Current Trends in Clinical Practice for the Minimal Invasive Medial Unicondylar Knee Arthroplasty

Unicondylar Knee Arthroplasty (UKA) is an early alternative surgical procedure for the unicondylar osteoarthritis or damaged knee joint with artificial prosthesis for the release of disabling painful condition and restoring the normal knee functions. Minimally UKA is one of the recent and the majority successful procedures in modern orthopedics for the osteoarthritis which is spreading throughout the worldwide. Recently, many orthopedic surgeons are expanding their abilities in this field. However, it needs good knowledge and well experience for the successful clinical outcomes. The minimal invasive approach is more efficient for short hospital stay and faster postoperative recovery with low morbidity of the patients after UKA. The aim of this article is to emphasize the steps in UKA based on modern facts: function of knee joint, diagnosis, less invasive approach for medial condylar replacement, radiographic evaluation, and earlier recovery, selecting the patient and implant survivorship with review of surgery.

CURRENT TRENDS IN THE DEVELOPMENT OF THE CHINESE SOCIAL SECURITY SYSTEM FOR PEOPLE WITH DISABILITIES

Abstract The Law on the Protection of Disabled Persons is the first social security legislation in China, with more than 50 laws directly related to the legal protection of the rights of persons with disabilities and career development for people with disabilities. In terms of employment protections for people with disabilities, it is important to focus on effective legal implementation, utilizing legal rulings to limit employment discrimination against people with disabilities. Educational protections are an important precondition for enabling people with disabilities to live with dignity; it should be guaranteed that people with disabilities are provided ＂reasonable accommodation＂ in educational conditions. A barrier-free environment is a necessary condition for social integration of people with disabilities. At the same time, it also benefits other members of the society such as the elderly. Through rehabilitation and exercise, most people with disabilities can return to the society to a significant degree. China should gradually expand the legislative scope of rehabilitation service fee reductions and legal categories of auxiliary appliances, issuing guiding documents that provide specifications for the relevant ＂dormant＂ legal provisions.

Current practices regarding middle turbinate resection among otolaryngologists

Objectives:Resection of the middle turbinate(MT)during endoscopic sinus surgery(ESS)has been a controversial topic among otolaryngologists for many years.Some studies advocate resection and have shown improved outcomes postoperatively,while studies favoring preservation show a decreased incidence of postoperative complications.The current practice pattern regarding this subject is unknown.The goal of this study was to learn the current practice of MT resection during ESS among otolaryngologists.Method:We performed an electronic anonymous survey of practicing otolaryngologists.Results:We found that the majority of the 252 responders stated that they will perform an MT resection in certain clinical situations,while there is a small subset that advocates never resecting the MT for inflammatory sinus disease(n=6,2.4%).Participants were significantly more likely to perform MT resection in patients undergoing revision compared to primary ESS for all conditions included.The complication of greatest concern among participants was iatrogenic frontal sinus obstruction,while empty nose was of the least concern.The majority of participants responded that MT resection was of extreme or moderate benefit for improved visualization and drug delivery postoperatively.When compared to general otolaryngologists,fellowship-trained rhinologists were less concerned about potential complications following MT resection and were more likely to perceive an extreme or moderate benefit from turbinate resection postoperatively.Conclusion:There remains debate over MT resection among otolaryngologists,but the results of this study show that the majority of participating otolaryngologists will perform a resection in certain clinical situations.
