Methods: The study used a descriptive cross-sectional design. A total of 146 nurses working in 6 long-term care facilities in South Korea self-reported their perception of EBP implementation, barriers to research utilization, and job satisfaction. Results: The level of job satisfaction was higher in nurses who were single, older, more experienced, and had a higher income. Lower perceived barriers to research utilization were associated with greater job satisfaction. However, there was no significant relationship between a level of EBP implementation and job satisfaction. Factors influencing job satisfaction included lower barriers to the organization and communication domains of research utilization, being unmarried and older. Conclusion: Decreasing barriers to research utilization may improve the job satisfaction of nurses working in long-term care facilities.展开更多

文摘Objective: To compare(i) the evidence-based practice(EBP) profiles and(ii) socio-demographic characteristics of nurses in 4 selected low-middle income countries(LMICs).Methods: A cross-sectional survey was conducted among 677 nurses from 4 hospitals in 4 countries(Afghanistan, Ghana, Iran, and Uganda). Results: The findings revealed that there are significant differences between the 3 EBP subscales(that is, practice/use, attitude, and knowledge/skill) across the 4 countries(P < 0.05). No significant EBP differences were identified across age groups and gender in the subscales of practice, knowledge, and attitude. Practice and knowledge subscales were significantly associated with working schedules(that is, par t-time and full-time)(P < 0.05). Analyses also revealed significant differences between attitude and knowledge subscales and working experience(P < 0.05).Conclusions: Nurses in the 4 countries have moderate practice, attitude, and knowledge/skil s of EBP. Nurses in LMICs require continuous professional development programs and suppor t to enhance their practice and knowledge regarding EBP in clinical settings.
文摘Objective:To understand the evidence-based practice readiness of developing evidence application projects in a 3A-level general hospital in Beijing,to identify barriers to evidence-based practice and to promote the successful application of evidence. Methods:All the clinical nurses who were going to participate in the application units of the evidence-based practice project were selected as the survey objects, and the self-made“Basic Information Questionnaire”and“Clinical Readiness to Evidence-based Nursing Assessment(CREBNA)”were used to conduct the survey. The total amounts and subscales were calculated. The factors that influence the score of the total scale were scored and analyzed. Results:The CREBNA total score was(119.87±19.18),the evidence subscale score was(47.94±8.54),the organizational environmental subscale score was(36.09±6.11),and the facilitator subscale score was(35.83±7.56).Univariate analysis showed that the total score of the scale was related to years of work, scientific research experience, knowledge of evidence-based care, and participation in evidence-based practice. Conclusion:The current evidence-based practice preparation situation is good.It is feasible to carry out evidence-based practice activities in the hospital. The follow-up development plan should be made based on the corresponding obstacle factors.
文摘Purpose: The purpose of this study was to identify the relationship between evidence-based practice (EBP) and job satisfaction among nurses working in long-term care facilities. Conclusion: Decreasing barriers to research utilization may improve the job satisfaction of nurses working in long-term care facilities.

文摘Objectives Even though guidelines are available to guide dysphagia identification and management practice, there is still a gap between evidence and practice, which requires improvement. The purpose of this study was to determine the effect of using tailored, multifaceted strategies to improve evidence-based post-stroke dysphagia identification and management practice in a community hospital. Methods Guided by the Knowledge to Action framework, the tailored, multifaceted strategies were developed and implemented for 5 months in a community hospital using a before-and-after study design. These strategies consisted of training intervention, policy intervention, and audit and feedback intervention.Nurses’level of knowledge and adherence, were collected in March 2019 and again in January 2020.Patients’quality of life and satisfaction were evaluated during the pre-intervention period(between February 2019 and April 2019)and the post-intervention period(between November 2019 and January 2020).Results A total of 55 patients with post-stroke dysphagia(28 in the pre-intervention period and 27 in the post-intervention period)and 17 registered nurses were recruited.Following implementation, there were statistically significant improvements in patients’outcomes(quality of life and satisfaction)and nurses’outcomes(level of knowledge and adherence). Conclusions This study assists in closing the research-practice gap by using tailored, multifaceted strategies to increase the use of evidence-based nursing care for dysphagia identification and management practices.
文摘Background:Evidence-based interventions(EBIs)are frequently adapted or modified during the implementation process to meet the needs of the target context, public health professionals,or health practitioners. However, little is known about how the EBIs of the same clinical problem at a different time(sustainability)and a different setting(scalability)should be adapted to facilitate implementation. To address this gap, this research aims to analyze the adaptations process of EBIs by taking post-stroke dysphagia screening as an example based on a series of empirical studies. Methods:We reviewed three best practice implementation projects of post-stroke dysphagia identification and management in China's Mainland, and conducted a comparative analysis of three projects of dysphagia screening practical decision-making according to core elements of evidence-based nursing(EBN).Results:We identified the core elements of EBN that influence decision-making in each best practice implementation project. The decision-making of screening time and tool for dysphagia in the three projects varied.Project 1:Post-stroke dysphagia was screened using Water Swallow Test(WST)before the first intake of liquid or food.Project 2:A sustainability study based on project 1,post-stroke dysphagia was screened using Standardized Swallowing Assessment(SSA)before the first intake of liquid or food, within 24 hours of admission.Project 3:A community scalability study based on project 2,post-stroke dysphagia was screened using Water Swallow Test(WST)before the first intake of liquid or food, within 24 hours after referral. Conclusions:Adaption of EBIs needs to fully consider the best available external evidence from systematic research, clinical expertise, patient preferences,as well as context.
