Background: It is said that after ICU discharge, 40% of patients suffer from acute stress symptoms, such as anxiety and depression, caused by “distorted memories” (memory loss and delusions) due to various treatment...Background: It is said that after ICU discharge, 40% of patients suffer from acute stress symptoms, such as anxiety and depression, caused by “distorted memories” (memory loss and delusions) due to various treatments in the ICU. This study focused on the quantitative and qualitative effects of ICU diaries on distorted memories that patients encountered during their time in the ICU. Aim: To assess improving distorted memories by providing information during ICU admission to patients to relieve the acute stress symptoms after ICU discharge. Design and Methods: This nonrandomized controlled trial included patients aged ≥20 years who had received treatment for ≥3 days in the ICU. Using ICU memory tool, the Hospital Anxiety and Depression Scale (HADS) and the Acute Stress Disorder Scale (ASDS) approximately 1 week after ICU discharge, then HADS and ASDS prior to hospital discharge, the change in mean values were compared between two groups (control and intervention) using a Wilcoxon signed-rank test. The intervention group was surveyed prior to hospital discharge using semi-structured interviews, and descriptions were analyzed by the content analysis method of Krippendorff. Results: Twenty-three participants were included in the control group, and 17 in the intervention group. When comparing subjects with distorted memories between the groups, we found reduced values for HADS anxiety (7.1 ± 3.8 to 5.7 ± 2.7, p = 0.011), HADS depression (8.6 ± 5.0 to 7.2 ± 4.3, p = 0.003), and ASDS (46.9 ± 13.8 to 43.8 ± 11.4, p = 0.012) in the intervention group. Based on the narrations, patients wanted to investigate facts in the ICU diary while having someone help them to remember these events. Conclusion: Improving distorted memories during ICU admissions may relieve acute stress symptoms. In addition, these patients needed someone to investigate facts from the ICU admission.展开更多

First, by combining theories anddiscussions from linguistic and literature, the author will compare both positive and negative perspectives andexamine how the parenthesis is used in different texts. Next, the author will focus on how the notions of parenthesisturn from initially being considered as surplus, subordinate, and inferior, to the re-evaluation of their role as asignificant and indispensable part of the text, and most importantly, as the sign of the author's presence in the text.Borrowing Laura Rascaroli's concept of "sonic interstices," the author will look at how the voice-over in the diaryfilm functions and relates itself to the images in the space created by parenthetical insertion. Finally, the author willexamine Jonas Mekas's diary film--The Song of Avila as a case study, analyzing the relationship between thevoice-over and the images in this film by applying my concept of a parenthetical structure.展开更多

We describe the development and validation of the Dai...Introduction: Fatigue is an important fibromyalgia (FM) symptom, but existing measures of fatigue are unlikely to meet regulatory standards for clinical trial use. We describe the development and validation of the Daily Diary of Fatigue Symptoms—Fibromyalgia (DFS-Fibro), a 24-hour recall, patient-reported outcome (PRO) measure of fatigue in FM that is administered electronically (ePRO). Methods: There were 3 phases of work: 1) item generation based on concept elicitation interviews with FM patients, with clinical relevance confirmed by expert clinician review;2) pilot testing/cognitive debriefing interviews with FM patients;and 3) psychometric validation using data from a methodology study with 145 FM patients. The measure was finalised based on both qualitative and quantitative findings. Results: Twenty-three items were generated in phase 1, some minor revisions were made following the pilot testing and cognitive debriefing (phase 2) but none were deleted. All patients found the measure easy to understand and use. Item reduction was conducted taking into account both the initial psychometric data and the earlier qualitative research, resulting in a final 5 item measure of the “symptom” of fatigue. The 5-item DFS-Fibro had very high internal consistency (alpha = 0.99), and strong test-re-test reliability (r > 0.84), convergent validity and known-groups validity. Conclusion: The DFS-Fibro has strong psychometric properties and strong face and content validity for the measurement of fatigue in FM.展开更多
