Due to differences in economic and medical conditions among countries around the world, there is currently no unified treatment plan for anti-HP. In China, empirical quadruple therapy is mainly used. With the abuse of antibiotics, many patients face the problem of secondary eradication after failure, and the resistance rate of HP is gradually increasing. After eradication failure, drug sensitivity cultivation is carried out to choose sensitive antibiotics for treatment. A new strategy is currently needed to address how to improve the eradication rate of HP during the first eradication. This article aims to discuss the first-line treatment plans and research progress for eradicating HP based on drug sensitivity testing before eradication. Compared with traditional empirical therapies, treatment based on drug sensitivity results can effectively improve the eradication rate of HP, and reduce drug resistance rates, and adverse reactions, among other benefits. .展开更多
BACKGROUND Pediatric abdominal infection is a common but serious disease that requires timely and effective treatment.In surgical treatment,accurate diagnosis and rational application of antibiotics are the keys to im...BACKGROUND Pediatric abdominal infection is a common but serious disease that requires timely and effective treatment.In surgical treatment,accurate diagnosis and rational application of antibiotics are the keys to improving treatment effects.AIM To investigate the effect of broad-spectrum bacterial detection on postoperative antibiotic therapy.METHODS A total of 100 children with abdominal infection who received surgical treatment in our hospital from September 2020 to July 2021 were grouped.The observation group collected blood samples upon admission and sent them for broad-spectrum bacterial infection nucleic acid testing,and collected pus or exudate during the operation for bacterial culture and drug sensitivity testing;the control group only sent bacterial culture and drug sensitivity testing during the operation.RESULTS White blood cell count,C-reactive protein,procalcitonin,3 days after surgery,showed better postoperative index than the control group(P<0.05).The hospital stay in the observation group was significantly shorter than that in the control group.The hospitalization cost in the observation group was significantly lower than that in the control group,and the difference between the two groups was statistically significant(P<0.05).CONCLUSION Early detection of broad-spectrum bacterial infection nucleic acids in pediatric abdominal infections can help identify pathogens sooner and guide the appropriate use of antibiotics,improving treatment outcomes and reducing medical costs to some extent.展开更多

Establishment of a highly efficient regeneration system for the mature embryo of wheat will provide a convenient tool for wheat tissue culture and transformation, thereby facilitating the transformation of foreign gen...Establishment of a highly efficient regeneration system for the mature embryo of wheat will provide a convenient tool for wheat tissue culture and transformation, thereby facilitating the transformation of foreign genes into wheat. By using the mature embryos derived from 20 different wheat lines including Shi 4185, Yumai 66, Lunxuan 987, CB037, Yangmai 6, Xinchun 9, Bobwhite, Han 6172, Zheng 9023, Jimai 20, Ningchun 4, and Jing 411, the effects of some factors including inoculation methods, initiating culture media, organic additives, antioxidants, and auxins on the regeneration from the explants were evaluated. The results indicated that the scraping embryo culture was better than the whole embryo culture, the Aa medium was better than the SD2 medium and dicamba was better than 2,4-D in increasing the regeneration frequency. An Adi medium was established in this study by adding silver nitrate, cysteine, ascorbic acid, dicamba, glutamine into the Aa medium at the concentration of 4,40, 100, 2, and 5 mg L^-1, respectively. By using the Adi medium and the scraping technique, the regeneration frequencies of the mature embryos of CB037, Lunxuan 987, Hart 6172, Yangmai 6, Bobwhite, Zheng 9023, Shi 4 185, and Jimai 20 became 85.6, 60,1, 46.0, 42.1,42.0, 34.0, 33.0, and 32.0%, respectively, which were about 5-8 times higher than that obtained from the conventional culture mediums and techniques. This novel regeneration system could be helpful in wheat transformation.展开更多

目的研究多学科协作诊疗(multi-disciplinary collaborative diagnosis and treatment,MDT)模式在临床医学专业整体思维培养中的效果。方法选取雅安职业技术学院临床医学院2024年3—6月的33名临床专业22级学生作为观察组,另选取雅安职...目的研究多学科协作诊疗(multi-disciplinary collaborative diagnosis and treatment,MDT)模式在临床医学专业整体思维培养中的效果。方法选取雅安职业技术学院临床医学院2024年3—6月的33名临床专业22级学生作为观察组,另选取雅安职业技术学院临床医学院2023年3—6月的33名临床专业21级学生作为对照组。观察组采取MDT模式进行教学,对照组采用传统教学。比较2组学生的考试成绩、整体思维能力、考核评价得分以及教学总满意度。结果观察组学生考试成绩中的临床多学科基础知识[(22.42±1.28)分]、临床专业知识[(22.05±1.96)分]、临床技术实操[(18.75±1.57)分]、临床试验设计和验证[(13.29±0.42)分]及医患沟通基本技能[(14.66±0.25)分]成绩均高于对照组[(18.02±2.38)分、(15.33±3.01)分、(13.28±2.64)分、(10.04±2.02)分、(11.50±1.12)分],差异有统计学意义(P<0.05)。观察组学生整体思维能力中的临床资料检索能力[(18.33±1.15)分]、临床实践操作能力[(17.18±2.86)分]、临床沟通应变能力[(18.19±0.97)分]、临床分析理解信息能力[(19.13±0.32)分]、临床团队协作能力[(18.33±1.67)分]及临床解决问题与创新能力[(18.43±1.07)分]评分均高于对照组[(12.06±2.11)分、(12.77±2.71)分、(13.28±2.24)分、(13.34±1.75)分、(12.34±1.92)分、(13.28±2.68)分],差异有统计学意义(P<0.05)。观察组学生考核评价中的课前任务[(95.11±4.35)分]、课堂参与度[(96.11±2.34)分]、方案可行度[(92.49±4.22)分]、操作规范[(93.17±2.33)分]、医患沟通[(93.75±3.58)分]、人文关怀[(95.77±3.28)分]、课后作业评价得分[(95.65±4.30)分]均高于对照组[(80.23±5.35)分、(83.38±3.44)分、(80.28±3.17)分、(83.65±2.78)分、(80.37±3.65)分、(84.38±4.49)分、(80.44±5.75)分],差异有统计学意义(P<0.05)。观察组的教学总满意度为100%,高于对照组的69.70%,差异有统计学意义(P<0.05)。结论在临床医学专业教学中采取MDT模式能有效提高医学生的考试成绩,培养学生的整体思维能力,提高教学总满意度,具有显著的教学效果。展开更多
Objective The influence of anti-tuberculosis (TB) treatment history on tuberculous lymphadenitis (TBLN) diagnosis is unclear. Therefore, this study aims to evaluate the diagnostic methods, including histology, mic...Objective The influence of anti-tuberculosis (TB) treatment history on tuberculous lymphadenitis (TBLN) diagnosis is unclear. Therefore, this study aims to evaluate the diagnostic methods, including histology, microbiology, and molecular tests, used for TBLN. Methods In this study, suspected patients with TBLN and having different anti-T8 treatment background were enrolled. All the samples were tested simultaneously by histology, ZiehI-Neelsen (ZN) staining, mycobacterial culture (culture), Xpert MTB/RIF (xpert), real-time PCR, and high-resolution melting curve PCR (HRM). Thereafter, the performance of these methods on samples with different anti-TB treatment background was assessed. Results In our study, 89 patients were prospectively included 82 patients with TBLN and 7 with other diseases. The overall sensitivities of Xpert, real-time PCR, histology, ZN staining, and culture were 86.6%, 69.5%, 58.5%, 43.9%, and 22.0%, respectively. The anti-TB treatment history revealed dramatic influences on the sensitivity of culture (P 〈 0.0001). In fact, the treatment that lasted over 3 months also influenced the sensitivity of Xpert (P 〈 0.05). However, the treatment history did not affect the performance of remaining tests (P 〉 0.05). For rifampicin drug susceptibility test (DST), the anti-T8 treatment showed only significant influence on the success rate of culture DST (P = 0.001), but not on those of Xpert and HRM tests (P 〉 0.05). Conclusion Other tests as welt as culture should be considered for patients with TBLN having retreatment history or over 1-month treatment to avoid false negative results.展开更多

In this study, rat cerebral cortical neurons cultured in vitro were pretreated with 10 ktM curcumin or post-treated with 5 pM curcumin, respectively before or after being subjected to oxygen-glucose deprivation and reoxygenation for 24 hours. Both pretreatment and post-treatment resulted in a significant decrease of cell injury as indicated by propidium iodide/Hoechst 33258 staining, a prominent increase of Nrf2 protein expression as indicated by western blot analysis, and a remarkable increase of protein expression and enzyme activity in whole cell lysates of thioredoxin before ischemia, after ischemia, and after reoxygenation. In addition, post-treatment with curcumin inhibited early DNA/RNA oxidation as indicated by immunocytochemistry and increased nuclear Nrf2 protein by inducing nuclear accumulation of Nrf2. These findings suggest that curcumin activates the expression of thi- oredoxin, an antioxidant protein in the Nrf2 pathway, and protects neurons from death caused by oxygen-glucose deprivation in an in vitro model of ischemia/reperfusion. We speculate that pharmacologic stimulation of antioxidant gene expression may be a promising approach to neu- roprotection after cerebral ischemia.展开更多
