In the digestion of amino acids,carbohydrates,and lipids,as well as protein synthesis from the consumed food,the liver has many diverse responsibilities and functions that are to be performed.Liver disease may impact the hormonal and nutritional balance in the human body.The earlier diagnosis of such critical conditions may help to treat the patient effectively.A computationally efficient AW-HARIS algorithm is used in this paper to perform automated segmentation of CT scan images to identify abnormalities in the human liver.The proposed approach can recognize the abnormalities with better accuracy without training,unlike in supervisory procedures requiring considerable computational efforts for training.In the earlier stages,the CT images are pre-processed through an Adaptive Multiscale Data Condensation Kernel to normalize the underlying noise and enhance the image’s contrast for better segmentation.Then,the preliminary phase’s outcome is being fed as the input for the Anisotropic Weighted—Heuristic Algorithm for Real-time Image Segmentation algorithm that uses texture-related information,which has resulted in precise outcome with acceptable computational latency when compared to that of its counterparts.It is observed that the proposed approach has outperformed in the majority of the cases with an accuracy of 78%.The smart diagnosis approach would help the medical staff accurately predict the abnormality and disease progression in earlier ailment stages. 展开更多

目的:分析桃红四物汤加减辨证和单纯西医治疗股骨头坏死临床疗效,探讨中医药治疗股骨头坏死作用机制,以期为股骨头坏死治疗方案选择提供依据。方法:选取2017年4月至2018年4月宝鸡市中医医院收治股骨头坏死患者78例作为研究对象,按照随... 目的:分析桃红四物汤加减辨证和单纯西医治疗股骨头坏死临床疗效,探讨中医药治疗股骨头坏死作用机制,以期为股骨头坏死治疗方案选择提供依据。方法:选取2017年4月至2018年4月宝鸡市中医医院收治股骨头坏死患者78例作为研究对象,按照随机数字表法将患者分为对照组和观察组,每组39例。对照组予髓芯减压手术治疗,术后常规预防感染、改善微循环,促进血循环等对症治疗。观察组在对照组基础上加用桃红四物汤加减辨证治疗,2组均同时治疗2周。观察2组患者治疗前、治疗末在血液流变学、红细胞聚集变化情况并比较;观察治疗前、治疗末Harris髋关节功能评分变化并比较;治疗过程中进行不良反应观察并比较;治疗末进行疗效比较。结果:1)2组患者治疗前全血高切、全血低切、全血中切、血浆黏度、红细胞聚集指数比较,差异无统计学意义(P>0.05),治疗末对照组较治疗前比较,差异无统计学意义(P>0.05)。治疗末观察组以上指标较治疗前显著下降(P<0.05),治疗末观察组患者以上指标显著低于对照组(P<0.05)。2)2组患者治疗前红细胞聚集程度狗情情况比较,差异无统计学意义(P>0.05),完成治疗后,2组患者红细胞无聚集率均较治疗前明显上升,且观察组明显高于对照组(P<0.05);3)2组患者治疗前Harris评分功能、疼痛、髋关节活动范围、下肢畸形和总分比较,差异无统计学意义(P>0.05),治疗末以上指标2组较治疗前均显著升高(P<0.05),治疗末观察组患者以上指标显著高于对照组(P<0.05)。4)完成治疗后观察组患者治愈率、总有效率明显高于对照组(P<0.05)。5)2组不良反应发生率上比较,差异无统计学意义(P>0.05)。结论:桃红四物汤加减能提高股骨头坏死疗效,改善股骨头微循环,提高髋关节功能,且不良反应少,能提高患者生命质量。 展开更多

康复护理对股骨颈骨折患者全髋关节置换术后功能恢复的影响 被引量:65

目的探讨康复护理对行全髋关节置换术治疗的股骨颈骨折患者术后功能恢复的影响。方法选择100例行全髋关节置换术的股骨颈骨折患者,按随机数表法分为观察组和对照组各50例,对照组术后采用常规护理,观察组术后采用针对性早期康复护理,比较... 目的探讨康复护理对行全髋关节置换术治疗的股骨颈骨折患者术后功能恢复的影响。方法选择100例行全髋关节置换术的股骨颈骨折患者,按随机数表法分为观察组和对照组各50例,对照组术后采用常规护理,观察组术后采用针对性早期康复护理,比较2组患者手术前后视觉模拟评分法(VAS)、生活质量(SF-36量表评分)、髋关节Harris评分以及临床疗效、并发症。结果 2组患者术后1周、1个月的VAS评分较手术前下降(P<0.05),且观察组低于对照组(P<0.05);手术1周、1个月后,2组患者SF-36量表评分较术前上升(P<0.05),且观察组评分高于对照组(P<0.05);术后1周、1个月、半年、1年时,2组Harris评分较术前升高(P<0.05),且观察组高于对照组(P<0.05);术后1年时,观察组疗效优良率92.00%高于对照组68.00%(P<0.05);观察组术后并发症8.00%低于对照组28.00%(P<0.05)。结论在股骨颈骨折患者全髋关节置换术后实施早期康复护理效果显著,可有效缓解疼痛,促进关节功能恢复,提高生活质量,临床应用价值高。 展开更多

Research on the characteristics of faults and their evolutionary history since the Cretaceous in the Suhongtu-Dagu depressions can provide a theoretical basis for geological evaluation of the coal seams in the Suhongtu Formation in the northern-central region of the Yin’e Basin.Using 3-D seismic-logging inversion techniques,seismic stratigraphic calibration,stratigraphic sequence delineation,and thickness calculations on the Suhongtu-Dagu depressions were carried out to clarify the planar and profile distributions of the faults,as well as the evolutionary history of these faults and the tectonic history of the depressions.The results of this study revealed that the distribution of the faults in the Suhongtu-Dagu depressions in the northern part of the Yin’e Basin varies with region,and the fault system was multi-period,orthotropic,north-east-trending,and north-north-east-trending,with a certain degree of inheritance in terms of the geological setting.Three types of faults were identified:Y-shaped fractures,reverse Y-shaped fractures,and parallel fractures,which can be classified as Paleozoic-Cenozoic continuous syncline faults and intra-depression faults from the top of the Permian to the Upper Cretaceous series and inter-stratigraphic adjustment faults within the Cretaceous System,respectively.The evolution of these faults can be divided into three phases:the controlling faults were the faults that existed before the Early Cretaceous and had been active since then;synclinal faults that formed during the Early Cretaceous;and modified faults that formed since the Early Cretaceous.The development and modification of the coal seams in the Cretaceous Suhongtu Formation in the Hari,Kuanzihu,and Babei sags were strongly controlled and influenced by a multi-phase complex fault system. 展开更多

目的探讨肝动脉阻力指数(Hepatic artery resistance index,HARI),miRNA-122a对脓毒症休克合并肝损伤的早期诊断及预后评估的临床价值。方法选择2016年12月至2019年02月沧州市中心医院EICU收治的176例脓毒症休克患者作为研究对象,根据... 目的探讨肝动脉阻力指数(Hepatic artery resistance index,HARI),miRNA-122a对脓毒症休克合并肝损伤的早期诊断及预后评估的临床价值。方法选择2016年12月至2019年02月沧州市中心医院EICU收治的176例脓毒症休克患者作为研究对象,根据有无肝损伤分为无肝损伤组(90例)、肝损伤组(86例)。肝损伤组患者再按肝损伤程度分为轻度肝损伤组(20例)、中度肝损伤组(25例)、重度肝损伤组(41例)。脓毒症休克合并肝损伤患者根据28d转归分为存活组(26例)和死亡组(60例)。选择健康体检者40例作为对照组。分析两组患者的临床资料,采用彩色多普勒超声测定肝动脉阻力指数(HARI),采用反转录-聚合酶链反应(RT-PCR)测定血清miRNA-122a表达量。采用受试者工作特征(receiver operating characteristic,ROC)曲线分析HARI、血清miRNA-122a对脓毒症休克合并肝损伤的早期诊断价值。采用二元Logistic回归分析影响脓毒症休克合并肝损伤患考预后的危险因素。结果①与对照组比较,脓毒症休克无肝损伤组、脓毒症休克合并肝损伤组患者的HARI、血清miRNA-122a呈递增趋势,差异有显著统计学意义(P<0.01)。ROC曲线分析显示,HARI、血清miRNA-122a诊断脓毒症休克合并肝损伤的AUC分别为:0.872[95%CI=(0.813,0.919)]、0.796[95%CI=(0.728,0.854)]。当HARI最佳临界值为0.738时,其诊断脓毒症休克合并肝损伤的敏感度为77.65%,特异度为83.53%。当miRNA-122a表达量最佳临界值为2.80时,其诊断脓毒症相关肝损伤的敏感度为71.76%,特异度为75.29%。当HARI联合miRNA-122a诊断脓毒症休克合并肝损伤的AUC为0.927[95%CI=(0.876,0.961)],最佳临界值为0.276时,其诊断脓毒症休克合并肝损伤的敏感度为91.76%,特异度为85.29%。②脓毒症休克无肝损伤组与轻度肝损伤组比较,HARI差异无统计学意义(P>0.05),而血清miRNA-122a差异有统计学意义(P<O.O1)。随肝损伤严重程度加重,HAR1、血清miRNA-122a呈递增趋势,重度肝损伤组显著高于轻度和中度肝损伤组,差异有显著统计学意义(P<0.01)。③与存活组比较,单因素分析显示,脓毒症休克合并肝损伤28d死亡组患者的肝损伤严重程度、APACHEⅡ评分、SOFA评分、PCT、Lac,miRNA-122a,HARI显著升高,差异有统计学意义(P<0.01)。④二元Logistic回归分析发现,肝损伤严重程度、APACHE D评分、SOFA评分、HARI,miRNA-122a是影响脓毒症休克合并肝损伤患者预后的独立危险因素。结论HARI联合血清miRNA-122a检测对评估脓毒症休克合并肝损伤具有较高的敏感度和特异度,且对预后评估有一定的临床价值。 展开更多
