AIM:To compare the surgical outcomes of glaucoma drainage device implantation(GDI)and trans-scleral neodymium:YAG cyclophotocoagulation(CPC)in the management of refractory glaucoma after Descemetstripping automated en...AIM:To compare the surgical outcomes of glaucoma drainage device implantation(GDI)and trans-scleral neodymium:YAG cyclophotocoagulation(CPC)in the management of refractory glaucoma after Descemetstripping automated endothelial keratoplasty(DSAEK).METHODS:This retrospective study on observational case series enrolled 29 patients who underwent DSAEK and posterior anti-glaucoma surgery(15 with GDI and 14 with CPC).The main outcome measures were intraocular pressure(IOP),glaucoma surgery success rate(defined as IOP of 6–21 mm Hg without additional anti-glaucoma operation),number of glaucoma medications,endothelial graft status,and best-corrected visual acuity(BCVA).RESULTS:The mean follow-up time was 34.1 and 21.0mo for DSAEK or glaucoma surgeries,both for the GDI and CPC groups.Both groups showed significant IOP reduction after glaucoma surgery.The GDI group presented a significantly higher success rate in IOP control than the CPC group(60%vs 21.4%,P=0.03).Both procedures significantly decreased the number of glaucoma medications(P=0.03).Forty percent and 57%of cases in the GDI and the CPC group,respectively,experienced endothelial graft failure during follow-up(P=0.36).Significantly worse BCVA after surgery was observed in the CPC group but not in the GDI group.CONCLUSION:Both GDI and CPC significantly decrease IOP in eyes with glaucoma after DSAEK.GDI is preferable to CPC in refractory glaucoma cases after DSAEK,as it manifests a significantly higher success rate for IOP control,similar endothelial graft failure rate,and relatively preserves BCVA than CPC.展开更多
AIMTo evaluate outcomes of combined pars plana vitrectomy and Baerveldt glaucoma implant (PPV-BGI) placement for refractory glaucoma.METHODSThe medical records of 92 eyes (89 patients) that underwent PPV-BGI were retr...AIMTo evaluate outcomes of combined pars plana vitrectomy and Baerveldt glaucoma implant (PPV-BGI) placement for refractory glaucoma.METHODSThe medical records of 92 eyes (89 patients) that underwent PPV-BGI were retrospectively reviewed, including 43 eyes with neovascular glaucoma (NVG) and 49 eyes with other types of glaucoma (non-NVG).RESULTSOutcome measures were visual acuity (VA), intraocular pressure (IOP), glaucoma medical therapy, complications, and success [VA>hand motions (HM), IOP≥6 mm Hg and ≤21 mm Hg, no subsequent glaucoma surgery]. Cumulative success rates for the non-NVG group and NVG group were 79% and 40% at 1y, respectively (P=0.038). No difference in the rates of surgical success were found between pars plana and anterior chamber tube placement. Preoperative IOP (mean±SD) was 30.3±11.7 mm Hg in the Non-NVG group and 40.0±10.6 mm Hg in the NVG group, and IOP was reduced to 15±9.5 mm Hg in the non-NVG group and 15±10.5 mm Hg in the NVG at 1y. Number of glaucoma medications (mean±SD) decreased from 2.7±1.3 in the non-NVG group and 2.8±1.3 in the NVG group preoperatively to 0.76±1.18 in the non-NVG group and 0.51±1.00 in the NVG group at 1y. Improvement in VA of ≥2 Snellen lines was observed in 25 (27%) eyes, although only 33% of non-NVG eyes and 2.3% of NVG eyes maintained VA better than 20/200 at 1y. Nonclearing vitreous hemorrhage was the most common postoperative complication occurring in 16 (17%) eyes, and postoperative suprachoroidal hemorrhages developed in 5 (5.4%) eyes.CONCLUSIONPPV-BGI is a viable surgical option for eyes with refractory glaucoma, but visual outcomes are frequently poor because of ocular comorbidities, especially in eyes with NVG. The location of tube placement does not influence surgical outcome and should be left to the discretion of the surgeon.展开更多

Refractory glaucoma was defined as eyes with an intraocular pressure(IOP) greater than21 mm Hg with maximally tolerated glaucoma medications, failed surgeries, or both. For all eyes with refractory glaucoma that underwent AGV implantation,data were collected on IOP, the best corrected visual acuity(BCVA) and glaucoma medications preoperatively and 4, 6, 12, 24 and 56 wk postoperatively. Logarithm values of IOP were calculated and compared. RESULTS: The study group was comprised of 30patients(30 eyes, 16 males and 14 females) with refractory glaucoma. Mean preoperative IOP was 39.3 ±13.8 mm Hg. Postoperative mean IOP was 15.7±7.1 mm Hg,19.6 ±12.8 mm Hg and 13.9 ±14.2 mm Hg at 12, 24 and56 wk respectively. BCVA was ≥6/60 in 11 eyes preoperatively, and five eyes had BCVA ≥6/60 at 56 wk postoperatively. Preoperatively, more than four medications were used to treat glaucoma in 21 eyes. At12 wk postoperatively, no medications were required to control IOP in 20 eyes. At 56 wk postoperatively, at least one medication was required to control IOP in 10 eyes.Over the entire follow up period, four eyes were treated with yttrium aluminium garnet(YAG) laser and 14 eyes required a second surgery. The AGV was removed in four eyes.CONCLUSION: AGV implantation reduced IOP and the number of medications required to control refractory glaucoma. However, there was a higher risk of decreased vision. Long-term follow up and prompt intervention are recommended.展开更多

AIM:To describe clinical findings and complications from cases of traumatic lens subluxation/dislocation in patients with secondary glaucoma,and discuss the multiple treating methods of operation combined with primary...AIM:To describe clinical findings and complications from cases of traumatic lens subluxation/dislocation in patients with secondary glaucoma,and discuss the multiple treating methods of operation combined with primary intraocular lens(IOL)implantation.METHODS:Non-comparativeretrospectiveobservational case series.Participants:30 cases(30 eyes)of lens subluxation/dislocation in patients with secondary glaucoma were investigated which accepted the surgical treatment by author in the Ophthalmology of Xi’an No.4Hospital from 2007 to 2011.According to the different situations of lens subluxation/dislocation,various surgical procedures were performed such as crystalline lens phacoemulsification,crystalline lens phacoemulsification combined anterior vitrectomy,intracapsular cataract extraction combined anterior vitrectomy,lensectomy combined anterior vitrectomy though peripheral transparent cornea incision,pars plana lensectomy combined pars plana vitrectomy,and intravitreal cavity crystalline lens phacofragmentation combined pars plana vitrectomy.And whether to implement trabeculectomy depended on the different situations of secondary glaucoma.The posterior chamber intraocular lenses(PC-IOLs)were implanted in the capsular-bag or trassclerally sutured in the sulus decided by whether the capsular were present.Main outcome measures:visual acuity,intraocular pressure,the situation of intraocular lens and complications after the operations.RESULTS:The follow-up time was 11-36mo(21.4±7.13).Postoperative visual acuity of all eyes were improved;28 cases maintained IOP below 21 mm Hg;2cases had slightly IOL subluxation,4 cases had slightlytilted lens optical area;1 case had postoperative choroidal detachment;4 cases had postoperative corneal edema more than 1wk,but eventually recovered transparent;2 cases had mild postoperative vitreous hemorrhage,and absorbed 4wk later.There was no postoperative retinal detachment,IOL dislocation,and endophthalmitis.CONCLUSION:To take early treatment of traumatic lens subluxation/dislocation in patients with secondary glaucoma by individual surgical plan based on the different eye conditions would be safe and effective,which can effectively control the intraocular pressure and restore some vision.展开更多

METHODS:...AIM: To evaluate efficacy of microcatheter-assisted trabeculotomy(MAT) in eyes with secondary glaucoma after congenital cataract surgery and explore its correlation with the different degree of trabeculotomy. METHODS: A retrospective analysis was conducted on patients who underwent the said procedure between September 2019 and September 2020. The patients were classified into two groups according to the degree of trabeculotomy(group 1: ≤240-degree;group 2: 240–360-degree). The intraocular pressure(IOP) and antiglaucoma drugs before and after operation was collected during the 12-month follow-up. RESULTS: Totally 27 eyes of 25 patients were included: 11(40.7%) eyes in group 1 and 16(59.3%) eyes in group 2. The mean IOP of all patients was 34.67±9.18 mm Hg preoperatively and 8.74±4.32, 9.95±5.65, 14.39±5.30, 16.02±4.37, 15.82±3.28, and 16.19±3.56 mm Hg 1 d, 1 wk, 1, 3, 6, and 12 mo after surgery, respectively. In all patients, there were significant differences in IOP at each time point(F=65.614, P<0.01). In each group, IOP after surgery was lower than that before surgery(all P<0.01), but there was no difference in the rate of IOP reduction between the two groups(P=0.246). Furthermore, the amount of anti-glaucoma medications reduced to 0.30±0.67(0–2) at 12 mo from 2.63±0.49(2–3) preoperatively(P<0.01), and there was no difference between the two groups(P>0.05). At the end of follow-up, the partial success rate was 81.8% in group 1 vs 93.75% in group 2(P=0.549). Various amount of intraoperative and postoperative hyphema occurred in all eyes, which spontaneously absorbed or cleaned through paracentesis and irrigation. No other serious complications was observed.CONCLUSION: MAT can effectively reduce IOP in patients with secondary glaucoma after congenital cataract surgery with a high success rate and safety. And it can be used as the first choice for the treatment of secondary glaucoma after surgery for congenital cataracts.展开更多
BACKGROUND Retinoblastoma(Rb)is primarily found in infants or young children.The most common primary presenting sign of Rb is leukocoria.Rb is very rare in children who are 10 years old or older.Timely and correct dia...BACKGROUND Retinoblastoma(Rb)is primarily found in infants or young children.The most common primary presenting sign of Rb is leukocoria.Rb is very rare in children who are 10 years old or older.Timely and correct diagnosis as well as proper treatment are the key factors affecting the prognosis of Rb.CASE SUMMARY A 10-year-old girl with symptoms of vision loss,redness,swelling and pain in the right eye for 2 mo was admitted to our Department of Ophthalmology.The visual acuity of the right eye was graded as hand movement.The intraocular pressure of the eye was 46.9 mmHg.No substantial space-occupying lesion or characteristic calcified plaque was found in the eye.The patient underwent anterior chamber irrigation under general anesthesia on the same day of admission,and 2 mL of irrigation solution was saved for pathological examination.Histopathological examination of the anterior chamber fluid revealed cancer cells.A diagnosis of Rb with masquerade syndrome was made.The patient underwent enucleation followed by 6 rounds of systematic chemotherapy.A follow-up examination almost 9 years later found no relapse of Rb.CONCLUSION For older pediatric patients who have secondary glaucoma and uveitis symptoms without a clear cause of the disease and have no space-occupying lesion found by imaging examination,aqueous humor or vitreous humor examination is recommended for timely and correct diagnosis and appropriate treatment.展开更多

Methods: Phacoemulsification and implantation of a foldable intraocular len was performed in 12 patients(13 eyes) with uveitis, secondary glaucoma and complicated cataract. The complications, intraocular pressures (IOP), and visual acuity were observed postoperatively.
Results: No severe complication was found in the patients postoperatively or within the operation procedure. The visual acuity was improved after the operation ( P < 0.05) .The intraocular pressures and/or the number of antiglaucoma medications reduced in 3months of the follow-up time.

Methods:The ophthalmic and systemic features,B-scan and UBM characteristics were recorded.Therapy and efficacy were analyzed. Results:Marfans syndrome,in this case,caused bilateral iris-lens diaphragm anterior dislocation,anterior chamber flattening, pupillary block, angle closure,and finally resulted in persistent increased intraocular pressure (IOP). After undergoing pars plana vitrectomy and lensectomy combined with anterior chamber reformation, the visual acuity of the patient's right eye increased from 6/150 to 6/7.5.(best-corrected) and that of the left eye was improved from 6/100 to 6/10 (best-corrected). The IOP of the right eye fell to 18 mm Hg, and the left eye to 12 mm Hg. Conclusion:Marfan′s syndrome can cause bilateral anterior chambers flattening,and induce secondary angle closure glaucoma.Combined pars plana vitrectomy, lensectomy and anterior chamberplasty can re-form the anterior chamber, control IOP and maintain visual function.展开更多
AIM: To determine the incidence and risk factors of secondary glaucoma after pediatric cataract surgery.METHODS: Two hundred and forty nine eyes of 148 patients underwent cataract surgery without intraocular lens (IOL...AIM: To determine the incidence and risk factors of secondary glaucoma after pediatric cataract surgery.METHODS: Two hundred and forty nine eyes of 148 patients underwent cataract surgery without intraocular lens (IOL) implantation (group 1), and 220 eyes of 129 patients underwent cataract surgery with IOL implantation (group 2) retrospectively, were evaluated between 2000 and 2011. The outcome measure was the presence or absence of post-cataract surgery glaucoma, defined as an intraocular pressure (IOP) ≥26mmHg, as measured on at least two occasions along with corneal or optic nerve changes.RESULTS: The mean follow-up periods of group 1 and 2 were (60.86 ±30.95) months (12-123 months) and (62.11±31.29) months (14-115 months) respectively. In group 1, 12 eyes of 8 patients (4.8% ) developed glaucoma. None of the patients developed glaucoma after surgery in group 2. The mean age of the patients at the cataract surgery was (2.58±0.90) months (1 month-4 months) and the average period for glaucoma development after surgery was (9.50 ±4.33) months (4-16 months) in group 1. Three of the 12 glaucomatous eyes were controlled with antiglaucomatous medication and 9 eyes underwent trabeculectomy+mitomycinC surgery.Onepatientunderwent a second trabeculectomy+mitomycin C operation for both of his eyes.CONCLUSION:The incidence of glaucoma after pediatric cataract surgery is very low in patients in whom IOL is implanted. The aphakic eyes after pediatric cataract surgery are at an increased risk for glaucoma development particularly if they underwent surgery before 4 months of age.展开更多

The aim of this study was to evaluate the clinical outcomes of Ahmed glaucoma valve implantation (AGVI) in neovascular glaucoma (NVG) and non- NVG patients. Methods This prospective, non-randomized study included 55 eyes of 55 patients with refractory glaucoma; 27 had NVG (NVG group) and 28 had non-NVG (non-NVG group). All of the patients underwent AGVI. The NVG group was adjunctively injected with intravitreal ranibizumab/bevacizumab (IVR/IVB) before AGVI. Intraocular pressure (lOP) was the primary outcome measure in this study. Surgical success rate, number of antiglaucoma medications used, best corrected visual acuity (BCVA), and postoperative complications were analyzed between the groups. Results All of the patients completed the study (follow-up of 12 months). Kaplan-Meier survival curve analysis indicated that the qualified success rates in the NVG and non-NVG groups at 12 months were 70.5% and 92.9%, respectively; this difference was significant (P=-0.036). The complete success rates in the NVG and non-NVG groups at 12 months were 66.7% and 89.3%, respectively (P=0.049). Compared with preoperative examinations, the postoperative mean lOP and use of medications were significantly lower at all follow-up time points in both groups (all P 〈0.05). There were significant differences in BCVA between the two groups at the 12-month follow-up (X2=9.86, P=0.020). Cox proportional hazards regression showed NVG as a risk factor for surgical failure (RR=15.08, P=0.033). Postoperative complications were similar between the two groups. Conclusions AGVI is a safe and effective procedure in refractory glaucoma, but the success rate of surgery was related to the type of refractory glaucoma. The complete and qualified success rates of NVG patient adjunctive anti-vascular endothelial growth factor treatment are still lower than those of non-NVG patients.展开更多
AIM:To compare the effect of phacoemulsification on intraocular pressure(IOP)in patients with primary open angle glaucoma(POAG)and pseudoexfoliation glaucoma(PXG).METHODS:A retrospective comparative case serie...AIM:To compare the effect of phacoemulsification on intraocular pressure(IOP)in patients with primary open angle glaucoma(POAG)and pseudoexfoliation glaucoma(PXG).METHODS:A retrospective comparative case series conducted at the Glaucoma Department at the Association to Prevent Blindness in Mexico.The study enrolled consecutive patients having phacoemulsification with intraocular lens(IOL)implantation and a diagnosis of POAG or PXG.Data about IOP values and number of glaucoma medications used was collected at baseline,1,3,6 and12mo postoperatively.RESULTS:The study enrolled 88 patients(88 eyes).After phacoemulsification,there was a statistically significant reduction in IOP values and glaucoma medications use compared to baseline in both POAG and PXG patients(P〈0.001).In the POAG group,a 20%decrease in IOP values was evidenced,and a 56.5%reduction in the number of medications used at the one-year follow-up.The PXG group showed a 20.39%,and a 34.46%decrease in IOP and number of medications used,respectively.A significant difference in the meanΔIOP(postoperative changes in IOP)was evidenced between groups(P=0.005).The reduction of the postsurgical IOP mean values in both groups,the POAG group showed a greater reduction in IOP values compared to the PXG group.CONCLUSION:In both types of glaucoma,phacoemulsification cataract surgery can result in a significant IOP reduction(20%)over a 12mo follow-up period.The number of medications used is also significantly reduced up to 12mo after surgery,especially in the PXG group.展开更多

All patients underwent complete ophthalmic examination and intraocular pressure (lOP) measurement before surgery and at ld, weekly for the 1st month, 3, 6mo, and ly after surgery and yearly afterward for 5y. lOP was measured by Goldmann applanation tonometry and/or Tono-Pen. Complications and the number of anti-glaucoma medications needed were recorded. Success was defined as lOP less than 21 mm Hg with or without anti-glaucoma medication and without additional glaucoma surgery. RESULTS: Mean age was 23.1±19.9y. All eyes had at least one prior glaucoma surgery, lOP was reduced from a mean of 37.2±6.8 to 19.2±5.2 mm Hg after 5y follow-up with a reduced number of medications from 2.64±0.59 to 1.81±0.4. Complete and qualified success rates were 31.5% and 46.0% respectively at the end of follow-up. The most common complications were encapsulated cyst formation in 51 eyes (41.1%), complicated cataract in 9 eyes (7.25%), recessed tube in 8 eyes (6.45%), tube exposure in 6 eyes (4.8%) and corneal touch in 6 eyes (4.8%). Other complications included extruded AGV, endophthalmitis and persistent hypotony. Each of them was recorded in only 2 eyes (1.6%). ~ CONCLUSION: Although refractory glaucoma is a difficult problem to manage, AGV is effective and relatively safe procedure in treating refractory glaucoma in Egyptian patients with long-term follow-up. Encapsulated cyst formation was the most common complication, which limits successful lOP control after AGV implantation. AIM:To describe the referral patterns of children with primary childhood glaucoma(PCG)or secondary childhood glaucoma(SCG)and their presenting symptoms in Northern Tanzania.METHODS:A retrospective observational study ...AIM:To describe the referral patterns of children with primary childhood glaucoma(PCG)or secondary childhood glaucoma(SCG)and their presenting symptoms in Northern Tanzania.METHODS:A retrospective observational study of children<17 y with PCG or SCG who were referred to Kilimanjaro Christian Medical Centre(KCMC)Eye Department between 2000 and 2013 was conducted.Presenting symptoms,age at presentation,place of origin,distance to hospital,type of glaucoma,visual acuity,optic disc appearance(vertical cupto-disc ratio)and type of referral were described.RESULTS:Seventy patients with PCG and 27 patients with SCG were included in the study.Median age at first presentation was 1 y in the PCG group(range 0-16 y)and 9 y in the SCG group(range 1-15 y).In both groups around 87%of the children presented already with low vision(logM AR>0.48,better eye).Most of the children(60%)and their caretakers presented on their own initiative,while 24%were sent by different general health cadres and 16%by eye care professionals.Buphthalmos was the main symptom mentioned as a trigger for presentation.CONCLUSION:The study shows that most of the children presented late resulting in advanced stages of glaucoma at the time of initiation of treatment.The majority attended the referral eye department on their own initiative with buphthalmos being the most commonly 452described symptom.Awareness creation among caretakers of children,general health and eye care providers,ideally embedded in general child health promotion activities,is needed to increase and accelerate referrals.展开更多
AIM:To report outcomes of patients after intraocular lens(IOL)repositioning or exchange for the version of the uveitisglaucoma-hyphema(UGH)syndrome that does not include closed loop anterior chamber IOL(nUGH).METHODS:...AIM:To report outcomes of patients after intraocular lens(IOL)repositioning or exchange for the version of the uveitisglaucoma-hyphema(UGH)syndrome that does not include closed loop anterior chamber IOL(nUGH).METHODS:Chart review of patients with nUGH who underwent IOL repositioning or exchange by one surgeon were reviewed.The main outcome measures were best corrected visual acuity(BCVA)as a decimal fraction preoperatively and postoperatively after IOL repositioning or exchange.Clinical findings evaluated included the presence of uveitis,hyphema,elevated intraocular pressure(IOP),and other complications such as pigment dispersion or vitreous hemorrhage.The number of anti-inflammatory and glaucoma medications were assessed before and after IOL repositioning or exchange.RESULTS:The study included 14 pseudophakic eyes.The median time at the onset of contemporary UGH after cataract extraction and IOL implantation(CE/IOL)was7.5 y.IOL repositioning or exchange was performed at a mean duration of 8.1±4.7 mo(median:4 mo)after onset of UGH.The mean BCVA was improved from 0.45±0.26 preoperatively after onset of UGH syndrome to 0.76±0.22(P=0.016)after IOL repositioning or exchange.Among the14 eyes,uveitis,elevated IOP,and hyphema were present preoperatively in 13,13,and 6 eyes,respectively.Uveitis and hyphema resolved in all cases after IOL surgery.The mean IOP was reduced from 26.4±4.5 mm Hg preoperatively to 14.7±4.9 postoperatively(P=0.01).The mean number of glaucoma medications used was reduced from 1.7±1.1 medications preoperatively to 0.8±1.08(P=0.04)postoperatively.CONCLUSION:IOL repositioning or exchange is an effective treatment in many cases for medically resistant contemporary UGH syndrome.展开更多

文摘AIM:To compare the surgical outcomes of glaucoma drainage device implantation(GDI)and trans-scleral neodymium:YAG cyclophotocoagulation(CPC)in the management of refractory glaucoma after Descemetstripping automated endothelial keratoplasty(DSAEK).METHODS:This retrospective study on observational case series enrolled 29 patients who underwent DSAEK and posterior anti-glaucoma surgery(15 with GDI and 14 with CPC).The main outcome measures were intraocular pressure(IOP),glaucoma surgery success rate(defined as IOP of 6–21 mm Hg without additional anti-glaucoma operation),number of glaucoma medications,endothelial graft status,and best-corrected visual acuity(BCVA).RESULTS:The mean follow-up time was 34.1 and 21.0mo for DSAEK or glaucoma surgeries,both for the GDI and CPC groups.Both groups showed significant IOP reduction after glaucoma surgery.The GDI group presented a significantly higher success rate in IOP control than the CPC group(60%vs 21.4%,P=0.03).Both procedures significantly decreased the number of glaucoma medications(P=0.03).Forty percent and 57%of cases in the GDI and the CPC group,respectively,experienced endothelial graft failure during follow-up(P=0.36).Significantly worse BCVA after surgery was observed in the CPC group but not in the GDI group.CONCLUSION:Both GDI and CPC significantly decrease IOP in eyes with glaucoma after DSAEK.GDI is preferable to CPC in refractory glaucoma cases after DSAEK,as it manifests a significantly higher success rate for IOP control,similar endothelial graft failure rate,and relatively preserves BCVA than CPC.
文摘AIMTo evaluate outcomes of combined pars plana vitrectomy and Baerveldt glaucoma implant (PPV-BGI) placement for refractory glaucoma.METHODSThe medical records of 92 eyes (89 patients) that underwent PPV-BGI were retrospectively reviewed, including 43 eyes with neovascular glaucoma (NVG) and 49 eyes with other types of glaucoma (non-NVG).RESULTSOutcome measures were visual acuity (VA), intraocular pressure (IOP), glaucoma medical therapy, complications, and success [VA>hand motions (HM), IOP≥6 mm Hg and ≤21 mm Hg, no subsequent glaucoma surgery]. Cumulative success rates for the non-NVG group and NVG group were 79% and 40% at 1y, respectively (P=0.038). No difference in the rates of surgical success were found between pars plana and anterior chamber tube placement. Preoperative IOP (mean±SD) was 30.3±11.7 mm Hg in the Non-NVG group and 40.0±10.6 mm Hg in the NVG group, and IOP was reduced to 15±9.5 mm Hg in the non-NVG group and 15±10.5 mm Hg in the NVG at 1y. Number of glaucoma medications (mean±SD) decreased from 2.7±1.3 in the non-NVG group and 2.8±1.3 in the NVG group preoperatively to 0.76±1.18 in the non-NVG group and 0.51±1.00 in the NVG group at 1y. Improvement in VA of ≥2 Snellen lines was observed in 25 (27%) eyes, although only 33% of non-NVG eyes and 2.3% of NVG eyes maintained VA better than 20/200 at 1y. Nonclearing vitreous hemorrhage was the most common postoperative complication occurring in 16 (17%) eyes, and postoperative suprachoroidal hemorrhages developed in 5 (5.4%) eyes.CONCLUSIONPPV-BGI is a viable surgical option for eyes with refractory glaucoma, but visual outcomes are frequently poor because of ocular comorbidities, especially in eyes with NVG. The location of tube placement does not influence surgical outcome and should be left to the discretion of the surgeon.

文摘AIM:To evaluate the long-term results and complications of ahmed glaucoma valve (AGV) implantation in refractory glaucoma.· METHODS:A retrospective review of 13 patients (13 eyes) with refractory glaucoma who underwent AGV implantation and had a minimum follow-up of 18 months was performed.All patients underwent a complete ophthalmologic examination and intraocular pressure (IOP) measurement before surgery and at 1 month,3 months,6 months,1 year after surgery and yearly afterwards.Complications and the number of antiglaucoma medications needed were recorded.· RESULTS:Mean age was 27.3±16.0 years.All eyes (100%) had at least one prior incisional surgery.Mean follow-up was 61.3±30.8 months.IOP was reduced from a mean of 35.0±7.0mmHg to 18.2±7.9mmHg at 12 months and to 17.0±4.1mmHg at 96 months (P <0.05) with a lower number of medications from baseline,76.9% patients required additional procedures to achieve the success criteria set by previously published series.The most common complications were encapculated cyst formation in eight eyes (61.5%) and tube exposure in four eyes (30.8%).· CONCLUSION:Encapsulated cyst formation was the most common complication which hindered succesful IOP control after AGV implant insertion for refractory glaucoma.Despite cyst excision with anti-fibrotic agents,successful IOP reduction was not achieved in 76.9% of the patients without antiglaucoma medication.
文摘AIM: To evaluate the outcomes of Ahmed glaucoma valve(AGV) implantation surgery for refractory glaucoma.METHODS: This one-armed historical cohort study was conducted in 2011. Refractory glaucoma was defined as eyes with an intraocular pressure(IOP) greater than21 mm Hg with maximally tolerated glaucoma medications, failed surgeries, or both. For all eyes with refractory glaucoma that underwent AGV implantation,data were collected on IOP, the best corrected visual acuity(BCVA) and glaucoma medications preoperatively and 4, 6, 12, 24 and 56 wk postoperatively. Logarithm values of IOP were calculated and compared. RESULTS: The study group was comprised of 30patients(30 eyes, 16 males and 14 females) with refractory glaucoma. Mean preoperative IOP was 39.3 ±13.8 mm Hg. Postoperative mean IOP was 15.7±7.1 mm Hg,19.6 ±12.8 mm Hg and 13.9 ±14.2 mm Hg at 12, 24 and56 wk respectively. BCVA was ≥6/60 in 11 eyes preoperatively, and five eyes had BCVA ≥6/60 at 56 wk postoperatively. Preoperatively, more than four medications were used to treat glaucoma in 21 eyes. At12 wk postoperatively, no medications were required to control IOP in 20 eyes. At 56 wk postoperatively, at least one medication was required to control IOP in 10 eyes.Over the entire follow up period, four eyes were treated with yttrium aluminium garnet(YAG) laser and 14 eyes required a second surgery. The AGV was removed in four eyes.CONCLUSION: AGV implantation reduced IOP and the number of medications required to control refractory glaucoma. However, there was a higher risk of decreased vision. Long-term follow up and prompt intervention are recommended.

文摘AIM:To describe clinical findings and complications from cases of traumatic lens subluxation/dislocation in patients with secondary glaucoma,and discuss the multiple treating methods of operation combined with primary intraocular lens(IOL)implantation.METHODS:Non-comparativeretrospectiveobservational case series.Participants:30 cases(30 eyes)of lens subluxation/dislocation in patients with secondary glaucoma were investigated which accepted the surgical treatment by author in the Ophthalmology of Xi’an No.4Hospital from 2007 to 2011.According to the different situations of lens subluxation/dislocation,various surgical procedures were performed such as crystalline lens phacoemulsification,crystalline lens phacoemulsification combined anterior vitrectomy,intracapsular cataract extraction combined anterior vitrectomy,lensectomy combined anterior vitrectomy though peripheral transparent cornea incision,pars plana lensectomy combined pars plana vitrectomy,and intravitreal cavity crystalline lens phacofragmentation combined pars plana vitrectomy.And whether to implement trabeculectomy depended on the different situations of secondary glaucoma.The posterior chamber intraocular lenses(PC-IOLs)were implanted in the capsular-bag or trassclerally sutured in the sulus decided by whether the capsular were present.Main outcome measures:visual acuity,intraocular pressure,the situation of intraocular lens and complications after the operations.RESULTS:The follow-up time was 11-36mo(21.4±7.13).Postoperative visual acuity of all eyes were improved;28 cases maintained IOP below 21 mm Hg;2cases had slightly IOL subluxation,4 cases had slightlytilted lens optical area;1 case had postoperative choroidal detachment;4 cases had postoperative corneal edema more than 1wk,but eventually recovered transparent;2 cases had mild postoperative vitreous hemorrhage,and absorbed 4wk later.There was no postoperative retinal detachment,IOL dislocation,and endophthalmitis.CONCLUSION:To take early treatment of traumatic lens subluxation/dislocation in patients with secondary glaucoma by individual surgical plan based on the different eye conditions would be safe and effective,which can effectively control the intraocular pressure and restore some vision.

文摘AIM: To evaluate efficacy of microcatheter-assisted trabeculotomy(MAT) in eyes with secondary glaucoma after congenital cataract surgery and explore its correlation with the different degree of trabeculotomy. METHODS: A retrospective analysis was conducted on patients who underwent the said procedure between September 2019 and September 2020. The patients were classified into two groups according to the degree of trabeculotomy(group 1: ≤240-degree;group 2: 240–360-degree). The intraocular pressure(IOP) and antiglaucoma drugs before and after operation was collected during the 12-month follow-up. RESULTS: Totally 27 eyes of 25 patients were included: 11(40.7%) eyes in group 1 and 16(59.3%) eyes in group 2. The mean IOP of all patients was 34.67±9.18 mm Hg preoperatively and 8.74±4.32, 9.95±5.65, 14.39±5.30, 16.02±4.37, 15.82±3.28, and 16.19±3.56 mm Hg 1 d, 1 wk, 1, 3, 6, and 12 mo after surgery, respectively. In all patients, there were significant differences in IOP at each time point(F=65.614, P<0.01). In each group, IOP after surgery was lower than that before surgery(all P<0.01), but there was no difference in the rate of IOP reduction between the two groups(P=0.246). Furthermore, the amount of anti-glaucoma medications reduced to 0.30±0.67(0–2) at 12 mo from 2.63±0.49(2–3) preoperatively(P<0.01), and there was no difference between the two groups(P>0.05). At the end of follow-up, the partial success rate was 81.8% in group 1 vs 93.75% in group 2(P=0.549). Various amount of intraoperative and postoperative hyphema occurred in all eyes, which spontaneously absorbed or cleaned through paracentesis and irrigation. No other serious complications was observed.CONCLUSION: MAT can effectively reduce IOP in patients with secondary glaucoma after congenital cataract surgery with a high success rate and safety. And it can be used as the first choice for the treatment of secondary glaucoma after surgery for congenital cataracts.
文摘BACKGROUND Retinoblastoma(Rb)is primarily found in infants or young children.The most common primary presenting sign of Rb is leukocoria.Rb is very rare in children who are 10 years old or older.Timely and correct diagnosis as well as proper treatment are the key factors affecting the prognosis of Rb.CASE SUMMARY A 10-year-old girl with symptoms of vision loss,redness,swelling and pain in the right eye for 2 mo was admitted to our Department of Ophthalmology.The visual acuity of the right eye was graded as hand movement.The intraocular pressure of the eye was 46.9 mmHg.No substantial space-occupying lesion or characteristic calcified plaque was found in the eye.The patient underwent anterior chamber irrigation under general anesthesia on the same day of admission,and 2 mL of irrigation solution was saved for pathological examination.Histopathological examination of the anterior chamber fluid revealed cancer cells.A diagnosis of Rb with masquerade syndrome was made.The patient underwent enucleation followed by 6 rounds of systematic chemotherapy.A follow-up examination almost 9 years later found no relapse of Rb.CONCLUSION For older pediatric patients who have secondary glaucoma and uveitis symptoms without a clear cause of the disease and have no space-occupying lesion found by imaging examination,aqueous humor or vitreous humor examination is recommended for timely and correct diagnosis and appropriate treatment.

文摘Purposes: To evaluate the effect of phacoemulsification in the patients with uveitis,secondary glaucoma and complicated cataract.
Methods: Phacoemulsification and implantation of a foldable intraocular len was performed in 12 patients(13 eyes) with uveitis, secondary glaucoma and complicated cataract. The complications, intraocular pressures (IOP), and visual acuity were observed postoperatively.
Results: No severe complication was found in the patients postoperatively or within the operation procedure. The visual acuity was improved after the operation ( P < 0.05) .The intraocular pressures and/or the number of antiglaucoma medications reduced in 3months of the follow-up time.
Conclusion: phacoemulsification is the best way to treat the patients with secondary glaucoma and complicated cataract caused by uveitis. Eye Science 2001; 17: 8 ~ 10.
文摘Purpose:To evaluate the long-term outcomes of a non-valved,.Chinese-made Hunan aqueous drainage device(HAD) in patients with refractory glaucoma,.compared to trabeculectomy.Methods:This was a retrospective observational case series,including 27 patients with refractory glaucoma who either underwent HAD implantation (n=11) or trabeculectomy (n=16).The mean follow-up was 27.9±13.5 (mean±SD) months.Intraocular pressure(IOP),visual acuity and postoperative complications were measured.Results:IOP was significantly lower at the last follow-up in both two groups compared with the baseline IOP (HAD:58.4 to 19.0 mmHg,P<0.001;trabeculectomy:58.4 to 23.7 mmHg,P<0.001).One week,1 month and 1 year after the operation,the average IOP of the HAD group was significantly lower than that of trabeculectomy group(P<0.05 at all time points).However,the IOP did not differ significantly between the two groups at the time of last follow-up.Conclusion:HAD implantation serves as a good option to control IOP in refractory glaucoma.
文摘Purpose:To report the clinical manifestations and treatment of a case of secondary glaucoma evolving from bilateral anterior chamber flattening caused by Marfan′s syndrome. Methods:The ophthalmic and systemic features,B-scan and UBM characteristics were recorded.Therapy and efficacy were analyzed. Results:Marfans syndrome,in this case,caused bilateral iris-lens diaphragm anterior dislocation,anterior chamber flattening, pupillary block, angle closure,and finally resulted in persistent increased intraocular pressure (IOP). After undergoing pars plana vitrectomy and lensectomy combined with anterior chamber reformation, the visual acuity of the patient's right eye increased from 6/150 to 6/7.5.(best-corrected) and that of the left eye was improved from 6/100 to 6/10 (best-corrected). The IOP of the right eye fell to 18 mm Hg, and the left eye to 12 mm Hg. Conclusion:Marfan′s syndrome can cause bilateral anterior chambers flattening,and induce secondary angle closure glaucoma.Combined pars plana vitrectomy, lensectomy and anterior chamberplasty can re-form the anterior chamber, control IOP and maintain visual function.
文摘AIM: To determine the incidence and risk factors of secondary glaucoma after pediatric cataract surgery.METHODS: Two hundred and forty nine eyes of 148 patients underwent cataract surgery without intraocular lens (IOL) implantation (group 1), and 220 eyes of 129 patients underwent cataract surgery with IOL implantation (group 2) retrospectively, were evaluated between 2000 and 2011. The outcome measure was the presence or absence of post-cataract surgery glaucoma, defined as an intraocular pressure (IOP) ≥26mmHg, as measured on at least two occasions along with corneal or optic nerve changes.RESULTS: The mean follow-up periods of group 1 and 2 were (60.86 ±30.95) months (12-123 months) and (62.11±31.29) months (14-115 months) respectively. In group 1, 12 eyes of 8 patients (4.8% ) developed glaucoma. None of the patients developed glaucoma after surgery in group 2. The mean age of the patients at the cataract surgery was (2.58±0.90) months (1 month-4 months) and the average period for glaucoma development after surgery was (9.50 ±4.33) months (4-16 months) in group 1. Three of the 12 glaucomatous eyes were controlled with antiglaucomatous medication and 9 eyes underwent trabeculectomy+mitomycinC surgery.Onepatientunderwent a second trabeculectomy+mitomycin C operation for both of his eyes.CONCLUSION:The incidence of glaucoma after pediatric cataract surgery is very low in patients in whom IOL is implanted. The aphakic eyes after pediatric cataract surgery are at an increased risk for glaucoma development particularly if they underwent surgery before 4 months of age.

文摘Background Neovascular glaucoma is a refractory disease, and difficult to manage. The aim of this study was to evaluate the clinical outcomes of Ahmed glaucoma valve implantation (AGVI) in neovascular glaucoma (NVG) and non- NVG patients. Methods This prospective, non-randomized study included 55 eyes of 55 patients with refractory glaucoma; 27 had NVG (NVG group) and 28 had non-NVG (non-NVG group). All of the patients underwent AGVI. The NVG group was adjunctively injected with intravitreal ranibizumab/bevacizumab (IVR/IVB) before AGVI. Intraocular pressure (lOP) was the primary outcome measure in this study. Surgical success rate, number of antiglaucoma medications used, best corrected visual acuity (BCVA), and postoperative complications were analyzed between the groups. Results All of the patients completed the study (follow-up of 12 months). Kaplan-Meier survival curve analysis indicated that the qualified success rates in the NVG and non-NVG groups at 12 months were 70.5% and 92.9%, respectively; this difference was significant (P=-0.036). The complete success rates in the NVG and non-NVG groups at 12 months were 66.7% and 89.3%, respectively (P=0.049). Compared with preoperative examinations, the postoperative mean lOP and use of medications were significantly lower at all follow-up time points in both groups (all P 〈0.05). There were significant differences in BCVA between the two groups at the 12-month follow-up (X2=9.86, P=0.020). Cox proportional hazards regression showed NVG as a risk factor for surgical failure (RR=15.08, P=0.033). Postoperative complications were similar between the two groups. Conclusions AGVI is a safe and effective procedure in refractory glaucoma, but the success rate of surgery was related to the type of refractory glaucoma. The complete and qualified success rates of NVG patient adjunctive anti-vascular endothelial growth factor treatment are still lower than those of non-NVG patients.
文摘AIM:To compare the effect of phacoemulsification on intraocular pressure(IOP)in patients with primary open angle glaucoma(POAG)and pseudoexfoliation glaucoma(PXG).METHODS:A retrospective comparative case series conducted at the Glaucoma Department at the Association to Prevent Blindness in Mexico.The study enrolled consecutive patients having phacoemulsification with intraocular lens(IOL)implantation and a diagnosis of POAG or PXG.Data about IOP values and number of glaucoma medications used was collected at baseline,1,3,6 and12mo postoperatively.RESULTS:The study enrolled 88 patients(88 eyes).After phacoemulsification,there was a statistically significant reduction in IOP values and glaucoma medications use compared to baseline in both POAG and PXG patients(P〈0.001).In the POAG group,a 20%decrease in IOP values was evidenced,and a 56.5%reduction in the number of medications used at the one-year follow-up.The PXG group showed a 20.39%,and a 34.46%decrease in IOP and number of medications used,respectively.A significant difference in the meanΔIOP(postoperative changes in IOP)was evidenced between groups(P=0.005).The reduction of the postsurgical IOP mean values in both groups,the POAG group showed a greater reduction in IOP values compared to the PXG group.CONCLUSION:In both types of glaucoma,phacoemulsification cataract surgery can result in a significant IOP reduction(20%)over a 12mo follow-up period.The number of medications used is also significantly reduced up to 12mo after surgery,especially in the PXG group.
文摘AIM: To evaluate the long-term results and complications of Ahmed glaucoma valve (AGV) implantation in a cohort of Egyptian patients. METHODS: A retrospective study of 124 eyes of 99 patients with refractory glaucoma who underwent AGV implantation and had a minimum follow-up of 5y was performed. All patients underwent complete ophthalmic examination and intraocular pressure (lOP) measurement before surgery and at ld, weekly for the 1st month, 3, 6mo, and ly after surgery and yearly afterward for 5y. lOP was measured by Goldmann applanation tonometry and/or Tono-Pen. Complications and the number of anti-glaucoma medications needed were recorded. Success was defined as lOP less than 21 mm Hg with or without anti-glaucoma medication and without additional glaucoma surgery. RESULTS: Mean age was 23.1±19.9y. All eyes had at least one prior glaucoma surgery, lOP was reduced from a mean of 37.2±6.8 to 19.2±5.2 mm Hg after 5y follow-up with a reduced number of medications from 2.64±0.59 to 1.81±0.4. Complete and qualified success rates were 31.5% and 46.0% respectively at the end of follow-up. The most common complications were encapsulated cyst formation in 51 eyes (41.1%), complicated cataract in 9 eyes (7.25%), recessed tube in 8 eyes (6.45%), tube exposure in 6 eyes (4.8%) and corneal touch in 6 eyes (4.8%). Other complications included extruded AGV, endophthalmitis and persistent hypotony. Each of them was recorded in only 2 eyes (1.6%). ~ CONCLUSION: Although refractory glaucoma is a difficult problem to manage, AGV is effective and relatively safe procedure in treating refractory glaucoma in Egyptian patients with long-term follow-up. Encapsulated cyst formation was the most common complication, which limits successful lOP control after AGV implantation. However, effective complications management can improve the rate of success.
文摘AIM:To describe the referral patterns of children with primary childhood glaucoma(PCG)or secondary childhood glaucoma(SCG)and their presenting symptoms in Northern Tanzania.METHODS:A retrospective observational study of children<17 y with PCG or SCG who were referred to Kilimanjaro Christian Medical Centre(KCMC)Eye Department between 2000 and 2013 was conducted.Presenting symptoms,age at presentation,place of origin,distance to hospital,type of glaucoma,visual acuity,optic disc appearance(vertical cupto-disc ratio)and type of referral were described.RESULTS:Seventy patients with PCG and 27 patients with SCG were included in the study.Median age at first presentation was 1 y in the PCG group(range 0-16 y)and 9 y in the SCG group(range 1-15 y).In both groups around 87%of the children presented already with low vision(logM AR>0.48,better eye).Most of the children(60%)and their caretakers presented on their own initiative,while 24%were sent by different general health cadres and 16%by eye care professionals.Buphthalmos was the main symptom mentioned as a trigger for presentation.CONCLUSION:The study shows that most of the children presented late resulting in advanced stages of glaucoma at the time of initiation of treatment.The majority attended the referral eye department on their own initiative with buphthalmos being the most commonly 452described symptom.Awareness creation among caretakers of children,general health and eye care providers,ideally embedded in general child health promotion activities,is needed to increase and accelerate referrals.

文摘AIM:To report outcomes of patients after intraocular lens(IOL)repositioning or exchange for the version of the uveitisglaucoma-hyphema(UGH)syndrome that does not include closed loop anterior chamber IOL(nUGH).METHODS:Chart review of patients with nUGH who underwent IOL repositioning or exchange by one surgeon were reviewed.The main outcome measures were best corrected visual acuity(BCVA)as a decimal fraction preoperatively and postoperatively after IOL repositioning or exchange.Clinical findings evaluated included the presence of uveitis,hyphema,elevated intraocular pressure(IOP),and other complications such as pigment dispersion or vitreous hemorrhage.The number of anti-inflammatory and glaucoma medications were assessed before and after IOL repositioning or exchange.RESULTS:The study included 14 pseudophakic eyes.The median time at the onset of contemporary UGH after cataract extraction and IOL implantation(CE/IOL)was7.5 y.IOL repositioning or exchange was performed at a mean duration of 8.1±4.7 mo(median:4 mo)after onset of UGH.The mean BCVA was improved from 0.45±0.26 preoperatively after onset of UGH syndrome to 0.76±0.22(P=0.016)after IOL repositioning or exchange.Among the14 eyes,uveitis,elevated IOP,and hyphema were present preoperatively in 13,13,and 6 eyes,respectively.Uveitis and hyphema resolved in all cases after IOL surgery.The mean IOP was reduced from 26.4±4.5 mm Hg preoperatively to 14.7±4.9 postoperatively(P=0.01).The mean number of glaucoma medications used was reduced from 1.7±1.1 medications preoperatively to 0.8±1.08(P=0.04)postoperatively.CONCLUSION:IOL repositioning or exchange is an effective treatment in many cases for medically resistant contemporary UGH syndrome.
