The paper formulates new principles that should form the basis for the development and creation of new environmental monitoring based on heavy UAVs and high-altitude so-called pseudo-satellites capable of operating for a long time at altitudes of 25 - 30 km. In order to develop such principles, this paper analyzes the radioecological situation in the territories of Donetsk and Luhansk regions of Ukraine for rapid and high-quality environmental cleanup and rehabilitation of areas with detected critical levels of environmentally hazardous pollutants. In order to quickly obtain fundamentally new environmental information, it is necessary to conduct multi-parameter, high-precision integrated monitoring of the Earth’s geospheres based on the latest methods and equipment for ground and remote environmental measurements, and new methods and technological means of clean, environmentally safe processing and final disposal. As the most appropriate technology, we propose mobile installations for plasma-chemical pyrolysis of medical waste directly at the place of its generation. 展开更多

How hypertensive patients in the rural areas use home blood pressure monitoring and its relationship with medication adherence: A primary care survey in China 被引量:1

Despite an increasing popularity of home blood pressure monitoring (HBPM) over the last few decades, little is known about HBPM use among hypertensive patients in the rural areas. A cross-sectional survey including 318 hypertensive patients was conducted in a rural community in Beijing, China, in 2012. Participants were mainly recruited from a community health clinic and completed the questionnaires assessing HBPM usage. Binary logistic regression models were used for the analysis of medication adherence with age, gender, level of education marital status, perceived health status, duration of hypertension, HBPM use, and frequency of performing BP measurement. Among the total population, 78 (24.5%) reported currently use of HBPM. Only 5.1% of the HBPM users cited doctor’s advice as the reason for using HBPM. Analysis of the risk factors of poor medication adherence by multivariable modeling indicated significant associations between the duration of hypertension (adjusted OR, 3.31;95% CI, 1.91-5.72;P 01), frequency of performing BP measurements (adjusted OR, 2.33;95% CI, 1.42-3.83; 展开更多

Telemonitoring in heart failure patients:Systematic review and metaanalysis of randomized controlled trials

BACKGROUND Home telemonitoring has been used as a modality to prevent readmission and improve outcomes for patients with heart failure.However,studies have produced conflicting outcomes over the years.AIM To determine the aggregate effect of telemonitoring on all-cause mortality,heart failure-related mortality,all-cause hospitalization,and heart failure-related hospitalization in heart failure patients.METHODS We conducted a systematic review and meta-analysis of 38 home telemonitoring randomized controlled trials involving 14993 patients.We also conducted a sensitivity analysis to examine the effect of telemonitoring duration,recent heart failure hospitalization,and age on telemonitoring outcomes.RESULTS Our study demonstrated that home telemonitoring in heart failure patients was associated with reduced all-cause[relative risk(RR)=0.83,95% confidence interval(CI):0.75-0.92,P=0.001]and cardiovascular mortality(RR=0.66,95%CI:0.54-0.81,P<0.001).Additionally,telemonitoring decreased the all-cause hospitalization(RR=0.87,95%CI:0.80-0.94,P=0.002)but did not decrease heart failurerelated hospitalization(RR=0.88,95%CI:0.77-1.01,P=0.066).However,prolonged home telemonitoring(12 mo or more)was associated with both decreased all-cause and heart failure hospitalization,unlike shorter duration(6 mo or less)telemonitoring.CONCLUSION Home telemonitoring using digital/broadband/satellite/wireless or blue-tooth transmission of physiological data reduces all-cause and cardiovascular mortality in heart failure patients.In addition,prolonged telemonitoring(≥12 mo)reduces all-cause and heart failure-related hospitalization.The implication for practice is that hospitals considering telemonitoring to reduce heart failure readmission rates may need to plan for prolonged telemonitoring to see the effect they are looking for. 展开更多

Objective:Home blood pressure monitoring(HBPM)is viewed as a facilitating factor in the initial diagnosis and long-term management of treated hypertension.However,evidence remains scarce about the effectiveness of HBPM use in the real world.This study aimed to examine the associations of HBPM use with blood pressure(BP)control and medication adherence.Methods:This prospective cohort study included hypertensive patients with high cardiovascular risk who were aged≥50 years.At baseline,information about types of BP monitor,frequency of HBPM,perception of anti-hypertensive treatment,and measured office BP were collected.During the 1-year follow-up(visits at 1,2,3,6,and 12 months),information on medication adherence was collected at each visit.The 2 major outcomes were BP control at baseline and medication adherence during the 1-year follow-up.A log-binomial regression model was used to examine the association between frequency of HBPM and outcomes,stratified by the perceptions of anti-hypertensive treatment.Results:A total of 5,363 hypertensive patients were included in the analysis.The age was(64.6±7.2)years,and 41.2%(2,208)were female.Of the total patients,85.9%(4,606)had a home BP monitor and 47.8%(2,564)had an incorrect perception of anti-hypertensive treatment.Overall,24.2%(1,299)of patients monitored their BP daily,37.6%(2,015)weekly,17.3%(926)monthly,and 20.9%(1,123)less than monthly.At baseline,the systolic BP and diastolic BP were(146.6±10.8)mmHg and(81.9±10.6)mmHg,respectively,and 28.5%(1,527)of patients had their BP controlled.Regardless of whether the patients had correct or incorrect perceptions of anti-hypertensive treatment,there is no significant association between HBPM frequency and BP control at baseline.During the 1-year follow-up,23.9%(1,280)of patients had non-adherence to medications at least once.In patients with an incorrect perception of anti-hypertensive treatment,those monitoring BP most frequently(daily)had the highest non-adherence rate(29.9%,175/585).Compared with those monitoring their BP less than monthly,patients who monitored their BP daily were more likely not to adhere to anti-hypertensive medications(adjusted relative risk=1.38,95%confidence interval:1.11–1.72,P=0.004).Conclusions:HBPM performance among hypertensive patients in China is,in general,sub-optimal.No association was observed between using HBPM alone and hypertension control,indicating that the effects of HBPM could be conditional.Patients’misconceptions about anti-hypertensive treatment may impair the role of BP monitoring in achieving medication adherence.Fully incorporating the correct perception of hypertension into the management of hypertensive patients is needed. 展开更多

Another reason for the need of home safety is specifically when the elderly person is alone or the kids are with baby-sitter and servant. Home security system i.e. HomeOS is thus applicable and desirable for resident’s safety and convenience. This will be achieved by turning your home into a smart home by intelligent remote monitoring. Smart home comes into picture for the purpose of controlling and monitoring the home. It will give you peace of mind, as you can have a close watch and stay connected anytime, anywhere. But, is common man really concerned about home security? An investigative study was done by conducting a survey to get the inputs from different people from diverse backgrounds. The main motivation behind this survey was to make people aware of advanced HomeOS and analyze their need for security. This paper also studied the necessity of HomeOS investigative study in current situation where the home burglaries are rising at an exponential rate. In order to arrive at findings and conclusions, data were analyzed. The graphical method was employed to identify the relative significance of home security. From this analysis, we can infer that the cases of having kids and aged person at home or location of home contribute significantly to the need of advanced home security system. In this paper,the Internet ofMedical Things(IoMT)is identified as a promising solution,which integrates with the cloud computing environment to provide remote health monitoring solutions and improve the quality of service(QoS)in the healthcare sector.However,problems with the present architectural models such as those related to energy consumption,service latency,execution cost,and resource usage,remain a major concern for adopting IoMT applications.To address these problems,this work presents a four-tier IoMT-edge-fog-cloud architecture along with an optimization model formulated using Mixed Integer Linear Programming(MILP),with the objective of efficiently processing and placing IoMT applications in the edge-fog-cloud computing environment,while maintaining certain quality standards(e.g.,energy consumption,service latency,network utilization).A modeling environment is used to assess and validate the proposed model by considering different traffic loads and processing requirements.In comparison to the other existing models,the performance analysis of the proposed approach shows a maximum saving of 38%in energy consumption and a 73%reduction in service latency.The results also highlight that offloading the IoMT application to the edge and fog nodes compared to the cloud is highly dependent on the tradeoff between the network journey time saved vs.the extra power consumed by edge or fog resources. 展开更多

目的探讨基于家庭监护系统的远程随访平台在起搏器植入术后患者中的应用及效果。方法采用便利抽样方法,选取植入具有家庭监护功能的双腔起搏器患者作为研究对象,将2021年1月至2022年3月收治的42例患者作为对照组,2022年4月至2023年6月... 目的探讨基于家庭监护系统的远程随访平台在起搏器植入术后患者中的应用及效果。方法采用便利抽样方法,选取植入具有家庭监护功能的双腔起搏器患者作为研究对象,将2021年1月至2022年3月收治的42例患者作为对照组,2022年4月至2023年6月收治的41例患者作为观察组。对照组采用常规门诊随访,观察组采用基于家庭监护系统的远程随访平台进行随访管理。干预6个月后,对两组患者的肩关节功能、自我护理能力、门诊随访次数、随访干预效果进行比较分析。结果两组各40例完成全程研究。干预6个月后,观察组门诊随访次数显著低于对照组,肩关节功能总分及日常生活能力、肩关节活动功能、肩关节疼痛3个维度得分显著高于对照组;观察组自护能力总分显著高于对照组(均P<0.05)。观察组干预总有效率为93.5%。结论基于家庭监护系统的远程随访平台,在患者居家期间为其提供专业支持,可改善起搏器植入术后患者肩关节功能、自护能力,降低门诊随访次数,提高随访干预效果。 展开更多

无线、远程对产妇血流动力学的连续监测技术的临床应用研究

探究一种创新的、无创的、反射式光体积描记术为基础的设备对产妇分娩和分娩期间血流动力学的连续监测情况。选取81例足月低风险产妇参与本研究。给产妇佩戴新型反射式PPG设备,该设备每5 s自动采集心输出量(CO)、血压(BP)、每搏输出量(... 探究一种创新的、无创的、反射式光体积描记术为基础的设备对产妇分娩和分娩期间血流动力学的连续监测情况。选取81例足月低风险产妇参与本研究。给产妇佩戴新型反射式PPG设备,该设备每5 s自动采集心输出量(CO)、血压(BP)、每搏输出量(SV)、全身血管阻力(SVR)、心率(HR),并将所有数据上传到基于智能手机的应用程序和数据云。使用Power BI分析工具(Microsoft)对收集的数据进行大数据分析。比较胎膜破裂、硬膜外麻醉、胎儿分娩和胎盘排出前后的平均测量值。结果显示,硬膜外麻醉与CO轻微升高相关(P<0.05),原因是HR和SV均无显著增加。在SVR反向下降时,血压保持稳定(P<0.05)。胎儿分娩与CO峰值相关,之后CO迅速下降(P<0.05)。平均血压在整个分娩过程中保持稳定,胎儿分娩时略有增加(P<0.05),反映出CO增加和SVR降低(P<0.05)。胎盘排出与CO的第二个峰值和SVR的降低相关。因此,一种新型反射式PPG设备可用于对产妇在分娩和分娩期间的血流动力学监测中。并利用这些数据确定正常值和异常值,以便在分娩和分娩期间制定安全方案。 展开更多
