Current Research Status of Traditional Chinese Medicine External Treatment for Diarrhea Type Irritable Bowel Syndrome

Irritable bowel syndrome(IBS-D)with diarrhea is a common gastrointestinal functional disease in clinical practice,which seriously affects the quality of life of patients.Cur‐rently,Western medicine has poor therapeutic effects,while traditional Chinese medi‐cine has unique advantages in relieving IBS-D symptoms and preventing recurrence.In recent years,especially with external treatment of traditional Chinese medicine,it has become a new treatment direction in clinical practice and has achieved good therapeutic effects.This article will provide a review of recent research on the treatment of IBS-D using traditional Chinese medicine external treatment methods.

The Characteristics of the Drug Composition in the Treatment of Insomnia by External Bath Prescription of Traditional Chinese Medicine

[Objectives]The research aimed to carry out a retrospective analysis on the characteristics of the drug composition in the treatment of insomnia by external bath prescription of traditional Chinese medicine,to provide ideas for the research and development of new Chinese medicine for insomnia.[Methods]The external bath prescriptions of traditional Chinese medicine related to insomnia were collected,and the appearance frequency of drug was analyzed,and statistical analysis of drug with high appearance frequency was conducted.[Results]In the external bath prescriptions of traditional Chinese medicine related to insomnia,the top ten herbs with high appearance frequency were as follows:Chaozaoren,Drgonsbones,Margaritifera,Yejiaoteng,Schisandrae Chinensis Fructus,Lilii Bulbus,Platycladi Semen,Poria,Coptidis Rhizoma,and Scutellariae Radix.According to the classification by efficacy,these herbs mostly have the effects of calming,nourishing and heat clearing.[Conclusions]Traditional Chinese medicine is most closely related to heart,kidney,liver and other viscera.Tranquilizing and sedative drugs should be chosen,which corresponds with the understanding on pathogenesis of insomnia in traditional Chinese medical science,and embodies the characteristics of external bath prescription of traditional Chinese medicine in the treatment of insomnia.

The method of external use of Chinese medicine in prevention of epidemic disease and its application in COVID-19

The ancient people of our country have accumulated rich experience in prevention of epidemic disease in the course of fighting the epidemic for thousands of years,and invented effective methods of external use of Chinese medicine for preventing epidemic,some of these methods have been still used today,in addition to ensuring the health and safety of ancient people in China.In the COVID-19 ravaged China today,all kinds of protective supplies for the general public is difficult to get,so the study of external use of Chinese medicine that preventing epidemic for modern society is still very significant and valuable.According to the characteristics of COVID-19 and the theory of combination of disease-syndrome,based on the applicability and feasibility for modern society,combed out three methods of external use of Chinese medicine,including medicine hanging,medicine smearing and medicine fumigation,and made corresponding application recommendations for hospitals,communities,families to prevent epidemic disease.

Research progress of external therapies of traditional Chinese medicine in the treatment of non-alcoholic fatty liver disease

Non-alcoholic fatty liver disease(NAFLD)is a common chronic liver disease characterized by diffuse hepatic steatosis.With the improvement of people's living standard,the incidence rate of NAFLD has been increasing,which has become one of the global health problems in 21st Century.However,there is no specific drug or standard treatment for NAFLD,which brings challenges to treatment.Acupuncture,moxibustion,massage and other external therapies based on the characteristics of traditional Chinese medicine have obvious curative effect in the clinical treatment of NAFLD,but the mechanism has not been systematically explained,which makes the clinical promotion evidence insufficient.This paper aims to summarize the researches on the treatment of NAFLD by external therapies of traditional Chinese medicine in recent years,and analyze its possible mechanism,so as to provide a scientific theoretical basis for future basic experiments and clinical research,and form a set of standardized clinical diagnosis and treatment scheme with the characteristics of traditional Chinese medicine.

External application of traditional Chinese medicine has a long history and definite curative effect on gout.By searching the latest literature on the treatment of acute gouty arthritis by external treatment of traditional Chinese medicine,the research progress of external treatment of acute gouty arthritis in recent years was summarized.According to the existing research results,the external treatment of Chinese medicine in the treatment of acute gout has better efficacy and higher safety,and has a broad application prospect.

Application of external treatment of traditional Chinese medicine in the treatment of cancer-related fatigue

External treatment of traditional Chinese medicine(TCM),also known as external treatment of internal diseases,has many advantages,such as simple,easy to operate,cost-effective and so on.Therefore,external treatment of TCM is favored by the majority of medical workers.By consulting the domestic literature reports on the treatment of cancer-related fatigue(CRF),it is found that the external treatment of TCM has a significant effect on the intervention of CRF,and has a good effect on reducing fatigue,improving the quality of life and prolonging the survival time.The purpose of this paper is to summarize the application of several external treatments in this disease,and to provide a variety of references for clinical treatment of CRF.

Study on the Correlation between Syndrome Differentiation of Malignant Pleural Effusion Treated by External Treatment of Traditional Chinese Medicine and Immunohistochemistry of Biopsy Tissue Based on Medical Video-assisted Thoracoscope

Objective:Guided by the theory of syndrome differentiation of yin and yang in traditional Chinese medicine surgery,through visual observation of internal medicine thoracoscope,comprehensive observation of pleural cavity and immunohistochemistry of biopsy tissue,to classify malignant pleural effusion according to syndrome differentiation,and to explore the scientific nature of its theory.Methods:From March 1,2014 to February 28,2015,40 cases of malignant pleural effusion were treated in Beijing Chaoyang Hospital affiliated to Capital Medical University.According to the proposed TCM diagnostic criteria for yin and yang syndrome differentiation,and collect age,gender,course of disease,clinical symptoms,tumor primary focus,histomorphological manifestations and immunohistochemical results and other related information,and carry out statistical data processing.Results:The positive syndrome was mainly metastatic lung adenocarcinoma,which accounted for the majority of all MPE cases,up to 75%.The immunohistochemical results of biopsy tissues were mainly CEA and TTF-1 positive;While pleural effusion caused by pleural mesothelioma was the main type of yin syndrome,and the results of immunohistochemistry combined with biopsy were mainly positive for D2-40,Calretinin,WT-1 and CK5/6.Conclusion:TCM syndrome differentiation of MPE based on internal thoracoscopy combined with biopsy immunohistochemical results has sufficient theoretical basis and certain scientific nature,and further clinical research is needed to verify its effectiveness and practicability in the future.

Advances in the Study on"Simmering Pus and Forming Flesh"in Treating Anorectal Diseases by External Treatment in Traditional Chinese Medicine

Due to the special perianal location,anorectal diseases are prone to form chronic refractory wounds after surgery,with Qi(气)deficiency and blood stasis as the main cause.As a classical theory in TCM medicine,"simmering pus and forming flesh"attaches great importance to harmonizing Qi and blood in the wound healing period,which gradually achieves significant efficacy in clinical application after anorectal disease surgery.Literatures in recent years on"simmering pus and forming flesh"method being applied to postoperative wound healing in all kinds of anorectal diseases(such as anal fistula,perianal abscess,anal fissure,mixed hemorrhoid,etc.)were reviewed,and advantages and characteristics of its clinical efficacy in anorectal diseases were summarized,so as to provide reference for postoperative wound healing and repair treatment in anorectal diseases.

Professor Renting Li's Combination of Internal and External Treatment of Traditional Chinese Medicine in Treating Radiation Enteritis

Radiotherapy is a common treatment for abdominal and pelvic malignancies with bone metastases.However,some patients develop radiation enteritis(RE)during the treatment or 2 months or more after the treatment,which seriously affects their quality of life.At present,the curative effect of western medicine is poor.Professor Renting Li believes that radiation kills tumor cells but at the same time,it damages the healthy Qi of the human body.Radiation causes heat and poison to accumulate in the intestinal tract,consumes and hurts Yin fluid,bums the vein,leads to deficiency of healthy Qi and poor detoxification,as well as cause blood stasis after a period of time.All these result in the combination of heat,blood stasis,and poison,manifesting as abdominal pain,diarrhea,mucus in stools,purulent and bloody stools,etc.Therefore,Professor Renting Li proposed the use of traditional Chinese medicine retention enema combined with oral traditional Chinese medicine to reduce symptoms,remove toxins,and improve the quality of life of patients.

Extreme Obesity Treated with Integrated Traditional Chinese and Western Medicine and Construction of an Innovative Obesity Diagnosis and Treatment Model: A Case Report

Background:The incidence of obesity is increasing each year,and is often accompanied by multi-system metabolic disorders,typically chronic diseases and multi-disease superposition disease states.Metabolic surgery is an important treatment option for patients with extreme obesity;however,surgical difficulties and perioperative complications are more serious in these patients.Therefore,there is an urgent need for a new comprehensive diagnosis and treatment model to reduce the weight of patients prior to surgery,subsequently reducing the risks of surgery.Case summary:This model combines traditional Chinese medicine(TCM),external TCM treatment,and health management with western technique of metabolic surgery.Obesity is recognized and treated by multidisciplinary,multi-channel,and multi-means,constituting an innovative diagnosis and treatment model of obesity in integrated traditional Chinese and Western medicine.The application of this model in the treatment of a patient with extreme obesity having multiple systemic diseases is described herein.Conclusion:The integrated traditional Chinese and Western medicine obesity diagnosis and treatment model can be used for precise syndrome differentiation,individualized treatment,and follow-up management of patients with extreme obesity,with strong scalability and significant clinical efficacy.

Diabetic foot is one of the chronic complications of diabetes,which is serious and expensive to treat.As an effective method for treating diabetic foot,Chinese medicine plays an important role in improving the clinical symptoms and reducing the suffering of diabetic foot patients.The methods of traditional Chinese medicine to treat diabetic foot include internal treatment method,external treatment method and comprehensive therapy.This article reviews the research results of traditional Chinese medicine treatment of diabetic foot in order to provide reference for the treatment of diabetic foot.

Evaluation of the Therapeutic Effect of Contrast-Enhanced Ultrasound on Different Types of Knee Osteoarthritis in the Elderly

Objective: To evaluate the curative effect of the Traditional Chinese Medicine (TCM) external therapy on knee osteoarthritis patients with different TCM constitutions using musculoskeletal ultrasonography and contrast-enhanced ultrasonography, and to explore the application value of contrast-enhanced ultrasonography in knee joint diseases. Methods: A total of 57 patients diagnosed with knee osteoarthritis in Shaanxi University of Traditional Chinese Medicine from December 2019 to May 2021 were collected, and they were divided into qi stagnation and blood stasis type group (23 cases) and cold-dampness obstruction type group (34 cases) according to the traditional Chinese medicine method. All patients were given acupuncture combined with TCM fumigation and washing. All patients underwent musculoskeletal ultrasonography and contrast-enhanced ultrasonography before and after treatment, observed and recorded relevant data, and compared the treatment effects between the two groups. Results: 85.96% (49/57) of knee osteoarthritis (KOA) patients had suprapatellar bursa effusion, 42.1% (24/57) had iliotibial band bursae effusion, some of which had poor sound transmission, and thickened synovium was seen in most effusions, 33.33% (19/57) had osteophyte formation. Compared with before treatment, the depth of suprapatellar sac effusion in the Qi stagnation and blood stasis type group decreased after treatment (P Conclusion: Musculoskeletal contrast-enhanced ultrasonography was used to quantitatively evaluate the efficacy of TCM external therapy on KOA for different TCM constitutions. Dynamic observation of synovial lesions of knee osteoarthritis provides a valuable imaging method for evaluating the efficacy of traditional Chinese medicine.

External Application of Chinese Medcine Qu Tong Ling（去痛灵）in Trealing 144 Cases of Cancer Pain

One hundred forty-four patients of cancer pain were treated with external application of QuTong Ling, and the effect of pain remission was observed. The total remission rate was 79. 2% with themaintaining time of 6. 42 1 . 24 hours, proving it was superior to butycinnamyl pyrazirum.

Study on Tongkuaixiao Babu Piaster (痛块消巴布剂) inTreating Cancer Pain

Objective: To explore the efficacy of Tongkuaixiao Babu plaster (痛块消巴布剂, TKXBBP) in treating cancer pain. Methods: In the clinical observation, sixty-five patients with moderate or severe cancer pain were randomly divided into two groups: 32 in the treated group (TKXBBP group) and 33 in the control group (Bucinnazine group). The therapeutic effects in relieving pain, improving quality of life (QOL), and the rate of satisfaction the patients felt of the two groups were compared respectively. Results:TKXBBP was effective in treating cancer pain. There wasn't any statistically significant difference in total effective rate (P>0.05), but the statistical difference was significant in obvious remission rate (P<0.05) between the treated and control group, and the effect on serious pain shown in the treated group was better than that of the control group (P<0.05). The difference in the initiation time of relieving cancer pain was insignificant (P>0.05), while in the remission period, the treated group showed its treatment was obviously superior to that of the control group (P<0.05). TKXBBP showed better effect in the improvement of QOL ( P<0. 05) and satisfaction rate, with significant difference between the treated and the control groups (P<0.01). Conclusion: TKXBBPs effect in treating cancer pain was obvious, its application was safe and convenient. It was shown that the external treatment with this kind of Chinese medicine had great advantage in treating cancer pain.

Alleviation of Cancerous Pain by External Compress with Xiaozheng Zhitong Paste(消癥止痛方)

Objective： To observe the clinical effectiveness of a topical application of Xiaozheng Zhitong Paste （消癥止痛方, XZ.P） in alleviating the cancerous pain of patients with middle/late stage cancer. Methods： By adopting a random number table, 124 patients enrolled were randomized into the treatment group （64 patients） and the control group （60 patients）. In addition to the basic therapy [including the three-ladder （3L） analgesia] used in both groups, topical application of XZP was given to patients in the treatment group for pain alleviation. The analgesic efficacy was recorded in terms of pain intensity, analgesia initiating time and sustaining time, and the optimal analgesic effect revealing time. Meanwhile, the quality of life （QOL） and adverse reactions that occurred in patients were recorded as well. Results： The total effective rate in the treatment group was 84.38% （54/64）, and in the control group it was 88.33% （53/60）, showing no significant difference between them （P〉0.05）, but the analgesia initiating time and the optimal analgesia effect revealing time in the treatment group were significantly shorter （both P〈0.01）. Moreover, XZP was better in improving patients＇ QOL, showing more significant improvements in the treatment group than those in the control group in aspects of mental condition, walking capacity, working capacity, social acceptability, sleep and joy of living （P〈0.05 or P〈0.01）. Lower incidence of adverse reactions, such as nausea, vomiting, mouth dryness, dizziness, etc., especially constipation, was noted in the treatment group （P〈0.05 or P〈0.01）. Conclusion： Applying an external compress of XZP showed a synergistic action with 3L analgesia for shortening the initiating time and the optimal effect revealing time, and could evidently enhance patients＇ QOL with fewer adverse reactions.
