Introduction: Several studies have reported a high prevalence of dental caries among the people with intellectual disabilities. In order to identify the sociodemographic, clinical and behavioral factors associated with it, we conducted a study in specialized centers welcoming subjects with intellectual disabilities in Ouagadougou (Burkina Faso). Objectives: The objective of this study was to identify the factors associated with it among intellectually disabled in the specialized centers of Ouagadougou. Methods: The study was cross-sectional analytical. The survey took place between November 2020 and January 2021 among people with intellectual disabilities in reception centers receiving a grant from the Ministry of Women, National Solidarity, Family and Humanitarian Action. Data collection was done with a questionnaire adapted from the World Health Organization (WHO) and dental caries was recorded according to WHO criteria in decayed, missing or filled teeth generating a DMF index. Logistic regression was used to identify factors associated with the presence of dental caries. Results: A total of 193 participants were included in the study with an average age of 12.4 years ± 7.9. The overall prevalence of caries measured with the DMF index was 58.6% 95% CI [51.2 - 65.8]. It was significantly associated with female gender (OR = 4.2;95% CI [1.4 - 12.5], p = 0,01), the clinical form of mental illness (epilepsy OR = 3.8 95%;CI [1.2 - 12.7], p = 0,02, trisomy OR = 5.0;95% CI [1.1 - 22.9], p = 0,03, motor autonomy OR = 0.2 95%;CI [0.1 - 0.7], p = 0,01) and at use of toothpaste OR = 9.33;95% CI [1.05 - 82.7], p = 0,04. Conclusion: Dental caries remains a very present pathology in most people living with an intellectual disability. Also, it is necessary to put in place prevention strategies to reduce its prevalence and improve access to oral care for these people. 展开更多

This study aimed to evaluate the Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC) of Coffea canephora aqueous extract (Cc), trigonelline (Tg) and 5-caffeoylquinic acid (5-CQA) against cariogenic microorganisms: Streptococcus parasanguinis ATCC 903 (SP), Lactobacillus rhamnosus ATCC 9595 (LR);and pathogens related to periodontal disease: Porphyromonas gingivalis ATCC 33277 (PG), Fusobacterium nucleatum ATCC 25586 (FN), Prevotella intermedia ATCC 49046 (PI) and Prevotella nigrescens ATCC 33563 (PN). Different concentrations of Cc (0.15625 to 10 mg/mL), Tg and 5-CQA (0.005 to 10.24 mg/mL) were tested. Chlorhexidine (0.05%) was used as positive control and the substances without the inoculum comprised the blank control. The Cc showed bacteriostatic action against SP (MIC = 5 mg/mL) and LR (MIC = 10 mg/mL). Tg showed bacteriostatic action against SP (MIC = 2.56 to 1.28 mg/mL), LR (MIC = 2.56 mg/mL), PG (MIC = 2.56 to 1.28 mg/mL), FN (MIC = 5.12 mg/mL), PN (MIC = 2.56 mg/mL), and PI (MIC = 2.56 to 1.28 mg/mL). Also, Tg showed bactericide properties against SP (MBC = 2.56 mg/mL), PG (MBC = 2.56 mg/mL), FN (MBC = 10.24 mg/mL), PN (MBC = 5.12 mg/mL), and PI (MBC = 2.56 mg/mL). Although 5-CQA has previously shown activity against Streptococcus mutans, in the present study, it showed no activity against all tested microorganisms. C. canephora extract only showed antibacterial activity against cariogenic microorganisms, not presenting action against periodontal pathogens. It was concluded that trigonelline presented the best effect against all pathogens tested, therefore coffee extracts with higher trigonelline content should be tested against these specific pathogens. 展开更多

Alkali production by oral bacteria is believed to have a major impact on oral microbial ecology and to be inibitory to the initiation and progression of dental caries. A substantial body of evidence is beginning to accumulate that indicates the modulation of the alkalinogenic potential of dental biofilms may be a promising strategy for caries control. This brief review highlights recent progress toward understanding molecular genetic and physiologic aspects of important alkali-generating pathways in oral bacteria, and the role of alkali production in the ecology of dental biofilms in health and disease. 展开更多

Objective: To explore the relationships between the daily total intake of fluoride, dental fluorosis and dental caries. Methods: An epidemiological method was used to investigate the daily total intake of fluoride, dental fluorosis, and dental caries among 236 and 290 children aged 8-13 years in a severe endemic area and in a non-fluorosis control area, respectively. The children were divided into eight subgroups according to each child' s estimated daily total intake of fluoride. The prevalence of dental fluorosis and dental caries in each group was calculated. Results: As expected, elevated levels of fluoride intake were significantly associated with a higher prevalence of dental fluorosis and an increasing amount of more severe defect dental fluorosis. When the daily total F intake was 2.78 mg/child/day, the prevalence of dental fluorosis was nearly 100%, with the prevalence of defect dental fluorosis increasing with increasing fluoride intake. There was also a significant negative(inverse) dose-response relationship between the daily total intake of fluoride and the overall prevalence of dental caries, the prevalence of which decreased when the daily total intake of fluoride increased up to 3.32 mg/child/day. However, at higher levels of daily total intake of fluoride the prevalence of dental caries increased, giving rise to a U-shaped dose-response relationship curve. Conclusion: It is important to monitor total fluoride exposure and protect children from excessive fluoride intake, especially during the years of tooth development. 展开更多

Objective:We applied hyperspectral imaging(HSI)system to distinguish early caries from soundand pigmented areas. It will provide a theoretical basis and technical support, for research anddevelopment of an instrument that could be used for screening and detection of early dentalcaries. Methods:Eighteen extracted human teeth(molars and premolars),with varying degrees ofnatural pathology and no degree of decay involving dentin were obtained.HSI system with awavelength range from 400 to 1000nm was used to obtain images of all 18 teeth containingsound, carious and pigmented areas. We compared the spectra of the wavebands at both 500 nmand 780 nm from the different tooth states, and the reflectance diference bet ween sound versuscarious lesions and sound versus pigmented areas, respectively. Results:There was a slight diference in refectance bet ween carious areas and pigmented areas at500 nm.A substantial difference was additionally noted in refectance bet ween carious areas andpigmented areas at 780 nm.Conclusion:The results have shown that the interference of tooth surface pigment can be elim-inated in the near-infrared(NIR)waveband,and the caries can be effectively identifed from the pigmented areas. Thus,it could be used to detect carious areas of teeth in place of the traditionalvisual inspection method or white light endoscopy.Clinical significance:The NIR difused light signal enables the identification of early caries frompigment and other interference, providing a reasonable detection tool for early detection andearly treatment of teeth diseases. 展开更多

Objective To analyze the prevalence and years lived with disability (YLD) from dental caries among children and adolescents and the time trends over the past two decades in Sichuan province, the largest province in west China. Methods Based on the Global Burden of Disease Study 2015 (GBD2015), which systematically assessed the epidemiological characteristics of major diseases and their transitions by country and region from 1990 to 2015, we extracted the estimated results for China. We then used the Bayesian meta-regression method to estimate the sex- and age-specific prevalences and YLDs from dental caries among children and adolescents under 15 years old in Sichuan province and compared them with global and national indicators for the same period. Results In 2015, there were almost 6 million cases of dental caries in children and adolescents (aged 〈 15 years) in Sichuan province, accounting for 6% of the total cases in China. For children under 5 years, the prevalence of deciduous caries was 55.9%, and the YLDs value was 10.8 per 100,000, while it was 24.3% and 5.1 per 100,000 respectively among 5- to 14-year-olds; for those aged 5 to 14 years, the prevalence of permanent caries was 21.5%, and the YLDs value was 11.5 per 100,000. From 1990 to 2015, the prevalence of dental caries for children under 5 years increased substantially, by 16.2%, and the YLDs increased by 8.7%. Among those aged 5 to 14 years, the prevalence increased and the YLDs decreased. Conclusion Dental caries remains a huge health burden in Western China. In contrast to the global and national data, the trend has increased rapidly over the past 25 years in this region. This work provides suggestions for the prevention and control for oral health in China with the policy of two-child. 展开更多

The estimate of dental caries among Chinese children at the microscale level using standard methodology remains unclear. In this study, we assessed and analyzed the disease burden of childhood dental caries in China by extracting data from the Global Burden of Disease, Injuries, and Risk Factors Study 2016 (GBD 2016). In 2016, the number of cases, prevalence, years lived with disability (YLD), and age-standardized YLD rate of dental caries was 93.0 million, 43.0%, 32,200 person years, and 14.8 per 100,000, respectively. Across 33 provincial units, the disease burden was highest in Hubei (YLD rate 28.6 per 100,000), lowest in Macao (9.1 per 100,000), while geographical clustering was not observed. Compared with 1990, the prevalence in 2016 decreased from 46.8% to 43.0%, and the YLD rate decreased from 16.5 per 100,000 to 14.8 per 100,000. Given the slight decrease in dental caries burden, the prevalence and disease burden remained high among Chinese children. Strategies for addressing the spatial inequity of childhood dental caries require geographical targeting. 展开更多

Biology is based on evolution;therefore, eu-kariotes like plants, animals and human beings are living in harmony with their procariote an-cestors. If this is obviously the normal way of life, then pathobiology follows the same basic principles. The highly complex harmony of the living world may be disrupted, and a punctuated disease starts which is different from the health equilibrium. To reflect this biological archetype on the main diseases in dentistry - caries and periodontitis - the avalanche model is being proposed based on scientific and clinical evi-dence. This way of argumentation does influ-ence the strategic planning of preventive and curative programmes in dental research as well as in daily practise. 展开更多

The rate of dental caries and periodontal diseases in the world is still high, regardless of progress in the oral and dental hygiene. The natural toothbrush or chewing stick called “Mis- wak” has been used since ancient history. In this research the efficacy of Miswak in preventing dental caries was investigated and compared with the efficacy of toothbrush and tooth- paste. The analytical and clinical trial method was applied for this research among high sch- ool’s students in the city of Yazd, Iran, 2006. Three hundred eighty second year’s students (190 cases and 190 controls) were examined dentally. Then the Miswak was distributed to the case group and required trainings were given to both groups. After one year, the examinations were repeated. For analyzing the data one-way variance analysis test, Kai square, Paired t-test and two variable analyses were used. In the beginning of this study, there were no significant differences between two groups (cases and controls) regarding their dental situation and the frequency of brushing their teeth (p-value = 0.162). In addition, there were no significant differences in DMFT between the two groups. The data collected at the end of the study showed a significant increase in DMFT in the control group (p-value = 0.000). There was 55% increase in the rate of dental caries in control group compared to case group (0.89 before the study and 1.38 after the study). The risk of dental caries for each tooth in control group was 9.35 times more than case group (9.14% and 0.98% respectively). Dental caries rate was detected slightly less in the case group at the end of this study. This might be as a result of the antimicrobial effects of Miswak. A longer study with more cases is needed to prove this suggestion. 展开更多

Introduction: Dental caries is the most prevalent chronic disease among schoolchildren. We aimed to investigate the association between dental caries index and socio-demographic status in schoolchildren by using a multilevel hurdle model. Materials and Methods: A cross-sectional epidemiological study was carried out on 906 primary school pupils in Kerman, Iran in 2012. The subjects were selected through a stratified cluster random sampling. At first, the whole target area was stratified according to two geographic areas, the north and south area. Then each area was stratified according to gender. In the next step, several schools were considered as clusters, which were selected randomly in each geographic area and gender strata. All the schoolchildren in these schools were included in this study as samples. Twelve-year-old pupils were examined for dental caries. The dependent variable in this study was the dmft/DMFT index. Eight variables, including socio-demographic information, were collected. Multilevel negative binomial hurdle model was employed for data analysis. Results: The prevalence of caries-free pupils was 30.1% and the mean dmft/DMFT was 3.6 ± 2.2. Negative binomial part of the adjusted model showed that the dmft/DMFT adjusted rate for females was 1.36 (95% CI: 1.10-1.79) times higher than males. Also, the dmft/DMFT adjusted rate for overweight pupils was 0.86 (95% CI: 0.74-0.98) times lower than those with normal weight. Logistic part of the adjusted model showed that the posibility of caries-free state in overweight pupils was 1.95 (95% CI: 1.22-3.11) times higher than those with normal weight. In addition, pupils whose fathers and mothers were workers and housewives, respectively, and those with a high maternal age were at a greater risk for caries experience than others (p < 0.05). Conclusion: BMI, gender, parent’s job and mother’s age at delivery were factors effecting on dental caries in schoolchildren. These pupils need more attention to dental care. 展开更多

Destruction of calcified tissue was caused by acids which are by product of carbohydrate metabolism of acidogenic bacteria consequent to dental caries. The purpose of this study was to assess the existence of acidogens potentially causing the dental caries and comparatively evaluation of efficacy of different toothpastes. The dental plaques of fifty persons belong to three age groups (1 - 20, 21 - 40, 41 - 60 year and above) were examined to identify microorganisms by the culture method. Thirty nine bacteria were isolated by spread plate method on BSMY I minimal media. Thirteen out of thirty nine, acidogens colonized in the dental plaques. Seven potentially acidogens CD17, CD26, CD27, CD28, CD29, CD34 and CD35 were treated with five different toothpastes. Inhibition effect of Triclosan and Fluoride containing tooth pastes were found more efficient. The results of the present study revealed that bacteria that commonly cause dental caries colonized in dental plaques of children and alcoholic person. Therefore, dental plaques must be considered a specific reservoir of colonization and subsequent dental caries. To reduce the dental problem triclosan and fluoride containing product should be recommended. 展开更多

Objectives: 1) To identify the hydrogeological characteristics and fluoride (F-) concentration in drinking water in the southeast of the state of Morelos, Mexico, and 2) to estimate the association between the prevalence and severity of fluorosis, dental caries, and water F-?concentration. Methods: A hydrogeological model was developed to determine the vertical distribution of lithostratigraphic units, rocks’ hydraulic conductivities, and groundwater flow characteristics. A total of 649 schoolchildren from two communities received oral examinations. One community was located in an area with 0.75 mg F-/L in the water supply and the other in an area with 1.6 mg F-/L. Fluorosis was assessed using the Thylstrup & Fejerskov Index (TFI) and the dental caries were assessed with the decayed, missing, and filled teeth (DMFT) index. The region groundwater was extracted from an inter-granular aquifer consisting of volcano clasts of intermediate volcanic chemical composition. Results: Oral examinations showed that 32.35% of the children had severe forms of fluorosis (TFI ≥ 4) in the community with 1.60 mg F-/L and 7.8% in the 0.75 mg F-/L community (p < 0.0001). DMFT = 0.53 (±1.28) and 0.42 (±0.95) in the community with 1.60 mg F-/L and 0.75 mg F-/L, respectively (p = 0.2259). Conclusions: The increasing extraction of underground water was associated with deeper water flows and higher F-?concentration. In the community with high F-concentration, a third of the children showed objectionable levels of fluorosis. Adequate groundwater management actions are required to reduce fluorosis risk. 展开更多

Objective: The study aimed to determine an association between dental caries and asthma among 12 to 15 years old children. Methods: This is a school-based cross-sectional study conducted from January to February 2016. A random sample of 544 children aged 12 - 15 years were enrolled from five private schools of Karachi. Dental caries was assessed using DMFT Index (Decayed, Missing, Filled teeth). The main exposure variable was asthma and information on it was collected through the International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire. Results: The data was analyzed using Cox Proportional Hazard algorithm. Crude and adjusted prevalence ratios with 95% CI were reported. Total 554 children, 306 (56.3%) boys and 43.7% girls participated in the study. Mean age of children was 13.2 ± 0.05 years. Total number of children with DMFT > 0 was 30.5%. The decayed component contributed largely (22.8%) to the DMFT score. Overall prevalence of asthma was 20%. Prevalence of caries in asthmatic children was 28.4% as compared to 31% among non-asthmatic children. Adjusted prevalence ratio of dental caries in asthmatic children was 0.8 (95% CI 0.6 - 1.3) after adjusting for carious food intake, age, oral hygiene index and dentist visit;the association between asthma and dental caries turned out to be in-significant. Conclusions: There was no association observed between asthma and dental caries among the children examined in this study. 展开更多

Although aluminum itself was reported to prevent dental caries, previous in vitro studies reported that the aluminum level in the whole saliva of children was independent of caries prevalence. Purposes of this study were to compare the elution levels of aluminum and calcium from deciduous enamel into acidic artificial saliva, and determine whether the degree of aluminum elution reflects individual caries risk. One hundred and eleven extracted human deciduous teeth were collected. Concentrations of aluminum and calcium eluted from sound regions of enamel into artificial saliva (pH 6.2 or 5.5) were determined using atomic absorption spectrophotometry. One hundred and four and 108 available data samples were obtained for aluminum and calcium evaluation, respectively, and were compared based on the sex, tooth type, caries history, and the number of the donor’s carious teeth at each pH. Calcium elution was not affected by the sex;however, more aluminum was eluted from boys’ than from girls’ teeth at pH 5.5. The aluminum release depended on the tooth type, and was significantly higher in incisors than in molars at a pH of 5.5. In relation to the caries history, aluminum (at pH 5.5) and calcium (at pH 6.2) dissolved more from sound regions of the enamel of caries-experienced teeth than from the enamel of teeth without caries experience. Moreover, aluminum markedly dissolved from enamel in artificial saliva at both pH 6.2 and 5.5 when the donor had more carious teeth, regardless of the presence or absence of caries experience in the sampled teeth. There is a possibility that the rate of aluminum elution from enamel into acidic fluid reflects individual caries sensitivity. 展开更多
