Constraint-induced movement therapy is an upper limb rehabilitation technique used in stroke patients with hemiplegia; however, studies of lower extremity rehabilitation are scarce. In this study, stroke patients with lower limb hemiplegia underwent conventional Bobath therapy for 4 weeks as baseline treatment, followed by constraint-induced movement therapy for an additional 4 weeks. The 10-m maximum walking speed and Berg balance scale scores significantly improved following treatment, and lower extremity motor function also improved. The results of functional MRI showed that constraint-induced movement therapy alleviates the reduction in cerebral functional activation in patients, which indicates activation of functional brain regions and a significant increase in cerebral blood perfusJon. These results demonstrate that constraint-induced movement therapy promotes brain functional reorganization in stroke patients with lower limb hemiplegia.展开更多

In the present study, the fluctuation function method and weighted curve fitting method were presented to make the calculation more simple and accurate. The vertical scales of fluctuation of typical layers of Tianjin Port were calculated based on a number of engineering geotechnical investigation data, which can be guidance to other projects in this area. Meanwhile, the influences of sample interval and type of soil index on the scale of fluctuation were analyzed, according to which, the principle of determining the scale of fluctuation when the sample interval changes was defined. It can be obtained that the scale of fluctuation is the basic attribute reflecting spatial variability of soil, therefore, the scales of fluctuation calculated according to different soil indexes should be basically the same. The non-correlation distance method was improved, and the principle of determining the variance reduction function was also discussed.展开更多

The shell studied here consists of three layers, of which the outer and inner skins are made of solid metal, while the core is FG porous metal foam. Partial differential equations are derived by utilizing the improved Donnell's nonlinear shell theory and Hamilton's principle. Afterwards, the Galerkin method is used to transform the governing equations into nonlinear ordinary differential equations, and an approximate analytical solution is obtained by using the multiple scales method. The effects of various system parameters, specifically, the radial load, core thickness, foam type, foam coefficient, structure damping, and Winkler-Pasternak foundation parameters on nonlinear internal resonance of the sandwich FG porous thin shells are evaluated.展开更多

Decompressive Hemicraniectomy has been utilized to treat brain swelling and mass effect secondary to these infarctions in an attempt to improve functional outcome. <strong>Aim: </strong>To evaluate the functional outcome of decompressive hemicraniectomy in management of malignant MCA infarctions. <strong>Methods: </strong>The study included 30 patients with malignant MCA infarctions operated upon by decompressive hemicraniectomy and duroplasty with pericranium or fascia lata graft in the period from June 2016 to January 2019. Pre-operative neurological condition, associated morbidity, location and extent of the infarction were assessed. Surgery was performed within 48 hours of the onset of stroke or 12 hours within deterioration of conscious level. Pre-operative CT scan as well as sequential post-operative CT was done. Functional outcome was assessed by the modified Rankin Scale (mRS) at the time of discharge and 3 months following surgery. <strong>Results: </strong>The study included 18 males and 12 females with a mean age of 54.7 years. The pre-operative GCS was <8 (5 - 7) in 11 patients and 8 or higher (8 - 13) in 19 patients. Good functional outcome (mRS 0 - 3) was achieved in 13 (43.3%) cases while poor outcome (mRS 4 - 5) occurred in 8 (26.7%) cases and mortality (mRS 6) occurred in 9 (30%) cases. <strong>Conclusion:</strong> Decompressive hemicraniectomy improves functional outcome in cases of malignant MCA infarction. Pre-operative GCS, age, volume of infarction, degree of midline shift, timing of surgery and associated morbidity are the most important factors affecting the outcome.展开更多

Gu’s Function shows that the trace metal ion concentration in water vapor has positive correlation to"specific electron affinity constant"last ionization potential Iz/ion valence Z and negative correlation to ion volume V and coordination number N(Gu, 1994). Gu’s Func-tion C=f[(Iz/Z)/VN]of the bond parameter in water vapor chemistry corresponds to the potential energy function Z’ e2/r in the Schrodinger Equation of quantum chemistry. In different ions with the same 2+ charge, the ion concentration of water-water vapor transfer may be much different. This shows that the 2+ charge of different ions has different attractive force (hydration force). This different attractive force of the charge can be scaled with the relative energy or charge size from Gu’s Function.展开更多

Methods: 897 adults over the age of 70 free of amnestic mild cognitive impairment and dementia and enrolled in the Einstein Aging Study made up the study sample. The PSS-14 was used to measure stress. Three cognitive domains (language, episodic memory, and frontal-executive) had previously been found using principle component analysis. Linear regression analyses were used to determine the relationship between the PSS subscales and cognitive domain function. Results: The study sample had a mean age of 79.1 years and 62.8% were female. Bivariate correlations show that the PSS-14 positively worded subscale of the PSS (PSS-PW) was significantly associated with all three cognitive domains (language: r = -0.15, p < 0.001;episodic memory: r = -0.16, p < 0.001;frontal-executive: r = -0.21, p <0.001) while the negatively worded subscale of the PSS (PSS-NW) was not significantly associated with any cognitive domain. In linear regression analyses adjusted for age, white race, gender, years of education, and depressive symptoms, the PSS-PW remained significantly associated with each of the cognitive domains. The PSS-NW was not associated with any cognitive domains in any model. The PSS-14 was significantly associated with language and episodic memory, but not the frontal-executive domain. Conclusion: Worse PSS-PW scores are associated with reduced cognitive function in the executive, memory, and language domains in nondemented older adults. The PSS-PW subscale correlated better with cognitive function than the overall PSS-14. Future research should evaluate the temporality of the association and if stress reduction therapies improve cognitive performance.展开更多

文摘Characterizing trait variation across different ecological scales in plant communities has been viewed as a way to gain insights into the mechanisms driving species coexistence. However, little is known about how changes in intraspecific and interspecific traits across sites influence species richness and community assembly, especially in understory herbaceous communities. Here we partitioned the variance of four functional traits(maximum height, leaf thickness, leaf area and specific leaf area)across four nested biological scales:individual, species, plot, and elevation to quantify the scale-dependent distributions of understory herbaceous trait variance. We also integrated the comparison of the trait variance ratios to null models to investigate the effects of different ecological processes on community assembly and functional diversity along a 1200-m elevational gradient in Yulong Mountain. We found interspecific trait variation was the main trait variation component for leaf traits, although intraspecific trait variation ranged from 10% to 28% of total variation. In particular, maximum height exhibited high plasticity, and intraspecific variation accounted for 44% of the total variation.Despite the fact that species composition varied across elevation and species richness decreased dramatically along the elevational gradient, there was little variance at our largest(elevation)scale in leaf traits and functional diversity remained constant along the elevational gradient, indicating that traits responded to smaller scale influences.External filtering was only observed at high elevations. However, strong internal filtering was detected along the entire elevational gradient in understory herbaceous communities, possibly due to competition. Our results provide evidence that species coexistence in understory herbaceous communities might be structured by differential niche-assembled processes. This approach ee integrating different biological scales of trait variation ee may provide a better understanding of the mechanisms involved in the structure of communities.
文摘BACKGROUND Functional constipation(FC)is a common functional gastrointestinal disease with various clinical manifestations. It is a physical and mental disease, which seriously affects patient physical and mental health and quality of life.Biofeedback therapy is the treatment of choice for FC,especially outlet obstructive constipation caused by pelvic floor dysfunction.High-quality nursing is a new nursing model in modern clinical work and a new concept of modern nursing service.AIM To explore the effect of biofeedback combined with high-quality nursing in the treatment of FC.METHODS A total of 100 patients with FC admitted to our hospital from March 2015 to July 2019 were selected for clinical observation. These patients were randomly divided into two groups of 50:Experimental group(biofeedback combined with highquality nursing treatment group)and control group(biofeedback group).RESULTS The constipation symptom score of the experimental group was significantly lower than that of the control group, and the difference was statistically significant(P<0.05).The anal canal resting pressure and initial defecation threshold of the experimental group were significantly lower than those of the control group, and the maximum squeeze systolic pressure of the anal canal of the experimental group was significantly higher than that of the control group(P<0.05).The Self-Rating Anxiety Scale and Zung’s Self-Rating Depression Scale scores of the two groups were significantly lower than before treatment. The Self-Rating Anxiety Scale and Self-Rating Depression Scale scores of the experimental group were significantly lower than those of the control group(P<0.05).The patient satisfaction score of the experimental group was significantly higher than that of the control group(P<0.05).CONCLUSION The application of biofeedback combined with high-quality nursing in the treatment of FC has significant advantages over pure biofeedback treatment, and it is worthy of promotion in clinical work.

文摘Reservoirs are an important water source in many densely populated areas in southwest China.Phytoplankton play an essential role in maintaining the structure and function of reservoir ecosystems.Understanding the succession in phytoplankton communities and the factors driving it are essential for eff ective water quality management in drinking water reservoirs. In this study, water samples were collected monthly at the surface layers from March 2016 to December 2019 in Hongfeng Reservoir, southwest China. The relationship between functional group succession was analyzed based on nonmetric multidimensional scaling analysis(NMDS),redundancy analysis(RDA),succession rate, and other analysis methods. The results showed distinct shifts in the community structure of phytoplankton functional groups within study period. The Cyclotella sp.was dominant in 2016 and 2017,and Pseudanabaena limnetica was the dominant group in 2018 and 2019.It appears that the phytoplankton composition and biomass are closely related to the water temperature and nutrient status in this reservoir. The results clearly showed that the permanganate index(COD_(Mn))was the key factor of dramatic phytoplankton functional group succession, and the change in succession rates was closely caused by total nitrogen concentration(TN).Therefore, the succession pattern and key factors of Hongfeng Reservoir revealed in this study were important guidance for the management of drinking water reservoirs in southwest China.A reasonable limit on exogenous nutrient input should be a priority, especially in high water temperature period.

文摘The scale of fluctuation is one of the vital parameters for the application of random field theory to the reliability analysis of geotechnical engineering. The non-correlation distance method was improved, and the principle of determining the variance reduction function was also discussed.

文摘In this article, the nonlinear dynamic responses of sandwich functionally graded(FG) porous cylindrical shell embedded in elastic media are investigated. The effects of various system parameters, specifically, the radial load, core thickness, foam type, foam coefficient, structure damping, and Winkler-Pasternak foundation parameters on nonlinear internal resonance of the sandwich FG porous thin shells are evaluated.
文摘<strong>Introduction:</strong> Malignant middle cerebral artery (MCA) infarction is a devastating entity that is associated with up to 80% mortality. Pre-operative GCS, age, volume of infarction, degree of midline shift, timing of surgery and associated morbidity are the most important factors affecting the outcome.
文摘This article reviews the constructs of stress, appraisal, coping, according to a transactional perspective, and executive function, and presents a stress control rating scale (ECOSTRESS), which design is founded in these constructs. This psychometric tool is useful in the assessment of cognitive control of stress, correlated with the function of dorsolateral prefrontal cortex. It has been validated for its use in the assessment of Portuguese people in situations of stress related to unemployment and economic insufficiency. Also, within the context of the cognitive control of stress, it is highlighted the usefulness of low resolution brain electromagnetic tomography (LORETA).
文摘We derive the conditions for the existence of the unique solution of the two scale integral equation and the form of the solution, according to the method of the construction of the dyadic scale function. We give the construction of the dyadic wavelet and its necessary and sufficient condition. As an application, we also develop a pyramid algorithm of the dyadic wavelet decomposition.
文摘Water Vapor Chemistry" as a new science was discovered and established from trace analysis in Gu’s laboratory(Gu, 1991; Gu et al., 1991). This different attractive force of the charge can be scaled with the relative energy or charge size from Gu’s Function.
文摘In this paper, we study the relationship between exponential dichotomy and quadratic Lyapunov function for the linear equation x△=A(t)x on time scales. Moreover, for the nonlinear perturbed equation x△= A(t)x + f(t, x) we give the instability of the zero solution when f is sufficiently small.
文摘Objectives: To examine the cross-sectional associations of the separate subscales of the Perceived Stress Scale (PSS) and tests measuring cognitive domains in older adults. Future research should evaluate the temporality of the association and if stress reduction therapies improve cognitive performance.
