We describe the development and validation of the Dai...Introduction: Fatigue is an important fibromyalgia (FM) symptom, but existing measures of fatigue are unlikely to meet regulatory standards for clinical trial use. We describe the development and validation of the Daily Diary of Fatigue Symptoms—Fibromyalgia (DFS-Fibro), a 24-hour recall, patient-reported outcome (PRO) measure of fatigue in FM that is administered electronically (ePRO). Methods: There were 3 phases of work: 1) item generation based on concept elicitation interviews with FM patients, with clinical relevance confirmed by expert clinician review;2) pilot testing/cognitive debriefing interviews with FM patients;and 3) psychometric validation using data from a methodology study with 145 FM patients. The measure was finalised based on both qualitative and quantitative findings. Results: Twenty-three items were generated in phase 1, some minor revisions were made following the pilot testing and cognitive debriefing (phase 2) but none were deleted. All patients found the measure easy to understand and use. Item reduction was conducted taking into account both the initial psychometric data and the earlier qualitative research, resulting in a final 5 item measure of the “symptom” of fatigue. The 5-item DFS-Fibro had very high internal consistency (alpha = 0.99), and strong test-re-test reliability (r > 0.84), convergent validity and known-groups validity. Conclusion: The DFS-Fibro has strong psychometric properties and strong face and content validity for the measurement of fatigue in FM.展开更多

This study focused on the quantitative and qualitative effects of ICU diaries on distorted memories that patients encountered during their time in the ICU. Aim: To assess improving distorted memories by providing information during ICU admission to patients to relieve the acute stress symptoms after ICU discharge. Design and Methods: This nonrandomized controlled trial included patients aged ≥20 years who had received treatment for ≥3 days in the ICU. Using ICU memory tool, the Hospital Anxiety and Depression Scale (HADS) and the Acute Stress Disorder Scale (ASDS) approximately 1 week after ICU discharge, then HADS and ASDS prior to hospital discharge, the change in mean values were compared between two groups (control and intervention) using a Wilcoxon signed-rank test. The intervention group was surveyed prior to hospital discharge using semi-structured interviews, and descriptions were analyzed by the content analysis method of Krippendorff. Results: Twenty-three participants were included in the control group, and 17 in the intervention group. When comparing subjects with distorted memories between the groups, we found reduced values for HADS anxiety (7.1 ± 3.8 to 5.7 ± 2.7, p = 0.011), HADS depression (8.6 ± 5.0 to 7.2 ± 4.3, p = 0.003), and ASDS (46.9 ± 13.8 to 43.8 ± 11.4, p = 0.012) in the intervention group. Based on the narrations, patients wanted to investigate facts in the ICU diary while having someone help them to remember these events. Conclusion: Improving distorted memories during ICU admissions may relieve acute stress symptoms. In addition, these patients needed someone to investigate facts from the ICU admission.展开更多
AIM: To investigate recruitment, retention, and estimates for effects of formula supplementation with Lactobacillus rhamnosus GG(LGG) on inflammatory biomarkers and fecal microbial community in infants with colic. MET...AIM: To investigate recruitment, retention, and estimates for effects of formula supplementation with Lactobacillus rhamnosus GG(LGG) on inflammatory biomarkers and fecal microbial community in infants with colic. METHODS: A prospective, double-blind, placebocontrolled trial was conducted in otherwise healthy infants with colic. We screened 74 infants and randomized and analyzed results in 20 infants [9 receiving LGG(LGG+) and 11 not receiving LGG(LGG-)]. LGG was incorporated in the formula(Nutramigen~®)(minimum of 3 × 107 CFU/d) in the LGG+ group. Fecal microbiota and inflammatory biomarkers, including fecal calprotectin(FC), plasma cytokines, circulating regulatory T cells(Tregs), and crying + fussing time were analyzed to determine optimal time points and effect sizes for a larger trial. RESULTS: Recruitment in this population was slow, with about 66% of eligible infants willing to enroll; subject retention was better(75%). These rates were influenced by parents' reluctance to volunteer their infant for a clinical trial and by their tendency to change formulas. The maximal difference of crying + fussing time was observed at day 14, comparing the 2 groups, with a mean difference of-91(95%CI:-76, 259) min(P = NS). FC showed no significant difference, but the optimal time to determine a potential effect was at day 90 [with a mean difference of 121(95%CI:-48, 291) μg/g stool], observing a lower level of FC in the LGG+ group. The fecal microbial communities were chaotic, as determined by Shannon's diversity index and not apparently influenced by the probiotic. No significant change was observed in plasma inflammatory cytokines or Tregs, comparing LGG+ to LGG- groups. CONCLUSION: Designing future colic trials involving a probiotic-supplemented formula for infants in the United States will require consideration for difficult enrollment. Infants with colic have major variations in feal microbiota and calprotectin, both of which improve with time, with optimal time points for measurement at days 14 and 90 after treatment.展开更多

[Method] A total of 36 Holstein cows at same parity and with similar milk yield were randomly divided into three groups, namely, control group, Chinese medi- cine group I and Chinese medicine group I1. The cows in the Chinese medicine group I were fed basal diet containing 4% compound Chinese herb additive; those in the Chinese medicine group II were fed basal diet containing 3% compound Chinese herb additive; and those in the control group were fed common basal diet. All cows in the three groups were analyzed comparatively. [Result] Supplementing some compound Chinese herb ad- ditive into basal diet increased milk yield and anti-heat stress ability of Holstein cows, besides, it also decreased cow diseases like cow mastitis. The Chinese medicine group I generated the highest economic benefit of practical production. [ Conclusion] Supplementing Chinese herb additive in- to basal diet is an effective way to relieve heat stress and increase milk yield.展开更多

As an action research, the report is an attempt to explore the role played by annotation in reading comprehension in English as a Foreign Language (EFL) classrooms. By means of learner diaries, the researcher found that annotation, as an efficient writing-to-learn strategy, is able to help EFL learners to research a deeper level of engagement and to encourage active reading. 文摘AIM: To investigate recruitment, retention, and estimates for effects of formula supplementation with Lactobacillus rhamnosus GG(LGG) on inflammatory biomarkers and fecal microbial community in infants with colic. METHODS: A prospective, double-blind, placebocontrolled trial was conducted in otherwise healthy infants with colic. We screened 74 infants and randomized and analyzed results in 20 infants [9 receiving LGG(LGG+) and 11 not receiving LGG(LGG-)]. LGG was incorporated in the formula(Nutramigen~®)(minimum of 3 × 107 CFU/d) in the LGG+ group. Fecal microbiota and inflammatory biomarkers, including fecal calprotectin(FC), plasma cytokines, circulating regulatory T cells(Tregs), and crying + fussing time were analyzed to determine optimal time points and effect sizes for a larger trial. RESULTS: Recruitment in this population was slow, with about 66% of eligible infants willing to enroll; subject retention was better(75%). These rates were influenced by parents' reluctance to volunteer their infant for a clinical trial and by their tendency to change formulas. The maximal difference of crying + fussing time was observed at day 14, comparing the 2 groups, with a mean difference of-91(95%CI:-76, 259) min(P = NS). FC showed no significant difference, but the optimal time to determine a potential effect was at day 90 [with a mean difference of 121(95%CI:-48, 291) μg/g stool], observing a lower level of FC in the LGG+ group. The fecal microbial communities were chaotic, as determined by Shannon's diversity index and not apparently influenced by the probiotic. No significant change was observed in plasma inflammatory cytokines or Tregs, comparing LGG+ to LGG- groups. CONCLUSION: Designing future colic trials involving a probiotic-supplemented formula for infants in the United States will require consideration for difficult enrollment. Infants with colic have major variations in feal microbiota and calprotectin, both of which improve with time, with optimal time points for measurement at days 14 and 90 after treatment.

文摘[Objective] To explore the effects of Chinese herb additive on milk yield and anti-heat stress ability of Holstein diary cow. [Method] A total of 36 Holstein cows at same parity and with similar milk yield were randomly divided into three groups, namely, control group, Chinese medi- cine group I and Chinese medicine group I1. The cows in the Chinese medicine group I were fed basal diet containing 4% compound Chinese herb additive; those in the Chinese medicine group II were fed basal diet containing 3% compound Chinese herb additive; and those in the control group were fed common basal diet. All cows in the three groups were analyzed comparatively. [Result] Supplementing some compound Chinese herb ad- ditive into basal diet increased milk yield and anti-heat stress ability of Holstein cows, besides, it also decreased cow diseases like cow mastitis. The Chinese medicine group I generated the highest economic benefit of practical production. [ Conclusion] Supplementing Chinese herb additive in- to basal diet is an effective way to relieve heat stress and increase milk yield.
文摘The perspective of literature formulations allows us to think of the Clinical Diary as an artistic product of Ferenczi’s unfolding experience of primary relatedness with his patients.The open and poetic style of his writing supports such an interpretation.Ferenczi’s own anxiety on surrendering to the experiment of mutual analysis was quite visible.Ferenczi undertook in intensive,genuinely interpersonal therapy:patient and analyst become both artist(writer)and medium.He was in fact the father of the post-Freudian approach to what he called the“traumatogenic”environing psychology of his patients.On a personal level,in his direct and forthright discussion of Freud and his work,he deeply influenced the creative unfolding of major psychoanalytic perspectives on theory and practice.The Clinical Diary is a personal novel of searching boundaries in psychoanalytical technical questions and also searching boundaries in human relations and in metapsychology of the conscious and unconscious processes.There are three storytellers:Ferenczi,his patients and Freud.Their storylines are mixed in special ways focusing on the secret questions of the analytical method.
文摘In this study, the author investigated insights of 15 college students majored in English as they explored their own use of annotation strategy and how they applied this strategy in effective reading process. As an action research, the report is an attempt to explore the role played by annotation in reading comprehension in English as a Foreign Language (EFL) classrooms. By means of learner diaries, the researcher found that annotation, as an efficient writing-to-learn strategy, is able to help EFL learners to research a deeper level of engagement and to encourage active reading. The findings also suggest that reading strategy training can be integrated with reading tasks in the EFL classrooms.

文摘Lu Yin庐隐(1898-1934)was a leading“new woman”,and she had scaled the heights of her writing profession in mid-Republican China.In her short 36-year life,she published over 200 works in all,including novels,essays,proses and travelogues.As the first May Fourth female writer who touched upon the theme of homosexuality and depicted a lesbian story without reservation,Lu Yin created a short story“The Diary of Lishi”in which she narrated the love between Lishi and her same-sex partner Yuanqing;but this finally ended in tragedy,which implied Lu Yin’s deconstruction of binary gender order.Meanwhile,this paper distinguishes“Old Friends on the Seashore”from lesbian or quasi-lesbian stories and argued that this fiction was more in terms of female same-sex camaraderie instead of lesbian love.
