儿童抽动障碍与链球菌感染关系的Meta分析 被引量:26

目的评估儿童发生抽动障碍(TD)与A组β-溶血性链球菌(GABHS)感染的关系。方法检索CNKI、万方、维普数据库以及Pubmed、Cochrane Library、Science Direct等数据库中关于儿童抽动障碍与GABHS感染证据的队列研究和病例对照研究,采用Review... 目的评估儿童发生抽动障碍(TD)与A组β-溶血性链球菌(GABHS)感染的关系。方法检索CNKI、万方、维普数据库以及Pubmed、Cochrane Library、Science Direct等数据库中关于儿童抽动障碍与GABHS感染证据的队列研究和病例对照研究,采用Review Manager 5. 1统计学软件,通过Meta分析方法探讨二者之间的关系。结果纳入TD与链球菌感染关系的研究共16篇文献,全部为病例对照研究,Meta分析结果显示,TD与GABHS感染相关,随机效应模型合并OR值为3. 03(95%CI:2. 03~4. 53),I^2=67%,敏感度分析,再次汇总OR值为2. 85(95%CI:2. 15~3. 78),I^2=33%。亚组分析显示合并OR值分别为:亚洲人群组2. 63(95%CI:1. 85~3. 76),欧洲人群组3. 56(95%CI:1. 77~7. 16),TD混合组3. 60(95%CI:2. 08~6. 24),单纯TS组2. 56(95%CI:1. 11~5. 91)。结论链球菌感染与儿童抽动障碍关系密切,GABHS感染会增加抽动障碍发生的风险。 展开更多

目的:研究儿童A组β溶血性链球菌(GABHS)感染的情况,对快速GABHS抗原检测方法进行评价。方法:随机选择呼吸道感染患儿1485例,对其进行快速GABHS抗原检测,对部分阳性者进行青霉素类药物治疗1周后复查,并与抗链O及咽培养比较。结果:1485... 目的:研究儿童A组β溶血性链球菌(GABHS)感染的情况,对快速GABHS抗原检测方法进行评价。方法:随机选择呼吸道感染患儿1485例,对其进行快速GABHS抗原检测,对部分阳性者进行青霉素类药物治疗1周后复查,并与抗链O及咽培养比较。结果:1485例呼吸道感染患儿中GABHS阳性率为23.2%(345例),主要为上呼吸道感染,下呼吸道感染则较少见。咽炎合并急性肾炎患者GABHS阳性率高达80%,与普通咽炎(21.6%)比较,P<0.01,有统计学意义,治疗1周后,复查抗链O阳性率改变较前无显著差异,GABHS抗原检测阳性率与咽培养无显著差异(P>0.05),抗原检测转阴率与临床症状转阴率无显著差异。结论:GABHS感染是小儿呼吸道感染疾病常见病因,常引起肾炎及风湿热等,快速GABHS抗原检测可作为其感染的诊断方法及疾病恢复的快速检测指标之一,对判断疾病转归很有帮助。 展开更多

Pediatric autoimmune neuropsychiatric disorders associated with group A streptococcal infections (PANDAS) is a concept that is used to characterize a subset of children with neuropsychiatric symptoms, tic disorders, or obsessive-compulsive disorder (OCD), whose symptoms are exacerbated by group A streptococcal (GAS) infection. PANDAS has been known to cause a sudden onset of reward deficiency syndrome (RDS). RDS includes multiple disorders that are characterized by dopaminergic signaling dysfunction in the brain reward cascade (BRC), which may result in addiction, depression, avoidant behaviors, anxiety, tic disorders, and/or OCD. According to research by Blum et al., the dopamine receptor D2 (DRD2) gene polymorphisms are important prevalent genetic determinants of RDS. The literature demonstrates that infections like Borrelia and Lyme, as well as other infections like group A beta-hemolytic streptococcal (GABHS), can cause an autoimmune reaction and associated antibodies target dopaminergic loci in the mesolimbic region of the brain, which interferes with brain function and potentially causes RDS-like symptoms/behaviors. The treatment of PANDAS remains controversial, especially since there have been limited efficacy studies to date. We propose an innovative potential treatment for PANDAS based on previous clinical trials using a pro-dopamine regulator known as KB220 variants. Our ongoing research suggests that achieving “dopamine homeostasis” by precision-guided DNA testing and pro-dopamine modulation could result in improved therapeutic outcomes. 展开更多

The Value of High Antistreptolysin O Titre as an Indicator of Tonsillectomy in Upper Egypt

Background: In this study, we aimed to evaluate the benefit of performing of tonsillectomy in patients with raised serum ASO titre only in absence or presence of group A beta heamolytic streptococci (GABHS) in throat swab. Materials and methods: In this prospective cohort study, 196 patients below the age of 14 were suffering from non-specific streptococcal infections, without fulfilling the clinical parameters used for the diagnosis of chronic tonsillitis 156 patients had a raised ASO titre above 200 iu/ml. Throat swab culture was performed in all patients. Results: The results showed that out of the 156 patients, 52 had positive throat swab for GABHS. All of the patients underwent tonsillectomy. Follow-up of the patients for one year after the operation, 88% of the first group with high ASO titre and positive throat swab showed improvement of symptoms;25% of the second group with high ASO titre and negative throat swab showed improvement of symptoms. Conclusion: Our study shows that isolation of GABHS from the patients tonsils by throat swab along with high ASO titre may be an indication of tonsillectomy in absence of any other indications. 展开更多
