The incidence rate of ETM, plasmic ET, serum superoxide dismutase (SOD) activity, malonyldialdehyde (MDA), complement C3 and C-reactive protein (CRP) were studied respectively.RESULTS The ET level of group A (35.92ng/L±8.30ng/L) was significantly reduced after 7 days of treatment (P<0.05) in contrast to that of group B (47.8ng/L±11.62ng/L), so did the level of MDA and CRP. But the SOD activity and C3 level in group A increased significantly (P<0.05).CONCLUSION YCHCQ decoction combined with EST had a beneficial effect for ETM in acute cholangitis.INTRODUCTIONEndotoxemia (ETM) is one of the most important physiopathologic causes of acute cholangitis and it is the trigger of cytokines and inflammatory factors. Objective: To assess the safety and effect of endoscopicsphincterotomy (EST) and endoscopic papillary bal-loon dilatation (EPBD) for choledocholithiasis.Methods: 328 patients with choledocholithiasis weresubjected to ...Objective: To assess the safety and effect of endoscopicsphincterotomy (EST) and endoscopic papillary bal-loon dilatation (EPBD) for choledocholithiasis.Methods: 328 patients with choledocholithiasis weresubjected to EST or EPBD; they included 174 patientswith single stone, 112 patients with two stones and 42patients with three stones (one patient with 20 stones).Patients with stones less than 10 mm in diameter un-derwent EPBD and those with stones larger than 11mm in diameter underwent EST.Results: EST and EPBD succeeded in 323 patients(98.5%), and failed in 5. Stones in 98 patients wereexcluded spontaneously after endoscopic therapy. 207patients were subjected to basket or balloon stone ex-traction. Stones in 14 patients were discharged bybasket lithotripsy. Four patients were given wavelithotripsy. Stones in 22 of the 323 patients were ex-tracted thoroughly after 2-3 times attempts. Totalcomplications were noted in 2.5% of the patients.Hemorrhage from the gastrointestinal tract was seenin one patient, cholangitis in 4 patients, and pancre-atitis in 3 patients. In 92 patients receiving digestivetract barium X-ray examination, 86 developed nobarium reflux to the baliary tract, 2 pneumobilia, 4barium reflux to the biliary tract.Conclusion: EST and EPBD are relatively safe and effec-tive in treatment of choledocholithiasis, and have fewcomplications.展开更多

METHODSThis retrospective cohort study was conducted with all patients who underwent endoscopic sphincterotomy during January 2006 to September 2015 at a tertiary academic center. Patients were grouped according to pre procedural usage of serotonin reuptake inhibitors (SRIs). Both groups were matched for demographic and clinical characteristics. Patients with thrombocytopenia, increased international normalized ratio, or a history of bleeding or coagulation disorders, concurrent use of other antiplatelet/anticoagulants were excluded from the study. RESULTSA total of 447 patients were included, of which 219 (45.9%) used SRIs and 228 (54.1%) cases did not. There was no significant difference in acute or delayed bleeding during endoscopic sphincterotomy between the two groups. (8.2% vs 12.3%, P = 0.16). CONCLUSIONThe use of SRIs was not associated with an increased risk of post-sphincterotomy bleeding. To our best knowledge, this is the first study to explore this association.展开更多

This trend is...BACKGROUND At our academic tertiary care medical center, we have noted patients referred for endoscopic retrograde cholangiopancreatography(ERCP) who increasingly require advanced cannulation techniques. This trend is noted despite increased endoscopist experience and annual ERCP volume over the same period.AIM To evaluate this phenomenon of perceived escalation in complexity of cannulation at ERCP and assessed potential underlying factors.METHODS Demographic/clinical variables and records of ERCP patients at the beginning(2008), middle(2013) and end(2018) of the last decade were reviewed retrospectively. Cannulation approaches were classified as "standard" or "advanced" and duodenoscope position was labeled as "standard"(short position) or "non-standard"(e.g., long, semi-long).RESULTS Patients undergoing ERCP were older in 2018 compared to 2008(69.7 ± 15.2 years vs 55.1 ± 14.7, P < 0.05). Increased ampullary distortion and peri-ampullary diverticula were noted in 2018(P < 0.001). ERCPs were increasingly performed with a non-standard duodenoscope position, from 2.2%(2008) to 5.6%(2013) and 16.1%(2018)(P < 0.001). Utilization of more than one advanced cannulation technique for a given ERCP increased from 0.7%(2008) to 0.9%(2013) to 6.6%(2018)(P < 0.001). Primary mass size > 4 cm, pancreatic uncinate mass, and bilirubin > 10 mg/d L predicted use of advanced cannulation techniques(P < 0.03 for each).CONCLUSION Complexity of cannulation at ERCP has sharply increased over the past 5 years, with an increased proportion of elderly patients and those with malignancy requiring advanced cannulation approaches. These data suggest that complexity of cannulation at ERCP may be predicted based on patient/ampulla characteristics.
