Peking Union Medical College(PUMC)launched the"4+4"Medical Doctor(MD)pilot program in 2018,admitting students with non-medical backgrounds from top universities,aligning with national medical talent training...Peking Union Medical College(PUMC)launched the"4+4"Medical Doctor(MD)pilot program in 2018,admitting students with non-medical backgrounds from top universities,aligning with national medical talent training policies to foster diverse and eager learners in medicine.On the occasion of the graduation of the first class of the"4+4"MD pilot class at PUMC in 2023,we reviewed the teaching reform in the pilot program and carried out a systematic survey and interviews with students,faculties,and management staff of the pilot class.This article reports on the measures taken by the pilot class at PUMC in enrollment and curriculum setting,and demonstrates the achievements of the pilot class in terms of student academic background structure,knowledge acquisition and skill learning,scientific research ability,and course evaluation.The results indicated that the pilot class had met the national demand for the"Medicine+X"talent training model.More specifically,with a diverse academic backgrounds,the pilot class graduates had academic levels comparable to the eight-year medical education graduates,and their scientific research abilities were satisfactory.The pilot program at PUMC will optimize the curriculum setting,strengthen the construction of faculty,learning resources,and teaching facilities,and reform the academic evaluation methods,thus deepening the reform of medical education and improving the"4+4"MD program as a novel medical education model.展开更多

Additionally results of CET-4, CET-6 and overseas internship from undergraduates of 2012–2015 grade, as well as the result of phased medical licensing examination and post-graduate entrance examination from undergraduates of 2012 were accordingly collected from the Teaching Management Department.Results: For the Chinese students in international classes, the average time of self-study was 161.49 min, 58.3% had preview before classes, and 90.7% had habit of review after classes. Thus the first time pass rate, total pass rate, first time excellent rate and total excellent rate of CET-4 and CET-6 of international classes were significantly higher than those of regular classes. The result of post-graduate entrance examination in 2016 showed that the score, pass rate and acceptance rate of international classes of 2012 grade were significantly higher those of regular classes(P < 0.01).Conclusions: Teaching-in-English reform in Hainan Medical University has achieved initial success. Chinese students from international classes are superior to those from regular classes in many aspects. However, there are still many problems, and effective measures should be implemented to promote teaching quality continuously.展开更多

We analyzed the length of stay (LOS) in a hospital f...Diabetes has become a growing concern in Japan, both medically and financially. The medical cost of diabetes was estimated at 1215 billion yen for fiscal year 2011. We analyzed the length of stay (LOS) in a hospital for type 2 diabetes mellitus patients who participated in educational programs to determine factors affecting LOS. Data on 991 patients obtained from 28 Red Cross hospitals in 2008 were used. For the analysis, we used the Box-Cox transformation model and Hausman test. The results revealed that patients aged 75 years and above and those with comorbidities and complications were prone to longer LOS. The analysis also revealed significant differences in LOS across the hospitals, even after controlling for patient characteristics. Finally, we applied the least squares method to determine the effects of hospital and regional factors and revealed that patients’ LOS was affected by the region’s average LOS. Regional corporations and networks appeared to be important in improving educational programs.展开更多
Objective:The study aimed to assess medication management compliance and mental health in elderly patients with hypertension.Method:The study evaluated medication compliance and mental health status of elderly hyperte...Objective:The study aimed to assess medication management compliance and mental health in elderly patients with hypertension.Method:The study evaluated medication compliance and mental health status of elderly hypertensive patients in China using simple random sampling.Data was collected using the Morisky Medication Compliance Questionnaire,Hospital Anxiety and Depression Scale,and a checklist.Ethical practices were strictly observed.Results:A study of 100 elderly hypertensive patients found poor drug management compliance,with female patients showing worse compliance.Female patients were more vulnerable to anxiety and depression.The study also found no significant association between gender,age,education level,marital status,living standards,and medication compliance.Barriers to medication management included food and daily necessities,lack of awareness about the importance of drug treatment,and basic family needs.The lowest-ranked barriers were lack of support from government health clinics,low income,and lack of family support.Conclusion:Based on the results,the study proposes an educational plan for elderly hypertensive patients and their families,to be evaluated and implemented by the hospital and township community service center.The plan aims to improve medication management and lifestyle modification compliance,encourage active participation,and provide access to medical and mental health clinics,support groups,and counseling services.展开更多

However, policies to control costs through the use of managed care and limiting resources are detrimental to academic medical centers and impede the advancement of medical sciTo survive the threats of managed care in the health care environment, acadendc medical centers must rely on their upper level managers to derive successful strategies. The methods used in this study include qualitative approaches in the form of key informants and case studies. In addition, a survey questionnaire was sent to 1h8 CEOs in all the academic medical centers in the U. S. The results are presented as frequency & percentage as appropriate at alpha level of P < 0.05;inferential statistics were generated. Paired t-test was used to perform the comparisons. <strong>Results:</strong> There was statistically significant difference in the knowledge level for the intensive care unit’s nurses regarding the intravenous medication administration during pre and post education program. Statistical analysis showed that there was a statistically significant between age, educational degree, critical units/wards, years of nursing experience and previous medication administration education program of the nurses and their knowledge during different phases of program intervention.<strong> Conclusion:</strong> Educational program on medication safety improves the knowledge of critical care nurses regarding intravenous medication errors. Within the context of developing a research presence in the Antarctic region, the first phase of the Chinese Polar Programs covered the period 2011-2015, which almost coincided with the 12th Five-Year Plan (2011-2015...Within the context of developing a research presence in the Antarctic region, the first phase of the Chinese Polar Programs covered the period 2011-2015, which almost coincided with the 12th Five-Year Plan (2011-2015). For the promotion of full understanding of the progress of Chinese expeditions and research in Antarctica, the observations and achievements of cruises during 2011-2015 are summarized in this paper. Four Antarctic cruises (28th-31st) were performed in the Prydz Bay and Antarctic Peninsula regions during the first phase of the Polar Programs. These cruises performed systemic collections of physical oceanographic and meteorological data to support further research on the ice-ocean-atmosphere interactions in Antarctica. Overall, 248 CTD/LADCP stations, 66 microstructure profiles, 507 XBT/XCTDs, 181 air sounding balloons, 58000 total gaseous mercury (TGM) concentrations, 452 aerosol samples, 294 atmospheric samples, 11 moorings, and 28 surface drifters were acquired or deployed during the four cruises. Using these extensive observations and other data, Chinese scientists have achieved new recognition in the fields of Southern Ocean physical oceanography and meteorology, as well as in other interdisciplinary subjects. These studies, which have been associated with scientific techniques, instrumentation, ocean circulation, water mass formation, energy transformation, and carbon uptake, have elucidated the dynamic mechanisms and potential effects of climate change in Antarctica. Finally, some observations based on experience gained during previous Chinese Antarctic Research and Expedition campaigns are summarized with advice for the improvement of future investigations in the Antarctic region.展开更多

文摘In order to fast transmission and processing of medical images and do not need to install client and plug-ins, the paper designed a kind of medical image reading system based on BS structure. This system improved the existing IWEB in the framework of PACS client image processing, medical image based on the service WEB completion port model. To realize the fast loading images with high concurrency, compared with the traditional WEB PACS, this system has the advantages of no client without plug-in installation, at the same time in the transmission and processing performance image has been greatly improved.

文摘Within the context of developing a research presence in the Antarctic region, the first phase of the Chinese Polar Programs covered the period 2011-2015, which almost coincided with the 12th Five-Year Plan (2011-2015). For the promotion of full understanding of the progress of Chinese expeditions and research in Antarctica, the observations and achievements of cruises during 2011-2015 are summarized in this paper. Four Antarctic cruises (28th-31st) were performed in the Prydz Bay and Antarctic Peninsula regions during the first phase of the Polar Programs. These cruises performed systemic collections of physical oceanographic and meteorological data to support further research on the ice-ocean-atmosphere interactions in Antarctica. Overall, 248 CTD/LADCP stations, 66 microstructure profiles, 507 XBT/XCTDs, 181 air sounding balloons, 58000 total gaseous mercury (TGM) concentrations, 452 aerosol samples, 294 atmospheric samples, 11 moorings, and 28 surface drifters were acquired or deployed during the four cruises. Using these extensive observations and other data, Chinese scientists have achieved new recognition in the fields of Southern Ocean physical oceanography and meteorology, as well as in other interdisciplinary subjects. These studies, which have been associated with scientific techniques, instrumentation, ocean circulation, water mass formation, energy transformation, and carbon uptake, have elucidated the dynamic mechanisms and potential effects of climate change in Antarctica. Finally, some observations based on experience gained during previous Chinese Antarctic Research and Expedition campaigns are summarized with advice for the improvement of future investigations in the Antarctic region.
文摘The Johnson and Johnson faulty hip implant case represents one of the most significant crises in medical device history,impacting nearly 93000 patients worldwide.In response to alarming failure rates and a global recall in August 2010,countries such as Australia,the United States,and the United Kingdom quickly implemented revision surgeries and reimbursement programs to protect patient safety.In stark contrast,India's response was alarmingly delayed;defective implants continued to be sold even after the global recall.By the time the import license was revoked,and the Central Drugs Standard Control Organization issued a recall notice,these implants had already been involved in 4700 surgeries across India.This paper explores the systemic weaknesses in India’s medical device regulatory framework that contributed to this delayed action,resulting in many patients suffering from serious health complications.It highlights deficiencies in monitoring and reporting mechanisms,inadequate regulatory oversight,and insufficient approval processes.Furthermore,the inability to trace affected patients and provide necessary compensation underscores significant gaps in regulation.Although subsequent legislative reforms were introduced,this paper argues that substantial loopholes remain,posing risks for future incidents.Thus,urgent,comprehensive,and enforceable regulatory measures are needed to increase patient safety.

文摘Graduate medical education(GME)has shifted its curricula from process-oriented approach to outcomes-oriented models.Program and faculty evaluation are methods by which educational curricula may adjust the teaching and learning environment to meet the needs and fills the gaps in GME.The measurement of educational outcomes is an essential for assessing teaching effectiveness in a shifting health care environment.In addition to trainee,program,and faculty evaluations,annual program review(APR)and evaluation and navigational changes made by the program education committee are essential to maintain effectiveness of an educational curriculum in a contemporary graduate medical training program.
