Design and Analysis for Enlarging Mechanism of Air Cylinder Stroke

In this paper,a new type of machinery which can enlarge the stroke of air cylinder is introduced. It is very useful when the original stroke of air cylinder cannot satisfy the work requirement.According to design requirements,its multi-objective mathematic model with constrained conditions is built. In order to solve this model, some methods are discussed and used such as penalty function method,variable metric method,Newton tangent method and bisection method. Based on the optimal result,motion curves of enlarging stroke mechanism are obtained. Results show that the motion characteristic of enlarging stroke of air cylinder mechanism is satisfying. In addition,we also consider the influence of different initial values of link length to optimal results and two important laws are got in this paper on the basis of optimal results.

Planning Method of Eco-environment Restoration Based on Enlarging Terrace and De-farming in Loess Hilly Region——A Case Study of Yangou Catchment

After expatiating the guiding ideology,contents,standards and principles of eco-environment restoration based on enlarging terrace and de-farming,this paper discussed the planning method and technical flow of enlarging terrace and garden plot in a small catchment of loess hilly region by means of GIS spatial analysis technology,and then the planning method was applied in Yangou catchment.The result showed that it is practicabl,and the areas of newly-built terrace and garden plot in Yangou catchment are at least 295.06 and 4.61 hm2,so that the areas of basic farmland and garden plot reach 359.23 and 622.69 hm2.After the land use structure is regulated,the forest coverage is 48.87%,and the permanent vegetation coverage is about 75% in Yangou catchment,while sediment reduction benefit is above 80% in slope land.In agricultural development,Yangou catchment can yield 1 645.13 tons of food supplies,above 9 340 tons of apples,and can feed 7 500 sheep every year.

Cognitive impairment in cerebral small vessel disease induced by hypertension

Hypertension is a primary risk factor for the progression of cognitive impairment caused by cerebral small vessel disease,the most common cerebrovascular disease.Howeve r,the causal relationship between hypertension and cerebral small vessel disease remains unclear.Hypertension has substantial negative impacts on brain health and is recognized as a risk factor for cerebrovascular disease.Chronic hypertension and lifestyle factors are associated with risks for stro ke and dementia,and cerebral small vessel disease can cause dementia and stroke.Hypertension is the main driver of cerebral small vessel disease,which changes the structure and function of cerebral vessels via various mechanisms and leads to lacunar infarction,leukoaraiosis,white matter lesions,and intracerebral hemorrhage,ultimately res ulting in cognitive decline and demonstrating that the brain is the to rget organ of hypertension.This review updates our understanding of the pathogenesis of hypertensioninduced cerebral small vessel disease and the res ulting changes in brain structure and function and declines in cognitive ability.We also discuss drugs to treat cerebral small vessel disease and cognitive impairment.

Enlarging ion-transfer micropore channels of hierarchical carbon nanocages for ultrahigh energy and power densities

Increasing the energy density of supercapacitor without sacrificing its high power is an everlasting pursuit in energy storage.Using ionic liquid electrolyte with high operating voltage can increase the energy density but usually at the expense of power density due to the large ion size,low ionic conductivity and high viscosity.Herein we demonstrate a simultaneous increase of the energy and power densities with ionic liquid electrolyte(EMIMBF4)mainly by enlarging the ion-transfer micropore channels of the electrode material,i.e.,the unique hierarchical carbon nanocages(hCNC).Boudouard reaction is adopted to tune the micropore size while remaining the hierarchical framework of hCNC.Meanwhile,the specific surface area,pore volume and conductivity are also increased under optimal activation temperature.Such a unique modification boosts the large-sized ion transfer,leading to the obvious decrease of equivalent series resistance and the dramatic increase of supercapacitive performance thereof.The optimized product exhibits an energy density up to 153.8 W h kg^(-1) at the power density of 1.8 kW kg^(-1),and maintains 54.0 W h kg^(-1) even at an ultrahigh power density of 480.1 kW kg^(-1).This study demonstrates an effective way to explore advanced electrode materials by the fine regulation of micropores and related properties.

Tumor necrosis factor-alpha, transforming growth factor-beta, degree of lower urinary tract symptoms as predictors of erectile dysfunction in benign prostatic hyperplasia patients

Objective: Erectile dysfunction (ED) is a condition of insufficient penile erection, consistently or recurrently, for sexual activity. Tumor necrosis factor-alpha (TNF-α) induces transforming growth factor-beta (TGF-β), which causes the transition of epithelial cells into mesenchymal cells that affect ED. This study aimed to evaluate the roles of TNF-α, TGF-β, degree of lower urinary tract symptoms, and prostatic volume for the presence of ED in benign prostatic hyperplasia (BPH) patients.Methods: Our study performed an analytic observational retrospective cohort study using secondary data from four hospitals in Bali, Indonesia, including medical records and other administrative data. The sample was BPH patients with several history qualifications.Results: Our sample was 83 respondents, ranging from 50 years to 80 years, 61 respondents with ED and 22 with non-ED. The International Prostate Symptom Score showed a significant result, which indicates that ED is more common in patients with higher International Prostate Symptom Score (p=0.002). Moreover, the TNF-α of ≥43.9 pg/mg and TGF-β of ≥175.8 pg/mL were significantly associated with the presence of ED in BPH patients (p<0.0001). Despite these results, prostate volume is not significant with ED (p=0.947).Conclusion: TNF-α, TGF-β, and lower urinary tract symptoms severity can predict the occurrence of ED in BPH, while prostatic volume was not significant.

Plea to radiologists:Please consider Mahvash disease when encountering an enlarged pancreas

Radiologists play a key role in establishing an early and accurate diagnosis,especially for rare diseases.Mahvash disease(OMIM 619290)is an autosomal recessive hereditary disease caused by inactivating mutations of the glucagon receptor and its main clinical consequences are pancreatic neuroendocrine tumors and in some cases,porto-sinusoidal vascular disease and portal hypertension.Untreated Mahvash disease can be lethal.The diagnosis of Mahvash disease has almost always been delayed in the past due to radiologists’unawareness of or unfamiliarity with the unique imaging features of Mahvash disease which are moderately to enormously enlarge pancreas with preserved pancreas contour and parenchyma without vascular involvement or lymphadenopathy.These features help differentiate Mahvash disease from other etiologies of diffusely enlarged pancreas such as diffuse pancreatic ductal carcinoma,diffuse pancreatic lymphoma,and autoimmune pancreatitis.Invoking Mahvash disease in the differential diagnosis of an enlarged pancreas has recently been shown to facilitate early diagnosis.To prevent missing the diagnosis of this significant disease,I sincerely ask radiologists to consider Mahvash disease in their differential diagnoses of diffusely enlarged pancreas.

Ethmoidal Polyposis, Adenoid Hypertrophy and Tympanic Membrane Perforation—A Case Report

Background: Ethmoidal polyposis coexisting with adenoid enlargement and tympanic membrane perforation is a rare condition in otolaryngological practice. Evidences have shown that antrochoanal polyp often times coexists with adenoid enlargement especially in the young which was in sharp contrast to ethmoidal polyposis occurring in the presence of adenoid enlargement. Moreover, the hidden location of nasopharynx housing the adenoids, coupled with the masking effect of bilateral sinonasal polyps, leads to inadvertent misdiagnosis of this complex pathology with subsequent difficulty in providing effective management. Case Presentation: A 24 year-old lady presented with recurrent bilateral nasal obstruction that became persistent associated with nasal discharge and anosmia. She also had recurrent right otorrhoea with associated hearing loss. Diagnostic rigid nasal endoscopy revealed mucoid discharge with bilateral polypoid masses filling both nasal cavities. Otoendoscopic finding revealed a small (about 5%) central tympanic membrane perforation. A clinical assessment of chronic rhinosinusitis with nasal polyposis complicated by chronic suppurative otitis media (CSOM) was made. A non-contrast CT scan of the paranasal sinuses showed isodense lesions in the nasal cavities, all paranasal sinuses and the entire nasopharynx. She had endoscopic sinus surgery and a nasopharyngeal clearance biopsy. The nasal, paranasal and nasopharyngeal masses had histologic confirmation of inflammatory nasal polyps and lymphoid (adenoid) hyperplasia respectively. Her condition improved remarkably with subsequent medical treatment. She was followed up for 8 months and no recurrence was observed. Conclusion: Sinonasal polyposis can coexist with adenoid hypertrophy and middle ear disease as a single pathological condition. Hence, a high index of suspicion and thorough evaluation become necessary for making timely diagnoses and instituting effective management.

Analysis of the Application and Effect of Homemade Medical Scrotal Support Shorts

Objective:To design and manufacture medical scrotal support shorts for patients with enlarged scrotum and observe its application and effect.Methods:40 patients with enlarged scrotum admitted to the basic surgery department from February 2021 to March 2023 were selected and divided into a test group and a control group according to their time of admission,with the test group using scrotal support shorts and the control group using ordinary shorts without scrotal support pockets or diapers.Results:The complication rate of skin injury in the scrotum and the surrounding inguinal area of the patients in the test group was significantly lower than that of the control group(P<0.01).The medical cost covered by patients in the test group was significantly lower than that of the control group(P<0.01)and the hospitalization satisfaction of the patients in the test group was significantly higher than that of the control group(P<0.01).The difference in the therapeutic effect of the test groups was statistically significant when compared with the control group(P<0.05).Conclusion:Homemade medical scrotal support shorts reduced the local enlargement of the scrotum and bleeding,but also protected the scrotum and the surrounding skin to prevent secondary injuries.The process of patient care was simple and promoted their recovery.The length of hospitalization was also decreased,the burden of health care costs was reduced,and the overall comfort and satisfaction of the patient was improved.

Hierarchically designed MoWSe_(2)/WO_(3)/C anode for fast and efficient Na+storage

Exploring anode materials with high energy and power density is one of the critical milestones in developing sodium-ion batteries/capacitors(SIBs/SICs).Here,the Mo and W-based bimetallic organic framework(Mo-W-MOF)with core-shell structure is first formed by a facile strategy,followed by a selenization and carbonization strategy to finally prepare multileveled Mo WSe_(2)/WO_(3)/C anode materials with core-shell petal like curled nanosheet structure.Between the petal(MoSe_(2))-core(WO_(3))structure,the formation of WSe_(2)flakes by partial selenization on the surface of WO_(3)serves as a heterogeneous connection between MoSe_(2)and WO_(3).The enlarged layer distance(0.677 nm)between MoSe_(2)and WSe_(2)can facilitate the rapid transfer of Na+and electrons.The density functional theory(DFT)calculations verify that the Mo WSe_(2)/WO_(3)/C heterostructure performs excellent metallic properties.Ex-situ X-ray diffraction(XRD),X-ray photoelectron spectroscopy(XPS),and transmission electron microscopy(TEM)confirm the activation process from the initial insertion reaction to the later conversion reaction.Resultantly,when employed as the anode of SIBs,a remarkable capacity of 384.3 mA h g-1after 950 cycles at 10 A g^(-1)is performed.Furthermore,the SICs assembled with commercial activated carbon(AC)as the cathode exhibits a remarkable energy density of 81.86 W h kg^(-1)(at 190 W kg^(-1))and 72.83 W h kg^(-1)(at 3800 W kg^(-1)).The unique structural design and the reaction investigation of the electrode process can provide a reference for the development of transition metal chalcogenides anodes.

Study on Biological Pathway of Carbon Dioxide Methanation Based on Microbial Electrolysis Cell

Realization of CO_(2) resource utilization is the main development direction of CO_(2) reduction.The CO_(2) methana-tion technology based on microbial electrolysis cell(MEC)has the characteristics of ambient temperature and pressure,green and low-carbon,which meets the need of low-carbon energy transition.However,the lack of the system such as the change of applied voltage and the reactor amplification will affect the methane production efficiency.In this research,the efficiency of methane production with different applied voltages and different types of reactors was carried out.The results were concluded that the maximum methane production rate of the H-type two-chamber microbial electrolysis cells(MECs)at an applied voltage of 0.8 V was obtained to be 1.15 times higher than that of 0.5 V;under the same conditions of inoculated sludge,the reactor was amplified 2.5 times and the cumulative amount of methane production was 1.04 times higher than the original.This research can provide a theoretical basis and technical reference for the early industrial application of CO_(2) methanation tech-nology based on MEC.

Endoscopic Kyoto classification of Helicobacter pylori infection and gastric cancer risk diagnosis

Recent advances in endoscopic technology allow detailed observation of the gastric mucosa.Today,endoscopy is used in the diagnosis of gastritis to determine the presence/absence of Helicobacter pylori(H.pylori)infection and evaluate gastric cancer risk.In 2013,the Japan Gastroenterological Endoscopy Society advocated the Kyoto classification,a new grading system for endoscopic gastritis.The Kyoto classification organized endoscopic findings related to H.pylori infection.The Kyoto classification score is the sum of scores for five endoscopic findings(atrophy,intestinal metaplasia,enlarged folds,nodularity,and diffuse redness with or without regular arrangement of collecting venules)and ranges from 0 to 8.Atrophy,intestinal metaplasia,enlarged folds,and nodularity contribute to gastric cancer risk.Diffuse redness and regular arrangement of collecting venules are related to H.pylori infection status.In subjects without a history of H.pylori eradication,the infection rates in those with Kyoto scores of 0,1,and≥2 were 1.5%,45%,and 82%,respectively.A Kyoto classification score of 0 indicates no H.pylori infection.A Kyoto classification score of 2 or more indicates H.pylori infection.Kyoto classification scores of patients with and without gastric cancer were 4.8 and 3.8,respectively.A Kyoto classification score of 4 or more might indicate gastric cancer risk.

Influence of enlarged section parameters on pressure transients of high-speed train passing through a tunnel

The influence of enlarged section parameters on pressure transients of high-speed train passing through a tunnel was investigated by numerical simulation.The calculation results obtained by the structured and unstructured grid and the experimental results of smooth wall tunnel were verified.Numerical simulation studies were conducted on three tunnel enlarged section parameters,the enlarged section distribution along circumferential direction,the enlarged section area and the enlarged section distribution along tunnel length direction.The calculation results show that the influence of the different enlarged section distributions along tunnel circumferential direction on pressure transients in the tunnel is basically consistent.There is an optimal enlarged section area for the minimum value of the pressure variation amplitude and the average pressure variation in the tunnel.The law of the pressure variation amplitude and the average pressure variation of the enlarged section distribution along tunnel length direction are obtained.
