Game Analysis of Regulation for Online Prescription Drugs under Customer Feedback Mechanism

Objective To study the way to better regulate the online sales of prescription drugs,and to provide reference for the adjustment of relevant policies since the online sales of prescription drugs has become an inevitable trend.Methods A game model was constructed for the strategy choice of pharmaceutical e-commerce platform,customers and government departments based on differential game theory and Nash equilibrium game model to analyze the pure strategy Nash equilibrium,Nash equilibrium dominant strategy of each subject and the mixed strategy Nash equilibrium under different conditions.Besides,Matlab was used to carry out simulation analysis.Results and Conclusion The study shows that:(1)Improving the credibility of the government and reducing the cost of government regulation can not only make the pharmaceutical e-commerce platform operate with high quality,but also give greater play to government functions;(2)The greater the influence of social evaluation on pharmaceutical e-commerce platforms,the lower the cost of high-quality operation of pharmaceutical e-commerce platform,and the greater the probability of customer choosing real evaluation strategy;(3)The greater the customers’perception of potential risk,the greater the compensation,and the lower the cost of reporting.Then,the greater the probability that government departments will choose strict regulation.Finally,the model solution and simulation analysis are combined to provide countermeasures and suggestions for the safety regulation of online sales of prescription drugs. 展开更多

To understand the current situation of institutional registration in Shaanxi Province after the implementation ofregistration system management in drug clinical trial institutions.Relevant information was collected on the“Announcement on the Accreditation of Drug Clinical Trial Institutions”issued by the National Medical Products Administration from 2005 to August 2022,the record management information system of drug and medical device clinical trial institutions,and the drug clinical trial registration and information publicity platform.A retrospective analysis was carried out in terms of institutional development,regional distribution,registered majors,principal investigators,and the number of drug clinical trials.After the implementation of institution registration,the number of drug clinical trial institutions in Shaanxi Province increased by 47.4%,884 principal investigators were registered,the number of registered majors expanded from 58 qualified to 117,and the professional scope increased by 50.4%.The policy of institution registration is conducive to promoting the rational use of medical resources and the development of drug clinical trial institutions and improving the healthy development of the pharmaceutical industry in Shaanxi Province. 展开更多

A novel method for the determination of two quinolone drugs norfloxacin (NOR) and levofloxacin (LVX) was described by capillary electrophoresis with electrochemiluminescence detection. The good relationship (r ≥ 0.9991) between peak area and concentration of analytes was established over two orders of magnitude. The limits of detection (LOD, S/N = 3) in standard solution are 4.8 × 10^-7 mol/L for NOR and 6.4 × 10^-7 mol/L for LVX, respectively. The limits of quantitation (LOQ, S/N = 10) in real human urine samples are 1.2 × 10^-6 mol/L for NOR and 1.4 × 10^-6 mol/L for LVX, respectively. The present method was successfully applied to the determination of NOR and LVX in human urine and the studv of oharmacokinetics of NOR. 展开更多

Background:Nonalcoholic fatty liver disease and its advanced stage,nonalcoholic steatohepatitis(NASH),are the major cause of hepatocellular carcinoma(HCC)and other end-stage liver disease.However,the potential mechanism and therapeutic strategies have not been clarified.This study aimed to identify potential roles of mi RNA/m RNA axis in the pathogenesis and drug combinations in the treatment of NASH.Methods:Microarray GSE59045 and GSE48452 were downloaded from the Gene Expression Omnibus and analyzed using R.Then we obtained differentially expressed genes(DE-genes).DAVID database was used for Gene Ontology(GO)analysis and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment pathway analysis.Protein-protein interaction(PPI)networks were used for the identification of hub genes.We found upstream regulators of hub genes using mi RTar Base.The expression and correlation of key mi RNA and its targets were detected by q PCR.Drug Pair Seeker was employed to predict drug combinations against NASH.The expression of mi RNA and hub genes in HCC was identified in the Cancer Genome Atlas database and Human Protein Atlas database.Results:Ninety-four DE-genes were accessed.GO and KEGG analysis showed that these predicted genes were linked to lipid metabolism.Eleven genes were identified as hub genes in PPI networks,and they were highly expressed in cells with vigorous lipid metabolism.hsa-mi R-335-5 p was the upstream regulator of 9 genes in the 11 hub genes,and it was identified as a key mi RNA.The hub genes were highly expressed in NASH models,while hsa-mi R-335-5 p was lowly expressed.The correlation of mi RNA-m RNA was established by q PCR.Functional verification indicated that hsa-mi R-335-5 p had inhibitory effect on the development of NASH.Finally,drug combinations were predicted and the expression of mi RNA and hub genes in HCC was identified.Conclusions:In the study,potential mi RNA-m RNA pathways related to NASH were identified.Targeting these pathways may be novel strategies against NASH. 展开更多

Objective:To explore the general differentiation and treatment of insomnia by Professor Gao Ying through drug clustering and group correspondence analysis,and provide reference for clinical diagnosis and treatment.Methods:Collect retrospective case data from outpatient system,use SPSS20.0 software to perform frequency and cluster analysis on high-frequency symptoms and drug data,and perform corresponding analysis on the clustered drug syndrome groups.Results:A total of 349 consultations in 204 patients were included.Cluster analysis of 35 symptoms and 40 flavors with a frequency of more than 10%resulted in a corresponding relationship between 7 symptom groups,6 drug groups and 5 drug syndrome groups.The medicine symptom group has a high degree of matching;the doctors distinguish and tre at insomnia with calming,clearing heat,nourishing yin,liver,spleen,qi and phlegm as the core treatment,with consistent decoction,two to pill,lily ground Huang Tang,Lily Zhimu Decoction,Wendan Decoction,Sini San,Xiao Chai Hu Tang,Xiaoyao San,etc.are commonly used prescriptions;the physician's experience is to add or subtract Danshen and Zao Ren drink,which has a wide range of applicability to various insomnia syndrome.Conclusion:Based on the cluster analysis of drug symptoms and group correspondence analysis,it can reveal the pathogenesis,treatment and class information hidden in the data of drug symptoms,which can reflect the general law of physicians'syndrome differentiation and treatment of insomnia.This method has a reference for the exploration of TCM clinical experience significance;The results of this study can provide feedback to guide the clinical diagnosis and treatment of insomnia. 展开更多

Cellular metabolism arouses the changes of substance in extracellular physiological microenvironment,and the metabolic level reflects the physiological state of cells.This paper developed a novel microphysiometer automatic analysis instrument based on multiparameter cell-based biosensors for quick drug analysis.This study included the multiparameter cell-based biosensors,cell culture chamber,drug auto-injection detection and analysis.The analysis instrument was capable of real-time detection for the acidic product and other chemical parameters generated by the cellular metabolism in the micro-volume.Finally,the paper employs human breast cancer cell line MCF-7 and drug experiments to verify the performance of microphysiometer,and study effects of different drugs on cell metabolism.Further,the research explores drug analysis method of the multiparameter microphysiometer.The results showed that the cell-based microphysiometer system provides a utility platform for rapid,long-term and automatic cell physiological environment detection and drug analysis. 展开更多

SpectraTr:A novel deep learning model for qualitative analysis of drug spectroscopy based on transformer structure

The drug supervision methods based on near-infrared spectroscopy analysis are heavily dependent on the chemometrics model which characterizes the relationship between spectral data and drug categories.The preliminary application of convolution neural network in spectral analysis demonstrates excellent end-to-end prediction ability,but it is sensitive to the hyper-parameters of the network.The transformer is a deep-learning model based on self-attention mechanism that compares convolutional neural networks(CNNs)in predictive performance and has an easy-todesign model structure.Hence,a novel calibration model named SpectraTr,based on the transformer structure,is proposed and used for the qualitative analysis of drug spectrum.The experimental results of seven classes of drug and 18 classes of drug show that the proposed SpectraTr model can automatically extract features from a huge number of spectra,is not dependent on pre-processing algorithms,and is insensitive to model hyperparameters.When the ratio of the training set to test set is 8:2,the prediction accuracy of the SpectraTr model reaches 100%and 99.52%,respectively,which outperforms PLS DA,SVM,SAE,and CNN.The model is also tested on a public drug data set,and achieved classification accuracy of 96.97%without preprocessing algorithm,which is 34.85%,28.28%,5.05%,and 2.73%higher than PLS DA,SVM,SAE,and CNN,respectively.The research shows that the SpectraTr model performs exceptionally well in spectral analysis and is expected to be a novel deep calibration model after Autoencoder networks(AEs)and CNN. 展开更多

In recent years,the emergence of nanotechnology experienced incredible development in the field of medical sciences.During the past decade,investigating the characteristics of nanoparticles during fluid flow has been one of the intriguing issues.Nanoparticle distribution and uniformity have emerged as substantial criteria in both medical and engineering applications.Adverse effects of chemotherapy on healthy tissues are known to be a significant concern during cancer therapy.A novel treatment method of magnetic drug targeting(MDT)has emerged as a promising topical cancer treatment along with some attractive advantages of improving efficacy,fewer side effects,and reduce drug dose.During magnetic drug targeting,the appropriate movement of nanoparticles(magnetic)as carriers is essential for the therapeutic process in the blood clot removal,infection treatment,and tumor cell treatment.In this study,we have numerically investigated the behavior of an unsteady blood flowinfused with magnetic nanoparticles during MDT under the influence of a uniform external magnetic field in a microtube.An optimal homotopy asymptotic method(OHAM)is employed to compute the governing equation for unsteady electromagnetohydrodynamics flow.The influence of Hartmann number(Ha),particle mass parameter(G),particle concentration parameter(R),and electro-osmotic parameter(k)is investigated on the velocity of magnetic nanoparticles and blood flow.Results obtained show that the electro-osmotic parameter,along with Hartmann’s number,dramatically affects the velocity of magnetic nanoparticles,blood flow velocity,and flow rate.Moreover,results also reveal that at a higher Hartman number,homogeneity in nanoparticles distribution improved considerably.The particle concentration andmass parameters effectively influence the capturing effect on nanoparticles in the blood flow using a micro-tube for magnetic drug targeting.Lastly,investigation also indicates that the OHAM analysis is efficient and quick to handle the system of nonlinear equations. 展开更多

Objective To study the development trend and hotspot of China’s drug supply chain in terms of bibliometrics to provide some reference for transforming and upgrading China’s pharmaceutical circulation industry.Methods Based on the data related to drug supply chain research from 1999 to 2019 collected by CNKI,Wanfang and VIP,software such as bibliographic information co-occurrence mining system(BICOMS)version 2.0,University of California Irvine Network(UCINET)version 6.0,and graphical clustering toolkit were used to measure and conduct visual analysis of the domestic drug supply chain research literature.Results and Conclusion Through literature analysis,it can be seen that the current research on drug supply chain in China mainly focuses on drug procurement and cost control,zero inventory control of hospital pharmacy,optimization and risk management of drug supply chain,and information of supply chain and logistics of pharmaceutical enterprises.Combining with the“Internet+”and other models,drug supply chain enterprise should optimize its management process according to the relevant national policy,promote information construction in all links of the supply chain,improve its transparency and ensure the quality and safety of drugs.The improvement of supply chain efficiency will enhance the transformation and upgrading of China’s pharmaceutical circulation industry. 展开更多

The properties and characteristics of the polymer used for the preparation of matrix drug delivery systems considerably influence their performance and the extent of drug release and its mechanism. The objective of this research was to examine the dimensional changes, and gel evolution of polymer matrices consisting of three different polymers Polyox, sodium alginate (hydrophilic) and Ethocel (hydrophobic), using an image analysis method. Furthermore to explore how these changes influence the release rate of a soluble drug namely, venlafaxine. All tablets displayed marked dimensional expansion and gel growth particularly those consisting of two hydrophilic polymers Polyox/sodium alginate (POL/SA/V) compared to those consisting of the hydrophilic/hydrophobic Polyox/Ethocel (POL/ET/V). Similarly the thickness of the gel layer in POL/SA/V matrices increased considerably with time up to 8 hours. In general our findings show that the POL/SA/V matrices, due to their thicker gel layer produced a more effective barrier which results in a more pronounced sustained release delivery. This accounts for the slower and smaller overall drug release observed with the POL/SA/V matrices compared to those containing POL/ET/V and indicates that the formation of a thick and durable gel barrier is a characteristic necessary for the preparation of sustained drug release systems. Moreover the solubility of venlafaxine in combination with the polymer’s properties appears to play an important role on the extent of drug release and the release mechanism. Overall the polymer mixtures examined comprise a useful and promising combination of materials for the development and manufacture of sustained release preparations based on these polymers. 展开更多

Mushrooms are well-known to possess a continuum of anticancer metabolites that are vital in the development of anticancer adjuvant drug leads based on natural products. Owing to the fact that conventional cancer therapeutic methods were failed to lessen mortality caused by cancer to the estimated level with occurrence of adverse side effects, anticancer agents isolated from natural mushroom sources unarguably make an experimental research area worth mass focus today. The current study was targeted on in vitro cytotoxicity and in silico predictive pharmacological analysis of a flavonoid compound isolated from Fulvifomes fastuosus mushroom. Targeted compound was isolated from the mushroom using different chromatographic methods and identified by NMR spectrometry and mass spectrometry. Cytotoxicity experiments were carried out using MTT assay and apoptotic cells were identified by ethidium bromide/acridine orange staining. The SwissADME tool, BOILED-Egg construction model and Swiss target protein prediction software have been used to perform in silico predictive pharmacological analysis. The isolated compound has been identified as 2-(3,4-dihydroxyphenyl)-6-[(E)-2-(3,4-dihydroxyphenyl)ethenyl]-5'-methylspiro[2H-furo[3,2-c]pyran-3,2'-furan]-3',4-dione by spectrometric methods. The result of MTT assay showed that 2-(3,4-dihydroxyphenyl)-6-[(E)-2-(3,4-dihydroxyphenyl)ethenyl]-5'-methylspiro[2H-furo[3,2-c]pyran-3,2'-furan]-3',4-dione has potent anticancer activity for hepatoma against Hep-G2 cell line (IC50 = 20.8 μg/ml) being less toxic to normal CC-1 epithelial cells (IC50 = 167.00 μM). The cells treated with compound ex-hibited apoptotic features such as cellular shrinkage, nuclear fragmentation and condensed cytoplasm. In summary, 2-(3,4-dihydroxyphenyl)-6-[(E)-2-(3,4-dihydroxyphenyl)ethenyl]-5'-methylspiro[2H-furo[3,2-c]pyran-3,2'-furan]-3',4-dione has shown potent anticancer properties against hepatoma with less cytotoxicity effect on normal cells. Furthermore, in silico study has revealed that properties of 2-(3,4-dihydroxyphenyl)-6-[(E)-2-(3,4-dihydroxyphenyl)ethenyl]-5'-methylspiro[2H-furo[3,2-c]pyran-3,2'-furan]-3',4-dione may contribute to making a high absorption and clearance of the test compound as not interfering with the therapeutic failure of the compound. The properties of 2-(3,4-dihydroxyphenyl)-6-[(E)-2-(3,4-dihydroxyphenyl)ethenyl]-5'-methylspiro[2H-furo-[3,2-c]pyran-3,2'-furan]-3',4-dione were compatible with well-known anticancer drug lapatinib. In conclusion, 2-(3,4-dihydroxyphenyl)-6-[(E)-2-(3,4-dihydroxyphenyl)ethenyl]-5'-methylspiro[2H-furo[3,2-c]pyran-3,2'-furan]-3',4-dione has a high tendency to act as a good anticancer adjuvant drug in the treatment of hepatoma. 展开更多

Background: Achieving the long terms goals of antiretroviral treatment (ART) requires a careful approach during treatment initiation that takes into account patient’s psychosocial state, availability and accessibility of treatment combinations, and adherence support. Adverse drug reactions that occur during the initial phases have a bearing on treatment outcomes and thus need to be monitored and treated. Objective: This study was done to assess length of time (survival time) it took for clinically significant adverse drug reactions to occur in patients taking Nucleoside Reverse Transcriptase Inhibitors (N(t)RTI) available for treatment of Human Immunodeficiency Virus (HIV) infection in Zimbabwe. Methods: A retrospective cohort of patient data collected from January 2009 to December 2012 was extracted from an Electronic Health Record database. Data from patients who were initiated on antiretroviral (ARV) drug regimens containing N(t)RTI drugs were analysed for survival time. A sample of 205 patient files was extracted for the time period for survival analysis using adverse drug reactions due to N(t)RTI drugs. Results: After data extraction, a total of 205 patient records were used in determining the time to event analysis of ADR’s in the cohort. The age range for the patients included in the study was 9 - 76 with a mean of 41 years (s.d = 14.8). Patients initiated on stavudine had a lower survival time before a clinically significant ADR compared to tenofovir (-365 days, p-value < 0.0005). Patients on zidovudine also had a less time before a significant reaction compared to those on tenofovir (-230 days;p-value = 0.008). Patients on zidovudine fared better compared to those on stavudine (-134 days;p-value < 0.0005). The mean survival time was highest for tenofovir (618 days), followed by zidovudine (388 days), and then stavudine (254 days).Conclusion: Patients on tenofovir have a longer survival time before a clinically significant adverse reaction. Treatment programmes need to continue commencing patients on tenofovir containing regimens as patients can be maintained for longer periods on this regimen. 展开更多

Classification of Herbal Drug Effects by Discriminant Analysis of Quantitative Human EEG Data

Clinical indications for herbal drugs very often only rely on traditional knowledge. Single plant-derived preparations are used for many purposes and cannot be classified to belong to a single category like calming or stimulating drugs. With respect to the brain a unique possibility exists to analyze drug effects by recording the EEG. It is common knowledge that many drugs change the frequency content of electric brain activity. Quantitative analysis of the EEG by Fast Fourier Transformation reveals parameters like spectral power, which can be processed further (CATEEM&#174;). Source density was determined from 17 channels of the quantitative EEG from 10 clinical studies recorded in a relaxed state with open eyes. Linear discriminant analysis was used to differentiate the effects of Placebo (circadian rhythm) from CNS-active herbal drugs in comparison to Valium&#174;. Calmvalera&#174;, L-Theanine, Lasea&#174;, Neurapas&#174;, Neuravena&#174;, Neurexan&#174;, Nutrifin Relax&#174;, Pascoflair&#174;(herbal calming drugs) as well as memoLoges&#174;, Zembrin&#174;(herbal stimulating drugs) induced different changes of the frequency content of brain electric activity. Discriminant analysis revealed that Nutrifin Relax&#174;, Pascoflair&#174;and Suntheanine&#174;could not be separated well from each other indicating a similar mechanism of action. The effect of Valium&#174;was projected at a very isolated position far away from the herbal preparations indicating a totally different mechanism of action. Zembrin&#174;and memoLoges&#174;grouped together with respect to the first three discriminant functions, but were different with respect to the 4th to 6th discriminant function. Lasea&#174;as anxiolytic drug and Neurapas&#174;as antidepressive drug were projected at isolated positions indicating their different clinical indications. The results indicate that discriminant analysis of human quantitative EEG data allows for unique pharmacological description of individual effect profiles of herbal drugs. 展开更多

The objective of this study was to acquire the information about residents of Changshou District’s knowledge levels and their behaviors of drug use so that implementation strategies of rational drug use can be formulated accordingly to promote and spread health education of rational drug use.Online and offline surveys were randomly conducted about rational drug use conditions of residents in Changshou District by questionnaires.Online questionnaires were sent to residents of Changshou District by Wenjuanxing,a professional platform used for surveys.Offline questionnaires were mainly distributed to residents of communities in Changshou District.All the online and offline statistics were analyzed and counted.309 questionnaires were distributed(176 online questionnaires and 133 paper questionnaires),and the effective recovery rate was 75.4%.According to investigation and survey,conditions of resident’s rational drug use were optimistic.Only 8.15%of residents who participated in the survey(or research subjects)took paracetamol tablets and Vitamin C Yinqiao tablets(also known as VC Honeysuckle Pills)at the same time when they had a cold or fever.Among 8.15%of residents,5.15%frequently took paracetamol,caffeine,and aspirin powder(also known as headache powder),80.26%checked expiration date of drugs before taking medicine,and>50%knew that drinking after taking medicines such as cephalosporin is prohibited.Common irrational problems caused by drug use were as follows:40%of research subjects took dietary supplements as drugs and 28%of them failed to know the correct usage and dosage of drugs.Proportion of drugs that were taken with irrational usage and dosage was antibiotics(64%),antibacterial(52%),drugs for patients with diabetes mellitus(36%),drugs for patients with hyperlipidemia,hypertension,and hyperglycemia(32%),vitamins(24%),drugs for the treatment of common cold and cough(20%),and others.Publicity of rational drug use should not be restricted to specific population,and knowledge of safe drug use is supposed to be actively popularized.Contents of rational drug use are as follows:Guide the public to correctly understand the functions of vitamins and dietary supplements strengthen publicity of rational use of drugs such as antibiotics,antibacterial drugs,drugs for chronic diseases,cold,and cough.Focus should be given on usage and dosage of drugs,use of antibacterial drugs,and repeated and excessive use of drugs caused by joint use of drugs with same ingredients of OTC drugs for the treatment of common cold and cough. 展开更多

Given the extremely high inter-patient heterogeneity of acute myeloid leukemia(AML),the identification of biomarkers for prognostic assessment and therapeutic guidance is critical.Cell surface markers(CSMs)have been shown to play an important role in AML leukemogenesis and progression.In the current study,we evaluated the prognostic potential of all human CSMs in 130 AML patients from The Cancer Genome Atlas(TCGA)based on differential gene expression analysis and univariable Cox proportional hazards regression analysis.By using multi-model analysis,including Adaptive LASSO regression,LASSO regression,and Elastic Net,we constructed a 9-CSMs prognostic model for risk stratification of the AML patients.The predictive value of the 9-CSMs risk score was further validated at the transcriptome and proteome levels.Multivariable Cox regression analysis showed that the risk score was an independent prognostic factor for the AML patients.The AML patients with high 9-CSMs risk scores had a shorter overall and event-free survival time than those with low scores.Notably,single-cell RNA-sequencing analysis indicated that patients with high 9-CSMs risk scores exhibited chemotherapy resistance.Furthermore,PI3K inhibitors were identified as potential treatments for these high-risk patients.In conclusion,we constructed a 9-CSMs prognostic model that served as an independent prognostic factor for the survival of AML patients and held the potential for guiding drug therapy. 展开更多
