BACKGROUND Patients with knee arthroplasty often have problems such as slow recovery of knee function,which may cause negative emotions and affect their postoperative rehabilitation.The application of sensory and moto...BACKGROUND Patients with knee arthroplasty often have problems such as slow recovery of knee function,which may cause negative emotions and affect their postoperative rehabilitation.The application of sensory and motor training in the Acknowledge,Introduce,Duration,Evaluation,Thank You(AIDET)communication mode in patients after knee arthroplasty can provide reference for the selection of postoperative rehabilitation training measures.AIM To explore the sensory and motor training effects in AIDET communication mode on knee function recovery and resilience of patients after knee arthroplasty.METHODS One hundred patients who underwent knee arthroplasty at our hospital between January 2022 and January 2024 were randomly divided into two groups.The control group(n=50)received routine rehabilitation training.In the observation group(n=50),the AIDET communication mode was used to perform sensory and motor training,in addition to routine rehabilitation training.The rehabilitation training was administered for 8 weeks.After surgery,knee function,balance ability,walking ability,proprioception,and resilience were compared between the two groups.RESULTS The New York Hospital for Special Surgery knee scores of the observation group at the time of discharge and 8 weeks after intervention were(65.23±6.84,84.53±5.27),which was higher than those of the control group(61.03±7.15,74.92±6.52)(P<0.05).The balance ability of the observation group at the time of discharge and 8 weeks after the intervention was higher than that of the control group,the time of time up to go test was shorter than that of the control group,and proprioceptive function was higher than that of the control group(P<0.05).The resilience level in the observation group after the intervention was higher than that in the control group(P<0.05).CONCLUSION Sensory and motor training in AIDET communication mode promotes knee function recovery of patients after knee arthroplasty,improves their limb walking ability and balance function,and increases their resilience level.展开更多

Ten medical students ...We studied the influence of doctor-patient communication skills training on brain functional architecture using resting-state functional MRI(rs-fMRI) with a regional homogeneity(ReHo) method. Ten medical students participated in the study. A 1-year long doctor-patient communication skills training program was conducted. RsfMRI data were collected at baseline, one month and one year after training. There was a significant increase in the communication skills test average scores between baseline and 1-month duration of training(P〈0.001). After one month of communication skills training, medical students had decreased ReHo in the right superior temporal gyrus compared with the baseline. After one year of communication skills training, students had increased ReHo in multiple regions and decreased ReHo in several regions(P 〈0.05, Alphasim corrected). The change of ReHo values in the superior temporal gyrus negatively correlated with the change of communication skills scale score between one month after communication skills training and baseline(r=-0.734, P= 0.036). The training program we used can be an effective approach of improving doctor-patient communication skills, and the training resulted in functional plasticity of the brain’s architecture toward optimizing locally functional organization.展开更多

However, inadequate functional health literacy among patients is a well-known barrier to eff...Patient involvement in the health care process has been documented as a critical component of successful disease management. However, inadequate functional health literacy among patients is a well-known barrier to effective doctor-patient communication, which has an adverse effect on health outcomes. This study aimed at investigating the association of functional health literacy and doctor-patient communication among HIV/AIDS patients in Homa Bay County, Kenya. It was a cross-sectional hospital-based survey conducted among 362 HIV/AIDS patients receiving care at the eight sub-county hospitals of Homa Bay county. Data was collected using a self-administered structured questionnaire. Cronbach’s alpha and confirmatory factor analysis tests were used to ascertain the reliability and validity of study instruments, while Logistic regression logistic analysis was used to measure the association between functional health literacy and doctor-patient communication. 51% of the respondents were females, and the majority (30.9%) of respondents fell in the age group of 45 years and above. The respondents’ functional health literacy levels were inadequate at weighted means scores of 3.23 [SD 1.31]. The study further established that functional health literacy significantly influenced doctor-patient communication [Nagelkerke R square = 0.318]. There is, therefore, a need to document the determinants of functional health literacy to improve it and make doctor-patient interaction an enjoyable and meaningful experience.展开更多

Methods: Learning was measured by comparing residents’ communication skills in a simulated consultation before and after a communication skills training. Communication skills were transcribed and tagged with a computer-assisted program. Potential variables associated with learning (residents’ characteristics, contextual characteristics and pre-training communication skills) were measured before the training and entered in regression analysis. Results: Fifty-six residents followed the training between 2002 and 2006. Poor pre-training assessment and supportive skills predicted the respective learning of these skills. Better assessment skills’ learning was predicted by copings (i.e. lower level of emotional coping), lower levels of self-efficacy and depersonalization. Better supportive skills’ learning was predicted by a lower work experience and associated with a higher training attendance rate. Conclusions: Predictors and correlates of assessment and supportive skills learning were different. Trainers needed to detect certain residents’ characteristics (i.e. depersonalization) in order to optimize assessment skills learning. Trainers needed to be aware that supportive skills are difficult to learn and to teach and may need more training hours.展开更多

Background: Communication difficulties due to cognitive impairment among residents with dementia disease may complicate care situations. Training can improve nurses’ communication skills and increase care quality. The validation method aims to facilitate communication with residents with dementia disease through empathic and confirmatory approaches. Evaluations of the validation method have primarily focused on the residents’ perspective, and reports on nurses’ experiences are sparse. Improved communication and relationships with residents after validation method training have been described previously. Videotaped data could provide additional information about these earlier results. Design: A descriptive qualitative design. Methods: Eight nurses participated in a year of validation method training, including videotaped conversations with eleven residents. Videotapes with at least five months between the first and last recording were analysed and compared qualitatively. Results: The analysis revealed an overall pattern: nurses’ movements within and between various paths when improving their communication skills. This was based on three sub-patterns: from controlling communication towards developing attentiveness in communication, from ambiguous communication towards developing coherence in communication, and from being open and attentive towards having a refined attuned communication. Conclusions: All nurses developed their communication skills during the programme, albeit to different degrees. The findings are in congruence with the experiences described by nurses, and so it is reasonable to believe that the programme helped to improve the nurses’ skills in communicating with residents with dementia disease. Relevance to clinical practice: A validation method training programme could give nurses the possibility to develop their skills in communicating with residents with dementia disease.展开更多

In order to study the on-board diagnosis network's security and reliability, a simulation model for the on-board diagnosis network of high-speed Maglev trains with the optimal network engineering tool (OPNET) was built to analyze the network's performance, such as response error and bit error rate on the network load, throughput, and node-state response. The simulation model was verified with an actual on-board diagnosis network structure. The results show that the model results obtained are in good agreement with actual system performance and can be used to achieve actual communication network optimization and control algorithms.展开更多

The paper describes the results and experience from the communication trainings which were gained within the projects: CR lacks the support of government agencies so as employers and research on the issue, medical staff are not professional enough in communication, there are more requirements for the quality of lecturers; there is no existing general system of monitoring of violent incidents; there is no relevant education at schools. The article contains also new general ways about how to resolve mentioned problems. These results include the special mini project where educated 550 staff members in the health care area in a short time period and taught them key skills for how to approach the aggressive patients-50.6% respondents used successfully the gained skills in their practice (negotiation with patients, aggressive related); 42.7% respondents also used successfully the gained skills in their private life. 文摘The on-board diagnosis network is the nervous system of high-speed Maglev trains, connecting all controller sensors, and corresponding devices to realize the information acquisition and control. In order to study the on-board diagnosis network's security and reliability, a simulation model for the on-board diagnosis network of high-speed Maglev trains with the optimal network engineering tool (OPNET) was built to analyze the network's performance, such as response error and bit error rate on the network load, throughput, and node-state response. The simulation model was verified with an actual on-board diagnosis network structure. The results show that the model results obtained are in good agreement with actual system performance and can be used to achieve actual communication network optimization and control algorithms.

文摘The user signal quality as well as the performance of transmission link experiences severe loss due to wireless channel fading and propagation loss in high-speed railway scenario.To improve the quality at the receiving end,spatial diversity was realized by means of cooperative communication technology based on the uncorrelated characteristics of the channels.The model of mobile communication system in high-speed railway was set up,and a cooperative scheme based on statistics was proposed.Mathematical analysis and simulation results show that the quality of the received signal and the performance of the transmission link are significantly improved using cooperative communication technology compared to that in non-cooperative communication mode.

文摘We present a temperature monitoring and warning system, which is based on wireless communi- cation technology and applied in train switchgear in this paper. The system is consists of three parts, including wireless temperature detection module, inter-vehicle transmission networks module and?remote monitoring server. The switchgear contact temperature data are collected via the wireless temperature detection module and exchanged in inter-vehicle wireless networking by Zigbee modules. Then the temperature of train switchgear cabinets can be monitored remotely through the GPRS wireless communication.

文摘The high-speed railway and high-way networks are now expanding at a phenomenal speed in Chinaand in many other parts of the world. The related broadband wireless communication over high-speed trains and highway vehicles is a very challenging task due to hostile transmission channel conditions. The demand for such services is growing rapidly, following the proliferation of laptop/tablet computers and smart phones. This motivates the research on wireless communications in the high mobility environments.
文摘The aim of this paper is to analyze the results from the implemented projects and trainings which were aimed to minimize violence in health care area in CR (the Czech Republic) in years 2010-2013. The paper describes the results and experience from the communication trainings which were gained within the projects: CR lacks the support of government agencies so as employers and research on the issue, medical staff are not professional enough in communication, there are more requirements for the quality of lecturers; there is no existing general system of monitoring of violent incidents; there is no relevant education at schools. The article contains also new general ways about how to resolve mentioned problems. These results include the special mini project where educated 550 staff members in the health care area in a short time period and taught them key skills for how to approach the aggressive patients-50.6% respondents used successfully the gained skills in their practice (negotiation with patients, aggressive related); 42.7% respondents also used successfully the gained skills in their private life. It was found out that with the used skills the courage by conflict resolution was created.

文摘The article is commonly about the impact of globalization on participants of pedagogical process in the condition of communication. The globalization leaves its tracks in separate spheres of the educational system as well as it impacts all spheres of modern life. The relation between a teacher and a student differs with its peculiarity in the contemporary life. A pupil or a student differs totally in classifying the information gained in his activity. A modem student demonstrates a special attitude to everyday events and gets an independent position. Such reality demands the teacher of the day to have a very high practice; the necessity of contemporary training in the process of training-education can be marked as the appearance of globalization. It occurs as a demand for globalization as well. All these items after being analyzed bring some conclusions.
