Evaluation of Clinician Training in Autism Screening, Care Management, and Patient Education

Objective: The demand for pediatric developmental evaluations has far exceeded the workforce available to perform them, which creates long significant wait times for services. A year-long clinician training using the Extension for Community Healthcare Outcomes (ECHO<sup>®</sup>) model with monthly meetings was conducted and evaluated for its impact on primary care clinicians’ self-reported self-efficacy, ability to administer autism screening and counsel families, professional fulfillment, and burnout. Methods: Participants represented six community health centers and a hospital-based practice. Data collection was informed by participant feedback and the Normalization Process Theory via online surveys and focus groups/interviews. Twelve virtual monthly trainings were delivered between November 2020 and October 2021. Results: 30 clinicians participated in data collection. Matched analyses (n = 9) indicated statistically significant increase in self-rated ability to counsel families about autism (Pre-test Mean = 3.00, Post-test Mean = 3.89, p = 0.0313), manage autistic patients’ care (Pre-test Mean = 2.56, Post-test Mean = 4.11, p = 0.0078), empathy toward patients (Pre-test Mean = 2.11, Post-test Mean = 1.22, p = 0.0156) and colleagues (Pre-test Mean = 2.33, Post-test Mean = 1.22, respectively, p = 0.0391). Unmatched analysis revealed increases in participants confident about educating patients about autism (70.59%, post-test n = 12 vs. 3.33%, pre-test n = 1, p = 0.0019). Focus groups found increased confidence in using the term “autism”. Conclusion: Participants reported increases in ability and confidence to care for autistic patients, as well as empathy toward patients and colleagues. Future research should explore long-term outcomes in participants’ knowledge retention, confidence in practice, and improvements to autism evaluations and care. 展开更多

Based on the CIPP(context,input,process,and product)model,this paper constructs an index system suitable for evaluating the teaching effect of VBSE(virtual business social environment)practical training from four aspects:context evaluation,input evaluation,process evaluation,and product evaluation.Taking the students participating in VBSE practical training in X university as the survey population,381 valid sample data were obtained through an online questionnaire survey,and the index weights were determined by factor analysis method.The score value of the VBSE practical training teaching effect was calculated based on the evaluation mean value of three indexes.The results showed that the context evaluation score was 1.56,the input evaluation score was 1.54,the process evaluation score was 1.51,and the product evaluation score was 1.48.Subsequently,this paper put forward some countermeasures from the aspects of optimizing course arrangement,improving hardware facilities,and enhancing team cooperation to provide a guideline for improving the effect of VBSE practical training. 展开更多

Based on this,it is concluded that evaluation of farmers' training is composed of demand evaluation,process evaluation and effect evaluation. Farmers' training evaluation system is evaluated including four grades of indices. Among them,the third grade index include demand investigation,satisfaction degree of peasant household,satisfaction degree of peasant household,organization degree,talent benefit,economic benefit,social benefit. The fourth grade index is the probability of demand investigation,training method,training content,training time,training textbook,teaching facility,tracking service,plan establishment,trainee,trainer,training evaluation,knowledge understanding degree,behavior change rate,growth rate of unit yield,growth rate of unit income,growth rate of output per worker,increasing rate of organization degree and participation degree. Connotation of evaluation index and the mathematical expressions of some indices are described in order to correctly evaluate the effect of training,to improve the quality of training,and to establish the training based on science. 展开更多

This paper describes training participation situation and evaluates training effects by matching methods in the case of coastal autonomous minority nationality areas of Yunnan province by using 2053 rural household data. The result shows that the average training participation from 2006 to 2008 is 26.39 percent. In addition, ATE is 18.33 percent, TT is 18.99 percent, TUT is 18.09 percent. And,TUT . It demonstrates that the rural labor force transfer training program is effective and well-directed in coastal autonomous minority nationality areas of Yunnan province of China. 展开更多

Then this paper employs the field survey data about Yichang and Jingzhou in Hubei Province to evaluate the farmer training satisfaction level in Hubei Province. Results show that farmers have high level of satisfaction on agricultural training in Hubei Province,and the average satisfaction level reaches 0. 8556; there are regional differences in the farmer training satisfaction level in Hubei Province; the index weight is not entirely directly proportional to the training satisfaction level in the evaluation index system. Finally,from training courses,training teachers,training organization and follow-up services,this paper brings forward the recommendations for improving farmer training satisfaction level in Hubei Province,improve farmer training system,further improve the effectiveness of training and promote farmers' quality. 展开更多

Background:China’s accelerating development and increasingly important role in global health engagement create a great demand for global health professionals including international consulting experts.This study reported the detailed development and evaluation of an international consulting training for global health workforce.Methods:Based on Kirkpatrick’s model,a mixed-methods approach was used to evaluate the effectiveness of the training.Quantitative and qualitative data on participants’reaction,learning,and application of the learned knowledge and skills were collected by a training evaluation survey at the ending of training and a follow-up interview in three months after the training.Results:Thirty-six participants attended the training and 34 of them completed quantitative investigation.The training satisfaction evaluations were positive,for which participants rated the training program highly and over 90%of them agreed with the usefulness of the training.About knowledge and skills change,participants showed improved consulting knowledge and skills from pre-to post-training(P<0.001).A total of 23 participants accepted follow-up interview,and most participants applied knowledge and skills learned from the training in their daily work or study.However,only 30.4%of participants applied their learning in the consulting program.The largest barrier of application was the lack of consulting opportunities.In addition,almost all the participants reported that they would be glad to attend more training courses in the future.Conclusion:The international consulting training program was well-received,and was feasible to improve the consulting service competence of global health professionals.According to participants’feedback,it is essential to develop and expand consulting training in the field of global health. 展开更多

Taking a high-speed train in China as an example,using computer simulation technology and comparing with the test data,the three current methods including linear stability analysis method,nonlinear stability analysis method and the field testing criterion are studied to evaluate stability of high-speed trains.A new stability evaluation method is proposed which can be used to evaluate lateral stability of high-speed vehicle based on the codes of UIC 515and UIC 518.From the viewpoint of taking the most unfavorable track conditions into account and improving the safety margin,the new method uses the root mean square of bogie lateral acceleration as a criterion to evaluate the lateral stability of high-speed trains.Numerical example shows that the proposed method not only considers the forced vibration caused by track irregularities in the actual practice,but also takes the instability self-excited vibration into account,so it can realize early warning of bogie slight unstable oscillation,meanwhile the method itself does not involve complex algorithms which has the possibility of engineering applications. 展开更多

Teaching and training through online tools were used by most Higher Education Institute(HEIs)worldwide during COVID-19 to cater to the needs of students who stay far away from educational institutions.After the pandemic,the virtual model of education and training became a trend.The students and teachers are also more used to this trend which is giving more opportunities to both learners and instructors.One of the notable benefits of virtual teaching and learning platform is that it provides a flexible environment to gain knowledge,skills,and attitude simultaneously along with formal off-line education.From the earlier studies it is found that the majority of studies have focused on traditional,offline training methods and only a few studies have focused on virtual training.Therefore,the present study examined the effectiveness of virtual training using the Kirkpatrick model.For this purpose,a research instrument based on the Kirkpatrick model was constructed and distributed among the UG and PG students in Mangalore City.A total of 143 responses were collected,of which 132 were completed responses and all of them were considered for analysis.Descriptive statistics and one sample t-test are employed to analyse and interpret the data.The findings revealed that virtual training is more effective compared to traditional and offline training methods.Henceforth,the training and education provided through virtual platforms made significant contributions to the employability of youngsters. 展开更多

The worldwide commonly used evaluation criteria for wheel derailment were summarized and their main problems were pointed out. The mechanism of train derailment was expounded on the basis of system dynamics stability concept. And the energy increment criteria were proposed to evaluate train derailment. By applying the criteria, the calculated results concerning 6 cases of freight train derailment on tangent railway line and 6 cases of freight train derailment on bridge were obtained, which are all in agreement with the practical situation. The safety, comfort and stability results concerning 3 cases of freight train running on bridge were analyzed. In addition, the running speed limits of freight train on the Yanconggou and Donggou bridge in the Beijing-Tonghua railway line of 50km/h and 60km/h, respectively, were proposed. And the running speed of freight train on the Nanjing Yangtze River Bridge can reach 70km/h. 展开更多

However, techniques and training have not been scientificall... Training in acupuncture techniques has a long history of thousands of years. It has been individually handed down from person (teacher) to person (student). However, techniques and training have not been scientifically evaluated because individual differences may exist among evaluators. In animal studies, some researchers have reported that acupuncture stimulation dilates blood vessels of the skin and skeletal muscles. These studies also reveal an association between skin temperature (ST) and blood circulation volume on the skin. Our previous studies have reported that acupuncture stimulation, especially that of electroacupuncture (EA), can elevate ST. Therefore, we monitored the instructive effects and level of EA techniques with ST and propose that we can bring monitoring ST into training/education of EA as a new index of technical assessment. Moreover, ratio of changes might be used as new criteria for retraining. Healthy students (n = 14) were given with 10 minutes of EA stimulation on the tibialis anterior: Zusanli (ST36) and Tiaokou (ST38). Their ST was monitored before and during stimulation as well as for 30 minutes after stimulation. All subjects showed a nominal increase in ST. At the time, ratios of changes were also calculated. Two subjects did not reach the average of 1.3%. This suggests that the technical level of the therapist was inadequate. Thus, observation of ST elevation and calculation of the average ratio of ST change (elevation) could be applied to a new scientific index of technical assessment in acupuncture treatment training. However, further research (e.g., larger-scale studies, adjustment for gender differences, or other age subjects) is required to support these findings. 展开更多
