In case 1, a fellow used a colonoscopy simulator while interacting with a standardized patient (SP), nurse, and attending physician all played by actors. The nurse and attending were instructed to display specific disruptive behavior and disregard the fellow unless requested to stop the disruptive behavior and focus on the patient and procedure. In case 2, the fellow was to calm an intern managing a patient with massive gastrointestinal bleeding. The objective in both scenarios was to assess the fellows’ ability to perform their duties while managing the disruptive behavior displayed by the actor. The SPs used checklists to rate fellows’ performances. The fellows completed a self-assessment survey.RESULTSTwelve fellows from four gastrointestinal fellowship training programs participated in the OSCE. In the “distracted care team” case, one-third of the fellows interrupted the conflict and refocused attention to the patient. Half of the fellows were able to display professionalism despite the heated discussion nearby. Fellows scored lowest in the interprofessionalism portion of post-OSCE surveys, measuring their ability to handle the conflict. In the “frazzled intern” case, 68% of fellows were able to establish a calm and professional relationship with the SP. Despite this success, only half of the fellows were successfully communicate a plan to the SP and only a third scored “well done” in a domain that focused on allowing the intern to think through the case with the fellow’s guidance.CONCLUSIONFellows must receive training on how to approach disruptive behavior. OSCEs are a tool that can assess fellow skills and set a culture for open discussion.展开更多

As referenced in a previously published article, the Obstetrics and Gynecology specialty is one of the service areas where these behaviors occur more frequently. Despite growing evidence of the ill effects of these types ofbehaviors many organizations are still having a difficult time in addressing these issues in an effective manner. Gaining a better understanding of the nature, causes, and impact of these behaviors is crucial to finding the right remedies for solution. Nobody intentionally starts the day planning to be disruptive, it's just that things get in the way. A combination of deep seated factors related to age and gender preferences, culture and ethnicity, life experiences, and other events that help shape values, attitudes and personalities, and more external factors related to training, environmental pressures, stress and burnout, and other personal issues all contribute to the mix. Given the complexities of today's health care environment, each person needs to recognize the importance of being held accountable for appropriate actions and behaviors that affect work relationships and care coordination that impact patient care. Early recognition, early intervention, and taking a pro-active supportive approach to improve individual behaviors will result in better relationships, less disruption, more satisfaction, and better outcomes of care.展开更多

The multiplication factor of the anomalous transport coefficient was adjusted to model the major disruptive discharge with double-null divertor configuration based on shot 61 916. The real-time feed-back control system for the plasma displacement was employed. Modeling results of the evolution of the poloidal field coil currents, the plasma current, the major radius, the plasma configuration all show agreement with experimental measurements. Results from the simulation show that during disruption, heat flux about 8 MW m-2 flows to the upper divertor target plate and about 6 MW m-2 flows to the lower divertor target plate. Computations predict that different amounts of heat fluxes on the divertor target plate could result by adjusting the multiplication factor of the anomalous transport coefficient. Here billions of USD are annually invested in developing new treatments, drugs, robots etc. to diagnose and cure diseases. Nevertheless, this study establishes that there is a fundamental shortcoming in the system that supports healthcare innovation: It is strongly biased towards micro-level innovation projects focusing on new products, alternative processes, and new financial solutions. The problem with this approach to support new projects is that the results are created as inventions within the system thus lacking holistic perspectives. This has consequently contributed with increasing costs that are out of proportion with existing budgets. Therefore this study seeks to analyze the current understanding of the Nordic healthcare system from a business model perspective. Here other aspects of the healthcare system are explored to determine if they could be redesigned to promote new types of innovation projects. The purpose of undertaking this task is to challenge the established patterns of the current healthcare innovation support practices. Here the vertical innovation process (VIP) framework, which is a systematic radical innovation model that seeks macro-level outcomes based on standalone inventions (see more below), is applied to analyze the current state-of-the-art in Nordic healthcare innovation projects. The results determine that very little attention is given to rethink and redesign the healthcare system at a macro-level, and it is discussed that stand-alone inventions ought to be rethought into the entire healthcare system to create a larger impact. Finally, it is argued that existing performance measures are inappropriate to foster projects that innovate the existing system: New measuring points should be developed to promote macro-level projects and to avoid the current rapid increase of costs in the Nordic healthcare system.展开更多

The key lies in how industrial clusters respond to disruptive innovation. The main obstacles to the development of disruptive innovation in industrial clusters are lock-in and cluster inertia, which originate from the negative effects of cluster system isomorphism. In order to break through the development barriers, industrial clusters need to adopt a targeted overall response strategy, including adopting bottom-up local subversive innovation policies, introducing external knowledge, encouraging spin-off entrepreneurial enterprises, and encouraging alliances and cooperation between incumbent enterprises and entrepreneurial enterprises, etc.展开更多

Fuel discharge contains multi-component fluid dynamics with phase changes, and, in the present study, the SFR safety analysis code SIMMER (Sn, implicit, multifield, multicomponent, Eulerian recriticality) was utilized as a technical basis. First, dominant phenomena affecting fuel discharge through the CRGT are identified based on parametric calculations by the SIMMER code. Next, validations on the code models closely relating to these phenomena were carried out based on experimental data. It was shown that the SIMMER code with some model modifications could reproduce the experimental results appropriately. Through the present study, the evaluation methodology for the molten-fuel discharge through the CRGT was successfully developed.展开更多

Methods: Forty-six AD patients aged 49 - 76 years were recruited for the study. Neurofeedback regulation was conducted using a BCI game designed to modulate EEG rhythms. Cognitive function was assessed using MMSE, MoCA, and ADAS-cog scales before and after a 10-day training period. EEG measurements were taken to evaluate changes in brain activity complexity. Statistical analyses were performed using SPSS25.0 software to compare pre- and post-training scores. Results: Post-intervention results showed significant improvement in the cognitive function of AD patients. The total scores of MMSE, MoCA, and ADAS-cog scales increased significantly (P < 0.01). Notable improvements were observed in memory, language, and attention domains. EEG complexity in the left frontal area also showed a significant increase (P < 0.05). Additionally, the disruptive behaviors of patients were significantly reduced, improving their overall quality of life. Conclusions: Neurofeedback therapy based on BCI games is a promising intervention for enhancing cognitive functions and reducing disruptive behaviors in AD patients. This innovative approach demonstrates significant potential for clinical application, providing a non-invasive method to improve patient outcomes. Further studies with larger sample sizes and long-term follow-ups are recommended to validate these findings and explore the specific effects of NFB training on different cognitive impairment levels.展开更多

Research based on "cool'' and "hot''executive functional theory and the dual pathway models, which are thought of as applied response inhibition and delay aversion, respectively, within the neuropsychological view of ADHD, has shed light on emotional responding before and after decontextualized stimuli, while CSTC circuit-related domains have been suggested to explain the different emotional symptoms of ADHD with or without comorbid DBD. This review discusses the role of abnormal connections in each CSTC circuit, especially in the emotion circuit, which may be responsible for targeted executive dysfunction at the neuroscience level. Thus, the two major domains – abstract thinking(cool) and emotional trait(hot) – trigger the mechanism of onset of ADHD.展开更多

Amongst the range of conceal- ment strategies, disruptive coloration is thought to visually fragment an animal's' outline, thereby reducing its rate of discovery. Here, I propose two non-mutually exclusive hypotheses for how disruptive camouflage functions, and describe the visual me- chanisms that might underlie them. (1) The local edge disruption hypothesis states that camouflage is achieved by breaking up edge information. (2) The global feature disruption hypothesis states camouflage is achieved by breaking up the characteristic features of an animal (e.g., overall shape or facial features). Research clearly shows that putatively disruptive edge markings do increase concealment; however, few tests have been undertaken to determine whether this survival advantage is attributable to the distortion of features, so the global feature disruption hypothesis is under studied. In this review the evidence for global feature disruption is evaluated. Further, I address if object recognition processing provides a feasible mechanism for animals' features to influence concealment. This review concludes that additional studies are needed to test if disruptive camouflage operates through the global feature disruption and proposes future research directions [Current Zoology 61 (4): 708-717, 2015].展开更多

The role of sexual selection in this process is controversial; however, most of the work has centered on mate choice while the role of male-male competition in speciation is relatively understudied. Here, we outline how male-male competition can be a source of diversifying selection on male competitive phenotypes, and how this can contribute to the evolution of reproductive isolation. We highlight how negative frequency-dependent selection (advantage of rare phenotype arising from stronger male-male competition between similar male phenotypes compared with dissimilar male pheno- types) and disruptive selection (advantage of extreme phenotypes) drives the evolution of diversity in competitive traits such as weapon size, nuptial coloration, or aggressiveness. We underscore that male-male competition interacts with other life-history functions and that variable male com- petitive phenotypes may represent alternative adaptive options. In addition to competition for mates, aggressive interference competition for ecological resources can exert selection on compet- itor signals. We call for a better integration of male-male competition with ecological interference competition since both can influence the process of speciation via comparable but distinct mecha- nisms. Altogether, we present a more comprehensive framework for studying the role of male-male competition in speciation, and emphasize the need for better integration of insights gained from other fields studying the evolutionary, behavioral, and physiological consequences of agonistic interactions.展开更多

Based on the author's 30-year engagement in the mountainous community Chizu Town, Tottori Prefecture, Japan, a unique participatory approach called ‘‘Zeroto-One Movement'' has been strategically studied. The study areas are found to have adaptively increased their coping capacity. Their unique participatory process is shown to be an adaptive process for SMART community governance under persistent disruptive risks—‘‘S'' represents small-sized and survivability-minded, ‘‘M'' modestscale and multiple-stakeholder involved, ‘‘A'' anticipatory and adaptive, ‘‘R'' risk-concerned and responsive, and ‘‘T''is transformative. Finally, the Case Station-Field Campus scheme is proposed to serve as a platform for studying the adaptive processes over a long period of time.展开更多

Experiment 1, in which the spots and their placement imitated the arrangement on the crab Carcinus maenas, tested whether high and low contrast spots touching the body outline (disruptive 'edge' morphs) made the artificial prey more difficult to detect than when the spots did not touch the outline (non-disruptive 'inner' morphs) against three different backgrounds: 'small', 'middle' and 'large' spot size. In Experiment 2, the range of spot sizes and their positions ('edge' and 'inner') on the crab morphs were varied to determine the most effective combination against the 'mid- dle' background. Altogether, 640 volunteers participated in these computer trials, representing a high degree of independent replication. All patterned morphs were significantly harder to detect than plain morphs, indicating that possessing at least some degree of background matching can provide camouflage. Both experiments demonstrated that various morphs, though not having the same spot sizes as the background, had similar or better survivorship as those with matching spot sizes--indicating that opti- mal camouflage did not come from background matching alone. In Experiment 2, edge-disrupted morphs consistently took longer to find than their non-disruptive counterparts. The relative effects of edge disruption, contrast, and background/prey spot size are clearly context-dependent, highlighting the complexity of prey concealment [Current Zoology 61 (4): 718-728, 2015].展开更多

The main obstacles to the development of disruptive innovation in industrial clusters are lock-in and cluster inertia, which originate from the negative effects of cluster system isomorphism. In order to break through the development barriers, industrial clusters need to adopt a targeted overall response strategy, including adopting bottom-up local subversive innovation policies, introducing external knowledge, encouraging spin-off entrepreneurial enterprises, and encouraging alliances and cooperation between incumbent enterprises and entrepreneurial enterprises, etc.
文摘The purpose of the present study is to develop a methodology to evaluate fuel discharge through the CRGT (control-rod guide tube) during CDAs (core-disruptive accidents) of SFRs (sodium-cooled fast reactors), since fuel discharge will decrease the core reactivity and CRGTs have a potential to provide an effective discharge path. Through the present study, the evaluation methodology for the molten-fuel discharge through the CRGT was successfully developed.

文摘Disruptive technologies provide a new paradigm for supply chain risk management and bring opportunities and challenges for the improvement of supply chain resilience(SCRes). This study summarizes the application cases of some disruptive technologies in the SCRes and analyzes the benefits and damages brought by disruptive technologies to the SCRes.The results show that disruptive technologies can provide the supply chain with flexibility, visibility, agility, and other capabilities at various stages of risk management.Hence, technology advancements greatly increase the level of the SCRes.Although disruptive technologies undermine the construction of SCRes,these damages can be eliminated through technology iteration or other disruptive technologies.Furthermore, disruptive technologies will provide better stability for the SCRes.The study also makes several suggestions for the use of disruptive technologies in the construction of the SCRes.
文摘Objective: This study aims to evaluate the feasibility and effectiveness of neurofeedback therapy based on brain-computer interface (BCI) games in enhancing cognitive functions and reducing disruptive behaviors in patients with Alzheimer’s disease (AD). Further studies with larger sample sizes and long-term follow-ups are recommended to validate these findings and explore the specific effects of NFB training on different cognitive impairment levels.

文摘The nutrient digestion, absorption and biological activity of bee pollen may be limited due to the complex pollen wall. Here, the effect of superfine grinding technology on the release of nutrients from bee pollen were investigated, and their antioxidant activities and in vitro digestion were explored in this study. Results showed that the content of nutrients in bee pollen increased after wall disruption.Among them, fat content increased by 22.55%-8.31%,protein content increased by 0.54%-4.91%,starch content increased by 36.31%-48.64%,soluble sugar content increased by 20.57%-29.67%,total phenolic acid content increased by 11.73%-86.98%and total flavonoids content increased by 14.29%-24.79%.At the same time, the antioxidant activity increased by 14.84%-46.00%.Furthermore,the active components such as phenolic compounds in the wall-disruption bee pollen were more readily to be released during the in vitro digestion, and easier to be absorbed because of their higher bioaccessibility.Antioxidant activities during in vitro digestion were also improved in walldisruption bee pollen. These findings provide evidence that bee pollen wall disruption was suggested, thus,it is more conducive to exerting the value of bee pollen in functional foods.

文摘The neurocircuitries that constitute the corticostriato-thalamo-cortical(CSTC) circuit provide a framework for bridging gaps between neuroscience and executive function in attention deficit hyperactivity disorder(ADHD), but it has been difficult to identify the mechanisms for regulating emotional problems from the understanding of ADHD comorbidity with disruptive behavior disorders(DBD). Thus, the two major domains – abstract thinking(cool) and emotional trait(hot) – trigger the mechanism of onset of ADHD.
文摘Camouflage is ubiquitous in the natural world and benefits both predators and prey. This review concludes that additional studies are needed to test if disruptive camouflage operates through the global feature disruption and proposes future research directions [Current Zoology 61 (4): 708-717, 2015].
文摘Natural selection has been shown to drive population differentiation and speciation. Altogether, we present a more comprehensive framework for studying the role of male-male competition in speciation, and emphasize the need for better integration of insights gained from other fields studying the evolutionary, behavioral, and physiological consequences of agonistic interactions.
文摘This article addresses the increasing need for participatory approaches to disaster reduction at the community level. Finally, the Case Station-Field Campus scheme is proposed to serve as a platform for studying the adaptive processes over a long period of time.
文摘Interactions between disruptive colouration and the match between prey and background spot size were manipulated in two experiments that used time taken by human 'predators' to find artificial prey (virtual crab morphs) against heterogeneous backgrounds as a measure of the their camouflage. The relative effects of edge disruption, contrast, and background/prey spot size are clearly context-dependent, highlighting the complexity of prey concealment [Current Zoology 61 (4): 718-728, 2015].
