Timely and objective information on flood-related crop loss, such as flooded acreage and degree of crop damage, is very important for crop monitoring and ...Floods often cause significant crop loss in the United States. Timely and objective information on flood-related crop loss, such as flooded acreage and degree of crop damage, is very important for crop monitoring and risk management in ag- ricultural and disaster-related decision-making at many concerned agencies. Currently concerned agencies mostly rely on field surveys to obtain crop loss information and compensate farmers' loss claim. Such methods are expensive, labor intensive, and time consumptive, especially for a large flood that affects a large geographic area. The results from such methods suffer from inaccuracy, subjectiveness, untimeliness, and lack of reproducibility. Recent studies have demonstrated that Earth observation (EO) data could be used in post-flood crop loss assessment for a large geographic area objectively, timely, accurately, and cost effectively. However, there is no operational decision support system, which employs such EO-based data and algorithms for operational flood-related crop decision-making. This paper describes the development of an EO-based flood crop loss assessment cyber-service system, RF-CLASS, for supporting flood-related crop statistics and insurance decision-making. Based on the service-orientated architecture, RF-CLASS has been implemented with open interoperability specifications to facilitate the interoperability with EO data systems, particularly the National Aeronautics and Space Administration (NASA) Earth Observing System Data and Information System (EOSDIS), for automatically fetching the input data from the data systems. Validated EO algorithms have been implemented as web services in the system to operationally produce a set of flood-related products from EO data, such as flood frequency, flooded acreage, and degree of crop damage, for supporting decision-making in flood statistics and flood crop insurance policy. The system leverages recent advances in the remote sensing-based flood monitoring and assessment, the near-real-time availability of EO data, the service-oriented architecture, geospatial interoperability standards, and the standard-based geospatial web service technology. The prototypical system has automatically generated the flood crop loss products and demonstrated the feasibility of using such products to improve the agricultural decision-making. Evaluation of system by the end-user agencies indicates that significant improvement on flood-related crop decision-making has been achieved with the system.展开更多

Background:China’s accelerating development and increasingly important role in global health engagement create a great demand for global health professionals including international consulting experts.This study repo...Background:China’s accelerating development and increasingly important role in global health engagement create a great demand for global health professionals including international consulting experts.This study reported the detailed development and evaluation of an international consulting training for global health workforce.Methods:Based on Kirkpatrick’s model,a mixed-methods approach was used to evaluate the effectiveness of the training.Quantitative and qualitative data on participants’reaction,learning,and application of the learned knowledge and skills were collected by a training evaluation survey at the ending of training and a follow-up interview in three months after the training.Results:Thirty-six participants attended the training and 34 of them completed quantitative investigation.The training satisfaction evaluations were positive,for which participants rated the training program highly and over 90%of them agreed with the usefulness of the training.About knowledge and skills change,participants showed improved consulting knowledge and skills from pre-to post-training(P<0.001).A total of 23 participants accepted follow-up interview,and most participants applied knowledge and skills learned from the training in their daily work or study.However,only 30.4%of participants applied their learning in the consulting program.The largest barrier of application was the lack of consulting opportunities.In addition,almost all the participants reported that they would be glad to attend more training courses in the future.Conclusion:The international consulting training program was well-received,and was feasible to improve the consulting service competence of global health professionals.According to participants’feedback,it is essential to develop and expand consulting training in the field of global health.展开更多

The current study evaluated how physicians in primary care team and consultation team perc...Objectives The in-hosptial palliative care consultation(PCC) is emerging as a routine service in some medical center in China. The current study evaluated how physicians in primary care team and consultation team perceive the PCC service for the purpose of investigating the effectiveness of this consultation model in a general hospital.Methods In-hosptial palliative care consultations have been carried out at Peking Union Medical College Hosptial by a dedicated consultation team, and 37 consultations were completed in 2016. A questionnaire was designed for physicians in terms of its benefits to patients,their family as well as the primary care team. Physicians who applied for consultation in 2016 formally(requested from the department other than the Geriatrics) and informally(by rotating residents and unemployed visiting doctors in geriatric department) were invited to participate in the survey by scanning a two dimentional code on social networking platform.Results There were 103 physicians participated in the survey, including primary care physicians from the department of Internal Medicine(n=8), Gynaecology(n=16) and Surgery(n=13), rotating residents(n=30), visiting doctors(n=16) in Geriatric department, and PCC team members(n=20). 94.0% of the non-PCC physicians agreed that PCC relieved the suffering of patients; 89.2% thought PCC improved the quality of patients' life; there were 91.6%, 95.2%, 90.4% physicians who felt it relieved the anxiety of patients, of family members and of care providers, respectively. There were 96.4% physicians who felt it could ease the tension in physician-patient relationship; 97.6% felt it lower the risk for medical negligence, and 96.4% of doctors who applied for PPC felt satisfied with PCC service in terms of process and achieving objectives of consultation. More primary-team physician agree "PCC service helps the physicians better understand palliative care" than PCC members(97.6% vs. 80%, P<0.05), while both were interested in learning more on palliative medicine(100% vs. 96.4%, P>0.05).Conclusion Palliative care consultation service in a general hospital is efficacious and acclaimed.The primary care physicians and the PCC members hold positive attitudes to the benefits that the PCC services bring to patients, family members, and physicians themselves. PCC for terminal patients in a general hospital may serve as a good modle for promotion of palliative care in China.展开更多

Methods: This is a secondary analysis of data from 52 men enrolled in the usual care control group of a randomized trial that focused on decision-making for newly diagnosed men with LPCa. We analyzed the patient-physician communication using the transcribed audio-recordings of real-time treatment consultations and a researcher-developed coding tool, including codes for communication behaviors (information giving, seeking, and clarifying/ verifying) and contents of clinical consultations (health histories, survival/mortality, treatment options, treatment impact, and treatment preferences). After qualitative content analysis, we categorized patient participation in communication about treatment-related clinical content, including “none” (content not discussed);“low” (patient listening only);“moderate” (patient providing information or asking questions);and “high” (patient providing information and asking questions). Results: Physicians mainly provided information during treatment decision consultations and patients frequently were not active participants in communication. The participation of patients with low and moderate cancer risk typically was: 1) “moderate and high” in discussing health histories;2) “low” in discussing survival/mortality;3) “low and moderate” in discussing treatment options;4) “none and low” in discussing treatment impacts;and 5) “low” in discussing treatment preferences. Conclusions: Findings suggest opportunities for increasing patient participation in communication about treatment decision-making for LPCa during clinical consultations.展开更多

The evaluation is based on the methods proposed by COSTANZA et al., and some modifications about unit value of forest and cropland system were made according to the real characters of ecosystem, climate, natural conditions etc., in Jilin Province. Total value of ecosystem services is about 554.404x10(9) yuan(RMB)/a, which is about 4.9 times of GDP of the corresponding period. The results of this study could be used as a fundamental work for the construction of ecological province, which was carried out from 2001, and could provide ecological information for decision-making. The implementation of an electronic care records service (CRS) at our hospital in 2008 has led to widespread press coverage of ensuing chaos in clinical administration. We wanted to know how this new electronic system impacted on our working patterns in outpatient clinics and – more specifically – on the time actually spent with the patients. Material & methods: This study was performed 4 and 12 months after implementation of CRS to assess its impact on the time distribution in clinic. Senior doctors were monitored with a stop clock during consultations. Timings for pre- and post-consultation administration, and the actual consultation with the patient were recorded. A total of 170 consultations were evaluated in this way. Results: The key findings were that the total time needed to spend on a urological outpatient of 16 minutes remains unchanged from the pre-CRS era, but a majority (57%) of this time is spent in administration on the computer without the patient involved. Conclusion: No more than 15 patients should be seen in a 4 hour outpatient clinic per doctor. This recommendation drawn up by BAUS before CRS remains still valid. Patient administration related to the consultation that has previously been done by administrative aides is now to be done by the doctors on the computer in the same consultation session. Intended to streamline patient pathways, this does reduce the quality interaction-time between doctor and patients significantly.展开更多

Many businesses have experienced difficulties in selecting a cloud service provider(CSP)due to the rapid advancement of cloud computing services and the proliferation of CSPs.Many independent criteria should be consid...Many businesses have experienced difficulties in selecting a cloud service provider(CSP)due to the rapid advancement of cloud computing services and the proliferation of CSPs.Many independent criteria should be considered when evaluating the services provided by different CSPs.It is a case of multi-criteria decision-making(MCDM).This paper presents an integrated MCDM cloud service selection framework for determining the most appropriate service provider based on the best only method(BOM)and technique for order of preference by similarity to ideal solution(TOPSIS).To obtain the weights of criteria and the relative importance of CSPs based on each criterion,BOM performs pairwise comparisons of criteria and also for alternatives on each criterion,and TOPSIS uses these weights to rank cloud alternatives.An evaluation and validation of the proposed framework have been carried out through a use-case model to prove its efficiency and accuracy.Moreover,the developed framework was compared with the analytical hierarchical process(AHP),a popular MCDM approach,based on two perspectives:efficiency and consistency.According to the research results,the proposed framework only requires 25%of the comparisons needed for the AHP approach.Furthermore,the proposed framework has a CR of 0%,whereas AHP has 38%.Thus,the proposed framework performs better than AHPwhen it comes to computation complexity and consistency,implying that it is more efficient and trustworthy.展开更多

More primary-team physician agree "PCC service helps the physicians better understand palliative care" than PCC members(97.6% vs. 80%, P<0.05), while both were interested in learning more on palliative medicine(100% vs. 96.4%, P>0.05).Conclusion Palliative care consultation service in a general hospital is efficacious and acclaimed.The primary care physicians and the PCC members hold positive attitudes to the benefits that the PCC services bring to patients, family members, and physicians themselves. PCC for terminal patients in a general hospital may serve as a good modle for promotion of palliative care in China.
文摘Objectives: To describe the communication behaviors of patients and physicians and patient par-ticipation in communication about treatment decision-making during consultation visits for local-ized prostate cancer (LPCa). Methods: This is a secondary analysis of data from 52 men enrolled in the usual care control group of a randomized trial that focused on decision-making for newly diagnosed men with LPCa. We analyzed the patient-physician communication using the transcribed audio-recordings of real-time treatment consultations and a researcher-developed coding tool, including codes for communication behaviors (information giving, seeking, and clarifying/ verifying) and contents of clinical consultations (health histories, survival/mortality, treatment options, treatment impact, and treatment preferences). After qualitative content analysis, we categorized patient participation in communication about treatment-related clinical content, including “none” (content not discussed);“low” (patient listening only);“moderate” (patient providing information or asking questions);and “high” (patient providing information and asking questions). Results: Physicians mainly provided information during treatment decision consultations and patients frequently were not active participants in communication. The participation of patients with low and moderate cancer risk typically was: 1) “moderate and high” in discussing health histories;2) “low” in discussing survival/mortality;3) “low and moderate” in discussing treatment options;4) “none and low” in discussing treatment impacts;and 5) “low” in discussing treatment preferences. Conclusions: Findings suggest opportunities for increasing patient participation in communication about treatment decision-making for LPCa during clinical consultations.

文摘In a digital world moving at a breakneck speed,consultancy services have emerged as one of the prominent resources for seeking effective,sustainable and economically viable solutions to a given crisis.The present day consultancy services are aided by the use of multiple tools and techniques.However,ensuring the security of these tools and techniques is an important concern for the consultants because even a slight malfunction of any tool could alter the results drastically.Consultants usually tackle these functions after establishing the clients’needs and developing the appropriate strategy.Nevertheless,most of the consultants tend to focus more on the intended outcomes only and often ignore the security-specific issues.Our research study is an initiative to recommend the use of a hybrid computational technique based on fuzzy Analytical Hierarchy Process(AHP)and fuzzy Technique for Order Preference by Similarity to Ideal Solutions(TOPSIS)for prioritizing the tools and techniques that are used in consultancy services on the basis of their security features and efficacy.The empirical analysis conducted in this context shows that after implementing the assessment process,the rank of the tools and techniques obtained is:A7>A1>A4>A2>A3>A5>A6>A7,and General Electric McKinsey(GE-McKinsey)Nine-box Matrix(A7)obtained the highest rank.Thus,the outcomes show that this order of selection of the tools and techniques will give the most effective and secure services.The awareness about using the best tools and techniques in consultancy services is as important as selecting the most secure tool for solving a given problem.In this league,the results obtained in this study would be a conclusive and a reliable reference for the consultants.

文摘The main purpose of this study is to give evaluation of ecological services of Jilin Province, Northeast China. To take this value into decision-making and GDP accounting system is considered to be one of the economic solutions for ecological problems. The evaluation is based on the methods proposed by COSTANZA et al., and some modifications about unit value of forest and cropland system were made according to the real characters of ecosystem, climate, natural conditions etc., in Jilin Province. Total value of ecosystem services is about 554.404x10(9) yuan(RMB)/a, which is about 4.9 times of GDP of the corresponding period. The results of this study could be used as a fundamental work for the construction of ecological province, which was carried out from 2001, and could provide ecological information for decision-making. Furthermore, the necessities for the further studies on the evaluation of ecological services and natural capital were discussed.
文摘Objective: Government targets to reduce waiting times are putting enormous pressures on outpatient services. The implementation of an electronic care records service (CRS) at our hospital in 2008 has led to widespread press coverage of ensuing chaos in clinical administration. We wanted to know how this new electronic system impacted on our working patterns in outpatient clinics and – more specifically – on the time actually spent with the patients. Material & methods: This study was performed 4 and 12 months after implementation of CRS to assess its impact on the time distribution in clinic. Senior doctors were monitored with a stop clock during consultations. Timings for pre- and post-consultation administration, and the actual consultation with the patient were recorded. A total of 170 consultations were evaluated in this way. Results: The key findings were that the total time needed to spend on a urological outpatient of 16 minutes remains unchanged from the pre-CRS era, but a majority (57%) of this time is spent in administration on the computer without the patient involved. Conclusion: No more than 15 patients should be seen in a 4 hour outpatient clinic per doctor. This recommendation drawn up by BAUS before CRS remains still valid. Patient administration related to the consultation that has previously been done by administrative aides is now to be done by the doctors on the computer in the same consultation session. Intended to streamline patient pathways, this does reduce the quality interaction-time between doctor and patients significantly.

文摘The advantages of a cloud computing service are cost advantages,availability,scalability,flexibility,reduced time to market,and dynamic access to computing resources.Enterprises can improve the successful adoption rate of cloud computing services if they understand the critical factors.Tofind critical factors,this studyfirst reviewed the literature and established a three-layer hierarch-ical factor table for adopting a cloud computing service based on the Technology-Organization-Environment framework.Then,a hybrid method that combines two multi-criteria decision-making tools—called the Fuzzy Analytic Network Process method and the concept of VlseKriterijumska Optimizacija I Kompromisno Resenje acceptable advantage—was used to objectively identify critical factors for the adop-tion of a cloud computing service,replacing the subjective decision of the authors.The results of this study determinedfive critical factors,namely data access secur-ity,information transmission security,senior management support,fallback cloud management,and employee acceptance.Finally,the paper presents thefindings and implications of the study.
文摘Many businesses have experienced difficulties in selecting a cloud service provider(CSP)due to the rapid advancement of cloud computing services and the proliferation of CSPs.Many independent criteria should be considered when evaluating the services provided by different CSPs.It is a case of multi-criteria decision-making(MCDM).This paper presents an integrated MCDM cloud service selection framework for determining the most appropriate service provider based on the best only method(BOM)and technique for order of preference by similarity to ideal solution(TOPSIS).To obtain the weights of criteria and the relative importance of CSPs based on each criterion,BOM performs pairwise comparisons of criteria and also for alternatives on each criterion,and TOPSIS uses these weights to rank cloud alternatives.An evaluation and validation of the proposed framework have been carried out through a use-case model to prove its efficiency and accuracy.Moreover,the developed framework was compared with the analytical hierarchical process(AHP),a popular MCDM approach,based on two perspectives:efficiency and consistency.According to the research results,the proposed framework only requires 25%of the comparisons needed for the AHP approach.Furthermore,the proposed framework has a CR of 0%,whereas AHP has 38%.Thus,the proposed framework performs better than AHPwhen it comes to computation complexity and consistency,implying that it is more efficient and trustworthy.
