The locking compression plates (LCP) are efficient tools in open reduction and internal fixation (ORIF), especially in osteoporotic bones. Two important factors of screw density and screw position can affect the functionality of the bone plate. Several studies have assessed the influence of the screw configurations on the bone-plate stiffness, but the effects of screw positions on the interfragmentary strain, εIF of LCP construct have not been investigated yet. In this study, finite element method was used to investigate the influence of screws number and position on the interfragmentary strain of LCP-femur system for a mid-shaft fracture. Results of this study showed that by insertion of screws closer to the fracture site, εIF decreases by 2nd degree polynomial function versus screw position, but by adding the screws from the ends of the plate, or by moving and placing the screws towards the fracture site, the reduction of εIF will be linear. Results of this study were compared and are in agreement with some studies in the literature, even though their scope was mostly stability of the bone-implant system, whereas our scope was focused on the interfragmentary strain. 展开更多

Comparison of joint function and trauma severity after arthroscopic headless compression screw fixation and conventional open reduction and internal fixation treatment of patellar transverse fracture

Objective:To study the differences in joint function and trauma severity after arthroscopic headless compression screw fixation and conventional open reduction and internal fixation treatment of patellar transverse fracture.Methods: 58 patients with patellar transverse fracture who accepted operative treatment in our hospital between August 2012 and December 2015 were collected and divided into open surgery group (n=30) and arthroscopic surgery group (n=28) according to the surgical approach. Before operation and 3 months after operation, the knee joint function of two groups of patients was evaluated respectively;3 d after operation, enzyme-linked immunosorbent assay (ELISA) was used to detect serum pain mediators, stress hormones and other trauma-related indicators.Results:Differences in the knee joint function index levels were not statistically significant between two groups of patients before operation (P>0.05);3 months after operation, the maximal knee joint flexibility and maximal extension levels of arthroscopic surgery group were higher than those of open surgery group (P<0.05);3 d after operation, serum prostaglandin (PGI2), dopamine (DA), neuropeptide Y (NPY), substance P (SP), glucocorticoid (Cor), renin (R), angiotensin 1 (Ang1), angiotensin 2 (Ang2), epinephrine (E), norepinephrine (NE) and aldosterone (ALD) levels of arthroscopic surgery group were significantly lower than those of open surgery group (P<0.05).Conclusions:Arthroscopic headless compression screw fixation can optimize the knee joint function and reduce the surgical trauma in patients with patellar transverse fracture. 展开更多

Objective:To observe clinical effects of pedicle screw fixation combined with cable wires and bone graft and cannulated compression screws on adult multi-segment lumbar spondylolysis.Methods:70 cases of patients with multi-segment lumbar spondylolysis were selected in our hospital.According to different surgical schemes,these patients were divided into the observation group(35 cases)and the control group(35 cases).The observation group received pedicle screw fixation combined with cable wires and bone graft and the control group received cannulated compression screw fixation.Macnab criteria were adopted to implement a therapeutic evaluation of two groups of patients to make an observation and comparison of the excellent and good rate of surgery and a series of indicators including perioperative clinical effects,intraoperative blood loss,duration of surgery,hospital length of stay(HLOS),visual analogue scale(VAS),Oswestry disability index and Japanese Orthopaedic Association(JOA)score.Results:The excellent and good rate of the observation group was 97.14%,and that of the control group was 82.86%,the difference between two groups was statistically significant(χ^(2)=6.248,p=0.012).The differences in intraoperative blood loss,duration of surgery and HLOS between two groups were statistically significant(t=-4.55,t=-4.55,t=-4.55;p<0.05).Oswestry index,VAS score and JOA score of the observation group were(2.4±0.9),(28.5±6.4)and(27.1±3.1)respectively,and these of the control group were(3.5±1.2),(37.1±7.8)and(21.3±2.7)respectively,the differences between two groups were statistically significant(t=4.338,t=5.043,t=8.347,p<0.05).Conclusions:Pedicle screw combined with immobilized implantation bone by wirerope has an excellent clinical effect on the treatment of adult multi-segment lumbar spondylolysis,and it has a series of advantages such as fast postoperative recovery,small surgical trauma and so on.In addition,this technique can also restore the stability of spinal segments and relieve pains to a greater degree. 展开更多

The purpose of this study was to compare the biomechanical stability obtained by using our technique featured an anatomical plate and compression bolts versus that of the conventional anatomic plate and cancellous screws in the fixation of intraarticular calcaneal fractures.Eighteen fresh frozen lower limbs of cadavers were used to create a reproductive Sanders type-Ⅲ calcaneal fracture model by using osteotomy.The calcaneus fractures were randomly selected to be fixed either using our anatomical plate and compression bolts or conventional anatomic plate and cancellous screws.Reduction of fracture was evaluated through X radiographs.Each calcaneus was successively loaded at a frequency of 1 Hz for 1000 cycles through the talus using an increasing axial force 20 N to 200 N and 20 N to 700 N,representing the partial weight bearing and full weight bearing,respectively,and then the specimens were loaded to failure.Data extracted from the mechanical testing machine were recorded and used to test for difference in the results with the Wilcoxon signed rank test.No significant difference was found between our fixation technique and conventional technique in displacement during 20-200 N cyclic loading(P=0.06),while the anatomical plate and compression bolts showed a great lower irreversible deformation during 20-700 N cyclic loading(P=0.008).The load achieved at loss of fixation of the constructs for the two groups had significant difference:anatomic plate and compression bolts at 3839.6±152.4 N and anatomic plate and cancellous screws at 3087.3±58.9 N(P=0.008).There was no significant difference between the ultimate displacements.Our technique featured anatomical plate and compression bolts for calcaneus fracture fixation was demonstrated to provide biomechanical stability as good as or better than the conventional anatomic plate and cancellous screws under the axial loading.The study supports the mechanical viability of using our plate and compression bolts for the fixation of calcaneal fracture. 展开更多

Objective:To investigate the effect of vertebroplasty combined with pedicle screw fixation in the treatment of senile osteoporotic vertebral compression fractures.Methods:Eighty-one patients with senile osteoporotic vertebral compression fractures were enrolled in our hospital from January 2015 to January 2019.They were randomly divided into a single group(40 cases)and a combined group(41 cases).),a single group was treated with pedicle screw internal fixation,and the combined group was treated with vertebroplasty.The recovery,pain and dysfunction index of the injured vertebrae before and after operation were compared between the two groups.The serum neurological function related indexes before and after operation were compared and the incidence of postoperative adverse events were recorded.Results:There was no significant difference in the recovery of the injured vertebrae between the two groups(P>0.05).The compression ratio,spinal stenosis rate and Cobb angle of the combined group were significantly lower than the single group(P<0.05).On the 3rd postoperative day,there were no significant differences between the two groups in Visual Analogue Scale/Score(VAS)and Oswestry Dability Index(ODI)scores(P>0.05).The VAS and ODI scores of the group were significantly lower than those of the single group(P<0.05).On the 3rd day after surgery,the neuron-specific enolase(NSE)and brain derived neurotrophic factor(brain-derived neurotrophic factor)were used.The levels of BDNF,S100βand Nerve growth factor(NGF)were not significantly different(P>0.05).At 3 months after operation,the level of BDNF in the combined group was significantly higher than that in the single group.The levels of NSE,S100βand NGF were significantly lower than that of the single group.The group(P<0.05);the incidence of adverse events in the combined group was significantly lower than that in the single group(P<0.05).Conclusions:Vertebroplasty combined with pedicle screw fixation for the treatment of senile osteoporotic vertebral compression fracture can effectively improve the recovery of postoperative vertebral body structure,postoperative pain and dysfunction in Jianing patients,improve postoperative neurological function To reduce the incidence of adverse events,with clinical promotion significance. 展开更多

Objectives To study the effects of applying locking compression plates in the treatment of patients with limb fractures on postoperative fracture healing.Methods:115 patients with limb fractures who were treated in our hospital from November 2019 to November 2020 were selected.In order to study the effective treatment method,the random-number table method was used in this study to divide the patients into two groups,namely the experimental group and the control group,and the locking compression plate treatment method and the pure plate and screw internal fixation treatment method were administered respectively to study their clinical application effects.Results:Compared with the control group,patients in the experimental group had a lower incidence of complications,shorter hospitalizations and shorter recovery time.Meanwhile,the experimental group had a better quality of recovery,and all data were significantly different from those of the control group,P<0.05,the intervention effect of the experimental group was better.Conclusion:The application of the locking compression plate in the treatment of patients with limb fractures is more conducive to promoting the postoperative healing of the patients'fractures,reducing the incidence of postoperative complications,and promoting the rapid recovery of patients,which has positive significance for clinical development. 展开更多

目的比较空心加压螺钉(CCS)和股骨颈动力交叉钉系统(FNS)固定治疗股骨颈骨折的疗效。方法将42例股骨颈骨折患者按照内固定方式不同分为FNS组(采用FNS固定治疗,15例)和CCS组(采用CCS固定治疗,27例)。比较两组切口长度、术中出血量、术中... 目的比较空心加压螺钉(CCS)和股骨颈动力交叉钉系统(FNS)固定治疗股骨颈骨折的疗效。方法将42例股骨颈骨折患者按照内固定方式不同分为FNS组(采用FNS固定治疗,15例)和CCS组(采用CCS固定治疗,27例)。比较两组切口长度、术中出血量、术中透视次数、手术时间、术后住院时间、并发症发生情况、下地时间、骨折愈合时间、步态情况。结果患者均获得随访,时间12~14个月。切口长度CCS组短于FNS组(P<0.05);术中透视次数、手术时间FNS组少(短)于CCS组(P<0.05);术中出血量、术后住院时间两组比较差异均无统计学意义(P>0.05)。两组均无切口感染、骨折不愈合等并发症发生;术后约3个月CCS组2例出现CCS松动及退钉现象,经减少活动后未影响骨折愈合。下地部分负重时间、骨折愈合时间FNS组均短于CCS组(P<0.05)。末次随访时两组患者均可部分负重行走。结论与CCS固定相比,FNS固定治疗股骨颈骨折术中透视次数少、手术时间短,且固定稳定、牢靠,在“早活动、晚负重”原则下利于患肢功能锻炼,更有助于促进骨折愈合。 展开更多

背景:舟状骨骨折是临床上常见的腕部骨折,主要采用经皮螺钉治疗,首先闭合复位骨折端,然后将空心加压螺钉精准置于舟状骨轴心并加压固定骨折端,能够促进骨折愈合,使腕关节获得良好功能。目的:评价一种新型克氏针导向器辅助经皮空心加压... 背景:舟状骨骨折是临床上常见的腕部骨折,主要采用经皮螺钉治疗,首先闭合复位骨折端,然后将空心加压螺钉精准置于舟状骨轴心并加压固定骨折端,能够促进骨折愈合,使腕关节获得良好功能。目的:评价一种新型克氏针导向器辅助经皮空心加压螺钉治疗舟状骨骨折的临床疗效。方法:回顾性分析2015年1月至2020年12月河北医科大学第三医院手外科收治的15例舟状骨骨折患者的临床资料。所有患者均采用新型导向器辅助经皮空心加压螺钉固定治疗。统计患者的骨折愈合时间、手术时间、透视次数、重返工作的时间及并发症。术后12个月采用改良Mayo腕关节评分对腕关节功能进行评估,测量患侧及健侧腕关节掌屈、背伸、尺偏、桡偏活动度及握力。结果与结论:所有患者均完成了12个月随访。术后X射线片示:空心压力螺钉位于舟状骨轴心位置。所有舟状骨骨折均获骨性愈合,平均愈合时间10.0周。手术平均历时55.7 min,术中平均透视次数10.9次,平均10.3周重返工作岗位。腕关节功能:优9例,良5例,可1例,优良率93.3%。患侧及健侧的腕关节掌屈、背伸、尺偏、桡偏活动度及握力相比较,差异无显著性意义(P>0.05)。所有患者均未发生伤口感染、骨折畸形愈合、螺钉松动及断裂等并发症。结果表明,应用新型导向器可以缩短手术时间,减少术中透视次数,提高螺钉精准度,缩短重返工作时间,腕关节功能恢复良好。该导向器使空心加压螺钉治疗舟状骨骨折手术更容易。 展开更多

为了实现空压机设备的能效提升,开发了1台额定功率为250 kW的双级喷油螺杆空压机,空压机的低、高压级均采用新型螺杆转子型线,压缩机在结构上采用了一系列创新设计,对级间压力、喷油量等关键运行参数进行了优化,并通过试验测试对所开发... 为了实现空压机设备的能效提升,开发了1台额定功率为250 kW的双级喷油螺杆空压机,空压机的低、高压级均采用新型螺杆转子型线,压缩机在结构上采用了一系列创新设计,对级间压力、喷油量等关键运行参数进行了优化,并通过试验测试对所开发双级喷油螺杆空压机样机的能效进行了评估。结果表明,在最佳转速为1330 r/min时,容积效率可达到95.6%,低压级和高压级压缩机的绝热效率则分别达到93.3%,88.6%;依据GB 19153—2019对样机能效进行评估,在100%,70%和40%负载下,样机均可达到定转速喷油回转空压机一级能效,考虑到空压机在额定负载与部分负载时的综合性能,其机组比功率可达到5.72 kW/(m^(3)·min),相较于一级能效限定值低了7.7%,达到了“双一级能效”,实现了同类螺杆空压机产品的能效升级。此外,通过试验研究了转速变化对双级喷油螺杆空压机低、高压级p-V指示曲线和效率的影响规律。经研究发现,低压级压缩过程p-V曲线低于等熵压缩过程,转速变化对其影响较小,而高压级压缩过程p-V曲线受气体泄漏影响显著,转速升高会导致高压级压缩过程曲线逐渐变缓;低压级和高压级压缩机效率随转速变化呈现出不同的变化规律,且高压级压缩机的容积效率和绝热效率均对转速变化非常敏感。研究结果可为螺杆空压机的能效升级提供参考价值。 展开更多

背景:Lisfranc韧带是维持足部横弓与纵弓的重要结构,Lisfranc损伤为一种严重的中足损伤。Lisfranc韧带损伤是复杂的,其治疗以及首选的固定方法有争议。目的:比较钢板联合Suturetape与钢板联合无头加压螺钉治疗第1,2跖骨近端粉碎性骨折Li... 背景:Lisfranc韧带是维持足部横弓与纵弓的重要结构,Lisfranc损伤为一种严重的中足损伤。Lisfranc韧带损伤是复杂的,其治疗以及首选的固定方法有争议。目的:比较钢板联合Suturetape与钢板联合无头加压螺钉治疗第1,2跖骨近端粉碎性骨折Lisfranc损伤的短期疗效。方法:回顾性分析东莞市中医院骨七科2019年1月至2022年6月因第1,2跖骨近端粉碎性骨折Lisfranc损伤患者48例,其中25例采用钢板联合Suture tape固定(观察组),23例采用钢板联合无头加压螺钉固定(对照组)。术前参照影像学资料,根据Myerson分类系统进行分型;术后随访根据骨折愈合时间、目测类比评分、美国足踝外科学会标准评估足功能恢复情况,并对两组术后并发症进行比较分析。结果与结论:①两组患者均顺利完成手术并获得随访,随访时间12-36个月,平均(18.0±5.42)个月;②两组患者手术时间、术中出血量比较差异均无显著性意义(P>0.05);③观察组较对照组骨折愈合时间稍长,差异有显著性意义(P<0.05);④术后3,6,12个月随访,观察组较对照组目测类比评分明显降低(P<0.05);⑤术后6,12个月,观察组术后各时间点随访足功能AOFAS评分较螺钉组明显改善(P<0.05),且均较术前显著提高(P<0.05);⑥术后观察组出现1例创伤性关节炎,对照组出现1例切口感染、1例螺钉断裂及2例创伤性关节炎,两组比较差异无显著性意义(P>0.05),考虑与样本量偏少有关;⑦提示对于第1,2跖骨近端粉碎性骨折Lisfranc损伤,实施钢板联合Suture tape内固定治疗Lisfranc关节损伤效果可靠,可改善患者足关节功能,且具有手术创伤小、术后并发症少、远期医源性创伤性关节炎风险低的优势,较无头加压钉更有利于足功能的恢复。 展开更多

目的比较股骨颈动力交叉钉系统(FNS)与4枚空心加压螺钉(FCCS)内固定治疗PauwelsⅢ型股骨颈骨折的疗效。方法将52例PauwelsⅢ型股骨颈骨折患者根据内固定方式不同分为FNS组(采用FNS内固定治疗,25例)和FCCS组(采用FCCS内固定治疗,27例)。... 目的比较股骨颈动力交叉钉系统(FNS)与4枚空心加压螺钉(FCCS)内固定治疗PauwelsⅢ型股骨颈骨折的疗效。方法将52例PauwelsⅢ型股骨颈骨折患者根据内固定方式不同分为FNS组(采用FNS内固定治疗,25例)和FCCS组(采用FCCS内固定治疗,27例)。记录两组手术情况、骨折复位及愈合情况、并发症发生情况,采用Harris评分标准评价髋关节功能。结果患者均获得随访,时间10~24个月。手术时间、术中透视次数FNS组短(少)于FCCS组(P<0.05),切口长度、术中出血量FNS组长(多)于FCCS组(P<0.05),住院时间两组比较差异无统计学意义(P>0.05)。术后采用Garden指数评价骨折复位质量,两组比较差异无统计学意义(P>0.05)。骨折愈合时间两组比较差异无统计学意义(P>0.05)。下肢静脉血栓、股骨头缺血性坏死发生率两组比较差异均无统计学意义(P>0.05),大腿外侧激惹症状发生率FCCS组高于FNS组(P<0.05)。末次随访时Harris评分优良率FNS组高于FCCS组(P<0.05)。结论FNS和FCCS内固定治疗PauwelsⅢ型股骨颈骨折均可获得较好的临床疗效,FNS内固定整体结构稳定性较高,操作更简便,手术时间更短,术中透视次数更少,术后大腿外侧激惹症状发生率低,早期疗效更好。 展开更多

目的研究加压空心螺钉及形状记忆合金骑缝钉在三关节融合术内固定中的生物力学差异,为形状记忆合金骑缝钉的临床应用提供参考。方法选择1例重度僵硬型马蹄足患者的足部二维CT数据,通过Mimics和Geomagic软件构建三关节融合术后模型,并利... 目的研究加压空心螺钉及形状记忆合金骑缝钉在三关节融合术内固定中的生物力学差异,为形状记忆合金骑缝钉的临床应用提供参考。方法选择1例重度僵硬型马蹄足患者的足部二维CT数据,通过Mimics和Geomagic软件构建三关节融合术后模型,并利用SolidWorks 2021软件建立三关节融合内固定几何模型;根据使用固定钉类型和组合方式(加压空心螺钉和形状记忆合金骑缝钉)分为A、B、C、D共4种固定方案;在ABAQUS软件中分析模拟中立位生理载荷下不同内固定方案模型的生物力学特性。结果方案D内固定模型距下、距舟、跟骰关节融合面端面最大位移均大于方案A、B、C;方案D内固定模型距舟关节、跟骰关节融合面内外侧位移之差分别为13.10%、13.60%,与其他3种固定方案相比融合面位移更接近于平行位移;方案D内固定模型距下、距舟、跟骰关节融合面最大von Mises应力均大于方案A、B、C。结论应用方案D(距舟关节和跟骰关节融合面使用骑缝钉内固定,距下关节融合面使用加压空心螺钉内固定)能够使三关节融合术后内固定融合面具有稳定性,并为其提供近似平行微动,产生适当的融合面应力使融合端面接触更紧密,促进骨痂生长,获得更佳的融合效果。 展开更多

目的探讨经皮空心钉联合改良滑动加压动力髋螺钉内固定治疗股骨颈骨折的疗效。方法将35例股骨颈骨折患者按内固定方式不同分为传统组(采用经皮空心钉内固定治疗,22例)和改良组(采用经皮空心钉联合改良滑动加压动力髋螺钉内固定治疗,13例... 目的探讨经皮空心钉联合改良滑动加压动力髋螺钉内固定治疗股骨颈骨折的疗效。方法将35例股骨颈骨折患者按内固定方式不同分为传统组(采用经皮空心钉内固定治疗,22例)和改良组(采用经皮空心钉联合改良滑动加压动力髋螺钉内固定治疗,13例)。比较两组手术时间、术后下地时间、住院时间、骨折愈合时间、并发症发生情况。采用Harris评分评价髋关节功能。结果患者均获得随访,时间6~36(19.57±9.45)个月。手术时间、住院时间两组比较差异均无统计学意义(P>0.05);术后下地时间、骨折愈合时间改良组均短于传统组(P<0.05)。两组均无骨折不愈合及股骨头坏死情况发生;股骨颈短缩率改良组低于传统组(P<0.05)。末次随访时Harris评分优良率改良组高于传统组(P<0.05)。结论改良滑动加压动力髋螺钉符合股骨颈解剖和生物力学特点,与经皮空心钉联合内固定治疗股骨颈骨折,具有患者术后下地时间早、骨折愈合时间短、股骨颈短缩率低、髋关节功能恢复好的优势。 展开更多
