group. Echocardiography was used to determine coronary lesion parameters, enzyme-linked immunosorbent assay (ELISA) was used to determine serum levels of homocysteine (Hcy) and coronary heart disease-related indexes, and the correlation between Hcy levels and coronary heart disease was further analyzed. Results:Serum Hcy level of observation group was higher than that of control group (P<0.05), absolute GLPSS value and E/A value under echocardiography were less than those of control group while E-DT and E/e value were higher than those of control group (P<0.05);serum adiponectin (APN) level was lower than that of control group while P-selectin, asymmetric dimethylarginine (ADMA), oxidized high-density lipoprotein (OX-HDL), MMP-2, MMP-9, lipoprotein-associated phospholipase A2 (Lp-PLA2) and Resistin levels were higher than those of control group (P<0.05);Hcy was negatively correlated with absolute GLPSS value, E/A value and APN level, and was positively correlated with E-DT value, E/e value as well as P-selectin, ADMA, OX-HDL, MMP-2, MMP-9, Lp-PLA2 and Resistin levels (P<0.05). Conclusions:There is direct correlation between serum Hcy levels and the severity of coronary heart disease in patients with H-type hypertension and coronary heart disease, it can be a reliable way to early screen for coronary heart disease and evaluate the illness, and it is also a new target of coronary heart disease intervention.
文摘Objective: To investigate the effect of telmisartan combined with Nifedipine Controlled Release Tablet on inflammatory factors, vascular endothelial function and left ventricular function in patients with coronary heart disease with mild to moderate hypertension. Methods:A total of 92 cases of patients with coronary heart disease with mild to moderate hypertension were selected as the object of observation, according to the random data table, they were divided into the control group (n=46) and observation group (n=46), and patients in the control group were treated with Nifedipine Controlled Release Table therapy, on this basis, the observation group patients were given telmisartan treatment, two groups were treated for 6 months. The levels of the blood pressure, inflammatory factors, vascular endothelial function and left ventricular function compared between the two groups before and after treatment. Results: There were no significant differences in the levels of SBP, DBP, hs-CRP, TNF-α, NO, ET-1, LVEF, LVEDD and LVESD in the two groups before treatment. After treatment, two groups of SBP, DBP, hs-CRP, TNF-α, ET-1, LVEDD and LVESD levels were significantly lower than those in the same group before treatment, and after treatment, the levels of SBP, DBP, hs-CRP, TNF-α, ET-1 and LVESD in the observation group were significantly lower than those in the control group, while there were no significant difference in the level of LVEDD between the two groups after treatment;Compared with level in the group before treatment, the levels of NO and LVEF in the two groups were significantly increased, and the observation group [(82.13±19.01) μmol/L, (52.83±7.45)%] was significantly higher than the control group ((67.37±13.08) μmol/L, (49.47±6.96)%)Conclusion: Telmisartan combined with Nifedipine Controlled Release Table in treating coronary heart disease with mild to moderate hypertension, can effectively control blood pressure, reduce the inflammatory stress, improve vascular endothelial function and left ventricular function of patients, has an important clinical value.

文摘Objective To evaluate the capability of wrist pulse analysis in distinguishing three physiolog-ical and pathological conditions:healthy individuals,coronary heart disease(CHD)patients without a history of ischemic stroke,and CHD patients with a history of ischemic stroke.Methods Study participants were recruited from Shuguang East Hospital,Yueyang Hospital of Integrated Traditional Chinese and Western Medicine,and Shanghai Municipal Hospital of Traditional Chinese Medicine,affiliated with Shanghai University of Traditional Chinese Medicine,from April 15 to September 15,2021.They were categorized into three groups:healthy controls(Group 1),CHD patients without a history of ischemic stroke(Group 2),and CHD patients with a history of ischemic stroke(Group 3).The wrist pulse signals of the study participants were non-invasively collected using a pulse diagnosis instrument.The linear time-domain features and nonlinear time-series multiscale entropy(MSE)features of the pulse signals were extracted using time-domain analysis and the MSE methods,which were subsequently compared between groups.Based on these extracted features,a recognition model was developed using a random forest(RF)algorithm.The classification performance of the models was evaluated using metrics,including accuracy,precision,recall,and F1-score derived from confusion matrix as well as the area under the receiver operating characteristics(ROC)curve(AUC).Results A total of 189 participants were enrolled,with 63 in Group 1,61 in Group 2,and 65 in Group 3.Compared with Group 1,Group 2 showed significant increases in pulse features H2/H1,H3/H1,W1,W2,and W2/T,and decreased in MSE_(1)-MSE7(P<0.05),while Group 3 showed significant increases in pulse features T5/T4,T,H1/T1,W1,W2,AS,and Ad,and de-creased in MSE_(1)-MSE_(20)(P<0.05).Compared with Group 2,Group 3 demonstrated notable increases in H1/T1 and As(P<0.05).The RF model achieved precision of 80.00%,61.54%,and 61.54%,recall of 74.29%,60.00%,and 68.97%,F1-scores of 70.04%,60.76%,and 65.04%,and AUC values of 0.92,0.74,and 0.81 for Groups 1,2,and 3,respectively.The overall accuracy was 67.69%,with micro-average AUC of 0.83 and macro-average AUC of 0.82.Conclusion Differences in pulse features reflect variations in arterial compliance,peripheral resistance,cardiac afterload,and pulse signal complexity among healthy individuals,CHD patients without a history of ischemic stroke,and those with such a history.The developed pulse-based recognition model holds the potential in distinguishing between these three groups,offering a novel diagnostic reference for clinical practice.
文摘Left ventricular remodeling index (LVRI) was assessed in patients with hypertensive heart disease (HHD) and coronary artery disease (CAD) by real-time three-dimensional echocardiography (RT3DE). RT3DE data of 18 patients with HHD, 20 patients with CAD and 22 normal controis (NC) were acquired. Left ventricular end-diastolic volume (EDV) and left ventricular end-diastolic epicardial volume (EDVepi) were detected by RT3DE and two-dimensional echocardiography Simpson biplane method (2DE). LVRI (left ventricular mass/EDV) was calculated and compared. The results showed that LVRI measurements detected by RT3DE and 2DE showed significant differences inter-groups (P〈0.01). There was no significant difference in NC group (P〉0.05), but significant difference in HHD and CAD intra-group (P〈0.05). There was good positive correlations between LVRI detected by RT3DE and 2DE in NC and HHD groups (t=0.69, P〈0.01; r=0.68, P〈0.01), but no significant correlation in CAD group (r=0.30, P〉0.05). It was concluded that LVRI derived from RT3DE as a new index for evaluating left ventricular remodeling can provide more superiority to LVRI derived from 2DE.
文摘AIM:To investigate whether serum adiponectin protects against cardiometabolic risk in a population sample with prevailing metabolic syndrome.METHODS:Middle-aged adults representative of a general population with baseline circulating adiponectin measurements(n=1224)were analyzed prospectively at a mean of 3.8 years’follow-up,using continuous values or sex-specific tertiles.Total adiponectin was assayed by an ELISA kit.Type-2 diabetes was identified by criteria of the American Diabetes Association.Hypertension was defined as a blood pressure≥140 mmHg and/or≥90 mmHg and/or use of antihypertensive medication.Outcomes were predicted using Cox proportional hazards regression analysis in models that were controlled for potential confounders.RESULTS:In models of multiple linear regression,sex hormone-binding globulin,fasting insulin(inverse)and,in men,age were significant independent covariates of serum adiponectin which further tended in women to be positively associated with serum creatinine.Cox regression analyses for incident coronary heart disease(CHD),adjusted for sex,age,non-HDL cholesterol,waist circumference and C-reactive protein,revealed significant inverse association with adiponectin tertiles in men but not women(HR=0.66;95%CI:0.32-1.38 for highest tertile).Cox regression for type-2 diabetes in a similar model(wherein glucose replaced non-HDL cholesterol),adiponectin tertiles appeared to protect in each gender.HR for incident hypertension roughly displayed unity in each of the adiponectin tertiles(P-trend=0.67).CONCLUSION:High adiponectin levels failed to protect against the development of hypertension and,in women,against CHD,presumably paralleling impairment in renal function as well.Involvement of adiponectin in autoimmune complex with loss of antioxidative-antiatherogenic properties may be underlying.

文摘Objective To explore the correlation between endothelia cells activation and cytokines (ET-1, NO) levels in patients with pulmonary hypertension (PH), and to discuss their roles in the development of PH. Methods Twenty patients with simple ventricular septal defect (VSD) were chosen as controls, and 30 patients with PH were studied. Plasma levels of ET-1 and NO were measured by radioimmunoassay or colorimetric method. Before cardiopulmonary bypass was established, the specimens from right lung were fixed with formaldehyde solution, embedded with paraffin and stained by SP immunohistochemistry. Intercellular adhesion molecule-1 (ICAM-1) expression was measured through the determination of the light density with computer imaging technology. Results Compared with that of the patients with simple VSD, the light density of ICAM-1 and plasma level of ET-1 increased in patients with PH; but plasma level of NO decreased (P<0.05). Positive correlation was observed between ICAM-1 and ET-1/NO (P<0.05). Conclusion Endothelia cells activation and imbalance of ET-1/NO might play an important role in the development of PH.
文摘Diagnosing the cardiovascular disease is one of the biggest medical difficulties in recent years.Coronary cardiovascular(CHD)is a kind of heart and blood vascular disease.Predicting this sort of cardiac illness leads to more precise decisions for cardiac disorders.Implementing Grid Search Optimization(GSO)machine training models is therefore a useful way to forecast the sickness as soon as possible.The state-of-the-art work is the tuning of the hyperparameter together with the selection of the feature by utilizing the model search to minimize the false-negative rate.Three models with a cross-validation approach do the required task.Feature Selection based on the use of statistical and correlation matrices for multivariate analysis.For Random Search and Grid Search models,extensive comparison findings are produced utilizing retrieval,F1 score,and precision measurements.The models are evaluated using the metrics and kappa statistics that illustrate the three models’comparability.The study effort focuses on optimizing function selection,tweaking hyperparameters to improve model accuracy and the prediction of heart disease by examining Framingham datasets using random forestry classification.Tuning the hyperparameter in the model of grid search thus decreases the erroneous rate achieves global optimization.
文摘Aim: The aim of the present study was to estimate the prevalence of coronary heart diseases (CHD) risk factors among Ghaza (Palastine) university students, to assess the CHD risk among them using a suitable scoring system and to identify how they perceive their risk of CHD. Methods: During the period from May 2008 to May 2009, 501 students were involved. Participants were subjected to the following activities;self administered questionnaire: including socio-demographic data, medical history of chronic diseases, family history about CHD, knowledge and perception of risk factors, anthropometric measurements in addition to laboratory testing. Results: The mean age was 20.8 ± 2.07 years;54% were females. The prevalence of hypertension and DM was 3.6% and 0.4% while it was 2.6% for hypercholesterolemia. The mean levels of LDL-C (88 mg/dl vs 85.5 mg/dl) and HDL-C (52.4 mg/dl vs 42.6 mg/dl) were higher among females than among males. Smoking was more prevalent among males than among females (33.1% vs 1.7%) with a total prevalence of 19%. Overweight and obesity were more prevalent among males (30.7% vs 22.5% and 9.6% vs 5.6%). In contrast to risk perception female students tend to be more knowledgeable than males with regards to different aspects of CHD. The overall level of perceived risk was moderate. Logistic regression analysis revealed that age and sex were associated significantly by higher level of total perception (p < 0.05). Conclusion: Health education and health promotion programs should be implemented and integrated within the primary health care sectors and directed to university students before admission. Further research needed to be implemented on larger sample to test knowledge and perception of the public in regard to their risk for heart disease including school children and community so that education could be provided in a more focused manner.

文摘Objective: To study the characteristics of pulse tracings in CHD, and objectively evaluate the significance of pulse signal in diagnosis and appreciation of therapeutic effect in patients with coronary heart disease(CHD), and accordingly provide with theoretic proofs for developing non-invasive technique of pulse diagnosis. Methods: By using the pulse detection system, pulse graphs in CHD patients, patients without CHD and "health" adults were collected and compared. Then characters of the pulse signal were analyzed with Hilbert-Huang transformation routine (HHT) and time-domain method respectively. Results: There existed characteristic change in pulse graph in CHD. ① h1,h3,h4,h3/h1,t,t5/t4 in time domain parameters of pulse graph increased and w1 was widened. ② 44% of C2 wave in HHT display showed chaotic and disorderly wave and irregularly wave amplitude in CHD. And 72% of C5 Wave exhibited in irregular wave with average wave amplitude over 10 gram-forces. These changes were significantly different from health adults. Conclusion: Characteristic wave of pulse graph extracted with methods of time domain or HHT routine might be considered as proofs for diagnosis and differentiation in CHD. Our researches prognosticate that pulse diagnosis can be used as an ancillary determination in occurrence of CHD for reasons of the advantage of convenient operation and non-invasion.
文摘Coronary atherosclerotic heart disease is a disease of myocardial ischemia and hypoxia,which often presents as chest pain,dyspnea,cold sweat and fatigue.Fatigue is the subjective experience of patients,which is easy to be ignored,and will lead to the decline of patients’quality of life and physical activity level,etc.,with a high incidence and great harm.The purpose of this paper was to review the concept,risk factors,assessment tools and intervention measures of coronary atherosclerotic heart disease fatigue in order to provide a reference for identifying and improving the fatigue of coronary atherosclerotic heart disease.
文摘Objective:To investigate the effect of psychological education combined with progressive muscle relaxation training on the symptom cluster and rehospitalization of elderly patients with coronary heart disease.Methods:This study is a longitudinal randomized controlled study involving 140 elderly patients with coronary heart disease.The patients were divided into two groups:an intervention group and a control group,with 70 cases in each group,via random number table.The patients in the control group received routine nursing,whereas those in the intervention group received psychological education combined with progressive muscle relaxation training for 4 weeks on the basis of routine nursing.The effect of the intervention was evaluated before intervention,at the end of 1 month,3 months,and 6 months after intervention.Results:At the end of 1 month,3 months,and 6 months,the sleep,fatigue,anxiety,and functional status of the patients in the intervention group were significantly better than those of the control group,with statistical significance difference(P<0.05).The rehospitalization rate of the intervention group was lower than that of the control group,and the difference was statistically significant(X2=10.685,P=0.001).Conclusion:Psychological education combined with progressive muscle relaxation training is effective in alleviating the symptom cluster of elderly patients with coronary heart disease and reducing their rehospitalization rate;thus,it should be popularized.
文摘Objective:To analyze the independent risk factors and establish a risk prediction model by investigating the readmission of elderly patients with coronary heart disease(CHD)within 1 year after discharge.Methods:A total of 480 CHD patients,who were hospitalized in the Affiliated Hospital of Hebei University from October 2019 to December 2020,were included in this study.A general data scale,mental health status scale,the Clinical Frailty Scale,Pittsburgh Sleep Quality Index,as well as the Family Adaptability and Cohesion Evaluation Scale were used to collect data.According to the number of readmissions due to CHD within 1 year after discharge,the patients were divided into two groups:the readmission group(n=212)and the no readmission group(n=268).General data,laboratory examination indicators,frailty,mental health status,sleep status,as well as family intimacy and adaptability were compared between the two groups.Logistic regression was used to analyze the independent risk factors for the readmission of these patients,and R software was used to construct a line diagram model for predicting readmission of elderly patients with CHD.Results:Five factors including body mass index(OR=1.045),low density lipoprotein(OR=1.123),frailty(OR=1.946),mental health(OR=1.099),as well as family intimacy and adaptability(OR=0.928)were included to construct the risk prediction model for the readmission of elderly patients with CHD within 1 year after discharge.The ROC curve showed that the area under the curve for predicting readmission of elderly patients with CHD was 0.816;Hosmer-Lemeshow goodness of fit test showed X2=1.456 and P=0.989;the maximum Youden index corresponding to the predicted value of risk was 0.526.The results showed that the model could accurately predict the risk of readmission in elderly patients with CHD within 1 year after discharge.Conclusion:This study constructed a line diagram model based on five independent risk factors of the readmission of elderly patients with CHD:body mass index,low density lipoprotein,frailty,mental health status,as well as family intimacy and adaptability.This model has good discrimination,accuracy,and predictive efficiency,providing reference for the early prevention and intervention of readmission in elderly patients with CHD recurrence.

文摘[Objectives] To explore the efficacy of Danlou Tablet( DLT) in the treatment of coronary heart disease( CHD) with phlegm and blood stasis syndrome and its effects on serum inflammatory factors. [Methods]One hundred and ninety-seven patients with CHD and phlegm and blood stasis syndrome in our hospital from January 2016 to January 2018 were selected and randomly divided into two groups: control group( n =98) treated with aspirin plus atorvastatin,and research group( n =99) treated with DLT and aspirin plus atorvastatin for one month. The clinical efficacy and incidence of adverse reactions were observed. Serum secretory phospholipase A2( s PLA2),lipoprotein-associated phospholipase A2( LP-PLA2),oxidized low-density lipoprotein( ox-LDL),monocyte chemoattractant protein-1( MCP-1) and World Health Organization Quality of Life( WHOQOL-100) scores were compared before and after one month of treatment. [Results] The total effective rate was93. 94% in the research group,which was higher than that in the control group( 79. 59%,P < 0. 05);the levels of serum s PLA2,LP-PLA2,ox-LDL and MCP-1 in the research group were lower than those in the control group after one month of treatment( P < 0. 05). There was no statistical significance of the difference in the total incidence of adverse reactions between the research group and the control group( P > 0. 05).After one month of treatment,WHOQOL-100 scores were higher in two groups,which were higher in the research group than that in the control group( P < 0. 05). [Conclusions]DLT can significantly reduce the level of serum inflammatory factors,improve the quality of life in patients with CHD and phlegm and blood stasis syndrome.

CHD is rapidly becoming a major cause of morbidity and mortality in developing nations as well and is now recognized as the leading cause of death worldwide.
文摘A critical appraisal is made of the evidence that depression is a causal risk factor for coronary heart disease. PubMed and Science Citation Index were searched for relevant papers. Forty eight papers satisfying inclusion criteria and reporting an association between a measure of depression and a coronary disease outcome were compared in terms of baseline assessment, exposure and endpoint definition, covariates measured and whether changes in, or treatment of, depression was assessed during follow-up. There was considerable variation in the definition of depression and coronary heart disease and contradictory findings are reported. Conventional risk factors for coronary heart disease were not assessed consistently or adequately. Only three of the forty-eight papers gave consideration to the time course of depression during follow-up and prior to study entry. Potentially confounding variables such as anxiety, personality traits and other psychiatric disorders were not taken into consideration in the majority of papers. Treatment of depression during the follow-up period was not mentioned in any of the papers. In light of identified methodological shortcomings and the inconsistent findings reported we suggest that there is as yet no convincing evidence that depression is an independent causal risk factor for coronary heart disease.
